    Although there are various methods of surgical treatment for glottic insufficiency following unilateral recurrent nerve paralysis, Isshiki thyroplasty type I is the procedure of choice. We recently devised a new method using ceramic blocks instead of silicone in Isshiki thyroplasty type I, in which the biological side-effects of silicone can be prevented. In developing this method, we discovered that fibrin glue is effective for fixing the ceramic block and stopping the bleeding of the wound. The simplicity of our method is a strong advantage.

    In the present study, the mode of action of Neurotropin on nasal allergic reaction was compared with that of ketotifen using guinea pigs (OS-animals), which resulted in non-specific hyperresponsibility by repeated nasal challenges with OS-oil. Daily administration of Neurotropin at doses of 10 to 50mg/kg i. p. for 8 consecutive days normalized the methacholine responsibility as well as the nasal response to the allergen in the passively sensitized OS-animals. Ketotifen at a does of 1mg/kg p. o. also inhibited nasal secretion markedly in the passively sensitized OS-guinea pigs, but decreased slightly the nasal methacholine hyperresponsibility. Thus, the effect of Neurotropin on nasal symptoms. i. e. watery secretion and sneezing, may be due to its inhibitory action on nasal hyperresponsibility.

    We investigated the clinical efficacy of Aztreonam (AZT) in 138 patients with otorhinolaryngological infections. A dose of 1g, 2g, or 4g of AZT was administered twice daily for a minimum of 5 days. The efficacy rate was 86% in chronic sinusitis (57 patients), 90% in chronic otitis media (40) and 80% in other diseaes (41). Out of 72 patients treated only with this antibiotic, 5 patients were assessed as excellent, 54 as good, 11 as fair, and 2 as poor, with an overall efficacy rate of 82%. Out of 66 patients treated with both AZT and other antibiotics which were effective to gram positive bacteria, 40 patients were assessed as excellent (p<0.001), 19 as good and 7 as fair, with an overall efficacy rate of 89%. The committee judged the total efficacy rate to be 97% in this study of 138 patients. Side effects included one patient with mild exanthem and two patients with mild diarrhea. None of the patients showed abnormal laboratory findings in the criteria as defined by Chemotherapy (Tokyo). From the above results, Aztreonam was considered to be a safe drug which is useful for the treatment of otorhinolaryngological post-operative infection, espe cially when combined with other antibiotics.

    A case of pleomorphic adenoma of the trachea was presented. The patient was a 41-year-old man who complained of stridor and slight dyspnea. Laryngofiberscopy revealed a round mass arising from th membranous portion of the tracheal wall and occupying about half of the tracheal lumen, at the level between the cricoid cartilage and the second tracheal ring. General anesthesia was performed by intubation through a tracheostoma made at the lower position of the trachea. The tumor was then removed through a vertical and horizontal incision of the trachea just beneath the cricoid cartilage. Histopathological examination revealed a pleomorphic adenoma in the trachea, which is so rare that only 8 such cases have been reported in Japan.

    Thirty-eight patients (37 males and 1 female) with oropharyngeal carcinoma treated at the Department of Otorhinolaryngology, University of the Ryukyus Faculty of Medicine between 1978 and 1985 were reviewed. The pathologic types were all squamous cell carcinoma. The most frequent complaints were pain or abnormal sensation in the throat. The carcinoma was located on the lateral wall in 15 cases, on the superior wall in 13 cases, and on the anterior wall in 10 cases. The clinical stages of the 38 cases were stage I in 5 cases, stage II in 5 cases, stage III in 1 cases, and stage IV in 17 cases, Thirtysix cases were treated with a combination of radiation and chemotherapy and two were operated upon and then irradiated. The dose of radiation ranged from 50 to 80 Gray. Bleomycin was the chemotherapeutic agent used most frequently. The five-year crude survival rate according to TNM classification (UICC 1978) was 80.0% at stage I 60.0% at stage II, 18.2% at stage III, and 23.5% at stage IV. The overall survival rate was 34.2%. Twenty-five patients died from the disease, and in 19 of these cases, the primary disease was the cause of death. It was considered that operative treatment provided better results and that efforts to reconstruct postoperative defects were important in regard to the patients' quality of life.

