    The diagnosis and treatment of refractory mood disorders were reviewed with special focus on depression. Refractory depression is generally defined when adequate treatment with antidepressants fails. Recent neuroradiological studies have shown that cerebral atrophy mainly in the temporal lobe, subcortical small infarcts and reduced cerebral blood flow are frequently observed in patients with refractory depression. Switching to another class of antidepressant, augmentation with lithium or thyroid hormone, or electroconvulsive therapy is used for the treatment. Prospective studies based on treatment algorithms are required to establish standard treatment of refractory depression.

    We reviewed the records of 18 patients who had central retinal vein occlusion at Kure National Hospital. Of the 18 eyes, 5 (27.8%) were nonperfused type and 13 (72.2%) were perfused type. Of the 13 perf used-type eyes, 1 (7.7%) become nonperf used type during follow-up.
    Visual prognosis of nonperf used-type eyes were poor and their corrected visions were less than 0.1 in a year.
    Of the 13 perf used-type eyes, 6 (46.2%) were good in visual prognosis, whose retinal hemorrhage and macular edema disappeared almost completely. Of the 5 eyes without marked macular edema and visual impairment, 4 (80.0%) were good in visual prognosis. Of the 8 eyes with macular edema which had decreased visions, 2 (25.0%) were good in visual prognosis.
    Of the 9 treatments with hyperbaric oxygen, only 1 (11.1%) had improvement in visual acuity after the treatment. Hyperbaric oxygen treatment had no effect on visual prognosis.
    Of the 18 patients, 8 (44.4%) had hypertension, 8 (44.4%) had diabetes mellitus, and 2 (11.1%) had glaucoma.

    We compared the serum marker and liver biopsy before the interferon-α therapy to those at the time of the occurence of hepatocellular carcinoma (HCC) in 15 patients with chronic hepatitis C virus infection (HCV) complicating with HCC after IFN-α therapy. Of 15 patients, 5 became sustained serum HCV RNA negative and 10 continued HCV RNA positive after IFN-α therapy. The follow-up period was 60.0 and 54, 3 months respectively. At the time of the occurence of HCC compared with that before IFN-α therapy, patients with sustained HCV RNA negative showed the decrease tendency of serum hyaluronic acid values and significant improvement of liver fibrosis in liver biopsy in contrast to in patients with HCV RNA positive indicating the increase tendency of serum hyaluronic acid values and progression of liver fibrosis in liver biopsy. Because all the stage of liver fibrosis except 1 patient was F 3-4 before IFN-α therapy irrespective of the response to IFN-α therapy, we concluded that patients with chronic HCV infection with stage F 3-4 of liver

    To construct large scale database for clinical perinatal research, Clinical Obstetric Information Network (COIN) started in 1996.
    1. Participants were 40 national hospitals (Fukuyama, Hamada, Hirosaki, IMCJ, Kanazawa, Kasumigaura, Kobe, Kochi, Kumamoto, Kure, Kyoto, Kyushu MC, Maizuru, Matsumoto, Miechuo, Mito, Miyakonojo, Nagano, Nagasaki-chuo, Nagoya, Nara, Narashino, NCVC, Okayama, Okura, Osaka, Osaka-minami, Otake, Saga, Sagamihara, Sendai, Shibukawa, Shimonoseki, Takasaki, Tochigi, Tokyo MC, Tosei, Yamanaka, Yokohama, Zentsuji) in 2000.
    2. Total mothers were 16, 299 cases, 1, 601 (9.8%) of those were preterm deliveries. Maternal transfer were 1, 034 cases (6.3%), postpartum hemorrhage were 4, 259 cases (26.1%), and blood transfusion were 74 cases (0.5%). Maternal mortality rate was 6.0 (per 100, 000 live births).
    3. Total neonates were 16, 606 cases, 1, 637 (9.9%) of those were preterm between 28-36 weeks, and 109 cases (0.7%) were between 22-27 weeks. Early neonatal death were 41 cases, mortality rate was 2.5 (per 1, 000 live births). Fetal death after 22 weeks were 90 cases, rate was 5.4 (per 1, 000 total births). Perinatal mortality rate was 7.8 (per 1, 000 total births).

    Policy-based Medical Service Network of Nervous and Muscular Disorders is engaged in the 4 areas of intractable neurological disorders: muscular dystrophy, intractable diseases of the nervous system, epilepsy and spinal injury. It is an affiliation of 8 regional coordinating hospitals and 33 local specialized core hospitals organized under the leadership of the National Center of Neurology and Psychiatry (NCNP). All the member institutes are currently operated by the National Government; however, the all except for the NCNP will be reorganized into independent administrative corporations in the fiscal year of 2004. From the nature of the field this network must develop and standardize the treatment including gene therapy, involve in genetic counseling, improve the patient QOL, and communicate with the agencies engaged in the welfare and education of handicapped subjects. Through these activities it is supposed to integrate medical care, clinical researches, education/training and information offering.
    In this context this symposium was intended to deepen our understanding of the expected role of this network. The panelists were invited from the NCNP, two patient organizations, the Tokyo metropolitan government which organize a local medical service network and a pharmaceutical company which develops and provides drugs for the rare diseases.

    A rare case of leiomyoma uteri in 19-year-old girl is presented. The patient noticed abdominal distension and left low back pain. The ultrasonography and MRI revealed a lower abdominal solid tumor, and this tumor was diagnosed as leiomyoma uteri. This tumor caused left hydronephrosis, and was successfully removed by myomectomy. The weight of the leiomyoma node was 1, 110g.
