    A HPLC method was developed for the determination of histamine released from washed blood cells of allergic patients after exposure to allergens in the presence or absence of plasma. The blood cells used were preserved in a vital state at 4℃ during at least 6 hours after blood-collecting. The responses of blood cells to an appropriately diluted series of a mite feces extract (Dff) solution were evaluated in three patients' groups (68 cases of hyposensitized, 59 cases of newly visited and 44 cases of the others) with bronchial asthma. Washed blood cells from the hyposensitized groups had a tendency to release histamine easily even with lower concentration of the Dff solution. Both the sensitivities of washed blood cells to Dff or mite body extract (Dfb) and the responses of the cells to diverse antigens in either extract differed largely in individual cases. The titer of blocking antibody could be expressed as the amount proportional to the concentration of Dff or Dfb.

    The bronchodilatory response to procaterol delivered by a metered dose inhaler with a spacer device (Volumatic^<【〇!R】>) and a corniced extension tube was studied in a double blind manner in 18 ashmatic children, who were brought to our emergency room with acute attacks. The mean percent increase in forced expiratory volume in 1 second (FEV_1) with the Volumatic^<【〇!R】> was 55.6±39.3%, and 46.2±37.4% with the extension tube. There were no significant differences in the mean percent increase in FEV_1, forced vital capacity (FVC), maximal flow rate at 50% of vital capacity (V_<50>), maximal flow rate at 25% of vital capacity (V_<25>) and peak expiratory flow rate (PEFR) between the two devices. It is concluded that there is no difference in the bronchodilatory effects of the two devices. The corniced extension tube has the advantage of being less expensive and more convenient to carry around.

    To predict the clinical efficacy of house dust mite immunotherapy (IT) for bronchial asthma, clinical factors before IT were analyzed in relation to clinical efficacy. The factors analyzed were severity, skin test threshold, age at which IT was started, duration of asthma, onset of asthma, FEV 1.0%, serum IgE levels before IT, the presence of allergens other than HD, family history of atopic disease, complications with other allergic diseases, sex, seasonality of attacks and the maintenance dose. The clinical efficacy ranging from good response to no benefit was well discriminated by this analysis. The rate of discrimination was about 90%, indicating clinical usefulness of the method. In this study, skin test threshold seemed to be the most important factor, followed by FEV 1.0%. While severity and age have been reported to be important, those who show good FEV 1.0% and low skin test threshold, regardless of their severity or age, may be good candidates for IT.

    In this case the patient was a 62-year-old male with a chief complain of sensory disturbance of the extremities. In January 1987 he was diagnosed as having bronchial asthma by a neighboring doctor and began to receive prednisolone (PSL) therapy at his hospital on an ambulatory basis. Later on, in January 1990, he developed dyspnea, epigastralgia, ecchymoses and muscle weakness of the lower extremities, with which he was admitted to the neighboring hospital, where he was started on treatment of asthmatic attacks with 30 mg/day of PSL. However, because muscle weakness of his extremities became gradually worse, he was transferred to our hospital. Physical examination on admission revealed an absence of knee and ankle jerk, sensory loss in the bilateral hands, legs and feet and diminished muscle strength of the extremities. Significant laboratory findings included leukocytosis, eosinophilia (4743/mm) and elevated serum IgE concentration (4900 IU/ml). Histologic evidence of vasculitis with an associated infiltration by eosinophils was noted on Skin biopsy. These findings led to a diagnosis of Churg-Strauss syndrome. Treatment was started with PSL at 40 mg/day. However, as asthmatic attacks and eosinophilia were refractory to the conventional therapies, concomitant lobenzarit disodium (CCA), 240 mg/day was initiated. On increasing the dossage of CCA to 320 mg/day because of unsatisfactory symptomatic responses elicited at the initial dose level, the patient became free from an asthmatic attack with a return to normal of his eosinophilic count. He was discharged virtually asymptomatic while being placed on a regimen of PSL at 5 mg/day.

