    The majority of diabetes mellitus in Japan are non-insulin dependent diabetes mellitus ( NIDDM) .
    The prognosis of NIDDM is largely determined by macroangiopathy and microangiopathy. The new diagnostic level was proposed by a Working Group of the American Diabetes Association.
    A significant proportion of people with IGT also have risk factors that increase their likelihood of developing coronary heart disease and many will develop diabetes.
    Insulin resistance and compensatory hyperinsulinemia are strongly related to metabolic risk factors for coronary heart disease and the clustering syndrome of multiple risk factors is called syndrome X.
    In the pathogenesis of NIDDM, The two fundamental defects which are insulin resistance and glucose desensitization of B cell are caused by a combination of genetic and environmental factors that lead to the progression from normal glucose tolerance to diabetes.
    At present, a few genes were proven potential for causing some form of NIDDM, but the gene or genes causing the common NIDDM remained unknown.
    In the treatment of NIDDM, current recommendations based on results of DCCT emphasize the aim of achieving HbAlc values of <7.0 % and action should be taken when levels are >8.0 %. Hyperglycemia-diabetic microangiopatly hypothesis was proven one and for all. Now, new orally active hypoglycemic agents (α-glucosidase inhibitor and insulin sensitizer) are frequently used to help manage the glucose intolerance of NIDDM. A non-sulfonylurea, hypoglycemic agent which evoke a more rapid and short lived insulin release than sulfonylurea hypoglycemic agent will be able to use to improve post prandial hyperglycemia. We have to select the medicine according to the clinical course of NIDDM.

  • —A comparison between Pancuronium and Vecuronium—

    Effects of pancuronium bromide and vecuronium bromide used during induction of anesthesia on cardiovascular response were studied in patients undergoing valve replacement surgery. Anesthesia was induced with fentanyl, diazepam, and either pancuronium bromide (PCB group) or vecuronium bromide (VCB group) administered in a dose of 0.2 mg/kg. Control values were measured before induction of anesthesia. Cardiovascular variables were measured 3 and 10 minutes after tracheal intubation. Heart rate and systemic arterial pressure after tracheal intubation in the PCB group were not significantly different from control values ; however, in the VCB group, both parameters decreased significantly after tracheal intubation. Cardiac index, systemic vascular resistance, and rate-pressure products decreased significantly owing to the induction of anesthesia in both groups. Only heart rate measurements differed significantly between the two groups. The findings in the present study suggest that with regard to heart rate pancuronium bromide is more advantageous than is vecuronium bromide for use as a muscle relaxant during induction of fentanyl anesthesia for valve replacement surgery.

    The total number of new patients who visited our department during the past 12 years was 91, 035 or an average of 7, 586.3 per year.
    The total numbers of male and female patients ware similar. However more male patients tend to visit than do female patients until the age of 30, and among patients aged 40 years and older, female patients outnumber male patients. This tendency was always seen in the past 12 years.
    Traumatic injury was most common ailment classification, followed by spinal and inflammatory diseases.
    The total number of inpatients during the past 12 years was 7, 176 or an average of 598 per year. Traumatic injury was the most common ailment classification. The difference between outpatients statistics was degenerative joint disease is forrowing.

    The development of avascular necrosis of the femoral head is associated with high-dose cortisone therapy. To study the effect of high-dose cortisone therapy on blood supply of the femoral head, the diaphysis of the femur, tibia, kidney, and liver, microangiography was performed in 24 steroid-treated adult Japanese white rabbits. Weekly doses of 2.5 mg/kg of methylprednisolone acetate were given. Rabbits were killed after 2 and 4 days, and 1, 2, 4 and 6 weeks. Vascular changes were observed on X-ray films after microangiography and decalcification of bone. Five rabbits of the control group received no drugs.
    The nutrient arteries of the femoral head, femur and tibia were not stained extensively from proximal thick vessels to distal small vessels of capillary size at 2 to 4 weeks. Formation of numerous small arteries was seen at that time. At 6 weeks, nutrient arteries had formed again, and the vascular pattern was normalized. These findings were also observed in the kidney and liver. These results suggest that the blood supply by nutrient arteries was impaired owing to the effect of high-dose cortisone. Similar changes were observed in the kidney and liver. Vascular impairment probably occurs systemically and might produce bone ischemic changes leading to avascular necrosis.

    We generated questionnaire tables for quality of life (QOL) in patients with Parkinson's disease (PD) and cerebrovascular disease (CVD) to investigate in what aspects their QOL is low, especially in elderly patients with these two neurologic diseases. The questionnaire tables comprised background factors, physical health, functional health, psychologic health, and social health. The background factors included past history, number of family members (including patient), presence of PD or CVD, degree of disability, ability to walk and communicate, cognitive function, knowledge about social care system, will, satisfaction with rehabilitation and others.
    Fifteen items were generated in each category of physical, functional, psychologic and social health for a total of 60 items. Although we included several disease-specific questions in the area of physical health, we tried to include as many common questions between PD and CVD as possible. Each of the 15 items in these four categories were classified into three ranks.
    By using these tables, we would like to study in what aspects the QOL is low in elderly patients with PD and CVD and to investigate the effect of rehabilitation on QOL in these elderly patients.

    We treated three patients with ruptured aneurysms protruding from the dorsal wall of the internal carotid artery in 1994. This type of aneurysm is rare but has a high risk of introoperative aneurysmal rupture. The problems in diagnosing and treating such aneurysms are disscussed. An acute stage operation was performed in each of the three patients. One problem in diagnosis is an absence of angiographic findings. A video disc recorder and digital subtraction angiography were useful in diagnosing this type of aneurysm. However, preoperative rerupture occurred in two patients with high-grade surgical risks. When the brain is markedly swollen, minimal cerebrotomy might allow the parent artery and the aneurysm to be safely approached during surgery. Special attention must be paid to clip application. We believe that dissection of the arachnoidal adhesion from the aneurysmal dome should be avoided.
