    At the “Tachikawa Large-Scale Disaster Response Base”the disaster-related facilities of all relevant ministries and agencies have been stationed in a functional manner. In the event of a dis-aster the Base will act as a center, leading countermeasure activities, while the National Hospital Tokyo Disaster Medical Center will take charge of disaster-related medical care. During normal periods the Center will provide highly-advanced emergency medical care and comprehensive medical treatment, but will also commit itself to conducting clinical studies focusing on disaster-related medical care, as well as carrying out the education and training of medical practitioners, related health workers and the general public. In addition, the Center, as one of the core National Centers offering disaster-related medical care over a large-scale area, will make statements regarding its fundamental philosophy, function and role.

    In order to evaluate the nutritional state of patients with pancreatic diabetes, we measured BMI (body mass index), body fat, serum albumin, serum cholesterol, Hb, RBC (red blood cells), serum iron, serum TIBC (total iron binding capacity) in 11 cases (male) with pancreatic diabetes (due to chronic pancreatitis) and 25 cases (male) with primary diabetes (NIDDM). All of them had been treated in our outpatients clinic. Indices in the above 8 items were decreased in the pan-creatic diabetes patients compared with the primary diabetes patients. Especially, values of Hb, RBC, serum iron, serum TIBC indicated a significant decrease, and the decrease of serum iron was the highest (P<0.001 vs. primary diabetes). The rate of abnormal values was high in the above 4 items in the pancreatic diabetes patients. The present investigation shows that a high incidence of anemia was found in pancreatic diabetes patients. Furthermore, the anemia in a patient among them improved due to oral administration of iron preparation (no active peptic ulcer at that time). Iron-deficiency anemia should be taken into consideration as one of the common kinds of malnutrition recognized in patients with pancreatic diabetes.

    Prognosis and cause of death were studied in 80 patients with myotonic dystrophy in Japan. The subjects consisted of 48 male and 32 female patients. Ten of them were congenital form and the rest were adult form. Average age at death was 12. 6 years in the congenital form, 55.1 years in the adult form and 49.6 years in all cases. In an, analysis of cause of death, more than half of the patients died of pulmonary infection or respiratory failure. Most of the cases in this group were in the terminal stage of the disease, and the deterioration was preceded by aspiration of food. Ten percent of the cases died by suffocation with food or sputum, and the other 10 percent died of cardiac failure or arrhythmia. Thirteen percent died of coincidental disorders such as malignant tumors, cerebral hemorrhage or hepatic failure. Sudden death were observed in 10 percent of the cases. It was noteworthy that this group included the cases who were relatively young and were not much disabled. Proper care to avoid aspiration of food was considered to be very important in the terminal stage of this disease. A manual of periodical examination should be framed to prevent sudden death.

    Over an 18 months period, 31 cases of immediate breast reconstruction were performed after the department of plastic surgery was established in Kure National Hospital in 1994. The proce-dures of breast reconstruction in our basic strategy were latissimus dorsi myocutaneous (LDM) flap for a breast-conserving operation, transverse rectus abdominis myocutaneous (TRAM) flap for a modified radical mastectomy and vertical rectus abdominis myocutaneous (VRAM) flap for a radical mastectomy. The numbers of patients who were operated with TRAM, LDM and VRAM flaps were 17, 7 and 4 respectively. The remainders were operated with modified methods, which were LDM flap and saline bag, supercharged VRAM flap, and free TRAM flap respectively. The results of more than half patients were satisfactory, which were obtained in the questionnaire of the operated patients.
    An immediate breast reconstruction will be widespread rapidly with the recognition of the advantages and the establishment of the indications of this method.

    Various issues remain unresolved in atopic dermatitis, such as its etiology, treatments and en-vironmental factors, etc. In this synposium, these issues were discussed from the clinical aspects: the clinical trial aiming at the elimination of environmental antigens, the possible association between waxing of the disease and patient's anxiety about the treatments due to insufficient explana-tion, the importance of sufficient explanation to make patients understand both the nature of the disease and the treatments including a skin care and to relieve patients from the stress, the efficacy of corticosteroid ointment treatment appropriately directed by dermatologist, and atopic dermatitis associated Kaposi's varicelliform eruption.

    The two cases of remote cerebellar hemorrhage after supratentorial craniotomy were reported. On the basis of reviewing the literature, such a postoperative cerebellar hemorrhage is extremely rare complication.
    We speculate that change of post-operative posture and postoperative cerebrospinal fluid overdrainage seem to be related to the cerebellar hemorrhage. Post-operative rest and meticulous management of the drainage system were necessary to prevent this postoperative complication.
    Although rare, we should be alert to the possibilities of such a cerebellar hemorrhage when the condition of the patient after operation is unexplainable from the preceding craniotomy.

    A case of small bowel perforation due to the disseminated tumor from the pancreatic cancer was reported.
    A 75-year-old male was transferred to our surgical department with the diagnosis of refractory intestinal obstruction.
    His abdominal X-ray film just after complaining of disastrous abdominal pain showed free air in the abdominal cavity.
    On emergent celiotomy, there were numerous disseminated peritoneal nodules, a great deal of purulent fluids and a thumb-tip sized metastatic tumor of the small intestine causing small bowel perporation. Then, resection of small intestine including the perforation area and the metastatic tumor was performed.
    Intraoperative palpation revealed a hard mass in the pancreatic body.
    Postoperative diagnostic imaging (CT-MRI) also demonstrated pancreatic body tumor.
    These findings led to the diagnosis that primary lesion was the pancreatic cancer and the metastatic tumor of the small intestine from the pancreatic cancer caused small bowel perforation.
    The prognosis of a patient with small bowel perforation due to the disseminated tumor from the cancer originating from intraabdominal organ like pancreatic cancer is poor.
    However, surgical intervention for perforative peritonitis is significant enough because it can make the patient's sufferings chiefly pain free and it enables the patient's peroral intake.

    Squamous cell carcinoma of the skin rarely develops under the nail. However, among the subungual malignant tumors, squamous cell carcinoma is most common. In the present study, a 79-year-old male and a 65-year-old female with subungual radiation keratoses are reported, As for these two patients, the keratoses may have been caused by long-term superficial X-ray irradiation therapy for rigworms. When a patient has a history of iradiatiation therapy, and he complains the pain or flare of their nails, biopsy should be done immediately. Because the diagnosis of radiation keratosis should be suspected and treatment should be started before invasive carcinoma develops.
