  • Summary of the Cooperative Studies on Chemotherapy of Tuberculosis in National Sanatoria of Japan

    The vectorcardiographic findings of 80 hypertensive patients, especially of 43 cases with left ventricular hypertrophy, were studied employing the Grishman system.
    In all cases the standard 12 electrocardiograms and the chest roentgenograms were also examined. Forty-three among 80 hypertensive patients showed the left ventricular hypertrophy roentgenographically.
    In those 43 cases, maximal vector of the horizontal QRS loop as found to be longer than that of normal, and maximal vector of the sagittal QRS loop was found to be shorter than that of normal. The mean for the angle of the maximal QRS vector was +35° in the frontal plane, +111° in the sagittal plane and -4° in the horizontal plane. Furthermore it was found that the QRS maximal vector tended to direct more leftwards, superiorly and posteriorly, and that the maximal vector of the T loop removed to rightwards, anteriorly, and that the average spatial QRS-T angle was 28°.
    The figure of eight contour was observed frequently in the inscription of QRS loop in the frontal plane, and the counterclockwise direction was observed in some cases.
    The T vector loop in the frontal plane was found to be more oval in the cases with hypercholesterolemia, The ratio T magnitude/QRS magnitude was found to be smaller than that of normal in the horizontal plane.

    Post-operative complication of lobectomy was studied on one hundred tuberculous patients. The results were summarized as follows:
    1) Bacteriological finding.
    Prior to operation, 39 out of 100 patients were sputa negative on smear and culture, 2 of them were sputa positive on smear and were negative on culture, and 56 of them were sputa positive on culture. Resected materials from same 100 patients were examined. As a result, 22 were negative on smear and culture, 12 were positive on smear and negative on culture, and 66 were positive on culture. There were differences in findings of bacilli between of sputa obtained prior to operation and of resected materials. But significant difference was not demonstrated in ratio of resistant bacilli between of that sputa and of the resected materials.
    2) Histogical finding of bronchi of resected lung.
    Tuberculous changes located in the proximal parts of bronchi of resected lobes were found more frequently in cases with resistant bacilli than in those with non-resistant bacilli.
    3) During one or two years after operation, two bronchial fistulas combinated with pyothoraxes, one tuberculous pyothorax, and four cases (which were) progressive observed.
    4) Status of bacilli in sputa examined before operation and post-operative complication.
    There were no complication in 41 patients with negative sputa. One patient out of 30 with lowly positive sputa and 6 patients out of 18 with highly positive sputa had complication after operation.
    5) Post-operative chemotherapy and complication.
    Post-operative chemotherapy was performed as follows:
    A: SM, PAS, INH
    B: VM, PAS
    C: PZA, INH
    Post-operative complication were found in one patient out of 6 of group A which had SM·INH resistant bacilli, one who was treated with PZA plus INH and had SM·INH resistant bacilli, and four patients out of 18 of group B which had resistant bacilli against SM·PAS·INH.

    Studies on gastric X-ray figures by indirect blind photographing were made on 208 tbc. patients, who were treated with antibiotics.
    Concerning gastric forms, by our data, the ptosis form was found in relative higher percent, as by former reports on tbc. patients without chemotherapical treatments. This fact is very remarkable, because the gastric juice showed almost normal acidity, as in our first report described. If the gastric form means, in a way, the constitution of patients, the ptosis form must show the predispositions to tuberculosis, which can not be concealed by chemotherapy. So we believe the constitution of tbc. patient, as far as the gastric form concerned, as same in present time as in former time, regardless of chemotherapy. Moreover, by our data, the ptosis form was found in higher percent in younger age, against the fact that this form is found, in general, more in higher age. What does this mean? This must be a problem concerned to the predisposition to tuberculosis in younger age.
    Concerning to excretion time and tonicity of stomach, almost none to be described.
    About gastric rugae, an eminent difference was found between our data and former reporters. The slender type was found in very lower and the rough in relative higher percent, rerenced to former reports. This must be caused by the improvement of nutritive condition of tbc. patients by chemotherapical treatments.

    The author describes on the clinical, bacteriological, immunological and histological findings of four cases of the borderline group of leprosy, which is situated between the two main polar types (tuberculoid and lepromatous) of leprosy.
    The four cases concerned are identified as those of the borderline group according to the mixed features of the finding seen in each polar type, although, histologically, it is recognized that more tuberculoid pictures are seen than lepromatous in the wide spectrum of borderline group of leprosy.
