    It is well known that antipyretic-analgesics are not unusual provocative substances for asthmatic patients, and that there is a cross-hypersensitivity between these drugs and tratrazine. However, tartrazine-induced asthma without Aspirin intolerance has rare1y been reported. When we carried out the oral challenge test on a 46-year-o1d man who was suspected of suffering from drug-induced asthma, the patient was diagnosed as having tartrazine-induced asthma, but not as an aspirin-induced asthma. Plasma histamine level was 0.94ng/ml before the challenge, and 1.98ng/ml after the challenge, and at the same time FEV_<1.0> significantly decreased. S1ow reacting substance (SRS) released from polymorphonuclear neutrophils before the challenge was 2133U. These observations might indicate that chemical mediators play a significant role in tartrazine-induced asthma. The prognosis was much better after the elimination of tartrazine.

    We studied the effects of plasma lipoproteins upon phosphatidylinositol (PI), sterol and DNA syntheses of lymphocytes activated by various mitogens and also differentiation to IgM-bearing cells of pokeweed mitogen (PWM) activated lymphocytes. 1) PI, sterol and DNA syntheses of lymphocytes activated by T cell mitogens phytohemagglutinin P (PHA-P) and concanavalin A (Con A)) were significantly suppressed by very low density lipoprotein (VLDL) and low density lipoprotein (LDL), respectively. 2) Those three syntheses were not suppressed by VLDL or LDL in the lymphocytes activated by B cell mitogens lipopolysaccharide (LPS) and Staphylococcus aureus Cowan I strain (SAC)). 3) In the PWM-activated T cells, VLDL or LDL suppressed PI and sterol syntheses but did not DNA synthesis. And, VLDL or LDL could not suppress DNA synthesis of PWM-activated B cells. 4) VLDL, LDL and high density lipoprotein (HDL) accentuated the differentiation of B cells to IgM-bearing cells, respectively. However, high concentrations of VLDL suppressed the differentiation on the contrary.

    We measured the leve1s of IgE and IgG antibodies to Candida albicans by radioallergosorbent test and enzymelinked immunosorbent assay, respectively, using Condida yeast cells as solid phase, in sera from asthmatic patients, nonasthmatic chi1dren and women with vaginal candidiasis. We demonstrated that: In nonasthmatic patients, the IgG levels were high in cord blood but declined at about four months of age, after that increased concurrently with age again, whi1e the IgE levels were lower than those of the asthmatics and independent of age. In asthmatic patients, both the IgE and IgG levels were significantly higher than in nonasthmatic chi1dren and of women with vaginal candidiasis, and there was a significant relationship between the IgE levels and the results of immediate cutaneous response.

    Airway responses to methacholine and to exercise were studied in 46 asthmatics and 22 normal subjects. Methacholine sensitivity was measured from dose-response relationships and defined as the log concentration of methacholine producing a 20% fall on FEV_<1.0> (PC20FEV1.0). On a separate day, 6min. treadmill exercise at the heart rate of 90% maximum predicted was performed and the severity of exercise-induced bronchoconstriction (EIB) was measured. Methacholine sensitivity differed in asthmatics and noma1 subjects in that asthmatics had much higher airway sensitivity. Twenty-nine of the 46 asthmatics responded to exercise with more than 20% fall in FEV_<1.0> indicating EIB, but none of the normal subjects did. It can be said that for EIB to occur, airway hypersensitivity caused by methacholine must exist. Methacholine sensitivity and severity of EIB were significantly related although the correlation coefficient was low (r=-O.32, p<0.05). To determine the predominant site of airway obstruction, density dependent index (DDI) was measured using air and He-O_2 flow volume curves. In methacholine-induced bronchoconstriction, predominant sites of obstruction were all in the large airway, whereas in EIB they were either in the large or smal1 airway. Thus the airway response to exercise and methacholine differed in the same asthmatics and it was heterogeneous in terms of the predominant site of airway obstruction in EIB. In the 29 asthmatics who had post exertional large airway obstruction, the correlation coefficent between methacho1ine sensitivity and severity of EIB was larger (r-=O.48, p<0.01) than that in all the asthmatics. In the remaining 13 asthmatics who had post exertional small airway obstruction, no correlation existed. These resu1ts suggest that a cholinergic bronchoconstrictive mechanism is more important in post exertional large airway obstruction than in post exertional small airway obstruction.