    A 67-year-old female, who had noticed an. anterior neck mass 2 weeks before her first visit to a physician, was referred to our hospital. The neck mass was 3.5cm in diameter with a soft and movable nature. The tumor size was not altered by changes of position. No symptoms other than the neck mass were found. Ultrasonic examination showed that the tumor was located between the external and internal jugular veins, and demonstrated the flow of blood from the internal jugular vein to the external jugular vein during the Valsalva maneuver. These findings suggested a diagnosis of venous aneurysm. At the time of the operation, the communication between the internal and external jugular veins was confirmed. This case of venous aneurysm represents the first report of an abnormal venous communication emanating from an anomalous jugular vein. However, the possibility of venous compressin with the omohyoid muscle in developing this aneurysm cannot be ruled out.

    Seven benign tumors in the external auditory canal were studied clinicopathologically. These cases included 3 adenomas, 1 osteoma, 1 melanoma, 1 histiocytosis and 1 epidermal cyst. Although the degree of both acinus and duct formation varied in all cases with adenoma, apocrine secretion was observed as a common histological finding. These findings tend to support the concept that all adenomas originate in ceruminous glands along the external auditory canal. To date, there has been no recurrence following the surgery. Both the histiocytosis and epidermal cyst were reported as rare cases in the region of the external auditory canal. No evidence of malignancy as invasion and/or recurrence was observed clinicopathological ly in this case of histiocytosis.

    We administered three tablets of Metharmon-F, which contains 1.0mg of pregnenolone, 0.1mg of testosterone, 1.0mg of androstenedione, 5μg of estrone, 0.5mg of androstenediol and 7.5mg of desiccated thyroid per tablet, daily for 2-4 weeks to 37 middle-aged female patients with an abnormal sensation in the throat who had been resistant to prior therapy with lysozyme chloride for 2 weeks or more. At the end of administration, the efficacy of the drug with regard to the throat discomfort was marked in 27% of the patients, moderate in 30% and slight in 16%. The drug was also effective against various accompanying symptoms such as fatigue, headache or stiff shoulders with a certain degree of correlation to its effective ness for the throat symptom. A mild side effect (dry throat) was reported by one patient (3%), but it disappeared during the period of drug administration. Metharmon-F was found to compensate for the ineffectiveness of lysozyme chloride in the therapy of female patients with throat discomfort. Therefore, the combined use of the two drugs may produce satisfactory results in the therapy of such patients.

    A case of a 78-year-old woman with adenoid cystic carcinoma occurring in the oral floor was presented. This tumor is highly malignant and occurs frequently in the salivary glands, particularly in the subman dibular gland and the minor salivary glands. Light microscopy showed the tumor to be characterized by small cells in a cribriform or solid arrangement. Electron microscopy revealed the tumor cells to have round to oval nuclei and the cytoplasm to contain relatively well developed mitochondria and rough endoplasmic reticulum. Myoepithelial-like cells were also found. In the present case, it was difficult to determine whether the tumor had originated in the sublingual gland or in the minor salivary gland. The patient underwent a resection of the tumor. Six months after treatment there is no evidence of recurrence or distant metastasis. The histological types, the treatment and the prognosis of this tumor are discussed.

    Congenital fistulas and cysts of the first branchial cleft are relatively rare anomalies as compared to those of the second branchial cleft. About 40 such cases have been reported in japan. A case of first branchial cleft cyst occurring in a 45-year-old female is described. The case: In May, 1989, the patient was found to have a congenital cyst. The main body of the cyst was situated below the right angle of the mandible and anterior to the sternocleidomastoid muscle, with a fistula ascending through the parotid gland, lying deep inside the facial nerve and having a blind ending at the floor of the external auditory canal. The anomaly appeared to correspond to Work's classification type II. Imaging techniques including X-ray computed tomography and ultrasonography were useful for preoperative diagnosis.

