ジャーナル フリーWorking on outdoor farms affects elderly farmers’ health, especially those who have chronic diseases. This study aims to identify the risk factors related to perceived hot conditions on outdoor farms among elderly Thai farmers aged 60 years and older with chronic diseases. A cross-sectional study was conducted on 352 elderly farmers in nine sub-districts of Nong Suea District, Pathum Thani Province, Thailand via convenience sampling. Questionnaires were used as instruments to gather data about demographic and working factors. The risk factors associated with elderly Thai farmers’ perceptions of hot conditions on outdoor farms were identified by using binary logistic regression. The most common chronic illnesses were hypertension (35.8%), heart disease (34.4%), and diabetes mellitus (24.5%). More than 60.5% of the participants perceived their hot conditions to be high. The results of binary logistic regression show that low income and work duration ≤8.0 hours per day were risk factors related to perceived hot conditions on outdoor farms (P value < 0.05). These findings may be used by relevant authorities to support elderly farmers by emphasizing the importance of individual and work-related factors. ジャーナル フリーLipids including fatty acids (FAs), which are water-insoluble molecules, are not only a cellular energy source but also signaling molecules that induce and modulate the expression of various cellular functions. Fatty acid-binding proteins (FABP) bind FAs in the cytoplasm, and are thought to determine the cellular localization of FAs. In a previous observation, FABP5 was expressed in thymic epithelial cells (TEC) in the thymus and was influenced by FAs. Fatty acids have mostly inhibitory effects on various cell types, including cancer cells, but their effects on TEC have not been well investigated. In this study, we investigated the effects of long-chain FAs (LCFAs) and the involvement of FABP5 in cell proliferation using a serum-free primary culture system. The results showed that saturated fatty acids did not affect proliferation, but n-3 long-chain polyunsaturated FA (LCPUFA) reduced, n-6 LCPUFA increased, and retinoic acid strongly reduced the percentage of proliferating wild-type TEC. The proliferation of FABP5-KO TEC was more significantly affected by LCPUFA, suggesting that FABP5 is an important modulator of FA-mediated TEC proliferation. ジャーナル フリーChronic intermittent hypoxia (CIH) has been used as a model to mimic nocturnal apnea, which is associated with hypertension. One of the mechanisms for hypertension in patients with nocturnal apnea is an enhancement of the plasma membrane response to acute hypoxia in carotid body glomus cells. Hypoxia is known to induce depolarization via inhibiting TWIK-related acid-sensitive K+ (TASK) channels, one type of leak K+ channels, in glomus cells. The present experiment was undertaken to immunocytochemically investigate the effects of CIH on the expression and intracellular localization of TASK1 channels and p11 that critically affect the trafficking of TASK1 to the cell surface. The expression levels of TASK1 proteins and p11 and their intracellular localization in rat carotid body glomus cells were not noticeably affected by CIH, suggesting that the enhanced membrane response to acute hypoxia is not due to an increase in surface TASK channels.抄録全体を表示PDF形式でダウンロード (1366K) -

ジャーナル フリーPatients with minimal change nephrotic syndrome (MCNS) are prone to loss of motor skills due to urinary protein leakage, steroid myopathy, and other factors. Acute kidney injury (AKI) is a common complication that contributes to the loss of physical function. Rehabilitation is crucial, but its efficacy and safety are unknown. Here we present a case of a patient with MCNS complicated by AKI, who commenced rehabilitation after dialysis was discontinued and experienced improved mobility. The patient, a woman in her 70s, was admitted to our hospital with bilateral lower limb edema and decreased urine output for approximately 5 days. Treatment with prednisolone and furosemide was initiated, but then dialysis was initiated due to AKI. Rehabilitation was started after dialysis was discontinued. The patient’s muscle strength and physical activity improved, and her exercise capacity and exercise tolerance improved without adverse effects. Rehabilitation may contribute to the improvement of exercise capacity without worsening renal function and urinary protein in patients with MCNS complicated by AKI.抄録全体を表示PDF形式でダウンロード (596K) -

