    It is widely recognized that accumulation of hydrophobic bile acids within hepatocytes plays a key role in liver injury such as apoptosis. The aim of this study was to investigate whether apoptosis was induced by hydrophobic bile acid (glycochenodeoxycholic acid: GCDCA) in RLN-8 cells that were established in culture from a normal Donryu rat. RLN-8 cells were treated with GCDCA from 2 to 12 hr. A ladder-like DNA fragmentation pattern was demonstrated in the DNA extracted from RLN-8 cells treated with GCDCA for 12 hr. Moreover, the treatment with GCDCA for 6 and 12 hr showed a significant increase in the levels of single-strand DNA in apoptosic cells, compared with the untreated RLN-8 cells. RLN-8 cells treated with GCDCA for more than 6 hr significantly increased the release of cytochrome c from mitochondria and the caspase activity compared with those of untreated cells. In RLN-8 cells after GCDCA treatment, changes in mitochondrial membrane potential were observed. Treatment with GCDCA for more than 2 hr induced a significant increase in Fas mRNA expression, and Bax a mRNA expression was significantly increased for more than 4 hr in RLN-8 cells.
    These results demonstrated that GCDCA as a hydrophobic bile acid induced apoptosis in RLN-8. In RLN-8 cells, two pathways on apoptosis induced by GCDCA are suggested: (1) Fas stimulates caspase activation, promotes changes in mitochondrial membrane potential and releases cytochrome c from mitochondria, and (2) Bax binds to form a mitochondrial pore and releases cytochrome c into cytosol.

    We conducted an epidemiological survey to study the relationship between lifestyles and medical expenditures for 5, 802 persons who belong to a government medical insurance program. The results of our study show that subjects who had been directed to receive medical treatment by their family physician had not only good eating habits but higher medical expenditures. However, the people who had not been directed to receive medical treatment had both good eating habits and lower medical expenditures. This tendency was also seen in the case of health examinations. In the former group, the people who had health check-ups showed higher medical expenditures, though the later group showed lower medical expenditures. In particular, those who smoked showed higher medical expenditures than non-smokers. According to a simulation model of the economical effect of quitting smoking, there would be a-14 billion-yen decrease in medical expenditures as a result of a 10% decrease in smoking rate. This was also seen in alcohol consumption; 10% decrease in drinking rate would result in an 8-billion-yen decrease in medical expenditures. In addition, these effects were more remarkable in the former group of people who were directed to get periodical medical consultations.

    The ABCA3 gene is expressed only in the human lung at the limiting membrane of lamellar bodies in type II cells. In this study we investigated immunohistochemically the expression of ABCA3 in lung adenocarcinoma and compared the expression of surf actant apoprotein-A (SP-A) and thyroid transcription factor-1 (TTF-1) both used as a tumor maker of lung adenocarcinoma. 70 cases of adenocarcinoma were immunostained. Positive staining of ABCA3 was observed in 68.8% of cases, which were faintly stained granules in the cytoplasm of cancer cells. Positive cells mostly appeared in the margins of the tumor mass. Most positive cells were cuboidal and some positive cells were tall, columnar and with mucus filled cytoplasm. SP-A expression was observed in 65.7% of cases. Positive cells were nearly identical to ABCA3 positive cells, except for tall columnar cells. TTF-1 expression was observed in 85.7% of the cases stained only in the nucleus. Tumor masses were stained diffusely. Not only cuboidal cells but various sized cells which formed a solid nest were stained, whereas tall columnar cells were not. The expression of ABCA3 in well and moderately differentiated adenocarcinoma was significantly higher than that of the poorly differentiated. There was no statistically significant relationship between SP-A expression and histological differentiation, but the rate of positive cases in poorly differentiated adenocarcinoma was lower. TTF-1 was diffused in most adenocarcinomas indicating that most adenocarcinomas had the character of alveolar Type II cells and Clara cells. ABCA3 and SP-A were observed in well differentiated adenocarcinomas indicating strongly the character of origin. Only ABCA3 was expressed in the tall columnar cells with mucus suggesting that ABCA3 was concerned with not only the formation of pulmonary surfactant but also the production of mucus.

    We investigated the care services of brain stroke patients in long-term care insurance. The subjects were 17 patients (10 males and 7 females) who lived at home and received long-term care insurance. We interviewed patients and their families to determine how the care services were supplied to the patient. The results indicated that the care services allotted to each division of cases were not fully used. There were 5 patients who hat not received any care at all, and other patients who had received less than half of the maximum provision of care. Our results also show that some case services were not received because the service aid of the long-term care insurance was insufficient. We suggest that the care services of the long-term care insurance plan do not reflected enough the demand of patients and their families. In an effort to remedy this situation, we suggest that health care service plans be made by hospital teams (which included the rehabilitation medicine), public health centers, and social welfare and care managers or so-called“Community-based caring teams”of the longterm care insurance programs.

    Foreign bodies in the oral cavity such as a denture can be accidentally swallowed with food. Such foreign bodies are spontaneously discharged in most cases and rarely induce complications. We report a case of perforation of the duodenum due to an accidentally swallowed partial denture. An 82-year-old man was seen at a hospital because accidentally swallowing a partialdenture while eatlng the patient was referred to the hospital because the denture was not discharged for 3 days. Abdominal plain X-ray examination showed that the denture was in the upper abdomen. Endoscopic removal was unsuccessful because the denture was impacted around the horizontal part of the duodenum, and an abdominal x-ray examination showed that the denture had not moved. Five days later, the pating underwent laparotomy. There was perforation of the duodenum by metallic hooks of the denture protruding through the wall of the intestine. The perforated site was incised and the denture was removed, with a foreign body remaining in the intestine for a long time, careful management is required considering probable complications such as perforation.

    We administered general anesthesia two times to a Werdnig-Hoffmann disease boy. At the time of the first anesthesia, a muscle relaxant was not used. During general anesthesia induction, an intravenous anesthetic sleeping dose and an inhalation anesthetic were used. During the operation the use of only the inhalation anesthetic could manage the anesthesia safely. In addition, it was possible to accomplish general anesthesia safely during the 2nd anesthesia making use of only the inhalation anesthetic. We were able to safely conduct general anesthesia of a Werdnig-Hoffmann disease patient using only an inhalation anesthetic.
