    The purpose of this study was to clarify the relation between muscle fatigue and muscle hardness. Using a muscle hardness meter, we measured changes in muscle hardness of the extensor carpi radialis longus before and after wrist dorsiflexion- palmar flexion in 10 subjects. The wrist joint dorsiflexion- palmar flexion movement was done at a frequency of two times per second, 160 times with a rest of 15 minutes followed by measurent of muscle hardness on the muscle belly of the extensor carpi radialis longus. Muscle hardness rose significantly after exercise loading. The results prove that muscle fatigue affects changes in muscle hardness. The muscle hardness meter was easy to use and is useful as an indicator of muscle fatigue.

    Uremic patients often present immune defects, which may have resulted from both uremia itself and dialysis. We analyzed the expressions of CD3, CD19, CD4, CD8, CD56, CD28, and CD80 on the surface of peripheral blood mononuclear cells (PBMCs) by flow cytometry, and interferon ( IFN ) -γ, interleukin (IL) -4, IL-10, and IL-12 mRNA levels on PBMCs using quantitative real-time RT-PCR in 14 chronic renal failure patients before the initiation of HD (nondialyzed: ND group) and after the patients were treated with hemodialysis (HD) for 1 year (HD group) . Sixteen healthy individuals were studied as normal controls (NC group) . The number of lymphocytes, both CD3+ T cells and CD19+B cells decreased in the ND and HD groups, with a decrease in the number of CD4+T cells. The numbers of CD3-CD56+ natural killer (NK) cells were identical in all three groups. HD normalized the CD28 expression level on T cells, which was low in ND patients, while HD diminished the CD80 expression level on B cells. The mRNA levels of the four cytokines were comparable between the ND and NC groups ; however, the HD group showed a decreased expression level of IL-4 mRNA and increased expression levels of IL-12 and IL-10 mRNAs although the expression level of IFN-γ mRNA did not change. Moreover, an increase in the IL-10 mRNA expression level was notable. Conclusions. In ND patients, helper T and B lymphopenia and a deficiency in the CD28 costimulatory molecule may play critical roles in the development of immune defects. HD normalized the expression level of CD28 but altered the expression levels of cytokines, particularly that of IL-10 mRNA.

    We report two cases of retroperitoneal fibrosis. 1: A 78-years-old man was admitted to Totsuka Kyouritsu Daiichi Hospital with edema in the lower extremeties. On admission, bilateral pleural effusion was revealed by chest radiography and the palient's renal function had severely deteriorated (serum creatinine level: 9.9 mg/dl) . An abdominal computed tomographic scan showed the presence of a mass surrounding both the ureters and also bilateral hydronephrosis. 2: A 75-years-old man was admitted to Totsuka Kyoritsu Daiichi Hospital because of appetite loss and edema of the legs. The patient had undergone a surgical operation for an abdominal aortic aneurysm one year prior to admission. On admission, an abdominal computed tomographic scan showed the presence of a mass in the circumference of both the ureters. The serum creatinine level was 6.6 mg /dl. The diagnosis for both cases was retroperitoneal fibrosis, and prednisolone was initiated at the dose of 30 mg/day. Following treatment, the severity of fibrosis of the two cases decreased and the serum creatinine levels decreased to normal range. It has been reported that retroperitoneal fibrosis generally responds to high doses of corticosteroids. However, the two cases described in this paper responded to low doses of corticosteroids suggesting that even low doses for the initial treatment of retroperitoneal fibrosis should be considered.