ジャーナル フリーGiant cell tumor is a highly aggressive tumor characterized by a marked proliferation of pleomorphic, bizarre giant cells usually observed in the lungs. The importance of histopathological imaging and the clinical course of this tumor are unknown. The objective of our report was to investigate whether these components affect treatment outcomes and prognosis compared to conventional cancers. A 40-year-old woman with cervical cancer showed leukocytosis and elevated granulocyte colony simulating factor (G-CSF). The patient underwent a radical abdominal hysterectomy. Pathology revealed a poorly differentiated squamous cell carcinoma of the cervix, similar to giant cell carcinoma. The patient recovered from the disease and is alive 37 months after concurrent chemoradiotherapy (CCRT). Leukocytosis and G-CSF were normalized after treatment. This was our second case of giant cell carcinoma of the cervix. Cumulative data on giant cell carcinoma are limited, thus we considered the prognostic significance of the presence of giant cell carcinoma in uterine carcinoma.抄録全体を表示PDF形式でダウンロード (6838K) -

ジャーナル フリーWe investigated the probability of measuring the odor of thirdhand smoke using four odor-measuring monitors and three gas detector tubes. We measured the changes in tobacco odor on paper towels and cloth products over time. The results of the measurements were plotted against time to obtain an exponential approximation curve. The coefficient of x and the R2 values were calculated from this curve. The odor indicated by four types of odor-measuring monitors and three types of gas detector tubes decreased exponentially over time. The curves obtained from all measuring devices had coefficients of ex between -0.001 and -0.021, and R2 values of >0.8. The reproducibility of measuring relative odor levels through all measuring devices was high, suggesting a good capability of measuring thirdhand smoke odor. ジャーナル フリーAlthough surgical resection is the only available treatment to achieve long-term survival in biliary tract cancer, many cases are often identified at an advanced stage at the time of diagnosis. Radiotherapy may be an alternative option to prolong survival in cases with locally advanced unresectable disease. While there are some reports of long-term survival after radiotherapy for unresectable biliary tract cancer, it is rare that clinical symptoms are exhibited by peritoneal dissemination more than 8 years after radiotherapy and that resection can be performed. Our case was a 55-year-old female who had visited with a complaint of jaundice and was diagnosed with primary unresectable hilar cholangiocarcinoma. She received definitve chemoradiotherapy, and repeated receiving maintenance chemotherapy thereafter until clinical manifestation. During follow-up, she was diagnosed with stenosis of the sigmoid colon, which was attributed to peritoneal dissemination of cholangiocarcinoma. We herein report a rare case of primary unresectable hilar cholangiocarcinoma after chemoradiotherapy which was followed by chemotherapy that was controlled for more than 8 years but eventually caused colonic obstruction attributed to peritoneal dissemination.抄録全体を表示PDF形式でダウンロード (6424K) -

ジャーナル フリーPancreatic neuroendocrine carcinoma (NEC) as classified in the World Health Organization (WHO) 2010 was reclassified in the WHO 2017 as either neuroendocrine tumor (NET) G3 or NEC. An accurate diagnosis based on the WHO 2017 classification is important in order treating this disease appropriately. We report a case diagnosed as NET G3 that responded remarkably well to treatment with streptozocin. The patient would likely not have received the streptozocin treatment if she had been diagnosed with NEC. ジャーナル フリー84歳，女性．初診数ヶ月前より全身紅斑が出没していた．初診時，38℃台発熱，意識レベル低下，全身に一部紫斑を混じる粟粒大紅斑が播種していた．WBC（8,200/μl），異型リンパ球（3%），sIL-2R（4,030U/ml），CTで全身リンパ節腫大があった．左鼠径リンパ節生検では，リンパ節の構造が破壊され，高内皮細静脈の増生と異型リンパ球の浸潤がみられ，Angioimmunoblastic T-cell lymphoma （AITL）と診断した．AITLは発熱，全身性リンパ節腫脹などの激しい全身症状を呈する比較的稀な末梢性Ｔ細胞リンパ腫である．節外病変として，約半数で皮疹を呈し，初発症状の一つとなるが，多彩な臨床像をとり，非特異疹も多く診断に苦慮する場合がある．皮膚病理組織学的所見でAITLの特異所見が得られない場合，リンパ節の特徴的な組織像により診断される．皮疹の遷延，全身症状，異型リンパ球出現など血液検査値の異常が続く場合には，リンパ腫も鑑別として挙げる必要がある．抄録全体を表示PDF形式でダウンロード (11680K) -

