    Chironomid midge is found to be an important allergen in allergic children. RAST tests with extracts from adult midges(Tokunagayusurika akamusi)were carried out on 140 allergic patients aged from 5 m to 22 years. 18.6% of 140 patients showed positive RAST:group 1(age 5 m to 6 years), 3.2%, group 2(6 to 12 years), 7.7%, group 3(12 to 18 years)20.0%, group 4(18 to 22 years), 48.3%. Positivity increased according to the increase of age. Positivity in asthma, 22.2%, allergic rhinitis 14.3%, atopic dermatitis 5.3%, urticaria 0%. Coincidence of RAST score and prick test was 78.4% according to Vanselow's criteria. Positivity with RAST and prick tests was the third rank following mite and house dust. Positive rate of RAST in the patients living around Tokyo is higher(20.0%) than the patients living in Tokyo(17.8%)but there was no statistical significance. Peripheral IgE levels were significantly higher in RAST positive group(p<0.001)than RAST negative patients. D. f. specific IgE antibodies were higher in RAST positive patients than RAST negative patients but there was no statistical significance.

    117 patients with mould allergy or suspected mould allergy were investigated for 16 different mould allergens by Pharmacia RAST RIA. The results were as follows. 1)The frequency of positive RASTs(class≧2)varied from 3.4% to 23.3% depending on the allergen, and many patients were RAST-positive to more than one of the 16 mould allergens. 2)The frequency of positive RASTs in the patients who had developed symptoms under the age of 12 was high. 3)In the cage of patiens with positive RASTs to the moulds, total IgE was often over 1000IU/ml, but RAST class was almost always within class 2. 4)The mould allergen which produced positive skin test, showed the highest frequency of positive RASTs among the 16 different mould allergens, but some of the other mould allergens indicated almost the same frequency of positive RASTs. From these results, it seems that some of the patients(especially those with a younger onset age) with mould allergy or suspected mould allergy have their symptoms caused by more than one mould allergen, so RAST is very useful for the investigation of unknown mould allergens.

    This study examined whether or not exacerbation of bronchial asthma accompanies platelet activation in vivo. We measured thrombin induced ATP release from washed platelets, plasma β-TG(β-thrombogloblin)level, plasma PF4(platelet factor4)level and plasma β-TG/PF4 ratio in 16 normal controls and 15 asthmatic patients during symptomatic periods and asymptomatic periods. We also measured them during antigen provocation tests in 6 asthmatic patients and during acetylcholine inhalation tests in 6 patients. The following results were obtained. 1)Amounts of ATP released from washed platelets were significantly higher in asthmatic patients during symptomatic periods than during stable periods or than in normal controls, and significantly higher after antigen challenge than before challenge. 2)β-TG/PF4 ratios did not elevate either during symptomatic periods or after antigen challenge, while plasma PF4 and β-TG levels increased. 3)ATP release, PF4 level, β-TG level and β-TG/PF4 ratio did not change after acetylcholine inhalation. 4)These results suggest an existence of platelet"hyperreactivity"and a possibility of reduced bindings of PF4 to endothelial cells during symptomatic periods in asthmatic patients.

    Immunoreactive substance P(IR-SP)in the respiratory tract tissues of normal guinea pigs was measured with a specific radioimmunoassay using N-specific anti-serum R400. Male Hartley guinea pigs were passively sensitized with homologous anti-ovalbumin(OA)serum. After inhalation challenge with OA antigen, they were studied on any changes in respiratory pattern and resistance, and also tested for IR-SP in trachea, major bronchus and lower lungs, In normal guinea pigs, the respiratory tissue level of IR-SP was 0.1-10 pmol/gram wet weight, and its distribution order was major bronchus>lower lung>trachea. In guinea pigs passively sensitized with homologous anti-OA serum, antigen inhalation clearly induced both the early asthmatic response and the significant decrease in IR-SP in trachea, major bronchus and lower lung;the induction of such experimental asthma ran parallel to the decrease in IR-SP in respiratory tracts. IR-SP, however, returned to the cose levels of preexposure 6hr after antigen inhalation. These results suggest that SP participates in the immediate allergic response in respiratory tracts. They also show that SP may play an important role in the pathogenesis of airway hypersensitivity in asthma.

