    Preventive and curative medicine have progressed surprisingly in these days so that the average life span has been extended and 65 years old or more people occupied 7% of total population in 1965. (Japan)
    The number will increase constantly and it is expected that 15% or more of the total population will consist of these aged people in about 35 years. Although the importance of social welfare and rehabilitation have been stressed, we clinical doctors are forced to face the aging phenomenon in the treatment of these old patients.
    So-called aging phenomenon is not a simple phenomenon but a complicated physiological degenerative process, from which many acute or chronic ailments must come out. The treatment of the elder patient is, therefore, quite different from that of an adult patient or an adolescent patient even if the same diagnosis is made.
    Regarding the treatment of the aged patients, 2 important points must be recognized, as follows:
    1) Allow them to get out of bed as soon as possible.
    2) Encourage their motivation to be treated.
    Surgical treatment is, therefore, acceptable in elderely patients in thinking of prevention of many complications, i. e, respiratory problems, decubitus, disuse atrophy and so on.
    Otherwise complications which jeopardize their lives will not be avoided even if ortho-paedic operation itself is performed successfully.
    The final goal should be rehabilitation of these patients, namely, gainful reemployment

    Of 16 patients with metastatic liver tumor treated with MFC therapy, 13 (81%) showed an objective response of tumor shrinkage. Fifty per cent survival of the group with response was 7 months long after diagnosis. Three cases of them were demonstrated as a part of the serial presentation of the cases with well response (total, 8 patients). The activities of serum LDH, Al-P and GOT were also checked, which were closely related to the therapeutic response of the tumor

    Since Sept. 1966-Nov. 1973, 25 cases of juvenile gastric cancer were found in The Cancer Center of Shikoku-Matsuyama National Hospital. The ratio of male to female was 1: 0.9 in youth, and 1: 0.5 in the total cases of gastric cancer in the same period.
    2 cases in 12 female juvenile gastric cancer were found in the pregnancy, more advanced cases were found in the period of 1-12 months after delivery.
    The ratio of curative to non-curative resection was higher than those which have been reported.
    The stage as advancement of gastric cancer was compared in two periods and in the second period (1970-1973) much more cases in early stage were found than in the first period (1966-1970).
    4 cases of juvenile EARLY CANCER were observed.
    Our date shows that, recently, more cases of juvenile gastric cancer in early stage are observed, and the possibility of curative resection are better than former years

    The study was undertaken to investigate the clinical effectiveness of Rifampicin (RFP) in the treatment of 165 pulmonary tuberculous patients, particularly in regard to the negative conversion rate on culture, the occurence rate of resistant strain and the side effects of the drug.
    Fifty-nine % of the total cases had a history of pulmonary tuberculosis for more than 5 years, and 88% of the total cases were classified as I or II type (Gakkai classification).
    Sixty-six of 165 cases in which sputum culture were positive at the start of the treatment were treated with RFP for more than one year.
    The results of the investigation were as follows:
    1. Although the majority of the cases were far-advanced mixed type on chest X-ray and were not improved by former conventional antituberculous chemotherapy negative conversion rate on culture was about 70%. The rate of bacteriologic relaps in the cases which showed negative conversion during early 6 months and maintained it for more than 3 months was very small (6%).
    2. Negative conversion rate in the cases in which the drugs combined with RFP were not used previously was superior to that in the cases in which the combined drugs were used previously. Negative conversion rate was 89% in the former, and 65% in the latter respectively.
    3. Development of bacilli resistant to RFP was found in 10 of 14 cases (71%) after 4 months of treatment, and in 17 of 20 cases (85%) after 6 months of treatment. The effect of the combined drugs on the development of resistance to RFP was uncertain.
    There was not any serious side effect except slight elevation of GOT (8.0%) and loss of appetite (6.7%). 5 cases (3.0%) dropped out because of these side effects, 2 cases of severe tuberculosis died of hemoptysis, and the cause of hemoptysis was uncertain because these cases were not associated with thrombocytopenia.
    The dose of RFP was 450mg daily in the majority of patients

    Guanase activity of human serum and liver supernatant were measured using 8-azaguanine as substrate. The ammonia liberated was assayed by the direct cololimetric method.
    Determination of transaminase activity in serum was the most sensitive of the enzyme tests routinly used in the diagnosis of liver diseases and was therefore a standard procedure in the clinical laboratory. Being distributed in many body tissues, the serum glutamic oxalacetic transaminase (GOT) and glutamic pyruvic transaminase (GPT) were often increased in non-hepatic disorders, such as myocardial infarction, circulatory congestion, muscle trauma, and postoperative states. Hence, these conditions, as well as hemolysis, must be excluded in relating observed abnormalities of serum transaminase levels to hepatic injury.
    The activities of the serum guanase elevated markedly in liver diseases, but they did not elevate in myocardial infarction and postoperative states, in which the activities of the serum GOT and GPT elevated slightly. These data indicated that the serum guanase activities reflected the hepatic injury more specifically than the GOT and GPT activities. It was considered from these results that the determination of the serum guanase activities by the ammonia method might be useful for the clinical practice

    A suction method for skin extensibility measurement was introduced, and its application for the determination of vascular fragility (capillary fragility) was evaluated.

