    Aiming at the early completion of therapy, the authors tried a consolidation therapy using a massive Ara-C administration.
    The subjects included ten patients with adult acute nonlymphocytic leukemia who were treated at our hospital during the period between August 1985 and March 1988. After an initial remission was successfully induced, a dose of 1 to 3g/m2 of Ara-C was administered twice a day for four to six days. This was followed by the administration of daunorubicin 25 to 45mg/m2 for two days. The treatment was then discontinued without any maintenance therapy.
    Although the disease relapsed in three patients six to 16 months after the onset of remission, the state of remission was maintained for nine to 31 months in the other seven patients.
    The side effects included nausea and vomiting in all cases, conjunctivitis in four cases, dermatitis in three cases, hepatic dysfunction in two cases, and cerebellar dysfunction in two cases.
    This therapy resulted in almost the same degree of myelosuppression as did the remission therapy and no patient had fatal side effects during the treatment. Therefore, this therapy seemed safe as a concomitant adjuvant therapy to remission therapy.

    A possibility that dietary salt increases plasma glucose and insulin level in response to starchy food was investigated. When salt was added to bread, plasma glucose and insulin concentrations were higher than those with non-salted bread. Salt may increase the glucose and insulin level by accelerating the digestion of starch by amylase activity or accelerating the absorption of the liberated glucose by the small intestine.

    The pathogenic factors were investigated in patients with anti-HBs antibody positive chronic liver disease. In patients with chronic HBV hepatitis, the liver cell damage and liver function are considered to improve during the seroconversion from HBs antigen to anti-HBs antibody. For cases with anti-HBs antibody positive active chronic liver disease, the pathogenic factors should be considered other than HBV infection.
    In our 45 cases with anti-HBs antibody positive active chronic liver disease, the incidence of patients with alcohol abuse, blood recipient, serum autoantibody positive and without these episodes were 54.5%, 21.9%, 23.8% and 16.9%, respectively.
    In our study, the highest incidence of haptoglobin 2-2 occured in cryptogenic cases, although the incidence of haptoglobin phenotype in patients with anti-HBs antibody positive active chronic liver disease was comparable with that of the Japanese population. The incidence of haptoglobin 1-1 in cryptogenic cases was found different between our study and the European study.

    Mass-screening of neuroblastoma has been carried out for seven years. Among 104, 780 babies tested, 19 cases of neuroblastoma were found and treated. The incidence of detection was approximately 1:5, 500 babies tested. Among the babies, who gave negative results at 6 months' screening, 5 patients developed the tumor later. All of the five patients showed elevation both in VMA and HVA at the time of presentation.
    From the data, it was suggested that about 1/2-2/3 of the patients were saved by massscreening and treatment, although some cases among them might have regressed spontaneously without any treatment.
    To save pseudo-negative cases, a repeat examination might be necessary at 12 months of age.
    The tumors which were found by mass-screening, were not uniform in their character in spite of the early stage, and a case with chromosome abnormality of lp was reported.

    We have studied the outcome of epileptic seizures and social as well as occupational adaptability in 30 patients who underwent anterior temporal lobectomy for medically uncontrolled seizures. The surgical indication has been carefully scrutinized in accordance to our hospital criteria. As a result, complex partial seizures were satisfactorily controlled in all operated patients. The outcome has been followed up date for more than 1 year in 14 out of the 30 patients in which complex partial seizures were found completely controlled in 11 and markedly improved in frequency in 3 patients.
    The occupational placement of the 14 patients was also favorable; almost all patients being placed on a part-time job 6 months after the surgery and about a half on more or less full-time work 1 year and 6 months after the operation.
    Six patients were found to have failed in obtaining job in spite of the seizures being satisfactorily controlled. The reasons were their character disturbance which had been manifest before the surgery and made their social adaptation difficult.
    Needless to say, surgical treatments for epilepsy are indicated preferably for patients who are psychically intact and whose seizures are proven to be medication resistant.
    Nonetheless, as shown in our results, the improvement of the seizures is not an omnipotent key for the patients to become socially adaptable but socio-psychological intervention and appropriate drug therapy are mandatory in the post-surgery management.

    The authors reviewed the results of the treatment in 59 cases of metastatic brain tumors. Some of the cases underwent an operation and/or radiation therapy and others did not. The mean survival time and the survival rate were obtained for comparison.
    First, the mean survival time was surveyed for all cases. As compared with 6.3 months for the group in which the cases did not receive operation, the time for the group who received operation was 6.7 months. The value for the group in which the cases received radiation therapy was 8.4 months and was significantly higher (p<0.01) than the 3.2 months for the group in which the cases did not receive radiation therapy.
    Then, the authors examined for single metastasis. For the group of total removal, the rate of survival for the 2 months immediately after operation was significantly higher (p<0.05) than that for the group in which operation was not performed. However, for the time following 2 months post operation, the survival rate was reduced. For the group of radiation therapy with single metastasis, the rate of survival for the 7 months proved to be significantly higher (p<0.01) than that for the non-radiated group. For radiation therapy after total removal of single metastasis, however, no significant differences were found for the 4 months post operation, thus indicating the effectiveness of the operation. Despite additional radiation therapy after total removal of tumor, the authors experienced 3 cases of early recurrence.
    Based on these results, the authors formulated their therapeutic policy as follows:
    1) For metastatic brain tumors with symptoms, operations are indicated if two months or more survival time is expected even in cases of multiple metastasis.
    2) Although, in principle, radiation therapy should be performed after operation, it should not be used on cases with single metastasis where the total removal can be performed.

