Multiple non-tumorous lesions in bilateral basal ganglia, pons and cerebral peduncles as well as right cerebellar hemisphere by T2-weighted image of MRI in a 73-year-old woman with unsteadiness, somnolence, anorexia and bilateral Babinski's sign.
Although the steroid therapy worked for the initial 2 months, she took turn for the worse with reducing the steroid dosage and the findings of MRI demonstrated the new lesions in bilateral frontal white matters, corpus callosum, left middle cerebellar peduncle and left cerebellar hemisphere.
More 3 months later, at last, this case was diagnosed as malignant lymphoma by the brain biopsy and the whole brain radiation therapy (total 40 Gy) was performed. However, this treatment had never effect on her. The finding of autopsy was compatible with primary cerebral malignant lymphoma (PCML).
When MRI findings demonstrate the multiple non-tumorous lesions in the brain, it is important that we should make efforts to do the early accurate diagnosis using the brain biopsy with a mind to PCML.
