  • (Part 2) Studies on the Hospital Infection in the Wards

    The author has already reported the air condition of the operating room. This paper consists of investigation of air contamination of the wards in as well as of be clothes, preying as follows;
    1) Bedclothes already worn out, may be one of possible causes of contact infection and the bacteria isolated from them are strongly resistant to antibiotic drugs.
    2) The air of the wards was contaminated with bacteria from the blankets which the patients put them upon anothers under unsuitable heating.
    3) The persons who work in the wards would be subject to bacterial carriers with the contaminated air by use of unsuitable bedclothes.
    4) Electric vacuum cleaner is available to clean the wards to prevent air contamination from the origins.
    5) In phage typing, Staphylococci isolated from the wards were almost classified Type 80.

    After the various experimental conditions on the fractional analysis of serum protein by salting out method using phosphates were investigated; the values determined by this method were compared with those by Tiselius's electrophoretic method.
    The results of these investigations were as follows:
    1) As the turbidities corresponding to total protein or globulin, i. e, extinctions E1 or E2, and the A/G ratios calculated from these data, increase with time interval, the measurement should be performed within 30 minutes after developing the turbidities.
    2) While the extinction E1 and E2 at 37°C increase as much 17% as at 15°C, the average A/G ratio calculated at the same temperature decrease to 90% of the values at 15°C.
    3) The relative error at each measurements is ±11-3%.
    4) The extinction E1 and the concentration of to total protein correlates closely, i. e. the correlation coefficient is 0.89. Therefore, as long as the measurements of the protein content of unknown Samples are performed in parallel with the known, the content of the former are able to determine approximately by turbidimetry.
    5) The correlation coefficients of the procentual concentration of each protein fraction between measured by this and by Tiselius's method are: albumin, 0.89; α, β and γ globulin, 0.58, 0.90 and 0.87 respectively, These figures are considered to be in good agreement except γ-globulin.
    Because of the fractional analysis of plasma protein by salting out method using phosphates is simple and rapid to perform, and moreover the values obtained by this method correlates closely with those by Tiselius's method it seems to be suitable as a routine method for the fractional analysis of serum protein.

    There are many theories of pathogenesis of arteriosclerosis, however, in them anabolism of cholesterol is said to have some relation with arteriosclerosis, But patients who has arteriosclerosis do not always show hypercholesterinemia.
    We studied on decholesteric substance agreeing with Ahren's and many student's view point that hypercholesterinemia has possibility to cause arteriosclerosis in experimental medicine of the arteriosclerosis.
    In this report, we describe the decholesterinic effect of the iod-preparation. Jodlecithine is iodide compounds in which iod molecules bind between double ponds of lecithine, and this chemical compound is mixed by liquorice-powder (glycyrrhizin).
    1) When iod-lecithine was daily given 300γ to patients for 4 weeks, serumcholesterol decreased into about 70% in average and in about 60% of them. The patients who had sera-cholesterol over 250mg/dl showed the normal level in over 80% of them after taking the iod-lecithine for 4 weeks.
    2) When we had given 300γ daily to patients for 8 weeks, the decreasing of the seracholesterol titer was seen in about 80% of patients and was about 35% in average of all.
    3) There was no side effect. For example, thyroxin sometimes showes the serious side effect, i. e. chest pain and angina pectoris-like attack, We did not have any like these cases.
    In Japan, the people traditionaly believe that the eating of the sea-weeds may have good effect on the arteriosclerosis.
    The aged people has the hypofunction or degeneration of some endocrine glands. The thyroid gland and adrenal gland especially have large relation with the metabolism of the cholesterol.
    This Jod-lecithine supposed to affect the thyroid gland, then secondary the thyroid gland controls the hypercholesterinemia, The iod-lecithine has little effect on the B. M. R., so iod-therapy is useful or effctive for arteriosclerosis.

