As medical reform in Japan progresses rapidly, efficient execution of discharge plans and linking of regional medical services will become extremely important in the future. However, it is not appropriate to evaluate all these problems in a single framework.
In this symposium, the effective use of regional medical resources and the establishment of high-quality medical services were discussed, using the following key words: 1) medical care for the acute, subacute, and chronic phases, 2) network among hospitals, clinics, care facilities, and regional medical associations, and 3) teamwork among physicians, nurses, social workers, and other medical staffs concerned.
