    Die Geschichte der Hals-Nasen-Ohrenheilkunde im 19ten Jahrhundert, das praktisch der Gründungszeit dieses Fachs entspricht, wird in einer kurzgefassten Form beschrieben. Die Entwicklung wird in drei Abschnitten,(1) Otologie,(2) Rhinologie und Pharyngologie und (3) Laryngologie geschildert. Neben der allgemeinen Erklärung der Entwicklungsgeschichte finden auch die Beschreidbung über die relevanten Personlichkeiten in der Zeit, die zu der Enrtuwicklunrg beitruguen.

    We report a case of a 38-year-old woman with adult T cell leukemia (ATL) associated local tumor of the epiglottis and soft palate. Since her chief complaints were dyspnea and hoarseness due to the swelling of the epiglottis, she was initially diagnosed as having epiglottitis and not ATL. However, a high concentration of of serum anti ATL antibody and the histpathological findings of a soft palate when treated second time for dyspnea suggested the involvement of adult T cell leukemia in the epiglottis and the soft palate. The association of local mass lesions in the upper respiratory tract with ATL is rare. However, it is considered important to recognize the possibly involvement of ATL in the upper respiratory tract in south-western Japan.

    A 50-year-old female with fibrous dysplasia of the temporal bone is reported. Her external auditory canal had narrowed. Roentgenological examinations revealed ground-glass appearance of the temporal bone while a CT scan revealed a mass lesion with soft tissue density in the mastoid cavity. A histological examination revealed the typical features of fibrous dysplasia while a bone scan revealed an abnormality only in the right temporal bone. Based on the above findings, this case was diagnosed as monostotic fibrous dysplasia.

    Metastatic tumors of the the nose and paranasal sinuses are relatively rare. We herein report a case of prostatic cancer that metastasized to the maxillary bone. The patient was an 85-year-old male suffering from right zygomatic swelling and pain. He had undergone an operation to remove an adenocarcinoma of the prostate two years earlier. A histopathological examination of the maxillary tumor showed the presence of adenocarcinoma and positive staining by an immunohistochemical study for metastatic prostatic cancer. There have been 85 cases of metastatic tumors of the head and neck region in the Japanese literature. When one encounters a tumor of the nasal and paranasal regions, then the possibility of a metastatic tumor should be considered. In addition, the metastatic pathways to the head and neck are also discussed briefly.

    A 64-year-old male complained of a swelling in the right neck along with a sore throat in the pharynx and was diagonsed as having lung cancer (adenocarcinoma). The right tonsil was covered with dirty exudate. A biopsy showed tonsillar metastasis from adenocarcinoma of the lung. In spite of intensive chemotherapy, the patient became cachectic and died after 1.5 months. The metastasis to the tonsils was thought to be hematogenic. The clinical findings of tonsillar metastasis and a possible mechanism of the metastatic process are also discussed.

    The quality of life (QOL) of patients with head and neck cancer was evaluated by the use of a QOL checklist developed by the Japanese Ministry of Health and Welfare. This QOL checklist is considered to also be applicable to patients with head and neck cancer, but further studies are still called for in order to make a more precise evaluation in the head and neck field. The present study revealed that the total QOL score depends first of all on the patient's physical condition, secondary on their ability to lead an active life, thirdly, on psychological symptoms and fourth, on their ability to interact socially. In order to improve the QOL, therefore, an improvement in the patient's physical condition is necessary and thereafter the care of the psychological and social interaction problems should be dealt with. In patients with laryngeal cancer, no significant difference was observed between the laryngectomy group and the conservative treatment group. The main factor improving the total QOL score is thus considered to be improving the amount of social interaction in the conservative treatment group, while it is thought to depend on improving the patient's physical condition in the laryngectomy group.

    1) In order to elucidate whether or not chemotherapy enhances the anti-cancerous effect of hyperthermia. we analyzed the findings of experimental in vitro studies using head and neck carcinoma cells. 2) With regard to the temperature degree and the duration of heat treatment, the condition of heat treatment at 44° for 60 minutes showed the most anti-cancerous effect among all the conditions studied. 3) In terms of the combined treatment of heat and anti-cancerous drugs, e. g. CDDP, 5-FU, ADR, VCR, MMC, the anti-cancerous effect of combined treatment was higher than heat treatment alone. 4) With regard to the timing of the combined treatment, simultaneous treatment appeared to have a stronger anti-cancerous elect than each treatment administered separately.
    These findings thus suggest that simultaneous chemotherapy might possibly help augment the clinical effects of hyperthermia for patients with head and neck carcinoma.

    We recently devised new over-lay method for performing simple in-office myringoplasty. After denuding the epithelia around the perforation following surface anesthesia of the eardrum, a skin graft from the inferior portion of the cartilaginous ear canal was placed on the eardrum. This technique was performed on five patients with chronic otitis media, and resulted in a successful closure and a good hearing state in all patients. This new method offers many advantages including superior cosmetic results, the use of only light anesthesia, and no necessity to pack the ear canal.

