    The plaque technique of Cunningham and Szenberg was used to study the adjuvant effect of 25 kinds of purified saponins on IgM formation.
    1) The administration of onjisaponin A (dose : 1-20 μg/mouse), onjisaponin B (0.2μg), platycodin C (0.1μg), platycodin D (0.1μg) and platycodin I (10μg) was proved to increase the number of plaque-forming cells of the spleen after immunization with sheep red blood cells.
    2) The decrease of the number of plaque-forming cells was recognized by the administration of onjisaponin B (10μg), onjisaponin C (10μg), platycodin D (10 μg), chikuset-susaponin III (10μg), ophiopogonin D (10-100μg), glycoside K (1-100μg) and ginsenoside Rc (1-10μg) .
    3) Neither increase nor decrease of the number of plaque-forming cells was seen by the injection of chikusetsusaponin IV, chikusetsusaponin V, phytolaccasaponin B, phytolaccasaponin E, ophiopogonin A, ophiopogonin B, glycoside H1, glycoside H2, ginsenoside Rb1, ginsenoside Rb2, ginsenoside Rd, ginsenoside Re, ginsenoside Rf, ginsenoside Rg1and ginsenoside Rg2.
    4) No remarkable effect of the administration of saponins on the spleen index could be seen, as compared with animals immunized with sheep red blood cells alone.

  • -CLASSIFICATION AND DISCUSSION OF EXPERIENCED CASES-

    We have experienced 15 cases widely categorized in atypical leukemia, and tried to classify these cases into following 4 groups.
    Group 1. Leukemic change from preleukemic states. Four in 15 cases developed from the preleukemic stages such as aplastic anemia, sideroblastic anemia, and thrombocytosis.
    Group 2. Specific clinical features during the course of leukemia. In this group, the combination with CNS leukemia, leukemic pleuritis, and myeloblastoma were involved. We had six such cases.
    Group 3. Alternation of main leukemic cells during the course of leukemia. Two cases were observed in erythroleukemia and myelomonocytic leukemia.
    Group 4. The clinical features of so-called atypical leukemia. There were 3 cases called low percentage leukemia or smoldering leukemia.
    Fifteen cases of atypical leukemia were classified into 4 groups as described above and discussed in detail.

    The strenuous two-step test which charges 3 times the exercise load of single two-step test to a patient as much as possible in 3 minutes, was devised with the use of a 3-channel radiocardiograph, in an attempt to improve the rate of diagnosis of coronay insufficiency.
    Maximal heart rate during the strenuous two-step test ranged about 84 to 87% of maximal heart rate which was reported by Robinson.
    Positive responses were seen in 35.1% of 148 cases with cardivascular disease and in 50% of 42 cases with ischemic heart disease, in which the double two-step test was negative. In healthy normal subjects, only 7.1% of 197 cases gave findings interpreted as positive.
    In 49.4% of 79 cases with positive responses, the time of appearance of ST depression which was interpreted as positive reaction took place from during exercise to less than 30 seconds. Without the use of telemeter, the positive incidence may become lower.
    Complaints of patients during and after exercise loading consisted mostly of mild palpitations and byspnea dizziness in a few cases.
    It is concluded that this test is quite useful in the diagnosis of coronary insufficiency undetectable by the double two-step test.

    The effects of thiamine (B1), its related substances and histamine liberators on blood pressure were studied by intraventricular injection to the rats. The results were as follows :
    1) The lasting rise of blood pressure was observed by the injection of B1 (50 to 200μg/animal), thiaminic acid (200μg), desthiothiamine (200μg) and cocarboxylase (CoC, 10μg) . The similar effect was observed by the injection of histamine liberators such as compound 48/80 (0.01μg), polymyxin B (0.01μg) and d-tubocurarine (0.5μg) .
    2) This pressure rising effect of B1, CoC and histamine liberators was inhibited by the pre-treatment with 20μg of antihistaminics (tripelennamine) . The pre-treatment of procaine (0.01 μg) did not inhibited effect of B1and CoC.
    3) Each successive injections of B1, thiamine tetrahydrofurfuryl disulfide (TTFD), CoC and histamine liberators caused tachyphylaxis.
    4) Polymyxin B and B1or d-tubocurarine produced cross-tachyphylaxis, which also occurred in the case of d-tubocurarine and B1, TTFD or polymyxin B.
    From these results, it seemd that the blood pressure rising effect by the injection of B1and its related substances may be ascribed to the histamine releasing action of these substances.

    Effect of histamine and other agents on the operant behaviors in rats was studied, using a commercially available apparatus. Two schedules of the food reinforcement with a fixed ratio (FR-30) and a fixed interval (FI-60) and a schedule of discriminated avoidance response (DAR) were used for the conditioned behaviors.
    In the FR-30 and FI-60 schedules, histamine, 5-hydroxytryptamine (5-HT) and acetyl-choline (ACh), administered intraventricularly 5-20μg/rat, subcutaneously 5-10 mg/kg, produced the dose-related decreases in the behaviors. Although the potencies of histamine and 5-HT were almost the same, a large dose of 5-HT produced a long lasting inhibition. ACh was found to have the most potent action with a very short duration.
    In avoidance schedule, histamine and ACh were less suppressive than that in the food reinforcement schedule. 5-HT showed the same potency in the both kind of the schedules.

