    Oxidative stress is involved in the initiation and progression of atherosclerosis. Oxygen free radicals either on cell wall membrane or on LDL particles facilitate lipid peroxidation nonenzymatically in several clinical conditions related to oxidative stress such as aging, smoking, coronary ischemia-reperfusion and diabetes mellitus. We examined the antioxidant activities of thiazolidinediones (insulin sensitizer) and their effect on cardiovascular function in type 2 diabetic model rats, and also those of dihydropyridines (commonly used anti-anginal and antihypertensive agents) in cultured human endothelial cells. In the pre-diabetic or early diabetic stage, hyperinsulinemia or insulin resistance is thought to be closely associated with oxidative stress. We administered pioglitazone or troglitazone, and measured 8-iso-prostagalandin F and malondi aldehyde-thiobarbituricacid. This study demonstrated impairment of the antioxidative system and an accumulation of collagen in the myocardium or aortic wall, and confirmed the antioxidant effects of thiazolidinedione compounds.
    In the present review, we introduce the role of transforming growth factor (TGF)-β-mediating cardiovascular complications and the effects of thiazolidinediones on cardiovascular function. We also examined the effects of dihydropyridine-type calcium antagonists on lipid peroxidation in cultured human arterial endothelial cells. Endothelial cells were exposed to 1mM H2O2, and then treated with nifedipine, amlodipine or azelnidipine (a newly developed dihydropyridine). The antioxidant activity was evaluated by measuring 8-iso-prostaglandine F concentration. Azelnidipine exhibits potent antioxidant activities, which we also introduced in this review.

    30 cases admitted to this hospital in 2004 with community-acquired pneumonia were reviewed: 18 men and 12 women, with their average age being 73.21±17.15 years. In 28 cases upon admission a CT chest scan was, and in 18 of these both lungs showed infiltrative shadow (64.3%). According to the ranking criteria for community-acquired pneumonia developed by the Japanese Respiratory Society, three were mild cases, 21 were moderate cases, and six seriously ill cases. As for prophlogistic causative factor, Haemophilus influenzae, Streptococcus pneumoniae were each present in four cases and were the most frequent. There were four deaths during treatment (13.3%). There was extensive dementia in patients in four cases, with the average age being 83.5 years old. In the three cases where there was difficulty giving pneumonic treatment the patients had extensive dementia. Pneumonia treatment of dementia cases will be an important problem in future.

    Effect of chest physiotherapy to 63 amyotrophic lateral sclerosis (ALS) patients of early stage, 10 ALS patients who were introduced noninvasive positive pressure ventilation (NIPPV), and 30 ALS patients who had tracheotomy ventilation (TV) was studied. The chest physiotherapy created by National Center of Neurology and Psychiatry Kohnodai Hospital was carried out to ALS patients of early stage. Thorax assist breathing was performed to ALS patients who were introduced NIPPV. The program of leaving their sickbed was carried out for TV group of ALS patients.
    From the result, it was revealed that chest physiotherapy prolonged the period until the stage of TV. Moreover, the improvement of SPO2, the reduction of dyspnea, and the improvement of blood gas were accepted by using thorax assist breathing with NIPPV. It was thought that thorax assist breathing with NIPPV was the technology of new chest physiotherapy. After doing the program for TV group, ALS patients could leave their sickbed under the right breathing control. Leaving their sickbed prevent lung complication and improves the patient's quality of life for TV group of ALS.

    In order to investigate the effect of fractures on the serum levels of cross-linked collagen N-terminal telopeptide (NTX) in patients with osteoporosis, serum levels of NTX in the patients who were treated with total knee arthroplasty were analyzed as a fracture model. The results showed that the serum levels of NTX in most of the patients peaked 3 to 4 weeks after surgery, and returned to the levels before surgery at 6 months. These results suggest that the effect of fractures on the serum levels of NTX may show up 3 to 4 weeks after fractures in patients with osteoporosis.

    We developed a neonatal ambulance (a doctor car) and a perinatal medical information network system cooperating with the local government of Mie. In addition to the latest diagnostic and therapeutic apparatus, the neonatal ambulance car was equipped with a bio-information analyzing system in order to transport neonates safely. Furthermore, the information obtained in the ambulance car is transferred to our center supporting diagnosis and treatment. Pre-hospital care of newborn infants during transport has come true by the introduction of this system. In addition, we simultaneously developed a perinatai medical information network system in Mie. Using this system the neonatal ambulance can be called and the medical information of mother and infant during and after hospitalization is available, Since the neonatal ambulance “Sukusukugou” and perinatal medical information network system were introduced in 2003, the neonatal and the infantile mortality rate of Mw have improved to less than the national mean. These systems are useful for child health and development.

    We report an extremely rare case of malignant melanoma of the lung. A tumor-like shadow was detected in the left lung of a 48-year-old man by chest X-ray examination, Chest CT revealed a tumorous lesion with multiple small nodules around it. In addition, bronchoscopy showed a dark brown tumor that obstructed the lumen of left bronchus B4, and transbronchial lung biopsy (TBLB) was performed. A histopathological examination of a biopsy specimen showed tumor cells with large round or oval nuclei possessing characteristic basophilic nucleoli and notable variation in the amount of melanin pigments from cell to cell. The tumor cells were positive for S-100, HMB45 and vimentin but negative for keratin, EMA and LCA in immunohistochemical examinations. By these findings, the tumor was histopathologically diagnosed as a malignant melanoma. There were no tumorous lesions in other organs that might be suspected to be primary malignant melanoma although metastatic lesions were observed in the liver by CT. We conclude that these clinical and histopathological examinations indicate that this case was a very rare malignant melanoma that originated in the lung.