    Forty-nine patients with low-tone sensorineural deafness were retrospectively studied. They were all characterized by a high incidences in female as well as a high incidence in younger people from 30 to 49 years of age. Their most common complaint was that of fullness and recovery rate was 88%. We studied the statistical relationships between both prognoses and age, sex, a hearing level of 1000Hz and the level of low-tone hearing loss, and no statistical relationship was found among these factors. In addition, we also compared two groups, one consisting of patients with low-tone sensorineural deafness only and the othercomprising patients with low-tone sensorineural deafness and symmetric high-tone sensorineural deafness. The prognoses of the former group were found to be statistically better than these of the latter group.

    We evaluated the plateau method in regard to pure tone audiometry. The plateau in air conduction audiometry (APL) is as follow: APL=(IaA-right AB gap)+(IaA-left AB gap)
    Therefore, there is no plateau in air conduction audiometry when the bilateral AB gaps are equal to IaA (the interaural attenulation for air conduction). Even if the AB gap of the nontest ear is equal to IaA, the bone conduction threshold of the test ear can therefore be determined by using the plateau method.

    Test results suggest that the new “testing by sound series counting”(TSSC) method can differentiate retrocochlear deafness (due to cerebrovascular disease) from sensorineural hearing loss and also measure temporal lobe damage. TSSC counts short sounds within a stimulus sound series. Research confirms that the number evaluated decreases when the rate of presentation accelerates. The current study consisted of 10 study patients, 6 with cerebrovascular disease and 4 with sensorineural hearing loss. The TSSC involved 4 separate series of gradually increasing sound rates, with the first series set at one stimulus per second (1000ms/SOA). The subjects were instructed to respond to the number of stimuli presented. A correct response by the control subjects of at least 90% at 4 selected rates was slower than 6 times per second (167, 250, 500 and 1000ms/SOA). The limiting rate, the rate at which correct answers began to markedly decrease, was observed in (83%) of the 6 patients with cerebrovascular disease, and only one demonstrated correct answers that were comparable to the control group. This patient showed obscure findings of temporal lobe damage, even in computed tomography. The responses from the 4 patients with a sensorineural hearing loss were at least 90% correct at rates as fast as 167ms/SOA, which was similar to the normal subjects.

    The clinical efficacy of erythromycin treatment for chronic paranasal sinusitis in children (under 12 years of age) was retrospectively investigated. Sixty-four patients with chronic paranasal sinusitis were treated and followed. The efficacy of treatment was determined based on the intranasal findings and nasal X-rays. The intranasal findings after treatment for 3 to 4 months improved in 55%, while the nasal X-rays showed an improvement in 45%. We statistically studied the relationship between these results and the difference in age, sex, site degree of clouding in the nasal X rays, nasal allergy complications and the duration of the disease before beginning this treatment. The shorter the duration before initiating this treatment, the higher the response rate (P=0.045) for the nasal X-rays. In addition, the patients without nasal allergy had a statistically higher response rate in the nasal X-rays than those with nasal allergy (P=0.046). No relationship was found, however, in the other cases.

    We performed preservative treatment using U-PASTA® KOWA on a total of 9 patients, including 8 patients with ulcers and fistulas after surgery for head and neck cancer and 1 patient with thoracic esophageal cancer. U-PASTA® is a povidone-iodine agent consisting of purified sucrose and a germicide which has a promotive effect on wound healing and is considered to be superior with respect to safety, homogenicity, expansiveness, and availability. The use of this agent for post operative ulcers and fistulas resulted in an 80% or greater improvement rate of various symptoms including granulation, depth of the lesion, serous discharge, and purulent discharge. In addition, no side effects were seen. The usefulness rate was determined to be 89%. This agent was thus considered to be useful for the preservative treatment of postoperative ulcers and craterform lesions in both the otolaryngologic and head and neck surgical fields.

    The propensity to develop depression in patients with dizziness was examined with an SRQ-D (a self-rating questionnaire for depression). The findings showed that 167(39.2%) of 425 patients were in a depressive condition. A total of 30 patients (13 with peripheral labyrinthine lesions and 17 with no vestibular disorder) were treated with an anti-depressant drug (Dosulepin). After treatment, 19 of these 30 patients showed an improvement in their depressive state. In addition, in most of these patients, the symptoms of dizziness also improved.

    The clinical efficacy of Banan® dry syrup on children with otorhinolaryngological infectious diseases such as otitis media, sinusitis, tonsillitis and pharngolaryngitis occur at a high rate in children in the otorhinolaryngological field. We examined the efficacy and safety on Banan® dry syrup (CDPX-PR) in the infections of 291 patients ranging from six months to sixteen years of age. A total of 192 different strains of organisms were isolated from those infections; The most predominant organisms was the S. aureus. The efficacy rate of Banan® dry syrup was 84% for acute and an acute exacerbation of chronic infections. Side effects occurred in 2% of the patients, and mainly comprised gastrointestinal disorders. We thus conclude that Banan® dry syrup is useful in treating otorhinolaryngological infections.
