    The effects of the following glucocortiocids: hydrocortisone, prednisolone, betamethasone and triamcinolone acetonide, on histamine release were studied in human leukocytes in vitro. These drugs decreased histamine release in a dose dependent manner during IgE or Ca^<2+> ionophore-mediated stimulation. Hydrocortisone modulated histamine release at physiological plasma levels. Lag time was always observed before the manifestation of the glucocorticoid action to histamine release, and there seemed to be no difference among the four drugs tested. The inhibitory effects of the glucocorticoids were decreased when the leukocytes were preincubated with 17α-methyltestosterone or incubated in the presence of cycloheximide. These results were discussed with emphasis on the mechanism of glucocorticoid action to histamine release from human basophils.

    The role of neutrophils migrated in the sputum and bronchoalveolar lavage fluid of intractable asthmatics is not yet understood. The production of leukotrienes(LTs)and superoxide by various stimuli from neutrophil-rich fraction of 20 intractable, 55 non-intractable asthmatics and 18 healthy subjects was examined by high performance liquid chromatography and a cytochrome C reduction method. LTC_4, D_4 and B_4 production by calcium ionophore was significantly enhanced in neutrophil-rich fraction from asthmatics, compared with healthy subjects(p<0.01). Moreover, LTC_4 production from neutrophil-rich fraction of intractable asthmatics was also significantly higher than in those from non-intractable asthmatics(p<0.05). However. there was no significant difference in LTD_4 and B_4 production between both groups of asthmatics. LTC_4 production by IgG antibody and zymosan-activated serum was higher in neutrophil-rich fraction from intractable asthmatics than in those from non-intractable asthmatics, although less LTs production by IgE antibody was seen. Production of superoxide by concanavalin A was significantly increased in neutrophil-rich fraction from intractable asthmatics than in those from non-intractable asthmatics or healthy subjects(p<0.005). The production of LTC_4 stimulated by CaI and superoxide by Con A was related at r=0.61. LTs and superoxide released from inflammatory cells, especially neutrophils, may play an important role in the pathogenesis of intractable asthmatics, comparable to the role of histamine released from basophil-mast cells in mild and atopic asthmatics.

    Food allergy has been considered to be an important position of allergic children. We measured specific IgG_4 antibody titers to egg white, milk and soybean by ELISA, and compared with IgE RAST score of 3 food allergens and other factors. The following results were obtained. 1) IgE RAST score and IgG_4 antibody titers to 3 food allergens showed significant correlation: egg white(r=-0.3306), milk(r=0.8847), soybean(r=-0.4652). 2) Significant factors in IgG_4 antibody factors to egg white: atopic dermatitis, family history of allergic disease, severity of allergic disease. 3) Significant factors in IgG_4 antibody titers to egg white and soybean: sex, exercise-induced asthma, allergic rhinitis, IgE RIST, eosinophile counts, D.f.RAST score. 4) Significant factors in IgG_4 antibody titers to all 3 food allergens: age, onset age, allergic complications. Especially, IgG_4 antibody titers of all 3 food allergens were higher in proportion to the severity of allergic disease.

    Drug specific antibodies in sera from patients with drug-induced immunocytopenia were assayed by EIA using special glass beads. The specificity of anti-aspirin and anti-ampicillin antibodies against each drug was studied by the inhibition test. The following results were obtained. 1) IgG and IgM antibodies to ABPC, salicylate were detected in the sera of patients with drug-induced leuco- and thrombocytopenia. 2) In the patient, their antibody levels to drugs correlated well with their changes in fever, exanthema and blood profiles. 3) Inhibition tests using causative drugs and the absorbed sera with various kinds of antigens showed that their antibodies were specific against the causative drugs. 4) It was indicated that drug specific IgG and IgM antibody activity in the sera from patients with drug-induced immune leucocytopenia was high. These results suggest that EIA using special glass beads is useful for the identification of causative drugs in drugs-induced immunocytopenia.

