    A very rare case of ectopic parathyroid hyperplasia in the submucosa of the pyriform sinus was reported. The patient, a 47 years old male had chronic renal failure induced by hyperparath-yroidism. It was considered that this ectopic parathyroid gland was derived from the fourth brachial pouch and originated from embryologic development of the hypopharyngeal mucosa. The clinical findings and the treatment of ectopic parathyroid hyperplasia due to secondary hyperparathyroidism are discussed.

    The effects of the calcium ionophore A23187 on viral antigen expression in lymphoblastoid (Raji and P3HR-1), and epithelial (A2L/AH) cell lines was observed. After three days of culturing in the presence of ionophore, there was a significant induction of early antigen (EA) in the Raji cells. The viral capsid antigen (VCA) and early antigen (EA) increased dramatically in P3HR-1 and A2L/AH, the two EBV producer cell lines. When the calcium ionophore was used in conjunction with direct activators of protein kinase C (12-0-tetradecanoyl phorbol-13-acetate; TPA) a greater induction of viral antigens was seen than with either agent alone. It is postulated that activation of protein kinase C is required for the expression of the viral genome.

    A questionnaire investigation was conducted at the Keio Hospital Department of Otorhinolar-yngology over a period of 8 years, from 1977 to 1984 ; 322 pediatric patients (222 males and 100 females) diagnosed with vocal nodules were surveyed. As a result, we conclude that strobovideography is required to determine the appropriate surgical treatment in male patients. The severity of stiffness is judged by the degree of wave motion of the vocal fold.

    The phonatory organ of the chicken is called the syrinx and is located in the carina. There are two medial syringeal membranes and two lateral syringeal membranes in the syrinx and these membranes produce glottal sound by means of their vibration. An androgenic hormone was experimentally injected into a female chicken. As a result, its vocal character changed to a breathy hoarseness. The syringeal membranes were observed by scanning electron microscopy after androgenic dysphonia was detected in the voice. Organic changes such as epithelial keratinization and irregularities of collageneous fibers in the lamina propria were observed.

    It is well known that there are three types of bronchial reactions in asthma: Immediate response (IR), late response (LR) and delayed response (DR). It is also reported that similar responses are seen in the skin. In this study, we examined the three types of skin reactions in 74 subjects with Japanese cedar pollinosis. Twenty cases (27.0%) had a LR, 26 cases (35.1%) had a DR for a skin test against Japanese cedar. In comparison, only 15 of 140 cases (10.7%) of house dust allergy showed a positive LR for the HD skin test and 5 (3.6%), a DR for the HD skin test. There was no correlation between late onset skin response and severity of nasal symptoms. Late onset skin tests tended to be positive for elder patients. Eosinophils and IgE levels tended to be lower in the LR positive cases. The levels of CHSO increased in several cases, but there was no correlation to skin responses. These findings seem to warrant an examination of the correlation between skin and nasal responses.

    We report our findings on 2 cases of adenocarcinoma and one case of adenoid cystic carcinoma of the larynx. These carcinomas are known to arise from submucosal seromucous glands and their occurrence within the larynx is extremely rare. There are only 5 cases of the former and 10 of the latter documented in Japanese literature. In the ENT Department of the Hokkaido University Hospital, 472 cases of malignant laryngeal tumors have been treated in the past 15 years. Of these, Adenocarcinomas and adenoid cystic carcinomas accounted for 0.6%. Clinical and pathological features of these carcinomas differ significantly from those of squamous cell carcinoma in that they have the outlook of a submucosal tumor, and many arise from supraglottic regions where glands are abundant. A review of the literature for this study indicated that the primary choice of treatment for these tumors is aggressive surgery. Because of the long history of these tumors, one should keep in mind that a longer follow-up of these patients is necessary.

