    The Ultrasonic osteotome, a new type of osteotome, was evaluated for its safety and efficiency in laminectomy and osteotomy in cat and rabbits. The best set up amplitude was 65 to 75% and irrigation rate was 10 ml/min.
    We measured the spinal cord evoked potential during laminectomy and evaluated the influence on the spinal cord adjacent to that where laminectomy was performed using ultrasonic osteotome. The mean amplitude of postoperative spinal cord evoked potential was 67.7% (rabbits) and 77.6% (cat) of preoperative control and there was no sudden decrease in amplitude. There was no pathological findings in the postoperative spinal cord, such as bleeding, swelling or demyelination, caused by mechanical compression. There was no pathological difference in osteocytes by HE stain, under various condition with the amplitude and irrigation rate changed. On contact microradiography, a high intense area in the surface of osteotomy was found with an irrigation rate of 0 ml/min. Scanning electron microscopic results of the surface of the osteotomy after use of ultrasonic osteotome showed an irregular surface, while the surface made by chisel were smooth.
    There was no significant difference in the surface of the osteotomy site with any set up in amplitude and temperature. The temperature of the surface of the osteotomy site was well controlled by irrigation. Proper use of the ultrasonic osteotome enabled safe and precise osetotomy. Based on the results of this study, the efficiency of the ultrasonic osteotome for osteotomy during spinal surgery was confirmed.

    To investigate the effect of aluminum (Al) contamination in intravenous solutions administered to neonatal patients, Al was measured in 51 samples from various available components that are used for intravenous therapy. Aluminum contamination in intravenous solutions was arbitrarily classified as high (≥501μg Al/l), intermediate (51-500μg Al/l) or low (≤50μg Al/l) . The high group included solutions containing calcium and phosphorus ; the intermediate group included potassium phosphate, magnesium sulphate, and amino acids. We also measured serum Al levels in 55 neonatal patients (gestation : 21-42 weeks, birth weight : 426-3, 966g) . The serum Al concentration of normal controls on the first day of life was 7.0±3.3μg/l. The patients who were under treatment of intravenous calcium solution, had a high serum Al concentration (27.3±5.7μg/l) . The patients who were receiving parenteral nutrition, also had a high serum Al concentration (23.8±11.8μg/l) . We concluded that Al contamination in intravenous solutions should be minimized to prevent Al toxicity.

    Anthropometric data were collected from 1 month to 12 months (male, n=189; female, n=189), in order to assess the nutritional state. We have measured weight, height, chest circumference, head circumference, skinfold thickness (triceps, subscapular) and upper-arm circumference. Triceps skinfold thickness was increased rapidly during the first 6 months and then, gradually declined thereafter. Subscapular skinfold thickness was increased during the first 3 months and then significantly declined thereafter. Upper arm circumference was increased rapidly during the first 3 months and then, increased slowly. These results indicate that rapid weight gain during the first 3 months is correlated with the accumulation of fatty tissue, and the gradual weight gain in the later period is correlated with an increasing amount of muscular tissue. During infancy, there was no difference in skinfold thickness between males and females.

    In the present study, we examined the effects of rhBMP-2 on non-transformed human osteoblast-like cells. Biopsies of iliac bones were obtained from 4 patients during orthopaedic surgery. Osteoblast-like cells (HBM cells) were isolated from bone biopsies by collagenase and dispase digestion. All experiments were performed using cells within 3 passages. The cells excised showed alkaline phosphatase (ALP) activity, parathyroid hormon (PTH) response and 1, 25-dihydroxy vitamin D3 [1, 25- (OH) 2D3] -dependent osteocalcin production. They were treated for 3 days with graded concentrations of rhBMP-2 at a confluent state. Treatment with 250ng/ ml rhBMP-2 increased the ALP activity in all cases. rhBMP-2 stimulated the PTH response in 3 cases but not in one case. This polypeptide inhibited 1, 25- (OH) 2D3-dependent osteocalcin production in all cases. This study suggests that rhBMP-2 elicits differentiation of human osteoblasts.

    To clarify the cause of incurvated nail, the relation between the nail plate and the distal phalanx of the patient of incurvated/ingrown nail and normal subjects was compared, by measuring CT scan images. The investigation was made on 12 incurvated nails, 8 ingrown nails and 8 normal nails of the first toes which were clinically diagnosed as such. Each toe was scanned at 1mm intervals with the same CT scanner under the same conditions. The pictures taken on the film were scanned and input into a computer. The transverse width of the distal phalanx and the width of the nail plate were measured on each slice. The minimum transverse width of the distal phalanx in both stages of slightly and intensively incurvated nail was smaller as compared to that in the stages of normal nail and ingrown nail. The ratios of the width of the nail plate to the minimum transverse width of the distal phalanx in the stages of normal nail, ingrown nail and intensively incurvated nail were nearly equal, while the ratio in the stage of slightly incurvated nail was outstandingly large. Based on the results of this investigation, it can be considered that the transverse width of the distal phalanx is reduced first, and consequently, the incurvation of the nail plate occurs, then the width of the nail plate is reduced as the incurvation becomes more intense.