  • The Joint Study Units of National Sanatoria of Far

    1) Pulmonary tuberculosis, never received drug treatment before and caused by drug resistant tubercle bacilli (SM 10μg/ml, PAS 10μg/ml and INH 1μg/ml) were grouped in this study as the primary drug resistant patient. Frequency of such a patient in the period of 1958 to 1961 was 6.50% in 1958, 9.20% in 1959, 6.60% in 1960 and 7.00% in 1961, respectively, with the average of 7.33%.
    2) High therapeutic effect on 194 drug resistant tuberculosis patients, at the initial treatment, was obtained even with the drugs to which causative organism was resistant. Especially, combination usage of SM, PAS and INH brought the more faborable result. Percentage of culture positive for tubercle bacilli among patients with infiltrative exudative form treated with SM, PAS and INH among 6 monthes was observed in 20 to 30%, while being around 50% with productive fibrous form, Tendency of improvement in roentgenograph, at the initial treatment, was also noted in 80 to 90% of the former and about 30% of the latter case.
    3) In case the reversal of infectiveness was not resulted at the first therapy, the continuous therapy also failed to eradicate the bacilli.
    4) Of only two patients who had received for more than a year of therapy, the disease developed into serious advanced stage. Even in these cases, the advancement were accelerated due to the surgical failure and hemoptysis but not because of resistant bacilli.
    5) Therefore, therapeutic response of the tuberculosis patients, caused by drug resistant tubercle bacilli or by bacilli developed to be resistant in the course of treatment, was markedly differed, depending on the reactivity of host (phagocytotic activity, degree of immunity and sensitivity) at the site where drug and bacilli contact.

    In the central operating room in the Nagoya National Hospital, 4, 257 operations were performed during the year of 1961, among which 330 cases were under 10 years old and 146 cases were under 3 years old.
    205 cases were done by local anesthesia, 82 cases by general (69 intubation, 9 opendrop, 4 mask), 29 cases by spinal and 14 cases by intravenous anesthesia.
    For premedication, we use mostly Demerol and Atropine-sulfate in accordance with the body weight.
    In general anesthesia, most of the infants and children were inducted by cycropropane gravity method and intubated after the intravenous or musclar injection of S. C. C., then maintained with nitrous oxide, ether, cycropropane and halothane, by using of the Ohio infant circle apparatus.
    After the operation, we placed the patients in the oxygen tent and kept them in the recovery room until next morning. Anesthetic complications which we encountered were as follows, that is one acute cardiac arrest, one cause-unknown continous severe cyanosis during the operation, one classic ether convulsion and several cases of vomitting.

    In the field of mental healing, there are various methods, such as self-psychotherapy, dialog, group therapy, and therapeutic social club.
    Although dialog is the most important psychotherapy, this method, at the same time, involves the defects of limiting to the member of patients and it is also uneconomical in time and money.
    Group therapy is a dynamic and attractive method, but it is difficult to apply to the cases of psychosis.
    I believe, therefore, therapeutic social club is the most available and valid psycotherapy, because this method is economical and not limited to the member of patients.
    In order to put in practice, a therapeutic team should be organized among psychiatrists, psychologists, caseworkers, nurses, and nutrionists.
    They should think much of the human rights and must have familiar contact with the group of patients. As to the therapeutic room, I want to require a room big enough for about 200 patients coming together, and I should like to have the following three kinds of room in all, that is, a room of a television-set, a room in which they can play “Igo” and “Shogi, ” a room equipped with the table of ping-gong.
    It is desirable that the classification of officials among patients should be a chairman, two vice chairmen a commitee of the ward, a commitee of ping-pang, a commitee of tennis, a commitee of base-ball, and a commitee of “Igo-Shogi” etc.
    Patients meetings were held totally 27 times from October 1961 to October 1962.
    Sex Distinction: male 156-110, female 55-36.
    Age: 15-65 years old.
    Most of their opinions are about the problems of recreation.
    Among their opinions, 34 for sports and amusements, 17 for a tour by bus, and 12 for cinema.
    I could hear their opinions in a free and relaxed atmosphere, which could have never been heard from them in a consulting room.
    The mentally disturbed people seem to be those who failed in struggle for existence. To them it is the most important to give a self-confidence and to encourage their spontaneity.
    In order to produce satisfactory results, we should not direct and guide them but understand them and melt into them. The best therapeutic policy is to make them recover from their complex unconsciously
