    Every finding during a neck autopsy may be essential for accurately diagnosing and explaining the mechanism of death. In this prospective study in the Department of Forensic Medicine and Toxicology of Jordan University Hospital, 17 out of 95 neck autopsies revealed contusions of the laryngeal mucosa in the vocal or perivocal area. These contusions were found to be associated with various causes of death, including mechanical asphyxia (such as throttling, ligature strangulation, hanging, smothering, choking, plastic bag asphyxia, gagging, and inhalation of blood) and other causes (such as atypical drowning, carbon monoxide poisoning, head injury due to a road traffic accident, burns, electrocution, and brain edema). This study provides insight into the mechanisms of this important lesion and may contribute to a better understanding of the cause of death.

    This study investigated the permeation resistance of chemical protective gloves made of laminate film comprising nylon, ethylene-vinyl alcohol copolymer (EVOH), and other materials against different chemical substances to examine their usability in different work processes. The permeation resistance of the chemical protective glove was tested using the Japanese Industrial Standards (JIS) test method against twelve substances: acetone, acetonitrile, dichloromethane, ethyl acetate, n-hexane, methanol, tetrahydrofuran, toluene, 2-propanol, 1-butanol, 1,4-diethylene dioxide, and ethanol. After 480 min, no substance, except for methanol and ethanol, permeated at a standard permeation rate of 0.1 μg/cm2/min. Methanol and ethanol showed permeation at 1 min and 30 min elapsed, respectively. Hence, the gloves tested in this study exhibited permeation resistance to various chemical substances, and can thus be used in many work processes. Some film materials have short permeation time against certain chemical substances, but the chemical protective gloves tested in this study can be used at work sites, such as manufacturing sites, that require permeation resistance to different chemical substances.

    Elevated lactate levels are associated with a poor prognosis in patients with sepsis and shock. Intravenous glycerol administration is often used in Japan to treat patients with acute stroke or brain trauma, but such treatment can cause elevated lactate levels. We experienced a case of transient hyperlactatemia induced by intravenous glycerol administration in a patient with brain trauma. A 74-year-old woman underwent decompressive craniotomy because of loss of consciousness and brain edema. Glycerol was administered after the operation for management of the brain edema. Although the patient’s hemodynamics remained stable, her lactate level decreased and increased repeatedly. We recognized that the elevation in her lactate level was associated with the administration of intravenous glycerol. This case suggests that intravenous glycerol administration can induce transient hyperlactatemia.

    Neuroendocrine prostate cancer (NEPC) is a histological variant of prostate cancer and is characterized by aggressiveness and poor clinical outcomes. NEPC usually develops as a mechanism of treatment resistance in patients receiving hormone therapy for advanced prostate cancer. NEPC is sensitive to primary platinum-based chemotherapy, and has a short response duration. Second-line therapy is required in many cases, but clinical data on subsequent treatment after progression to first-line chemotherapy is limited. Here we report our experience of four cases of NEPC treated with second-line chemotherapy. Progression-free and overall survival rates were very low in three of the patients. One patient received multidisciplinary therapy using systemic and local chemotherapy and radiation therapy and survived for 24 months after initiation of second-line chemotherapy. Multidisciplinary therapy with chemotherapy and radiation is a promising option for improving the survival of patients with NEPC.

    Tarsal tunnel syndrome (TTS) is a nerve entrapment of the posterior tibial nerve. This uncommon condition frequently goes undiagnosed or misdiagnosed even though it interferes with the daily activities of workers. Here we discuss the return to work status of a 37-year-old male patient who manages a manufacturing plant. He was identified as having Tarsal Tunnel Syndrome as a result of a foot abnormality and improper shoe wear. He had moderate pes planus and underwent tarsal tunnel release on his right foot. What are the determinant factors in defining a patient’s status for returning to work after a tarsal tunnel release? We conducted a literature review using PubMed, Science Direct, and Cochrane. The Indonesian Occupational Medicine Association used the seven-step return-to-work assessment as a protocol to avoid overlooking the process. Duration of symptoms, associated pathology, and the presence of structural foot problems or a space-occupying lesion are factors affecting outcome. Post-operative foot scores, including Maryland Foot Score (MFS), VAS, and Foot Function Index, can be used to evaluate patient outcomes. Early disability limitation and a thorough return-to-work assessment are needed.