    A study of detailed computer analysis of the anthropometric data of the palpebral fissure in 99 Japanese (63 males, 36 females) was performed with Standardized photogrammetry of the face. In each subject, 6 standard points were stipulated and 17 anthropometric measurements were taken. 8 indices were calculated to illustrate in measurements, 7 measurements as well as these indices were processed statistically for significant differences by sex, and right and left sides. The male and female inclination of palpebral fissure, and the male length of palpebral fissure was significantly different in right and left sides. The length of the cornea, the length of the palpebral fissure, the exposing index of cornea, the widening index of palpebral fissure, and the width of palpebral fissure on the right and left, and the inclination of palpebral fissure the right, and the intercanthal distance and the width of face was significantly different by sex. These measurements and indices were compared with the Uchida's beauty index of the palpebral fissure. As a result, 28.37% of the males and 28.65% of the females were within this index.

    A progressive elevation of plasma gonadotropins and sex steroids during puberty in females has been well documented. The existence of inhibin, a nonsteroidal gonadal regulator of FSH secretion, was reported. However, the changes of inhibin and its physiological role during human puberty has not been well understood. In the present study, the concentrations of plasma inhibin together with those of FSH and estradiol were measured by RIA in pubertal girls. Six healthy 6-year-old girls were selected ; serum samples were collected yearly for 6 years. Plasma inhibin levels decreased progressively with age from 6 to 9 years and rose significantly from 9 to 10 years. A significant rise in plasma FSH levels from 7 to 9 years was observed and the levels increased with advancing age. Estradiol levels tended to rise from 8 to 11 years old. Although statistically it was not significant, the tendency of inverse correlation was noticed between the levels of inhibin and FSH. This longitudinal study demonstrated for the first time the decrease of inhibin concentrations from 6 to 9, suggesting that the changes in inhibin are closely related to the initiation of FSH rise in prepubertal girls. The increase in inhibin levels from age 10 may be due to the follicular development before menarche.

    There have been many histopathological studies on the gallbladder in cholecystitis, but reports of inflammatory changes in the cystic duct are few. Through operative findings, the inflammatory changes in the cystic duct seemed to be more mild than those in the gallbladder in cholecystitis. In this study, we comparatively evaluated the inflammatory changes between the gallbladder and cystic duct. A total of 103 patients were entered in this study ; 95 patients had received cholecystectomy, and 8 specimen were obtained from autopsy. In 24 patients (47.1%) with cholecystolithiasis and in 3 patients (30.0%) with cholecystocholedocholithiasis, the degree of infiltration of the inflammatory cells in cystic ducts was more mild than those in the neck of the gallbladder. The degree of fibrosis of the wall of the cystic duct was similar to that of the neck of the gallbladder, or it was more mild. In the correlation of the infiltration of the inflammatory cells and the fibrosis of the wall, the patients with fibrotic changes in the gallbladder had more advanced infiltration of inflammatory cells. As for the accumulation of elastic fibers, in 11 patients (19.7%) with cholecystolithiasis and 1 patient (10.0%) with cholecystocholedocholithiasis, it was more prominent in the cystic duct than in the neck of gallbladder. Among patients with cholecystolithiasis and cholecystocholedocholithiasis, none with accompanying mild infiltration of inflammatory cells and fibrosis of the wall, had severe accumulation of elastic fiber. In this study, we found that inflammatory changes of the cystic duct was less noted than that of the neck of the gall bladder. Cystic duct has a helical-line structure which produces two converse flows of bile. This structural characteristic may play a role in defending the choledochus from the spread of cholecystitis.

    We assessed self tendon grafting using the patellar ligament in rabbits by electron microscopic observation of the progress of ligament repair following grafting. Using adult Japanese white rabbits, central region, comprising one-third of the total length of the patellar ligament was extirpated with or without the bone under systematic anesthesia, and the pediculate graft an free graft pieces were collected for use in the reconstructive operation of the anterior cruciate ligament. The animals were sacrificed 4, 12 and 24 weeks after grafting to collect the grafted ligaments. The ligaments were prefixed with a mixture solution of 4% paraformaldehyde and 5% glutaraldehyde. Subsequently, the ligaments were postfixed with 1% osmium tetroxide and embedded in an Epon-Araldite mixture. Ultrathin sections were stained with uranylacetate and lead citrate, and examined with an Hitachi HS-9 electron microscope. In the grafted ligaments, the fibroblasts appeared to be enlarged and also increased in number, compared with those in the normal patellar ligament. Many enlarged granular endoplasmic reticula and well developed Golgi apparatus were observed in the cytoplasm of the fibroblasts in the grafted ligaments 4 and 12 weeks after grafting. The nuclei which were of angled in shape were fever in number, and most of these were elliptic in shape. For the collagen fibrils, biphasic distribution of thick fibrils (100-300nm in diameter) and thin fibrils (10-20nm in diameter) were observed in the normal patellar ligament. However, the grafted ligaments exhibited uniform distribution of the fibrils mostly with diameter of 30-60 nm ; the density of the distribution was slightly sparse compared with those of the nomal patellar ligament. No large difference was observed in the growing processes of the grafted tissues between the pediculate grafts and free grafts. From these findings, it was suggested that the grafted ligaments in the reconstructive operation of the anterior cruciate ligament by the use of the patellar ligament may be recomposed in the transplanted area and transformed into the structure similar to the anterior cruciate ligament. Further, there was no large difference in the grafting processes between the pediculate grafting and free grafting.