    Effect of B1and related agents on the operant behaviors in rats was studied, using a commercially available apparatus. Two schedules of the food reinforcement with a fixed ratio (FR-30) and a fixed interval (FI-60) and a schedule of discriminated avoidance response (DAR) were used for the conditioned behaviors.
    In the FR-30 and FI-60 schedules, B1, B1-P and B1-PP administered intraventricularly 10-50 μg/rat or subcutaneously 100-200 mg/kg, produced the dose-related decreases in the behaviors. In the DAR schedule, B1, B1-P and B1-PP did not produce significant alteration.
    The suppressive effect in the food reinforcement schedule by B1, histamine B1-P and B1-PP was partially antagonized by the simultaneous administration of tripelennamine, an antihistaminic.
    A large dose of B1caused an elevation of histamine level in the brain of mouse approximately 25%.
    From these results, a possibility that the suppressive effect of B1on the operant behaviors may be responsible for histamine in the brain, is discussed.

  • -AVERAGE NUMBER OF GLOMERULI AND THEIR AVERAGE AREA-

    In 89 autoptic kidneys, the average number in a given zone (1 mm2) and the average area of glomeruli were determined by means of manual optical picture analyzer (abb. MOP) . Their variations with age were discussed and also their relations with the renal weight were studied.
    The results were as follows ;
    1) The weight of the kidney tended to increase from the first day of birth, reach at the level almost equal to the adult one in the late teens, and reduce rather gradually after the 70's.
    2) The average number of glomeruli present in a given zone (1 mm2) was about 30 on the first day of birth ; then, it reduced to about 5, which is equal to the adult one, in the late teens, but tended to reduce gradually after the 40's.
    3) The average area of glomeruli was about 4, 000μ2on the first day of birth, then was increasing to about 20, 000μ2in the early teens to be almost equal one. In the adults after the 20's, the average area of glomeruli did not change.
    4) The relations between the renal weight and the average number of glomeruli in a given zone (1 mm2) and between the renal weight and the average area of glomeruli were studied on adults' kidneys, but no correlative relation was observed.

  • “CLINICAL EVALUATION OF A NEW MINOR TRANQUILIZER, FLUTAZOLAM (MS-4101) BY A DOUBLE BLIND METHOD.”

    Using the double blind method, new tranquilizer MS-4101 (Ro 7-6102) and placebo were compared in surgical patients. Either 10mg of MS-4101 or placebo was given orally, once daily, to patients for 3, 5 or 7 days before the day scheduled for surgical procedures. A simple scoring system based on the observation of the patient's behavior and answers to standardized questions has been used. Results were statistically analyzed. Anxiety grade of patients were classified according to Manifest Anxiety Scale (MAS) . Judging from the global evaluation of the effects based on the score of “anxiety”, “getting to sleep” and “state of sleeping”, MS-4101 was effective in the high anxiety group, particularly in the 7 days-therapy group.

    Macroglobulinemia of Waldenström is a disease of the reticuloendotherial system characterized by a marked increase in the serum gamma macroglobulin concentration.
    This syndrome bears some clinical resemblance to both multiple myeloma and chronic lymphatic leukemia and therefore has to be differentiated from these diseases especially the former.
    The differentiation from multiple myeloma is made by longer and more benign clinical course, the lack of osteolytic bone lesions and the infrequent finding of Bence Jones proteinemia.
    The next clinical chemical findings may be necessary to diagnose as Waldenström's macroglobulinemia : (1) characteristics of IgM in serum protein is monoclonal, (2) concentration of monoclonal IgM is greater than 1.0 g/dl in serum, and (3) monoclonal IgM fraction in serum is more than 15 per cent of total serum protein with ultracentrifugation.
    In this paper the results of clinical laboratory findings which examined 2 cases of Waldenström's macroglobulinemia in our hospital have been investigated and the criteria for the diagnosis of this disease have been reevaluated.

    During general anesthesia with a modified neuroleptanesthesia in a 18 year old male with a spinal cord tumor, the authors experienced so-called mass reflex characterized by profound hypertension, increased pulse pressure and cutaneous flushing when the tumor was removed, and then arterial oxygen tension decreased from 171 to 90 in mercury.
    But we succeeded in overcoming the elevation of blood pressure with the alcuronium having a sympathetic ganglion blocking action.
    Generally we have been used a light anesthesia in paraplegics, but since such patients have these dangers this method induced the elevation in blood pressure was not desirable, and particularly a modified neuroleptanesthesia should not be used.
