    Superficial hemosiderosis of the central nervous system (SS) is a rare disease. However, patients with this disease eventually suffer from complete hearing loss, blindness and dementia. Therefore, the quality of life of such patients becomes very low. On the other hand, the early-stage diagnosis of this disease has recently become possible, based on the characteristic MRI findings.
    In the present the paper, the authors summarized the clinical features of 35 Japanese cases and described the pathological findings observed in one typical case. The predominant symptoms at the onset are a diminution of hearing ability and unsteady gait, especially in the otolaryngological or neurological fields. The possibility of the presence of SS patients among many patients with these two common symptoms exists. Therefore, it is recommended that doctors, especially otolaryngologists and neurologists, should pay close attention to this particular disease.

  • A GROUNDWORK TO PROMOTE HEALTH POLICY-BASED MEDICAL SERVICES

    All Japanese national hospitals and sanatoria are required to perform policy-based medical services, one of which is the medical treatment of intractable diseases. We investigated the catchment area of the Department of Clinical Neurology (DCN) of the National Iwate Hospital (NIH) and the current medical services of the area, to assess whether the hospital can successfully provide an intractable neurological diseases (IND)-based service.
    The catchment area of DCN-NIH's medical service for IND, which was determined by the distribution of the dwelling places of patients with IND who were treated at DCN-NIH, was within a radius of about 30km of the NIH. This area extended over Iwate and Miyagi prefectures, and contained a total population of 515, 000. In the area, in total, 20.1% of patients with IND were being treated at DCN-NIH, and if the area is restricted to within a radius of 15km of the NIH, the percentage increases to 49.1% of patients. No hospitals and clinics within and around this medical service area were expected to compete with NIH as providers of medical service for IND.
    These findings suggest that the number of patients who were treated at DCN-NIH may markedly increase when DCN-NIH increases its medical service for IND, and are positive encouragement for DCN-NIH to commit to concentrating on medical services for IND.

    Though, Hackethal bundle nailing is known as the technique for the internal fixation of humeral shaft fracture commonly, we've adapted this technique for proximal humeral fracture fixation with changing some surgical procedures from 1988. It is reported that the results of retrospective study of 28 patients treated with this technique (H-method). As with an average bone union period, in the case of 2 parts fracture, it was 2.9 months with conservative therapy, 2.8 months in the patients treated by the H-method. As with the JOA score, in the case of 2 parts fracture, patients with conservative therapy obtained 95 points on average, patients treated by the H-method 91.0 points, In the case of 3 parts fracture, patients with conservative therapy obtained 85 paints on average, patients treated by the H-method 95. Thus, in the case of 3 parts fracture, patients treated by the H-method had better results. Shoulder range of motion recovery period, was 3.7 months on average in patients with conservative therapy and 2.9 months in patients treated by the H-method, so the H-method was better. The surgical technique of H-method was easier and less invasive with good results as a treatment method for 2 parts and 3 parts fractures. Therefore, the H-method may be an excellent surgical method applicable not only to humeral shaft fracture but also to proximal humeral fracture.

    Gene researches have cleared the cause of various types of muscular dystrophy. However, there is a long way to reach the clinical application of the results to patients with muscular dystrophy. In the meantime, therapies for cardiac and respiratory failure of muscular dystrophy have improved the life span of patients. As a result, life styles of patients have changed to stress individuality and sociality in spite of severe illness, and medical cares and life cares are needed to maintain the quality of life of patients. In our symposium, we took the psychological problems that appear in daily lives in accordance with improving the quality of life of patients, and showed that staffs of various professions should take part to solve these problems. We should know that the cooperation of various professions is essential for daily care of the patients even if therapy for muscular dystrophy has developed.

    The periopertive pulmonary thrombo-embolism (PTE) has increased in Japan. PTE is a fatal disease, therefore the prophylaxis of PTE is very important. We report the two cases of managements of general and epidural anesthesia after the insertion of inferior vena caval (IVC) filter.
    One of two patients had been diagnosed as PTE and the other patient had been pointed out deep vein thrombosis in lower extremity preoperatively. Epidural anesthesia is reported to improve lower extremity blood flow and to prevent the increase in plasminogen activator inhibitor. We conclude that epidural anesthesis in combination with a prophylaxic IVC filter insertion without heparin therapy is an useful technique, because epidural anesthesia can be used effectively without the risk of hematoma formation of heparin therapy.

    Multiple non-tumorous lesions in bilateral basal ganglia, pons and cerebral peduncles as well as right cerebellar hemisphere by T2-weighted image of MRI in a 73-year-old woman with unsteadiness, somnolence, anorexia and bilateral Babinski's sign.
    Although the steroid therapy worked for the initial 2 months, she took turn for the worse with reducing the steroid dosage and the findings of MRI demonstrated the new lesions in bilateral frontal white matters, corpus callosum, left middle cerebellar peduncle and left cerebellar hemisphere.
    More 3 months later, at last, this case was diagnosed as malignant lymphoma by the brain biopsy and the whole brain radiation therapy (total 40 Gy) was performed. However, this treatment had never effect on her. The finding of autopsy was compatible with primary cerebral malignant lymphoma (PCML).
    When MRI findings demonstrate the multiple non-tumorous lesions in the brain, it is important that we should make efforts to do the early accurate diagnosis using the brain biopsy with a mind to PCML.

    A 49-year-old man was referred to our hospital for further evaluation of neutrophilia. The karyotype of bone marrow cells indicated they were Philadelphia (Ph1) chromosome positive and, therefor, the patient was diagnosed as having chronic myelogenous leukemia (CML). He was administered natural interferon-alpha (IFN-α), 3 million IU daily, from November 1999. During the first three months, a major partial cytogenetic response was obtained (13.2% Ph1 -positive cells in February 2940). However, during the same period he had an episode of agitation and in April a delirium appeared. The symptoms resolved when IFN-α was withdrawn. There is no abnormal sign in head-MRI and electroencephalogram study and he had no history of psychiatric illness. Neurologic toxicity has been described in the patients under treatment with IFN-α. However, IFN-α had induced a major cytogenetic response, thus rechallenge and continuation of IFN-α contributed in prolong his survival. He maintains major partial cytogenetic response during treatment with IFN-α, 3 million IU weekly.
