    Recently, myringoplasties have been increasingly performed. The fascia, dermis, homogeneous fascia and homogenous dura have all been frequently used as materials for closing ear drum perforations. In addition, artificial membranes such as the chitin and collagen membranes have also been employed as closure materials. In our effort to follow up on this trend, we performed myringoplasty using a thin split-thickness skin graft on 24 ears of in 23 outpatients with chronic purulent otitis media for the closure of an ear drum perforation. The perforations were successfully closed in 21 of the 24 ears. The three important aspects of this method are considered to be: First, in 17 of the 24 ears, the perforations were closed with one skin graft. Second, in five of the 24 ears, side-slipping of the skin graft prevented perforation closure. In four of five ears, the perforations were closed by a second graft. A third graft was also planned in one of these five ears. Third, in two of the 24 ears, the recurrence of otitis media caused by acute rhino-pharyngitis prevented a closure of the perforation closure. The results of this clinical study confirmed that myringoplasty using a free skin graft is effective for closing ear drum perforations caused by trauma, paracentesis and ventilation tube insertion.

    We used a KTP laser to treat hypopharyngeal hemangiomas in two patients. The hemangiomas in two cases were located in the posterior surface of the right arytenoid region, and regressed rapidly after laser exposure and thereafter showed a good postoperative course. Our findings demonstrated that the KTP laser is the best known method for the treatment of localized hemangiomas since its wave length is highly absorbed by hemoglobin. It is thought that the KTP laser will thus be used more frequently in the future for the treatment of such vascular-rich lesions as hemangiomas in the field of otolaryngology.

    An unusual case of external nasal schwannoma in a 19-year-old male with a two-year history of a slowly expanding small external nasal mass in herein presented. As far as we could determine, only one previous case of external nasal schwannoma has been reported in the literature. This subcutaneous elastic mass(measuring 1.0 × 1.5cm in diameter) was located between the left lateral cartilage of the nose and the left lateral crus of the cartilage alaris major. After being diagnosed as a solid tumor, based on the CT, local findings and history, an excision was successfully made through the left limen nasi. The diagnosis was then pathologically confirmed to be a combined type Antoni-A and -B schwannoma. While the origin of the tumor could not be clarified, the tumor was thought to have originated from either a branch of the anterior ethmoidal or the infraorbital nerve.

    Various histologic types of tumors can originate in the nasal septum. We herein report twenty-three cases of tumors of the nasal septum which were treated over the past fifteen years. The pathological diagnoses of the tumors are as follows; twelve hemangiomas, two cases of verrucous vulgaris, one papilloma, three pleomorphic adenoma, three malignant melanomas, one adenocarcinoma and one malignant lymphoma. All benign tumors were surgically resected. Malignant tumors were treated by combination therapy of surgery, radiation and/or chemotherapy. A histologic examinations is thus considered to be mandatory for such cases since the optimum treatment depends on the histological findings.

    We report a case in which a fragment from a grinder was found to have remained in the nasal vestibule as a foreign body for 20 years. The patient was initially treated as a furuncle of the nose. However, a black and hard foreign body was found after a discharge of pus, and the patient was thus sent to our hospital. At examination, the patient remembered an accident which had taken place 20 years earlier. This case demonstrates the importance of taking a detailed past history, not only in cases with a foreign body, but in all such cases.

    We experienced a 58-year-old male patient who complained of a left earache and left hearing loss in conjunction with left facial paralysis. He was diagnosed as having Wegener's granulomatosis. His audiogram showed a severe mixed hearing loss in the left ear. Computed tomography demonstrated that mastoid air cells on the left were occupied with soft tissue without any evidence of bony erosion. A histological examination of the external ear mucosa demonstrated granulomatous inflammations. ANCA serological tests are thus called for when angitis is suspected since early treatment can prevent a progression of the disease, which may include prominent pulmonary or renal involvement.

    An 86-year-old man presented with apocrine cancer of the retro-auricular skin, associated with an infiltration of the parotid gland. The tumor was resected while preserving the facial nerve. After the left cervical lymph nodes were dissected, the wound was closed using a local flap. About one year and ten months later, the patient underwent a reoperation and radiotherapy because of peripheral facial paralysis and tumor recurrence in the neck. There have only been a small number of reports on apocrine cancer (a type of syringocarcinoma) and thus its epidemiological features remain obscure. Histologically, it has the features of adenocarcinoma, but its cytoplasm is eosinophilic. According to previous reports on the prognosis of this cancer, a relapse is common and metastasis is not rare. In particular, cutaneous apocrine cancer is said to have the potential to be highly malignant also showing rapid growth and metastasis. A surgical resection is the most reliable treatment for this cancer. Although there have only been a few reports on the use of radiotherapy and chemotherapy for this cancer, the prognosis may improve with a combination of various treatments.