    The activity of natural killer (NK) cells in asthmatic children was examined using K562 cells as the target cells. 1. In control group, an elevation in the activity of NK cells was observed in children between 2 and 5 years of age. Age-related difference was not seen in children over 6 years. Sex-related difference, lower activity in females, was observed among children over 11 years of age. 2. The activity of NK cells was low in asthmatic children. This tendency was evident in agroup of children with asthmatic attacks induced by infections. Age-related difference was also, seen in this group, with the lowest activity recorded in children over the age of 11 years. Mononuclear cells adherent to glass did not influence NK activity. 3. Enhancement of NK activity by interferon (IFN) was observed in bronchial asthma patients as well as control subjects. When exposed to IFN at high concentrations, the enhancement rate of NK activity was higher than that of control group. 4. In relation to treatment, a decreasing tendency was seen in the group treated with theophylline.

    Eosinophil peroxidase (EPO) is presumed to be important for the specific function of the eosinophils in IgE mediated hypersensitivity. Nevertheless, no evidence supporting this presumption has been presented. We determined EPO content in peripheral blood eosinophils which were collected and purified from a total of 11 patients with such diseases as nasal allergy, asthma, drug eruption, Kimura's disease, eosinophilic lymphadenitis and hyperosinophilic syndrome. In addition, we determined both the amount of hydrogen peroxide produced in histamime oxidation with diamine oxidase and the amount of EPO necessary for degradation of hydogen peroxide produced. The enzyme activity of EPO per one eosinophil was 1.25×1O^<-4>in average purpurogallin units, and the total EPO of approximately 500 eosinophils was supposedly sufficient for degrading as much hydrogen peroxide as the oxidation of 1 mg of histamine generates as a final product. The present results suggest that the eosinophil leukocytes possibly contribute to cleansing site involved in IgE mediated reaction by promoting the degradation of hydrogen peroxide in the catabolism of histamine.

    In vitro methods for determination of the potency of allergen extracts, including radioallergosorbent test (RAST) inhibition, RAST direct titration, and quantitation of the major allergen by radioimmunoassay, were evaluated. In a comparative study of the relative potency of the four sugi (Cryptomeria japonica) pollen extracts and the four mite (Dermatopphagoides farinae) extracts, results of these in vitro methods correlated highly with those obtained from end point of prick test. These in vitro methods were sensitive and reproducible enough for practical usage and may be applicable for standardization of allergen extracts. In addition, quantitation of the major allergen by radioimmunoassay had an advantage over the other methods, since the radioimmunoassay could determine absolute amounts of the major allergen in the extracts.

    The prevalence of bronchial asthma and wheezing and occurrence of remission of asthma are examined in 57, 761 school children in Western Japan. On addition to this epidemiological research, the modified ATS-DLD children's questionnaire was employed. The results were as follows: 1) The prevalence-rate of asthma was 3.83% in males, 2.49% in fenmales and 3.17% overall in the age group .It was slightly lower in o1der children. 2) These rate are statistically different in accordance with the "life styles" of the children, i.e., the rate ate 3.83% for children in urban areas, 3.07% in middle areas not clearly classifiable as urban or rural, and 2.62% in rural areas. 3) There was no correlation in the prevalence-rate of asthma to smoking in the family or to home-heating. 4) The prevalence-rate of asthma in children with family histories of majior allergy was 2.4 times higher than in those without such family histories and the rate in children with family histories of asthma was 3.8 times the rate in those without such histories. 5) The prevalence-rate of wheezing was 4.17% in males, 3.62% in females and 3.90% overall. The rate among sixth graders was half that of first graders. 6) The rate of remission of asthma was 1.26% in males, 0.72% in females and 0.99% overall. This rate increased according to grade level in school.