    ロボット支援下手術は,拡大視・立体視・多関節機能を有する鉗子などにより精密な手術が可能であるが,特有の体位セッティング・ポート配置・ドッキングなどの操作が必要で,手術時間や麻酔時間の延長につながる.当院では(1)体位を低位砕石位から開脚位へ変更,(2)臍のカニューラを安定して保持するためにHasson coneを取り付け,(3)カニューラのオブチュレータ(内筒)をブラントオブチュレータからブレードレスオブチュレータに変更,(4)チームを固定,(5)婦人科全員で定期的にドッキングのトレーニングを施行することにより,コンソール開始までの時間短縮を図ってきた.本研究ではその成果を検討した.対象は産業医科大学病院で2019年4月から2022年7月までに産婦人科で良性子宮疾患および子宮体癌IA期に対してロボット支援下子宮全摘出術を行った全77例とし,症例1-40の前半群と,症例41-77の後半群とで,コンソール開始までの所要時間を比較した.結果は,体位セッティング時間・ポート配置時間・ドッキング時間とも,前半群と比較して後半群で有意に時間短縮した.前半群では挿管終了からコンソール開始までに91.5分(53-131分)所要していたのに対し,後半群では59分(37-126分)であった.使用機材の適切な選択やチーム教育により,コンソール開始までの時間は短縮可能である.

    子宮頸部発生の中腎型HPV非依存性腺癌(中腎癌)は希少であり,その治療方法は確立されておらず,化学療法感受性についての検討も不十分である.症例は30歳,未経産.不正性器出血を主訴に受診,子宮頸癌を疑った.診察,生検および画像診断から子宮頸部腺癌IIA2期と診断し,手術予定としたが,SARS-COV-2感染症に罹患したため,当時のSARS-COV-2罹患後手術リスク評価を鑑み8週以上の手術待機の方針として,パクリタキセル・カルボプラチン(Paclitaxel and Carboplatin: TC)療法を2コース実施した.部分奏効と判断し,広汎子宮全摘出術を施行した.摘出検体の病理組織学的診断の結果,中腎癌IIA2期,ypT1b2N0M0と診断した.術後はTC療法を4コース追加投与し,13カ月再発を認めていない.子宮頸部中腎癌に対し,術前化学療法(Neoadjuvant chemotherapy: NAC)としてTC療法を実施し,奏効を確認しえた最初の報告をする.

    Digital and technological solutions constitute a key enabler to achieving better workplace safety and health outcomes. Fundamentally, the success of their implementation is underscored by a need for employers, employees, industry stakeholders and policymakers to collaborate in adopting a “digital first” mindset. This review provides a background on the evolution of work and the workforce in post-independence Singapore, and introduces pertinent local workplace safety and health trends. It delves into how a “digital first” approach may be adopted and effected, followed by challenges and opportunities in the digitalization of Singapore’s workplace safety and health landscape. Illustrative examples are used to highlight applications of digital and technological solutions in the control of occupational hazards. In our discussion, workplace strategies are built around the hierarchy of controls framework, whereas worker-related strategies are divided into workspace optimization for productivity/performance enhancement, training/education, and instituting surveillance/open reporting mechanisms. We demonstrate that with an open and forward-looking mind, and well-executed change management, we will be able to capitalize on technological advances to improve work and working conditions for all workers.

    The exact definition of a personal health record (PHR) is not clear, but it is generally defined as an electronic record of an individual’s medical examination results and medication history that can be accurately tracked by the individual and family members. There are many opportunities to obtain personal PHR data in the field of occupational health, and the amount of information in a worker’s PHR is enormous. It is necessary in this field to comprehensively integrate and accumulate not only information from medical examinations, but also records of the work environment and work records as personal health information.

    Digital health implementation is crucial to make the greatest benefit to a country’s citizens, including its workers. This article describes the implementation of digital health in both general health and occupational health. An internet search regarding the implementation of digital health, telemedicine, and telehealth in Indonesia was performed during July – August 2023. Publications released from the Ministry of Health Republic of Indonesia, the Ministry of Labour, Manpower and Transmigration of the Republic of Indonesia, as well as from various sources regarding digital health and telemedicine, were used in the writing of this article. Digital health transformation in Indonesia has been set as national goal since 2021 and some challenges still present as potential setbacks of nationwide digital health implementation.