    The PM-carbinol olfaction test (PM-test) was performed on Japanese adults with olfactory disturbance or normal olfaction. The threshold level of the PM-test was compared with results reported in the United States and with results obtained from T & T olfactometry. The results are as follows: 1) The mean of the PM carbinol threshold for twenty adults with normal olfaction was 8.5 dS. This was higher than that of the United States. 2) In the twenty patients who revealed mild or moderate hyposmia, the PM-thresholds correlated with that of T & T olfactometry. 3) In the ten patients who suffered from severe hyposmia or anosmia, the PM-carbinol thresholds were off the scale.

    The intracordal injection technique was developed by Brtinings in 1911. This treatment has been accepted as a vocal rehabilitation for dysphonia caused by deficiencies of glottal closure. Most of the studies made using this technique have centered on injectable substances, but there are still many unaddressed problems with the injection technique. In this study, we measured the mechanical force required to perform intracordal injection of a constant amount of substance. In the conventional method, a large needle is required to reduce the necessary mechanical force. However, leakage of the injected substances could not be prevented from the pinhole which reamains after injection. We modified this technique by adapting a CITO JECT, a syringe normally used in the anesthesia of the peridontal ligament, to make the collagen injection. With this method, used in conjunction with a laryngoscopic fiberscope to aid in sighting the injection process, the force required has been reduced to 1/4 that of the conventional technique

  • A double-blind comparative clinical trial with Cefotiam (CTM)

    The clinical efficacy, safety, safety and usefulness of Flomoxef (FMOX) in treating suppurative otitis media (acute otitis media and acute exacerbation of chronic otitis media) were investigated by means of a double-blind comparative clinical trial using Cefotiam (CTM) as the control antibiotic. The total daily dosage for both antibiotics was 2 grams (potency), administered by intravenous injection in two 1 gram doses. Treatment was usually continued for seven consecutive days. The total number of patients in the study was 234.186 of these patients were used to analyze the clinical efficacy (94: FMOX analysis, 92 CTM analysis). 1. Committee analysis of the clinical efficacy of treatment for both acute and chronic forms of purulent otitis media showed efficacy rates of 58.5% for the FMOX group and 54.3% for the CTM group. Efficacy rates determined by the investigators' evaluations gave FMOX an efficacy of 68.1% and CTM, 59.8%. The differences in efficacy of the two drugs for both analyses were not statistically significant. For treatment of acute exacerbation of chronic suppurative otitis media, the committee rated FMOX to have a clinical efficacy of 57.5%, and CTM, 50.0%. The efficacy of FMOX was significantly higher (p=0.031). For cases of acute purulent otitis media, the committee rated the efficacy of FMOX to be 64.3% and CTM to be 90.0%. In these cases, the efficacy rate of CTM was significantly higher (p=0.032). 2. The bacterial eradication rates were not significantly different. Eradication of the causative microbes was 67.1% for the FMOX group and 61.4% in the CTM group. 3. Side effects developed in 7 Patients undergoing FMOX treatment, and in 4 patients undergoing CTM treatment. Abnormal laboratory test values were detected in 6 FMOX patients and 7 CTM patients. There were no significant differences between the two groups with regard to the incidence of side effects or abnormal laboratory findings. On the basis of the above results, FMOX is concluded to be an antibiotic whose usefulness can be favorably compared to CTM in treating types of suppurative otitis media, including acute exacerbation of chronic suppurative otitis media.

    The antibacterial activity of dibekacin (DKB) is more potent against Staphylococcus aureus and such gram-negative aerobes as Branhamella catarrhalis, Haemophilus influenzae, Klebsiella pneumoniae, Proteus spp. and Pseudomonas aeruginosa than against gram-positive anaerobic cocci and Streptococcus spp., indicating that the usefulness of DKB in the treatment of sinusitis depends on the causative bacteria and patient. In the case of chronic sinusitis, DKB is apt to be efficacious because the isolation frequency of DKB-susceptible gram-negative aerobes is high. DKB has good in vivo transfer to lesions and good in vivo retention, and when these are taken into account, DKB can clearly be judged a useful antibiotic for the treatment of sinusitis.
