ジャーナル フリーThe term vertigo is so difficult to define that there is no conclusive definition so far advanced which is convincing to everybody.
There are 44 different definitions of vertigo proposed up to the present by medical workers in various parts of the world. The earliest one is BÁRÁNY's given in 1980. These different definitions of vertigo may be classified in different ways. In formal terms there can be 4 different categories of definition: one which identifies vertigo with 1) hallucination, 2) a syndrome, 3) a kind of sensation, or 4) a mode of consciousness. In terms of contents, 4 different categories of definition are also distinct: one which regards vertigo as 1) an abnormality in the sensation of movement, 2) an abnormality in the sense of relativity in space, 3) disturbance of equilibrium, or 4) disturbance of consciousness. A number of different definitions can be derived from different combinations of these concepts.
None of these combinations has been supported by an overwhelming majority of authors. Nor is there any tendency for these concepts to change with the current of the times. It seems that all the definitions so far proposed are an expression of personal perference of authors who give them.
The author has proposed to define vertigo as “disturbance of equilibrium as is felt by the patient himself (1975)”.抄録全体を表示PDF形式でダウンロード (803K) -

ジャーナル フリー172 tympanograms were recorded in sound-conductive loss to assess their value in the diagnosis of secretory otitis media, and in the suggestion of the resolving stage of effusion in the middle ear.
The tympanograms were found in 4.1% of type I, 5.1% type III, 49.5% type IV, 61.0% type V, 25.4% type VI, 61.2% type VII, and 62.8% type VI, as based on the tympanogram classification by Yokoyama et al.
Type I (A) and III (As) tympanograms having simultaneously the middle ear pressure from -80 to -100 mmH2O and the lowering of the peak were considered to be indicative of secretory otitis media.
Type VI and VII tympanograms, which did not correspond to and of JERGER'S classification and the others, have been frequently found clinically among patients with flaccid tympanic membrane, and the resolving stage of middle ear effusion.抄録全体を表示PDF形式でダウンロード (828K) -

ジャーナル フリーEin malignöses Melanom im Hypopharynx eines 64 jährigen Patienten wurde berichtet.
Er war sich des Fremdkörpergefühls im Kehlkopf vom März 1977 bewußt. Am 14.1. 1978 wurde die umfangreiche Resektion einschließlich des Tumors in der Allgemeinennarkose gemacht. Trotzdem nach den etwa 9 Monaten war die Metastase am Hals, der Mundhöhlenschleimhaut und dem Lendenwirbel entstanden, und im Juli 1979 war er gestorben.
Kein Bericht über das maligne Hypopharynxmelanom war in Japan, soviel wir gelesen haben. Diesmal berichteten wir den seltenen Fall vom malignen Melanom, das aus dem Hypopharynx original entstanden zu sein schien.抄録全体を表示PDF形式でダウンロード (5167K) -

ジャーナル フリーA case of primary septal adenoid cystic carcinoma was reported. A 55-year-old man, farmer, complained of a severe nasal bleeding. The posterior half of nasal cavities were full of tumor, but the site of origin was not confirmed because of much bleeding. The CT scan suggested that it was arose and localized in the posterior septum. Histopathological examination showed adenoid cystic carcinoma of cribriform pattern. He received 60Co irradiation (6200 rads) combined with 5-fluorouracil, but the tumor was radioresistant relatively. Then it was completely removed with a sufficient amount of normal septal tissue by Denker's method. No recurrence and distant metastasis were noted for 7 months after the surgery. As the course of adenoid cystic carcinoma is very prolonged, it is necessary to follow up for more than 5 years.
The primary septal adenoid cystic carcinoma is extremely rare, only 5 cases have been reported in the world on our search of literatures.

ジャーナル フリーTwo cases of ingestion of paraquat, a weeding agent, were reported in this paper. They were successfully managed with forced diuresis and steroid hormone.
As described in many previous reports, initial management of paraquat ingestion case shoud involve removal of the poison from the gut as soon as possible. However, the both were not the cases for such a treatment, because they had been admitted in our hospital at over 72 hours after ingestion of paraquat. Forced diuresis and steroids were then applied to the patients in order to wash the dissolved paraquat out of blood circulation and to prevent a developement into the pulmonary fibrosis.抄録全体を表示PDF形式でダウンロード (1757K) -