    Sixty-three patients who underwent surgery for salivary gland tumors between 1975 and 1990 were clinically reviewed. Three factors, consisting of hardness, pain and immovability, were found significantly more frequently in malignant tumors than in benign tumors. Some clinical findings also strongly suggested malignancy and these included the presence or two of the above mentioned factors for malignancy. Benign tumors were therefore considered to be suggested by a combination of two of the three factors indicating a benign tumor which included tumor softness, an absence of pain and movability.

    A 22-year-old male presented with a complaint of swelling in his left upper neck. The swelling was elastic soft and movable. The CT and sonography findings demonstrated a cystic lesion, and the final diagnosis indicated a lateral cervical cyst. After performing an aspiration of the cyst, 99.5% ethanol was instilled for 16 minutes as a sclerosing agent, and thereafter was thoroughly cleaned out with saline solution. A CT scan 14 months after the ethanol instillation revealed no evidence of recurrence. This is the first time that ethanol has been applied to cysts in the field of otolaryngology and appears to be a safe and non-surgical treatment.

    We studied the uptake of antibiotics in the nasal and paranasal mucosa cultured by the collagen gel method. The uptake of the administered antibiotics was measured under various bioassay conditions. The time course study of the uptake of antibiotics in the cultured mucosal tissue demonstrated an increase in the concentration of antibiotics up to 2 hrs after the initiaion of the culture, and thereafter the concentration of antibiotics became constant after 2 hrs. It was thus found that the tissue uptake of the antibiotics depends on the concentration of the administered antibiotics. There was significantly higher uptake of antibiotics in the paranasal mucosa than that in the nasal mucosa. These findings thus suggest that the tissue culture by the collagen gel method is useful in monitoring the tissue uptake of the administered antibiotics.

    The positive rates of allergen-specific IgE antibodies were examined using the MAST system for a comparison between nasal allergic patients and healthy adults in Osaka. The patient group consisted of 47 males and 52 females, while the control group had 50 males and 50 females. The age of the subjects in both groups ranged from 20 to 29 years old. For the males patients, Dermatophagoides farinae showed the highest positive rate; 51%, while Japanese cedar was 47%, and thereafter was followed by timothy grass, sweet vernal and house dust (HD). In female patients, the highest positive rate was exhibited for Japanese cedar: 46%, while D. farinae was 40%. Although the positive rates of D. farinae and HD were highest in the male group compared with the female group, no difference in either group was observed for pollen allergens. Overall, the positive rates in the patients group tended to exceed those of the control group, especally regarding pollen allergens. In addition, a significant difference in the positive rate was seen in the patient group compared with the control group. The number of positive allergens was greater in both male and the patient group. As mentioned above, males tended to be sensitized more easily than females in both the patient and healthy adults groups.

    E0659 nasal gel spray was administered to both nostrils in single doses 6μg/spray and 30μg/spray, and multiple doses of 30μg/spray three times daily for 7 days in healthy volunteers.
    1. No adverse reactions or abnomal laboratory findings related to the text drug were observed.
    2. A bitter taste was experienced as a subjective symptom related to the test drug, but the symptom was transient and was not considered to be a contraindication for the continuation of treatment.
    3. No injury to the nasal mucosa was observed.
    4. The nasal ciliary movements were not affected.
    E0659 nasal gel spray was well tolerated both after a single-dose administration and after a 7-day t. i. d. administration regimen.

    To determine the optimal dose of E0659 nasal gel spray for the treatment of perennial allergic rhinitis, a double-blind comparative study was carried out in which the drug was administered in 3 doses twice daily(6μg/spray in group L, 15μg/spray in group M, and 30μg/spray in group. H).
    1. The final global improvement rating was higher in group M than in groups L or H.
    2. The utility rating was higher in group M than in groups L or H, and the difference in the utility rating was significant for groups L and M.
    3. No adverse reactions or abnormal laboratory findings related to the test drug were observed except for a “nasal hemorrhage” observed in one patient.
    4. The findings of this study therefore suggest the optimal dose of E0659 nasal gel spray to be 15μg/spray.

    To determine the optimal regimen for E0659 nasal gel spray in perennial allergic rhinitis, a multicenter single-blind study was carried out by administering E0659 15μgispray in both b.i.d. and t.i.d. regimens,
    1. The final global improvement rating was 52% for the b.i.d. group and 43% for the t.i.d. group, based on “good to excellent” responses. Therefore, the result between the b.i.d. and the t.i.d. regimens were considered to be clinically similar.
    2. The overall safety rating based on “no safety problems” was 97% for b.i.d. and 98% for the t.i.d. group, so no difference in the overall safety rating was observed between the b.i.d. and the t.i.d. regimens.
    3. The utility rating was 57% for the b.i.d. group and 48% for the t.i.d. group based on a finding of “useful to very useful”, so again the findings for the b.i.d. and t.i.d.regimens were found to be similar.
    Based on the above findings, the b.i.d. treatment regimen is therefore considered to be the optimal regimens for E0659 nasal spray.