    Serum soluble intercellular adhesion molecule-1 (sICAM-1) levels were measured in 54 asthmatic patients and 32 healthy subjects by sandwich enzyme-linked immunosorbent assay. Serum sICAM-1 levels in the asthmatic patients not undergoing steroid therapy were significantly higher than those in the healthy subjects and those in the asthmatic patients undergoing steroid therapy (p<0.01 and p<0.001, respectively). Serum sICAM-1 levels in the asthmatic patients during attack were not significantly different from those in the patients in remission. We studied changes in serum ICAM-1 levels in 13 asthmatic patients in remission and during attack and found that serum ICAM-1 levels during attack were significantly higher than those in remission (p<0.05). The origin of serum soluble ICAM-1 in asthmatic patients still remains unclear, but the up-regulated expression of ICAM-1 on the endothelium of the respiratory tract may contribute to the recruitment of eosinophils, neutrophils and lymphocytes to the airway in bronchial asthma.

    Peripheral blood eosinophils from normal healthy subjects were purified on a Percoll gradient and were incubated with H_3^<32>PO_4. ^<32>P-labeled eosinophils were stimulated by IL-5 under various conditions. After the reaction was stopped, SDS/PAGE and autoradiography were performed to determine the incorporation of ^<32>P into proteins. ^<32>P incorporation into the bands with molecular weights of 52 kDa and 65 kDa was obtained. ^<32>P incorporation into the band of 52 kDa was presumed to be non-specific. ^<32>P incorporation into the band of 65 kDa increased when it was incubated with 1〜3 U/ml IL-5. IL-5-induced ^<32>P incorporation into the band of 65 kDa was dependent on the temperature of the reaction system and the incubation time with IL-5. However, IL-5-stimulated eosinophils generated no significant superoxide anion as measured by MCLA-dependent chemiluminescence method.

    In the present study, we used the skin chamber method to determine histamine and tryptase released by continuous loading of a single antigen on the skin in order to evaluate dermal reactions produced and thereby obtain greater insight into the role of type I allergic reactions in children with atopic dermatitis. The subjects were 46 children, 23 males and 23 females aged 10.6±4.3 years on the average, who were being treated at the National Children's Hospital for moderate or severer atopic dermatitis. Mite antigen stimulation was carried out by the skin chamber method upon admission and histamine and tryptase levels in collected antigen solution were determined. Histamine levels in the chamber were increased significantly at 6, 12 and 24 hrs after stimulation compared to the control levels (p<0.01). Tryptase levels were increased 2 hrs after stimulation but decreased with time thereafter. Histamine and tryptase levels were significantly correlated 2 hrs after stimulation, with a correlation coefficient of 0.954 (p<0.01). No significant correlation was observed 24 hrs after stimulation. These findings indicate that children with severe atopic dermatitis have great skin reactivity and very sensitive to stimulation by antigens. Mast cells and basophils are thought to be involved in immediate and delayed type reactions, respectively.

    An 18-year-old male with a lifelong history of atopic dermatitis and recurrent severe Staphylococcus aureus abscesses was found to have hyperimmunoglobulinemia E. We measured IFN-γ production by peripheral blood mononuclear cells cultured with a protein A non-producing strain of staphylococcal antigen and serum specific IgG subclass antibodies to this antigen in this patient. IFN-γ production, when cultured with non protein A produced by this antigen in this patient was almost the same as that in healthy subjects. IFNγ m-RNA was detected at the dose of 0.0625 μg total RNA. The levels of IgG1, IgG3 and IgG4 were higher in this patient than in healthy subjects, while IgG2 was lower. We thus identified a case of hyperimmunoglobulinemia E syndrome with normal IFN-γ production.

    Neuropeptides and cytokinecs are known as substances which produce various effects on local disease sites. Many studies of these substances role in rheumatoid arthritis (RA) have been carried out in connection with synovial effusion and synovium, but very few in connection with sera. We studied the substances in sera from patients with RA, hypothesizing that the serum levels should reflect the degree of RA activity. We sclected nine neuropeptides and cytokines, and measured their serum levels in twenty-five RA patients. For comparison, ten healthy persons were also studied. The results showed that low levels of CRF, ACTH and cortisol, and the high levels of SP, IL-6 and TNF-α in sera from the patients with RA compared with those from healthy persons. IL-6 and TNF-α correlated with RA activity indexes. We concluded that some neuropeptides and cytokines should act actively on RA through the circulatory blood. The mesurement of serum neuropeptides and cytokines in patients with RA is a useful indicator of the degree of RA activity.
