    The central effects of CS-088, angiotensin II type 1 receptor blocker on intraocular pressure (IOP) in male pigmented rabbits were investigated. Fifty μL CS-088 was administered to the lateral eye, orally, or intravenous by from the auricular vein (i.v.) . For intracerebroventricular (icv) treatment, a guide cannula was inserted two weeks before injection into the lateral ventriulus following Fifkova's brain map ; ten μL CS-088 was subsequently injected (5 μL/min) to the lateral ventriculus. After the treatments, IOP was measured with a pneumatonometer and blood pressure (BP) at the right lower limb was monitored without anesthesia. IOPs were significantly decreased bilaterally after 30 min topical administration of 6 % CS-088 to the lateral eye compared to a phosphate buffer treatment of the control group (p<0.05, n=6) ; BP showed no change. With 6 % CS-088 oral administration, TOP and BP were not changed. TOP and BP decreased significantly at 60 min after 6 % CS-088 i. v. treatment (p<0.05, n=5) . IOP did not change with 0.06% CS-088 icy treatment. These results suggest that topical use of CS-088 might decrease IOP in the pigmented rabbit by a sympathetic nerve effect and its local effect.

    Since asthmatic bronchitis is defined as chronic inflammation, antiinf lammatory therapy such as an inhaled steroid and a leukotriene receptor antagonist have become very important. In addition, it has recently been reported that theophylline has an anti-inflammatory effect. We examined the role of theophilline in a guinea pig model of allergen-induced chronic lung inflammation. Guinea pigs, after intraperitoneal (Day 1) and intranasal (Day 15 and 22) ovalbumine (OVA) sensitization, and subse uent OVA inhalation for 2 weeks showed an increase in infiltrating eosinophils and airway resistance and an enlarged wall area, indicating an asthma-like development. The animals were treated with theophylline 7 mg/kg/d or leukotriene receptor antagonist 10 mg/kg/d during the study period. The dose-response relationship between airway resistance and nebulized acetylcholine was obtained before and after treatment. Animals treated with theophylline showed decreased of infiltrating eosinophils, the wall area in cartilaginous airways, and airway resistance. Results also showed that theophyline was able to inhibit infiltration of eosinophils even in the smooth muscle of cartilaginous airways where steroid inhalation therapy could not. On the other the hand, animals treated with leukotriene receptor antagonist showed only a decreased in airway resistance. These results suggest that theophylline has the same or more of an anti-inflammatory effect on asthma than leukotriene receptor antagonist.

    The hypothesis that the pre-care relationship between the primary caregiver and the cared-for affects the caregiver's sense of burden, the relationships of the primary caregiver's sense of burden with other individuals and environmental factors was evaluated. The subjects were 125 female primary caregivers for care insurance recipients aged 65 years and over. The Zarit Burden Scale, and characteristics of primary caregivers, the care environment, and the cared-for were assessed. The subjects were divided according to their relationships with the cared-for into good-relations, fair-relations, and poor-relations groups. Both the total score of the Zarit scale (model I ) and the total score of the 14 items extracted by factor analysis of the Zarit scale (model II) were significantly higher in the poor-relations group. Through swap-wise multiple regression analyses performed using Models I and II as dependent variables and the questionnaire items as independent variables, 5 of the 7 items in which the scores were significantly higher in the poor-relation group were adopted for model II . As a result, from the assessment of the connection between the pre-care relationship and the intensity of the caregiver's sense of burden, model II was shown to more strongly reflect the caregiver's sense of burden in the three groups. The primary caregivers in the good-relations group were often housewives and working women, but those in the poor-relations group were divided into part-time workers and caregivers. In the poor-relations group, many of the cared-for were the caregivers' parents-in-law, and often living together began when care giving became necessary. Further, in the poor-relations group the time the cared-for had been receiving care services tended to be longer, and the family's understanding was significantly poorer. These features were in agreement with the higher level of disability, and the significantly lower intellectual and consciousness levels measured by the Karasawa method and the N-ADL level in this group. In this study, a few ADL items were significantly correlated with the caregiver's sense of burden; the correlations with items such as “sitting up and walking” and “eating, ” which require frequent assistance, were particularly high. These results suggest the validity of the study's hypothesis.