    We studied the distributions of airborne pollen with a Durham sampler in Fukuoka City from 1972 to 1987. We examined the correlations between the annual variations in the Cryptomeria j. and Cupressaceae pollen counts and the annual changes in meteorological conditions. Annual changes in the day on which the Cryptomeria j. pollination season started were also examined. The Cryptomeria j. pollens were the most counted, followed by Cupressaceae. Cryptomeria j. pollinated from February to March, and Cupressaceae from March to April in Fukuoka City. The variations in the annual pollen counts for Cryptomeria j. and Cupressaceae were remarkable, and the day on which the pollination season of Crytomeria j. started varied widely each year. The sum of these pollen counts correlated with the mean temperature in July of the previous year(r=0.878, p<0.001) and the beginning of the pollination season correlated with the mean temperature in January(r=-0.765, p<0.001). The variation in the number of new patients with pollinosis at the otorhinolaryngology department of Kyushu University correlated with the Cryptomeria j. and Cnpressaceae pollen counts(r=0.906, p<0.01). Therefore, we could make pollen forecast and it would be useful for treatment of the pollinosis.

    There have been many cases in which the values of Dermatophagoides farinae(D.f.) and Dermatophagoides pteronyssinus(D.p.) have been over the limit when they have been determined by a Phadebas RAST unit(PRU) made by Pharmacia Co. I calculated the actual value of D.f. and studied the influence of D.f. on 226 asthmatic children. The results obtained were as follows; 1. The mean value of PRU was 145.3+15.3(mean+S.E.) PUR/ml on D.f.(RAST). 2. The peak of the actual value occurred at the age of 4 yeers old. 3. There were good correlations between D.f. and IgE (RIST)・H.D.(RAST)・eosinophil(r=0.75・r=0.88・r=0.49). 4. The value of D.f. was differed significantly under the influences of complication and environment. 5. The value of D.f. was differed slightly according to family history. The value of D.f. was beyond my expectations and acquired sensitization by environmental factors helped to trigger the onset of allergic symptoms.

    Sixty nine subjects with bronchial asthma, atopic dermatitis, urticaria, allergic rhinitis and gastrointestinal allergy were investigated for food allergens by Pharmacia RAST RIA to study the clinical significance of offending allergens. The following results were obtained. 1) In egg allergic subjects, a decreasing reactivity pattern of egg white, egg yolk and chicken was found. The RAST for egg white was suggested to be a screening test for the egg allergy. 2) In cow's milk allergy, the ratio of positive RAST to casein higher than that of whey protein such as β-lactoglobulin and α-lactoalbumin. Casein showed almost same allergenicity as cheese. Positive RAST results to beef were found in some cases of cow's milk allergic subjects. 3) Wheat, rice, buckwheat, soybean and peanut were clinically important allergens, and showed high positive RAST ratio. Sesame seed was also found to be one of the clinically important allergens. 4) The usefulness of RAST to fish and crustacea was recognized, whereas there was a few subjects with fish and crustacea allergy. In meats, fruits and vegetables allergy, further investigation should be necessary. These results were suggested that the Pharmacia RAST RIA was useful to make a diagnosis of IgE-mediated food allergy.

    The usefulness of a commercially available whole blood theophylline assay kit(AccuLevel^<TM>, Siva Co.Ltd.) was studied. The principle of the kit is based on enzyme immunochromatography. The kit has several advantages: the sample volume employed to assay is very small(only 12 μl), reaction time is short enough(within 25 minutes), everything used in the test is contained in the kit, the procedure is very easy and so on. The coefficients(r) between the values obtained by the AccuLevel^<TM> kits of two different lots and fluorescence polarization immunoassay(FPIA) were 0.994 and 0.985, respectively. In conclusion, AccuLevel^<TM> was found to be a very reliable and useful assay kit for the purpose of monitoring of the real time theophylline levels in blood and giving appropriate treatment to patients with bronchial asthma.

    To elucidate the effect of substance P(SP) on acetylcholime(ACh) release from vagal nerve terminals, we performed the following experiment using guinea-pig tracheal smooth muscle. We measured the contractile responses to electrical field stimulation(EFS) or ACh in the presence and absence of SP or capsaicin. The following results were obtained: 1)Contractions evoked by electrical field stimulation(EFS) increased in the presence of a subthreshold dose of SP(a dose which did not induce any contraction), as compared with the control. 2)In contrast, SP did not alter the response to ACh. 3)Capsaicin also increased contractions evoked by EFS. 4)Tetrodotoxin or atropine suppressed contractions evoked by EFS completely. These results suggest that SP increases ACh release from vagal nerve terminals without affecting smooth muscle sensitivity. Furthermore, they suggest that endogeneous SP has the same effect on the vagal nerve terminals.