ジャーナル フリー症例は40歳女性．食後に生じる低血糖症状を主訴に当科紹介受診した．75g経口ブドウ糖負荷試験では，1時間後血糖245 mg/dl，2時間後血糖196 mg/dlであり耐糖能異常と診断した．しかし，6時間後血糖はインスリン遅延過剰分泌に伴い46 mg/dlと低血糖を呈しており，反応性低血糖と診断した．単純糖質を避けるなどの食事指導を行うとともに耐糖能異常・反応性低血糖に対してボグリボース0.6 mgの内服を開始した．低血糖症状の頻度は一旦減少していたが，徐々に低血糖頻度が再度増加した．詳細な問診により月経2-3日前に低血糖の頻度が多いことが判明したため，月経前後の血糖変動を確認するためFlash Glucose Monitoring（FGM）を装着した．FGMでは月経3日前より食後血糖の増悪とともに，反応性低血糖の出現を認め，一方月経4日後には食後高血糖は改善し，反応性低血糖も消失したことを確認した．月経前にはボグリボースの飲み忘れがないように服薬指導を行うとともに，月経数日前には昼食後の補食をするように指導を行い，低血糖出現頻度は減少した．これまで本例のように月経前に増悪する反応性低血糖症をFGMで評価し得た報告はない．反応性低血糖の診断や病勢評価，治療において，月経周期の血糖への影響についても考慮する必要がある．抄録全体を表示PDF形式でダウンロード (611K) -

ジャーナル フリー製鉄所勤務の42歳男性，初診の2週間前から両手背の掻痒感を自覚していた．両手背に苔癬化をきたした紅斑局面を認めた．職場で使用している手袋のパッチテストを行い，手袋のゴム部分で陽性となった．製鉄工場で勤務していることから，金属パッチテストを実施し亜鉛で陽性反応を示した．ゴムの加硫促進助剤に含まれる亜鉛によるアレルギー性接触皮膚炎と診断した．職業性皮膚疾患のなかでも接触皮膚炎は最多であり，原因物質も様々である．ゴム（ラテックス）に対するアレルギー反応はⅠ型アレルギーとⅣ型アレルギーがあり，Ⅳ型アレルギー反応としてはチウラムなど加硫促進剤に対するアレルギーが多い．本症例のように亜鉛アレルギーがある場合，ゴムに対してもアレルギー反応を起こすことがあり，注意が必要である．抄録全体を表示PDF形式でダウンロード (3042K) -

ジャーナル フリー慢性疾患をもった成人期にある患者への看護実践の基礎的能力を習得するための専門科目「成人看護学実習（慢性期）」では，新型コロナウイルス感染症流行前は，全3週間，内科系病棟および外来診療部門・入院支援室で臨地実習を行ってきた．しかし，新型コロナウイルス感染症の流行拡大に伴い，2020年5月はオンライン実習に切り替えて実施し，2020年10月からは本学大学病院での臨地実習が再開した．臨地実習は，本学の新型コロナウイルス対応マニュアルに基づいた実習基準のもと，病院滞在時間に制限が設けられ，病棟実習は患者と会話や接触をしないシャドーイングを中心とした実習を構築した．教育上の工夫として， 1名の患者を原則2名の学生で受け持ち，午前と午後に分散して実習を行った．また，会話を通して知り得る患者情報の取得や看護援助の実施は，学生自身が実践する姿勢をもって実習に臨むことができるように，シャドーイングの視点を明確に，具体的な内容を明文化させ，看護師を媒介者として患者と関わる経験となるよう支援した．その結果，学生は自分が考えた看護計画に基づく援助内容を媒介者である看護師の実践をもとに評価し，さらに，看護師が臨床判断のもと実践した援助と比較し，臨床判断の根拠を踏まえた実践的な学びにつながった．一方で，援助的人間関係の構築過程や看護技術の実践は経験できず，課題が残った．抄録全体を表示PDF形式でダウンロード (468K) -