    Expression of the c-erbB-2 oncoprotein (ErbB-2) and the p53 gene product was examined in the operative specimen from 68 human breast cancer patients. These patients were detected immunohistochemically by the streptavidin-biotin method using a commercial kit (SABPO kit, Nichirei Co., Inc., Tokyo) and monoclonal anti-mouse antibodies specific for the intracellular domain of the protein. Expression was evaluated as positive when specific membranous staining for c-erbB-2, and nuclear staining for p53, were observed in a majority of cancer cell nets. The expression of the estrogen receptor (ER) as a marker of hormone dependency was also examined ; serum levels of these two gene products were measured (c-erbB-2: mAbl Triton Diagnostics Co., Inc. Alameda, CA) (p53: Ab-2 Oncogene Science Co., Inc., Uniondale, NY) . Tissue c-erbB-2 and p53 products were positive in 22.1% (15/68), and 25.0% (17/68) of the components, and serum c-erbB-2 and p53 protein levels were positive in 19.1% (13/68) and 7.4% (5/68) . Oncoprotein (c-erbB-2 or p53) tissue expression-positive patients showed a tendency to be estrogen receptor negative and 8 cases were positive on p53 protein expression out of 25 scirrhous breast cancer. The data might provide a better characterization of the human breast cancer phenotype, and two expressions of the c-erbB-2 and p53 protein in the tissue would be useful for judgements of prognosis.

  • -Changes of Spinal Cord Function and Prospective of The Post-Operative Outcome in Spinal Cord Tumor Patients-

    We measured the ascending spinal cord evoked potentials (ASCEPs) in 65 spinal surgery or spinal cord surgery cases. In 11 spinal cord tumor patients (age, 18 to 73 years ; average 51.6 years.), the relationships among the changes in intraoperative ASCEPs, spinal cord function, and postoperative outcome were analyzed. The control group included 54 spinal surgery or spinal cord surgery patients who underwent intra-operative ASCEP monitoring. The typical ASCEPs consisted of 2 negative peaks (N1, N2) . Before spinal cord decompression, N1 showed, 0.7 to 7.5μV (averaged 2.7μV) in amplitude ; this peak apparently increased after laminectomy and dural incision (0.8 to 15.9μV, average 4.8μV after dural incision) . Post-operatively, the motor score recovered from 4.3 points to 5.0 points and the recovery rate was 16.9%. On the other hand, the sensory score recovered from 6.6 points to 9.2 points and the recovery rates was 71.2%, indicating that more sensory recovery was obtained by the surgery. If the patients were separated to an H group (amplitude increased to more than 200%) and an L group (less than 200%), the post-operative recovery rate of the sensory score was apparently dominant in the H group (H group, 118.8% ; L group, 41.9%) . Thus, the prospect of good post-operative outcome seemed possible with ASCEP monitoring applied during spinal cord tumor surgery.

    An autopsy case of pleomorphic carcinoma of the gall bladder is reported. An 81-year-old woman was admitted complaining of upper abdominal pain and hepatomegaly. Laboratory data showed elevated serum levels of CEA and CA19-9, and abdominal CT and US revealed SOL in the liver. Cytological findings of the ascites disclosed hepatoma-like tumor cells, showing large atypical mono/multi nuclear cells with some prominent and large eosinophilic nucleoli. PAS-positive granules were present in the cytoplasma. At autopsy, white tumorous tissue seen in the gall bladder, showed a direct invasive growth pattern to liver. Histologically, severe atypical large tumor cells and bizarre cells were seen, showing typical histological characteristics of pleomorphic carcinoma. Focally, tubular adenocarcinoma was associated. Immunohistologically, some tumor cells were positive for EMA, and others were positive for vimentin. From these findings, pleomorphic carcinoma was considered to be deepithelialized carcinoma which originated from the cholecystic mucosa.

    Many methods of posterior lumbar interbody fusion (PLIF) have been reported since Cloward introduced the procedure in 1943 for treatment of lowback pain due to ruptured interververtebral discs. We report a case of lumbar spinal canal stenosis with instability treated with PLIF using a new type of prosthesis : a thin walled, threaded tubular shell of titanium. A 53yearold man was admitted complaining of lowback pain, pain and numbness of the left lower extermity, and left drop foot. Lumbar spinal canal stenosis at the L4-5 level with instability was diagnosed, and PLIF was performed using the threaded fusion cage system. We exposed the posterior aspects of L4 and L5 using standard methods. The herniated nucleus pulposus of L4-5 was first resected, then two threaded fusion cages were inserted into the L4-5 intervertebral space. Chips of cancellous bone, taken from the nearby iliac crest, were tightly packed into the cages. Although this fusion device was introduced by Ray in 1991, this is its first reported use in Japan.
