Food allergic diseases causing damage to many human organs are increasing dramatically. Therefore the management of food allergres in clinical practice is needed. We studied the effect of Hochu-Ekki-To on liver and small intestine damage in a mouse model of food allergy. First, NC/Jic strain mice with high levels of serum IgE were sensitized with ovalbumin (OVA), and the mice were treated with Hochu-Ekki-To or saline. We organized the mice into 4 groups : Group 1 and 2 were sensitized and treated with low dose (300 mg/kg/day) and high dose (1000 mg/kg/day) Hochu-Ekki-To, respectively, Group 3 was sensitized and treated with saline and Group 4 was nonsensitized. Second, mouse serum, liver and small intestinal samples were collected. The levels of serum ALT were detected using spectrometry. The number of multinucleated hepatocytes was counted, and hepatic nuclear size and small intestinal villous edema were measured using an image analyzer. Immmunohistochemical staining for IL-4, IL-6, TNF-α and CD4 detection were performed. The experimental results showed that treatment with Hochu-Ekki-To significantly reduced serum ALT levels, and significantly inhibited food allergy induced hepatocyte degeneration in terms of multinucleation and nucleus size, and villous edema in the small intestine, compared to sensitized mice using saline. In addition, cytokine expressions of IL-4, IL-6, TNF-α and CD4 decreased significantly in mice treated with Hochu-Ekki-To. Consequently, we can conclude that Hochu-Ekki-To improved liver and small intestine damage caused by food allergies in mice.

Cell proliferation, cell adhesion and tumor suppressor gene for actinic keratosis (AK), keratoacanthoma (KA), Bowen disease and squamous cell carcinoma (SCC) were studied. The expressions of Ki-67 and p53 were significantly different between AK and Bowen disease and between KA and SCC. The expressions of E-cadherin was also significantly different not only between AK and Bowen disease and between KA and SCC, but also between Bowen disease and SCC. These results appear to indicate that when evaluating malignancy, knowledge of Ki-67 and p53 expressions is useful for differential diagnosis between AK and Bowen disease and between KA and SCC. In addition, evaluation of E-cadherin expression appears useful for differential diagnosis not only of AK versus Bowen disease and KA versus SCC, but also of Bowen disease versus SCC.

A total of 66 patients with primary AML (40 men, 26 women, Median age 50 years) and 29 patients whose disease subsequently relapsed (15 men, 14 women, Median age 53 years) were analyzed retrospectively. The patients were all treated in our hospital with the same chemotherapy regimen between March 1987 and February 2000. The remission rate of 87.8% was obtained in patients with fresh disease, whereas the rate was 51.7% in patients with relapsed disease. The respective median remission durations were 16 and 7 months, and the median survival times were 22 and 10 months. Remission duration was associated significantly with WBC count and number of post-induction chemotherapy courses in patients with fresh disease. Longer overall survival was related to sex, WBC count, and number of post-induction courses in fresh cases. Multivariate analysis in relapsed cases revealed that the remission rate was associated with 1st remission duration. Achievement of the second remission was the sole significant factor for survival in relapsed cases. Based on these observations it was concluded that considerations on prognostic factors at the start of initial treatment would help in planning better total treatment strategies including stem cell transplantation.

The 26 patients who were diagnosed with sleep apnea syndrome (SAS) at Fujigaoka Hospital of Showa Univ., department of snore outpatients were treated with a template, a type of oral appliance for temporomandibular joint dysfunction (TMD) . Evaluation of the template therapy was carried out by apnomonitor inspection such as apnea index (AI), desaturation index (DI), AI + DI, lowest saturation pulse O
2 (lowest SpO
2) and average saturation pulse O
2 (mean SpO
2) . The overjet and the overbite were measured as the dental occlusal condition. Whole indexes from the apnomonitor were improved by wearing the template. The mean overbite was 2.7 mm and the mean overjet was 3.9 mm (normal length : 2.4 mm) . A wider overjet results in a backward intrusion of the lower jaw making the oral cavity narrower. The template widers the oral cavity the same as a UPPP operation. From these clinical findings it is thought that the template is effective for SAS patients.

Anterolateral thigh perforator (ALT p) flaps were used in 3 patients who required reconstruction of the upper or lower extremities. Case 1 was a 34-year-old man who had a skin defect with exposure of the tendon and tendon defects on the dorsum of the right hand caused by a traffic accident. Since the patient required future tendon grafts, a thin ALTp flap was selected. Case 2 was a 42-year-old man who had an open fracture of the distal tibia and fibula. After an open reduction and screw fixation, the patient had a skin and soft tissue defect with exposure of the fracture and screw. A thin ALT p flap was used in the reconstruction. Case 3 was a 37-year-old man who had a skin defect on the dorsum caused by a degloving injury of the right hand. Although an ALT p flap was selected, a flow-through anastomosis was performed because the distal area and fingers require stable blood circulation. The flaps remained intact after surgery and had no postoperative complications. At present, more than 6 months after surgery, no problems have occurred with the flaps, and circulation to the distal area is good. Hence, we conclude that an ALT p flap is useful in reconstruction as a thin flap and as a flow-through free flap.

Solitary tumors of the ribs are relatively rare. In this study, we report a patient with a solitary benign tumor of the 9th rib. A 30-year-old female consulted our hospital with right thoracic pain without etiological factors. At the 9th rib, pressure pain was noted and was exacerbated during movement. Plain radiography showed swelling of the right 9th rib and a clear-cut spindle-shaped transparent bone area. CT revealed a high-density phyma at the site. Contrast-enhanced MRI revealed a high signal intensity area with a relatively clear border on T2-weighted images. On T1-weighted images, a low signal intensity area was detected. Bone scintigraphy and gallium scintigraphy showed solitary abnormal accumulation in the right 9th rib, and tumorectomy was performed. Histopathological findings did not suggest malignancy, and fibrous dysplasia was diagnosed. There have been no abnormalities such as relapse for 7 months after surgery.
Tumors that develop in the ribs comprise only 2.4% of all bone tumors. In these tumors, the incidence of fibrous dysplasia is 26%, making it highest among rib tumors. Metastatic bone tumors comprise the second highest percentage (18.6%), followed by osteochondroma (11.6%) . Fibrous dysplasia frequently develops in patients 10 to 19 years old, without gender difference. Monostotic fibrous dysplasia is observed in most patients. On plain radiography, frosted glass-like bone lesions are characteristic of fibrous dysplasia. The tibia shows the highest incidence, followed by the femur, rib, and humerus. In our department, we previously encountered a patient with eosinophilic granuloma and a patient with osteoid osteoma. These rib lesions are very rare. In this study, we compare the present patient with these two patients, with a review of the literature.

62 years old female had suffered from plantar epidermal cysts for over 10 years. The first clinical examination showed multicystic pearl-like cysts and keratinization extending to the sole. An adequate wide excision and histopathological studies indicated squamous cell carcinoma. A secondary operation was performed to excise partially the calcaneal bone and perform reconstruction with a medial plantar flap. No disfunction and no local recurrence or metastasis have been noted since the patient's surgery two and a half years ago. This success is, we believe, due to quick and accurate diagnosis and appropriate surgery which include various reconstructions.