    We evaluated skin prick tests on children according to new positive criteria(wheal≧3mm, erythema≧10mm). The concordance of prick tests and RAST-scores was significantly better when the new criteria were used instead of the criteria laid down by the scratch test committee. The frequency of positive reactions in prick tests performed on children younger than 12 was significantly better according to the new criteria. There was no difference in skin test reactivity between males and females. More than 10% of the cases showed larger than 5mm in erythema 24-48 hours after prick test(late reaction). The frequency of the reaction was closely related both with total-IgE and immediate-type skin test reactivity. So, those new criteria are more effective in evaluating skin prick tests on children, especially those under 12 years of age. We should also be aware of the late reaction which is closely related to immediate reaction when making evaluation.

    The inhibitory effect of 2% disodium cromoglycate(DSCG)eye drops on histamine release in tears was examined by an ocular provocation test with antigen in 11 patients with Japanese cedar pollinosis during the off-season. DSCG eye drops were administered to the right eye, while the left eye received a placebo, and five min later a cedar antigen solution(1:20w/v)was applied to the eyes. Lacrimal levels of histamine were determined by radioimmunoassay. Lacrimal levels of histamine in the placebo-treated eyes for 0-5 min and 5-10 min after antigen administration were 4.3±4.0ng/ml and 3.6±3.7ng/ml, respectively. In contrast, they were 1.5±2.7ng/ml and 1.7±2.6ng/ml in the DSCG-treated eyes, showing significant inhibitory effect of the drug on histamine release(p<0.01 at 5 min;p<0.01 at 10 min). The excellent effect of DSCG eye drops in suppressing histamine release was also demonstrated by their inhibition rates, which were 73.8% at 5 min and 67.5% at 10 min after provocation. There results indicate that 2% DSCG eye drops are an effective drug for prophylactic treatment of Japanese cedar pollinosis.

    We have investigated the inhibitory effect of selective alpha-2 adrenoceptor blocker, 2[2-(4, 5-dihydro-1H-imidazol-2-yl)-1-phenylethy1]pyridine dihydrochloride sesquihydrate (DG-5128), in a double-blind, randomized, crossover study(placebo, DG-5128 100 mg, 200 mg), on 18 patients with steroid-dependent asthma. DG-5128 significantly(p<0.05), dosedependently imporved final global improvement rating, global utility rationg, the forced expiratory volume in 1 second(FEV_<1.0>), asthmatic symptoms and rales on auscultation. Improvement of final global improvement rating in DG-5128 100 mg group was 47.0% and that in DG-5128 200 mg group was 64.7%. The results of this study showed that DG-5128, selective alpha-2 adrenoceptor blocker, is endowed with the ameliorative effect for bronchial asthma, especially intractable asthma and that it will be a usefull new drug for asthma.

    We searched for anti-endothelial cell antibodies, which were postulated to be associated with the vasculopathy of the central nervous system(CNS), in the sera of patients with some neuroimmunological disorders. Serum samples were obtained from 21 patients with multiple sclerosis(MS), 10 patients with neuro-Behcet disease, 10 patients with sarcoidosis, 7 patients with amyotrophic lateral sclerosis and 25 healthy normal controls. Anti-endothelial cell antibodies were measured by an ELISA method using cultured rat brain endothelial cells and were detected by indirect immunofluorescence using human umbilical cords and rat brain as the substrate. The levels of anti-endothelial cell antibodies were significantly increased in the sera of patients with MS(p<0.001), neuro-Behcet disease(p<0.001)and neuro-sarcoidosis(p<0.001), especially those with an exacerbation. Positive labelling of endothelial cells from rat brain was found in sera from patients with MS and neuro-Behcet disease. Positive staining for anti-endothelial cell antibody was detected on the endothelium of both rat brain and human umbilical cords in the patients with sarcoidosis. These results suggest that anti-endothelial cell antibodies may play a pathogenetic role with the organ-specificity in the central vasculopathy of some neuroimmunological disorders.