    The population of people in Japan over 65 years old is expected to exceed 30% by 2025. As the society ages, there are not only healthy workers and employers, but also an increasing number of diseased or injured workers. Falls, the most common occupational hazard, increase in incidence with age. The management of osteoporosis and prevention of bone fractures from falls are emerging in elderly female workers, to reduce the loss of work productivity. Rheumatoid arthritis is a representative musculoskeletal disease that causes functional decline because of joint damage mainly in working women, but appropriate treatment improves disease activity and work productivity in workers with rheumatoid arthritis. It is also important not only to digitize subjective information by converting it into digital form (digitization), but also to digitalize the physiological information related to health, labor and disease (digitalization). In the future, artificial intelligence (AI) will be able to analyze vast amounts of physiological information (big data) obtained from workers and patients via the Internet of things (IoT), which will improve the information value linked to health promotion and optimal treatment practices, and contribute to the Digital transformation (DX).

    This paper discusses the role of the workplace in digital occupational health as part of an increasingly digitalized working life. Digital occupational health can be considered to consist of at least the following digitalized components: a) occupational health services and data, b) human resource data, c) group-level field data collected from the work environment and employees, and d) individual-level field data collected for personal use only. These data and related processes form a basis for so called data-driven management of occupational health and safety. To collect such data and keep it updated, it is important to pay attention to: a) worker acceptance, b) user friendliness, c) data validity, integrity, and protection, d) adequate resources, and e) ethical and effective use of the data. The current literature suggests that there are promising mobile and wearable devices and eHealth solutions to support worker health. To use them effectively, it is good to pay attention to the implementation process in the workplace. Ultimately, trust and collaboration among all parties are the cornerstones for gaining benefits from digital occupational health.

    Stress is a common part of working life, but knowledge is lacking on how to identify it early and with little effort on the part of the employee. We investigated whether simple stress reports and computer usage data could be useful tools for long-term assessment of stress in real life. 38 experts responded to a baseline questionnaire on need for recovery (NFR) and psychological distress (General Health Questionnaire, GHQ12). Their computer usage for work was recorded for 5 months, during which they filled in a 4-month simple diary and a 2-week detailed diary on, for example, stress and productivity. Salivary cortisol and heart rate variability were collected on 3 consecutive days. Generalized estimating equations models were used for the analyses. High NFR and GHQ12 predicted self-reported stress during work, and a decrease in (some) mouse usage features, but not keyboard usage features, over the following months. Some mouse usage features were associated with stress and productivity. The results provide some support for the usefulness of simple stress questions and mouse usage features in assessing long-term stress in real life.

    This mini review explores the transformative potential of big data analysis and artificial intelligence (AI) in reforming occupational medicine in Indonesia. Emphasizing the preconditions, case studies, and benefits, it underscores the role of big data in enhancing worker well-being. The review highlights the importance of informative health big data, especially in high-risk industries, with examples of case studies of AI implementation in occupational medicine during the COVID-19 pandemic and other relevant scenarios. While acknowledging the challenges of AI implementation, the essay identifies the role of academic and professional organizations as pioneers in big data utilization. Six potential benefits that are identified, including improved patient care and efficient resource allocation, demonstrate the transformative impact of big data analysis. The proposed pathway of preparation underscores the need for awareness, skill enhancement, and collaboration, addressing challenges in data management and stakeholder engagement. The conclusion emphasizes continuous assessment, feasibility studies, and commitment as essential steps in advancing occupational medicine through big data analysis.

    There is clear scientific evidence that physical activity helps to prevent depression and anxiety. Utilizing mobile health (mHealth) technologies to enable physical activity is promising, but the evidence of the effectiveness of mHealth interventions on physical activity and mental health is inconsistent. We recently developed a native smartphone app to prevent depression and anxiety by promoting physical activity. One of the app’s strengths is that it adopts a deep-learning model and automatically estimates psychological distress from users’ physical activity patterns. We conducted a single-arm, 1-month feasibility trial to examine the implementation of the app and its effectiveness in promoting physical activity and improving depression and anxiety. As a result, we did not observe any significant improvement in physical activity or psychological distress. For implementation aspects, the participants used the app less. The conclusion of the presentation is that mHealth interventions are promising for the improvement of physical activity and mental health among workers, but, at this stage, their effectiveness is unclear. There are challenges to be addressed, especially in implementation.
