Purpose: in order to investigate what kind of roles are supposed to be taken by the psychiatric department of general hospital in urban area for the benefit of the community, we followed up outpatients who mostly came from the community.
Research procedure: as a preliminary survey, we examined 415 new outpatients regarding their ways of attendance to outpatient clinic at the psychiatric dept, of Konodai National Hospital using their outpatients' case histories.
As a first step, we focused our attention on schizophrenic patients and examined the case histories of schizophrenic patients who were received as fresh outpatients from January to December in 1966.
Secondly, we examined the treatment histories and the way of adjustment using a questionnaire and home visiting interview.
Results: as the results of a preliminary survey, we found that the outpatients who were recom-mended to attend to hospital remarkably dropped out from the beginning month to the third month on their treatment process. The order of dropping out of outpatients is (1) neurotic patients, (2) schizo-phrenic patients, (3) epileptic patients.
According to the results of follow up study of schizophrenic patients during one year, the rate of patients who were residents of Ichikawa City was 15 percent. The rate of patients who had been continued to receive medical treatment was found to be 40 percent of all schizophrenic patients accepted as fresh cases for one year, and 25 percent patients of these continued cases was receiving medical treatment at the other clinics.
Also it became clear that the ways of adjustment of 60 percent discontinued patients were unex-pectedly good, and a quite few of them were not hospitalized and well adjusted.
The rate of these successful patients in Ichikawa City was 25 percent of whole patients. On the other hand, most patients who had ever been hospitalized were hospitalized for the second time.
From the above results, we found a large number of schizophrenic patients have not always necessity of hospitalization, and discussed repletion of outpatient clinic of hospital, community medical resources and cooperation of these resources.