    Short latency somatosensory evoked potentials (SSEPs), auditory brain stem response (ABR) and electrically, elicited blink reflex were investigated in 17 severely handicapped patients, aged 2 to 21 years. All of them were bed-ridden and exhibited severe difficulty in swallowing, due to central motor deficits.
    In SSEPs, eleven cases showed loss of N1, low amplitude of N1 or prolongation of the interpeak latency of P3-NJ, suggesting brain stem or thalamo-cortical dysfunction. In two cases of them, the peak latency of P3 was prolonged. In ABR, prolongation of the interpeak latency of I-V or low amplitude of components were found in 8 cases. These cases were suggested to have the brain stem dysfunction. Prolongation of the peak latency of R2 and R2' or loss of R2 and R2' in blink reflex, suggesting brain stem or upper central dysfunction, were observed in 11 cases.
    SSEPs, ABR and blink reflex, objective and non-invasive neurophysiological examinations, are useful to evaluate the brain stem and cerebral function in severely handicapped patients.

    It has been demonstrated that the immunosuppressive therapy is clinically indispensable for organ transplantation. Based on various parameters, we studied pharmacokinetics of cyclosporin in 15 renal transplantation patients. We initially measured the drug concentration by RIA method but we recognized the values in RIA method to be unreliable, and so we measured the drug concentration by HPLC as a preferable method. Almost all cases were taking mizoribine in combination with cyclosporin in which decreased the side effects by reducing doses of cyclosporin. There were eight cases who have been kept in good control on cyclosporin trough level of under 30ng/ml.
    Doses of cyclosporin should be the reflection of the clearance and the time after the renal transplantation.
    In the future, such variables as absorption, renal and liver function, time after renal transplantation and individual disparity in AUC should be taken into consideration.

    The concentration of serum digoxin was measured in 130 inpatients to see the correlation among dosage, age and the serum digoxin concentration.
    The following results were obtained; The correlative equation was Y=0.47+0.74D/(140-A) (Y: serum digoxin concentration, ng/ml; D: dose/weight, mcg/kg; A: age, yr.), and the coefficient of correlation was 0.59. By putting Y=1.0 on the equation, the maintenance dose of digoxin can be calculated from weight and age, that is; the dose=0.71 (weight) (140-age).

    A vial-form preparation of thrombin is the only commercially available one, which is easily mistaken for an injectable preparation. Therefore, we developed a tablet of thrombin to make oral administration easier and studied the pharmaceutical characteristics of the developed thrombin tablet. From a pharmaceutical study, it was concluded that the results of weight variation and disintegration tests of the tablet conformed to the standards of the Japanese Pharmacopeia XI and the activity of thrombin was about 90% of the original vial forms.
    Therefore, a tablet containing 10, 000 units of thrombin was prepared and administrated to patients with esophageal varices after endoscofic sclerotherapy. This trial administration was effective in 9 out of 10 cases which was favorably comparable to the vial form preparation.
    From these results, it was concluded that the thrombin tablet is useful and makes oral administration easier.

    The excretion values of polyamine in the urine of 207 patients with breast carcinoma, 14 patients with benign breast diseases and 44 normal subjects were studied especially in relation to their alteration by the extension of malignancy in breast carcinoma patients.
    Urinary polyamine was not significantly elevated during early stages, but it was elevated in advanced stages of breast carcinoma. Excretion values of polyamine in the urine of patients with breast carcinoma without lymphnode metastasis and recurrence were 57.4±74.4 (positivity 74.8%) and 93.3±67.0 (100%) μmol/g. crnn, respectively. The results suggest measruement of urinary polyamine may be of value in monitoring tumor progression and metastatic invovlement.

    In our hospital about one hundred patients with chronic pulmonary disease need to be served with chest physical therapy. To surmount the difficulties caused by a lack of staff, place and time, we conducted pulmonary rehabilitation classes as group training of chest physical therapy beginning in July, 1985. Classes were held about once a month, and until September, 1987, 20 classes were held. We assessed this trial particularly on the basis of effectiveness, drawbacks and room for improvement. In this trial, patients' confidence in professional staff (doctors and nurses) became stronger and patients came to take advice by staff members more readily. Overburden of the staff may require closing of the classes unless professional physical therapists can be employed to continue them. In these classes we were unable to teach patients individiually and in detail. Although the number of patients with chronic pulmonary disease increased in our hospital, the number of hospital admittance as emergencies decreased.