    Critical paths are used to supply standard quality of perioperative management to patients. Twelve kinds of CPs were used for 256 patients from April 2002 to March 2003.
    Five patients were able to suspend CPs early ; we analyzed the variance factors in these patients. Complication occurred during surgery in two patients. The operative procedure was altered in one patient. Two patients did not need to continue CPs because of early discharge from the hospital ; it is supposed that these two discharged patients completed their CPs by education from the staff or by change of CP. However, because of post-operative variances, it will be necessary to use new CPs for patients with severe complications or altered operative procedures.

    In order to suppress the intracellular calcium overload in ischemia-reperf usion injury, KB-R7943, a selective Na+/Ca2+ exchanger inhibitor was added to the preischemic perfusates and its myocardial protective effect was studied. Rat isolated perfused hearts were divided into three experimental groups which were perfused for three minutes before ischemia with the fluids containing the following : 10 μM/l KB-R7943 solution (KBR group), KCl solution with a potassium concentration adjusted to 20 mM/l (KCL group), and 10 μM/l KB-R7943 solution with 20 mM/l of potassium concentration by adding KCl (KBR-KCI group) ; the control group was made ischemic by simply blocking coronary perfusion. After normothermic ischemia for 30 minutes, the hearts were reperf used, and the recovery rates of cardiac function (heart rate [HR], left ventricular contraction [LVC], maximum left ventricular contractive force change rate [LVdc/dt], coronary flow [CF] ) and intracellular calcium concentration were measured and compared with preischemic values. For the KBR-KCl group, cardiac function was also investigated after 60-minute ischemia (KBR-KCl 60 group) . After thirty minutes reperf usion, LVC, LVdc/dt and CF in the KBR, KBR-KCl and KBR-KCl 60 groups showed good recovery compared to those of the control group. the KBR-KCl group had an even better recovery in LVC, LVdc/ dt and CF than in the KBR group 30 minutes after perfusion. There were no significant differences among the measurements of the KBR-KCl and KBR-KCl 60 groups. For the kinetics of intracellular calcium, no intracellular calcium overload during ischemia-reperf usion was observed for the KBR-KCl and KBR groups compared to the control and KCl groups. Administration of KB-R7943 before ischemia suppressed intracellular calcium overload during ischemia-reperf usion and reduced ischemic-reperfusion injury. These results suggest that the addition of KB-R7943 to cardioplegia could have a cardioprotect effect against ischemicreperfusion injury.

    MRL/Mp-lpr/ lpr (MRL/lpr) mice spontaneously develop systemic lupus erythematosus (SLE) -like disease. The natural history of the pulmonary involvement and the underlying mechanism of leukocyte infiltration into the lungs of MRL/lpr mice and SLE patients remains to be elucidated. The aim of this study was to investigate the expression profiles of chemokine and the chemokine receptor in the lung of the SLE-prone mouse. The expression of IP-10 and its receptor, CXCR3, were evaluated by immunohistology, reverse transcription polymerase chain reaction (RT-PCR), Southern blot analyses and flow cytometer. We used M RL/Mp-+/+ (MRL/+) mice without Fas gene abnormality and C57BL/6 (B6) mice as the control. Lung infiltration and inflammation in MRL/ lpr mice progressed according to monthly increase in age. There was a significant increase in mononuclear cell infiltration into the lung perivascular, peribronchial lesions. Using RT-PCR techniques, the expression of IP-10 mRNA was greater in lungs of MRL/ Ipr mice after 4 months of age than in those of MRL/+ and B6. Macrophage-like cells are likely important sources of IP-10 determined by immunohistology. Furthermore, flow cytometric analyses as well as immunohistology revealed expressed CXCR3 increased infiltrating CD4 positive T lymphocytes in 4 and 6 months of age. Expressed CXCR3 also increased infiltrating CD8 positive T lymphocytes in 4 and 6 months of age. Our present results suggest that IP-10 expression in the lung likely plays a crucial role in lung inflammation associated with Thl cells through CXCR3.

    We report on a 59-year-old male who developed skin cancer many years after receiving radiation therapy and carbon dioxide snow-freezing therapy. About 50 years ago, the patient underwent radiation therapy and carbon dioxide snow-freezing therapy for nevus of the Ota of the left buccal region, the orbital region, and the forehead (fractional dose and total dose unknown) . About 6 months prior to evaluation at this hospital, the patient noted tumor growth on the skin in the areas previously treated, and protruding lesions were observed on the left buccal region, the orbital region, and the forehead. Histopathological examination showed squmous cell carcinoma. At surgery, protruding lesions and scar tissue were completely excised, and split-thickness skin graft was used in the areas of skin loss. After 2 years of follow-up, no local recurrence or metastasis has been observed.