ジャーナル フリーIt is well known that the house dust is an important allergen in allergic diseases of the respiratory organs. However, it was appeared that one of the mites, Dermatophagoides farinae (Df), played an important role as the allergen of the house dust in nasal allergy. On the other hand, the other mite, Dermatophagoides pteronyssinus (Dp), which occurs in the house dust in Japan more than Df, was cultured and the allergen was separated from the extract of Dp by Dr. Ishii. In order to clarify the clinical significance of the mite (Dp) in the nasal allergy, we observed the skin reaction in the patients with nasal allergy using the new allergen from the extract of the mite (Dp).
The results obtained were as follows:
1) The positive rate of the mite and the house dust in the skin reaction were 91.3% and 91.3%, respectively. Further, the overall agreement was 100%.
2) The significant correlation between the mite (Dp) and the house dust was observed in the size of wheal in the skin reaction of these allergens, but it was not observed in the size of redness.
From the above results, it was suggested that the mite (Dp) was an important allergen of the nasal allergy as well as the house dust.抄録全体を表示PDF形式でダウンロード (466K) -

ジャーナル フリーPhysiological roles of the canine cricopharyngeus muscle (Cp. M.), as upper esophageal sphinctor, were investigated by various investigation methods. The obtained results were mainly compared with those obtained from hyopharyngeus muscle (Hp. M.). Electromyogram recordings from Cp. M. and Hp. M. in vivo showed that during deglutition, Cp. M. stopped but Hp. M. generated the burst discharges. During phonation, the electrical activity of Cp. M. was not appeared but that of Hp. M. was generated. Electrical stimulation evoked a twitch tension in both muscles, but the durations of active state and of twitch were longer in Cp. M. than those of Hp. M. The critical stimulation frequency of a complete tetanus formation was lower in Cp. M. than that in Hp. M. The K-induce contracture was produced by in Cp. M. by excess K solution but not in Hp. M. The mean membrane potentials and shapes of action potential recorded from Cp. M. and Hp. M. were nearly the same, but the membrane capacitance was lower and the membrane resistance was higher in Cp. M. than those of Hp. M. However, the obtained results suggest that both muscle tisses are composed of at least two different muscle fibers. Succinic dehydrogenase and myosin ATPase staining methods showed that Cp. M. was composed mainly of intermediate slow muscle (82%) and a part of muscle of red and white fast muscles (18%). The reversed sequeses were observed in Hp. M.(slow muscle 22% and fast muscle 78%). The above results obtained from Cp. M. and Hp. M. suggested that the physiological function of these muscles in pharyngea are controlled by neural activity as a somatic muscle, but the differences in electrical and mechanical properties also contribute to maintain the coordinative activity of pharyngea.抄録全体を表示PDF形式でダウンロード (9648K) -

ジャーナル フリーFundamental and clinical evaluations of the new macrolide family antibiotic MOM were performed with the results which may lead to the following conclusions:
1) In vitro antibacterial activity: The minimum inhibitory concentration of MOM was measured by an agar plate dilution method using heart infusion agar (Eiken). MOM inhibited the growth of 80 strains of coagulase positive Staphylococcus aureus at ≤ 0.2 to ≥ 100 μg/ml. Clinical isolated Streptococcus pyogenes and Streptococcus pneumoniae strains were inhibited at ≤ 0.2 to 1.56 μg/m1 of MOM.
2) Results of clinical treatment: When MOM was orally administrated to 33 cases of representive infection in the Otorhinolaryngologic field, it was excellent in 15 cases, good in 14 cases and poor in 4 cases. When the cases in which it was excellent and good were considered together, good results were obtain in 29 cases, that is a ratio of effectiveness of 87.9 per cent.

ジャーナル フリーIn order to evaluate the efficacy of pipemidic acid (PPA) on sinusitis, a well controlled study was carried out according to the same protocol in cooperation with 11 clinics in Japan. The results obtained were as follows.
1. PPA was used for 153 patients with sinusitis. The 148 cases judged by doctors in charge consisted of 35 cases of acute sinusitis, 33 cases of acute exacerbation of chronic sinusitis and 80 cases of chronic sinusitis. The 115 cases judged by the committee consisted of 28 cases of acute sinusitis, 29 cases of acute exacerbation of chronic sinusitis and 58 cases of chronic sinusitis. The bacteriological efficacy was investigated in 117 cases.
2. As to the overall clinical efficacy of PPA, the ratio of effectiveness were 48.0% in doctor's judgement and 59. 3% in committee's judgement.
3. In the cases of infections due to GNB, the ratio of effectiveness was 58.1% in doctor's judgement and 70.4 % in committee's judgement, and PPA was confirmed to have superior efficacy against infections due to GNB.
4. With respect to the bacteriological results, PPA eliminated causative organisms in 58.1%. It was remarkable that PPA eradicated GNB and indicated organisms in the ratios of 68.4% and 82.4% respectively.
5. With respect to the subjective symptoms, objective findings and X-ray findings, the improvement ratio of PPA was higher for GNB infections than for the infections of other organisms.
6. Incidence of side effects was 9.2%. They were mainly gastrointestinal disturbance and were not serious. From the above results, PPA was considered to be excellent chemotherapeutic agent for sinusitis due to GNB.抄録全体を表示PDF形式でダウンロード (7308K) -
