  • —Some Novel Bioactive Neuropeptides on Feeding Regulation—

    Novel neuropeptides of G-protein coupled receptor (GPCR) ligands are shown to localize in the brain and perform a range of physiological functions including feeding regulation. Here, we will describe the distribution and localization of these neuropeptides identified very recently and to examine their involvement in neuronal networks, particularly feeding regulation. This review concerns some novel GPCR ligands of feeding-regulation factors such as orexin, ghrelin and galanin-like peptide (GALP), or those described by our research group and others, and neuronal interactions among these neuropeptides in the hypothalamus. Cross-talk among several of these neuropeptide-containing neuron types in the hypothalamus plays a role in determining feeding states. We show structural and functional characteristics of novel neuropeptides and summarize the known interactions between several of these neuron types and leptin-targeting neurons in the hypothalamus. Research in this field will serve an important role in clarifying neurologically-based causes for appetite dysfunctions and help in establishing therapies for people suffering from such conditions.

    We investigated that the usefulness of DaniScan® for control of the environment in bronchial asthma patients attending our hospital. In October 2002, we had nine patients (5 men and 4 women; mean age 42.3 years old) with a positive RAST reaction to mites. We investigated the relation between the DaniScan® score, RAST score, HRT score, and exacerbation of asthma. Subsequently, we gave pamphlets to the patients regarding control of the environment. In October 2004, we carried out a clinical investigation of six of these patients for a change of symptoms, therapy for asthma, RAST score, HRT score, and DaniScan® score. Using Fisher official approval and Wilcoxon official approval, p<0.05 was estimated as those significant with a correlation with a correlation coefficient estimated as p<0.10 for the analysis between 2 groups. The patients of positive DaniScan® had a high RAST score and HRT score. It was thought that with the high score (score 3-4) of, DaniScan® was related to the expression of asthma symptoms. DaniScan® could easily and accurately measure the degree of mite allergen revelation in the house and proved to be useful for control environment. The decrease of RAST and HRT scores as a result of control of the environment suggested exacerbation of asthma. However, because only a few cases were considered in this study, DaniScan® is yet to be determined effective in control of the environment. More studies are needed.

    Several reports have indicated cervical cancer, commonly squamous cell carcinoma, arising from cervical intraepithelial neoplasm (CIN) and related to human papillomavirus (HPV) infection. About 30 types of HPV among 100 types which exist in the natural environment cause human genital organ infection. Several studies have established a strong relation between infection with the high-risk types of HPV and the development of cervical cancer and its precursors. In the current follow-up protocol, patients with cytological abnormalities are uniformly examined with periodical cytology in order to find progressive change. Introduction of testing for the high-risk types of HPV may keep to determine individual follow-up schedules. We investigated HPV-DNA by means of PCR using cytology specimens and in situ hybridization (ISH) of biopsy specimens of abnormal cytology patients. In 30 patients with abnormal cytology from class-III to class-IV, 29 patients were infected with HPV and 25 were high-risk types. Out of three patients in which ISH was performed, two patients showed positive staining in the nuclei of epithelial cells. Our results showed a high positive rate for infected HPV, which is similar to past reports. Testing of HPV-DNA for abnormal cytology cases can individualize the risk of patients and should be involved in follow-up protocol.

    We analyzed the relation between blood pressure levels and lifestyle factors by logistic regression analysis. These data were collected through a database operated by the Health Information Systems Research Association. The results of the blood pressure levels and data concerning lifestyle factors were collected by questionnaire. Unhealthy lifestyles were reflected in higher lifestyle scores. The results indicated that obesity scores had a significantly positive relation to blood pressure levels in all age groups of males and in females 40-59 years of age. In addition, perceived health scores showed a significantly positive relation in male groups under 40 and over 60 and in females over 50 years of age. Stress scores had a negative relation in females over 60 while smoking scores were negative in males 40-49 and over 60. The exercise score had a positive relation in the male group under 40. Obesity is a well-known factor of increasing blood pressure, but we showed that perceived health significantly relates to increases in blood pressure. Many lifestyle factors such as obesity, food intake, stress, alcohol consumption, smoking, and exercise showed significant differences in perceived health groups classified by sex and age as indicated by one-way ANOVA. Our results suggest that improving lifestyles might help to improve perceived health, as well as reduce blood pressure.

    Two cases with persistent inflammatory symptoms involving the cheek were treated. Signs included trismus and refractory fistula due to a chip of wood as a traumatic foreign body in the masseter muscle. Case 1 could be diagnosed by CT, while case 2 could be diagnosed by both CT and ultrasonography. Following removal of the foreign body, symptoms improved in both cases. X-rays were unable to detect the wooden foreign body. The early stage of diagnosis is important to prevent persistent inflammation as a result of an inaccurate initial diagnosis. Ultrasonography, CT and MRI are useful for diagnosing a wooden foreign body. A traumatic foreign body in the masseter muscle is rare but can be encountered in clinical practice. Therefore, it is necessary to consider a foreign body when a patient shows signs of persistent inflammation of a cheek such as lockjaw, refractory fistula, and abscess.

    A 88-year-old man, who had bloody stool on 26th August 2004, was admitted to a nearest hospital. He had massive bloody stool and per-shock state on 4th September 2004, was sent to our hospital. Angiography examination was suggested blooding area in cecum. It is difficult that he was performed because his general condition was very severe, so that IVR was performed to embolization of the blooding area. He escaped shock state and performed ileocecal resection. He recovered himself, discharge our hospital. He was well, in this time. Embolization of IVR to put the waiting surgery on the mind seemed to take effective method for the senior case that had become bleeding shock by the gastrointestinal bleeding.