    Recently increase of aged patients has been discussed in several fields. We have few information, however, about aged patients in psychiatric field. In this paper, the present condition of aged patients (older than age of 60) in national mental sanatorium was studied as the joint study of national mental sanatorium in 1972 and 1973.
    Method
    As primary survey, the age, sex and diagnosis of all patients in national mental sanatoria were surveyed in April 1, 1973.
    On the basis of this data, lists for investigation about aged patients was made.
    According to these lists, aged patients of each mental sanatorium were surveyed by attendant psychiatrists. We took statistics about aged patients by using these lists.
    Results
    1) Aged patients were 13.6% (575 cases) of all patients (4, 291 cases) in national mental sanatorium.
    2) Although, lists of 575 cases were collected, number of effective objects were 540 cases.
    percentage of each diagnosis of 540 cases is as follows,
    Schizophrenia 36.7%
    Senile dementia 14.4%
    Arteriosclerotic mental disorder 12.6%
    Other functional senile mental disorder 11.5%
    This data suggests that increase of aged patients in national mental sanatorium depends on increase of chronic schizophrenic patients.
    3) Concerning with distribution of aged cases, peak of schizophrenic patients was found between age of 60 to 64, and other senile mental disorder older than age of 75.
    4) Aged psychiatric patients with chronic psychical illness were 71.196.
    5) Defect cases were 49.4%.
    Moreover, the data suggests that these patients might have dementia

    Among the 200 cases diagnosed as exanthem subitum clinically in our pediatric department during 5 years from April 1970 to June 1975, 4 patient showed symptoms of central nervous system involvement such as convulsions, bulging fontanels, restlessness, vomiting etc.
    No abnormal findings of cerebrospinal fluid except for the elevation of opening pressure at the time of lumbar puncture. Two of them were suspected of the occurence of slight hemiparesis from the 4th day to the 10 day (Ca one week) of the illness.
    The cause of exanthem subitum is still unknown, but the condition is generally ascribed to the viral infection. Serological studies were carried out on 40 paired serum samples of 40 of the exanthem subitum cases. Viral antigen such as adeno, echo, coxsackie, measles, rubella, polio I, II and III, mycoplasma and EB virus were prepared for them. Positive data were obtained each one of adeno and rubella, 4 of EB virus. Among the 4 cases with complication of nervous systems, only one (case 3) showed high EB virus antibody titre

    Lung infection due to M. kansasii is relatively rare disease in this country (Co-operative study group of National chest hospitals: Kekkaku 48: 203-211, 1973). It occurs usually in Tokyo area. Only three cases were found in the area other than Tokyo area. This case is the fourth case which has been found in the area out of Tokyo.
    The patient is a 34-years-old male and the disease appeared in his right upper lobe as a solitary, thin-walled cavity without pericavitary lesion. The lesion disappeared during the course of chemotherapy with SM-INH-PAS and successively RFP-EB-SM

    We have reported a case of primary leiomyosarcoma of the lung. This is the 24th case report in Japan.
    The patient, a 65-years-old man, was brought to the Kyoto National Hospital by ambulance because of difficulty in breathing and chest pain. Roentgenologic examination of the chest showed complete opacification of the right hemithorax. Bloody pleural fruid was obtained by thoracentesis, it contained no tumor cells. Difficulty in breathing became more severe and the patient died on the fourth hospital day.
    At autopsy, there was a large tumor mass in the right upper lobe, and this was diagnosed as leiomyosarcoma histologically. There was no metastasis except for regional lymphnodes.
    Electronmicroscopy showed large and bizarre nuclei, a remarkable conglomeration of mitochondria mainly at the nuclear poles, pinocytic vesicles and myofilaments.
    In the diagnosis of the histological types of pulmonary sarcoma, electronmicroscopic study is very useful.
    Primary leiomyosarcoma of the lung has a relatively good prognosis after operation as compared to other pulmonary sarcomas. Early diagnosis, therefore, is especially important

    Demand failure was caused by trouble in pacemaker or input insufficiency to pacemaker without any trouble in generator.
    We have experienced two cases which is considered to be low input to pacemaker to sence. This is caused by fall of myocardial potential, rise of electrical resistance around electrode or poor contact of electrode with myocardium.
    On implantation of pacemaker, stable position of electrode, low stimulation threshold is important. At the same time, sufficient input to gain sencing is also important.
    From the above consideration, we shoud take care of endocardial potential to manage pacemaker

    Statistical observation was made on in-patients of the Pediatric Ward and Newborn Care Unit at Okura National Hospital from March 1972 to May 1975.
    Total numbers of in-patients were 1929. In 1972, 26.5% of 313 in-patients were respira-tory diseases, 21% of them were neurological diseases and 13.4% were from maternity ward. In 1973, new maternity ward was built up and newborn care unit removed from pediatric ward. In 1975, 31.4% of 191 patients were diseases of newborn, and 23.7% of pediatric ward patient were neurological diseases.
    34% of neurological patients came from Setagaya and Komae district where our hospital was located, while 77% of the other patients (patients without neurological disease) came from the district.: 66% of neurological patients came from other district.
    Total number of neurological patients was high compared with the data of other hospital