    Prostaglandin El (PGE1) exerts a vasodilating action and inhibits platelet functions. PGE1 therapy by intravenous drip infusion has been applied to patients with idiopathic sudden sensorineural hearing loss. We administered Limaprost alfadex, which is the first oral drug to contain PGE1, to 67 cases of sudden sensorineural hearing loss as outpatient therapy. The results were compared retrospectively with those of 79 patients who did not receive PGE1 therapy. We evaluated that outcome using the criteria of the Ministry of Health and Welfare in Japan, recovery rate and complete cure rate. There was no statistically significant difference in outcome between patients treated with oral PGE1 therapy and those not treated with PGE1 therapy. Also, upon detailed examination of 16 cases which showed complete recovery, we were unable to find common patterns.

    Two cases of carcinosarcoma of the hypopharynx were presented. The first patient was a 52-year-old female complaining of dysphagea. Laryngofiberscopy and pharyngography revealed a round mass arising from the posterior wall of the hypopharynx and occupying about half of the lumen. The tumor was completely excised with part of the pharyngeal wall using right iateral and suprahyoid pharyngotomy. Histopatholgical examination revealed a so-called carcinosarcoma. The second patient was a 75-year-old male complaining of dysphagea and dyspnea. Laryngofiberscopy and CT scan revealed a round mass arising from the right pyriformis sinus and almost obstructing the supraglottic space. Because of post cricoid invasion, the tumor was completely excised along with the larynx. Histopathological examination revealed a carcinosarcoma. To date, there has been no evidence of local recurrence or metastasis in either case. Previously, only 7 cases of carcinosarcoma of the hypopharynx have been reported in Japan.

    We determined platelet-activating factor (PAF)-induced aggregation of platelets in patients with nasal allergy before and during specific hyposensitization therapy. In addition, we determined the IgE, IgG level in the serum and eosinophil counts in the circulating blood. Before hyposensitization therapy, it was observed that the rate of PAF-induced aggregation of platelets tended to be more increased in the patients with a high level of serum IgE or IgG, compared with that in the patients with a low level of IgE or IgG. On the other hand, during hyposensitization therapy, the correlation between PAF-induced maximal aggregation rates and serum IgE, IgG levels changed from a positive correlation to a negative correlation. We supposed that the increase in the rate of PAF-induced aggregation of platelets was caused by IgE and IgG molecules binding to each Fc receptor on the platelet membrane. We further surmised that the reactivity of platelets was reduced during hyposensitization therapy because of decreased numbers of IgE and IgG molecules binding to the platelet membrane. Moreover, it appeared that the IgE and IgG molecules bound to the platelet membrane stimulated platelet activity. These findings suggest that PAF-induced aggregation of platelets in nasal allergies may be useful in evaluating the efficacy of specific hyposensitiza- tion therapy.

    Histamine contents were quantitatively evaluated in the nasal mucosa of toluene diisocianate (TDI)-sensitized guinea pigs, of the experimentally-induced hypersensitivity model, which were treated with or without the topical administration of glucocorticoids. Histamine contents were significantly increased after TDI sensitization for 4 weeks. Treatment with the newly developed glucocorticoid, fluticasone propionate (FP), designed for topical use, tended to normalize the elevated histamine contents. Single applications of TDI caused the rapid reduction of histamine in the nasal mucosa of TDI-sensitized guinea pigs. The reduction of histamine content due to fluticasone application may suggest that several mechanisms, including inhibitions to the accumulation of histamine-containing cells and/or to the reduction of histamine content in each cell and to the reduction of the rate of histamine release from mast cells, might contribute to the clinical efficacy of this glucocorticoid for the treatment of allergic rhinitis.