    A 34-year-old female was admitted to our hospital, with complaints of epigastralgia, nausea and general fatigue on 4th October, 1985. Examination for gastro-intestinal system revealed no abnormal findings. However the patient mentioned that amenorrhea began within 2 years after her second delivery, and occasional headache happened.
    Incidental x-ray films of the skull showed balloon like sella suggesting a diagnosis of empty sella. The test of pituitary gland showed decrease of plasma LH, FSH, and TSH. All symptoms disappeared one month following administration of T3 (75μg/day). Gonadal functions recovered gradually to physiological cycle following 2 years and a half of T3 administration.
    An empty sella syndrome with gastro-intestinal symptoms and endocrine abnormalities is rather rare. T3 administration recovered the hormonal abnomalities.

    A 47-year-old man had acromegaly for over the past 20 years. In June 1057, he developed severe fatigue and decrease of bodyweight. He was admitted to our hospital with these complains and the diagnosis of diabetes mellitus.
    Blood examination showed hyperglycemia (BS: 884 mg/dl) and high titer of growth hormone (10.81 ng/ml). He was treated by human insulin for diabetes mellitus. X-P and CT showed arachnoid cyst or empty sella, but MRI showed clear adenoma in the sella turcica.
    He has been treated by bromocriptine and his necessary insulin unit is decreasing.

    A 56-year-old man was admitted to our department because of abdominal pain and a mass with tenderness in the left side of the abdomen. Laboratory data showed a leukocytosis. An abdominal plain film revealed two niveau formations. An abdominal echogram and an oral gastrographin meal examination revealed obstruction of distal jejunum and mild dilatation of jejunal loops.
    This patient was operated for acute abdomen. The surgical exploration revealed a small amount of straw-colored ascitic fluid and dilated small intestinal loops. A short segment of distal jejunum with reddish discoloration and induration was found, that caused obstruction of the small intestine. Two other similar segments were found at distal ileum and ceacum. These three segments were resected, and two live worms, about 3cm in length and about 0.1cm in diameter, were found, whose heads were burrowing into the mucosa.
    The diagnosis of intestinal anisakiasis was made postoperatively.

    We reported a case of gastric schwannoma which was encounterd at operation for early gastric cancer. The patient was a 59-year-old man with a chief complaint of abdominal distention. The endoscopic examination revealed a superficial elevated lesion in the posterior wall of the antrum and the diagnosis of gastric cancer (tubular adenocarcinoma) was made with the biopsies. Subtotal gastrectomy was performed. At the operation, in addition to the carcinoma, a subserosal mass (4×3×3cm) was also found in the lesser curvature of the middle corpus. Histologically, palisading arrangement of spindle shaped cells was significant. With Azan staining and silver impregnation, the tumor cells were considered to be non-muscular origin and diagnosis of schwannoma was made.

    A 17-year-old female was admitted to our hospital with hematemesis. Endoscopic examination disclosed a depressed lesion on the anterior wall of the gastric fornix. The covering mucosal folds showed hypertrophy and disruption, characteristic of IIc lesion.
    By biopsy the lesion was diagnosed as poorly differentiated adenocarcinoma. The resected stomach had a depressed lesion, 14×17mm in size, showing the same findings as observed at endoscopy. Histological examination revealed infiltration of carcinoma limited within the mucosa. Lymph node metastasis was negative.
    The incidence of early gastric carcinoma in young adults under 30 years of age is apparently limited. Especially any case under 20 years of age is extremely rare. In this paper, a case of early gastric carcinoma in a 17-year-old female was reported with a review of the literature.

    Pregnancy in the minor uterus of uterus bicornis rarely results in delivery of viable infants, having been reported in only 9 cases worldwide. In the present case, twin pregnancy of a female and a male fetuses in the two uteri resulted in survival of both infants despite two episodes of rupture of the minor uterus during pregnancy. This report is the first of successful delivery of twin pregnancy in a woman with uterus bicornis unicollis with rudimentary horn.

    It has been well known that the mental disturbance is one of the major side effects of adrenocortical steroid drugs. Emotional changes such as euphoria and/or depression were frequently observed in some patients treated with steriods. Recently, however, a variety of mental disorders other than emotional disturbances has been reported. In this paper, three cases of steroid induced psychosis are reported.
    Case 1: The patient was a 70-year-old male with pulmonary silicosis treated with dexamethasone (2-3 mg/day) who became a hypomanic state. Dexamethasone was switched to betamethasone in addition to administration of diazepam and thioridazine. One month later, the psychotic symptomes disappeared.
    Case 2: The patient was a 32-year-old female with SLE treated with dexamethasone (0.5-2.5 mg/day) who was presented in an acute hallucinatory state. Dexamethasone was exchanged for prednisolone, and levomepromazine was added. In 10 days, the symptoms cleared rapidly.
    Case 3: The patient was a 54-year-old male with bronchial asthma treated with the combination of hydrocortisone (500mg inj/day) and prednisolone (10-40 mg/day) who developed an auditory hallucination. The steroid was tapered and then discontinued while, haloperidol was prescribed. In 7 days, the symptoms cleared rapidly.
    All cases had neither antecedent episodes of psychosis nor psychotic patients in the families.
