    Among 397 patients undergoing surgery for breast cancer, 226 patients were carried out hypotensive anesthesia administering through dripping intravenous infusion of a 0.1-0.05% solution of trimethaphan (Arfonad®) in order to investigate the safety and utility of hypotensive anesthesia. In this report, the bleeding volume and urine volume during the operation under hypotensive anesthesia, and the effect on operation time and anesthesia time were studied. In all cases general anesthesia was conducted by a modified NLA in order to regulated the systolic blood pressure between 80 and 100mmHg. The bleeding volume during the operation was 114.10±79.30ml (mean±S.D.) in the hypotensive anesthesia group and 627.54±234.79ml in the non-hypotesive anesthesia group. Thus a clear and marked reduction in bleeding volume was observed in the hypotensive group. The urine volume during the operation was 1.91±1.09ml/min. in the hypotensive group and 2.77±1.61ml/min. in the nonhypotensive group. Thus the reduction in urine volume was clearly observed in the hypotensive group, resulting no abnormality as oliguria or anuria. The operation time was 130.26±29.2 min. in the hypotensive group and 142.18±32.43min. in the non-hypotensive group, so that a curtailment of operation time was markedly observed in the hypotensive group. In the hypotensive anesthesia group a tendency was observed for a shortening of the anesthesia time, but there was no significant difference as the operation time showed. In the hypotensive group no patient required blood transfusion during the operation, so that there was no danger of hepatitis dut to blood transfusion. Based on the above results, it can be said that hypotensive anesthesia, if adequately regulate the hypotensive condition, is an excellent method for as a countermeasure in preventing the loss of blood during an operation.

    11 cases of relatively unocommon benign disease, which showed lymph node swelling and skin tumor were investigated by ANAE-staining to estimate the changes of T-cells and plasma cell series in the lesion's imprints. A case of mal-absorption syndrome had a high percentage of T-cell in mesenteric lymph node, but it suggested the relative increasing sign according to B cell depletion. A case of eosinophilic granuloma was shown a slight T-cell increasing in cervical lymph node. 2 cases of Castleman-s lymphoma, hemangioma cavernosum, foreign body granuloma, state after lymphangiography, PVP deposit and pseudolymphoma were obtained no significant findings. Systemic diseases, amyloidosis and mast cell reticulosis, were revealed slight decreasing tendency of both T-cell and plasma cell series. There are several reports that the transition from eosinophilic granuloma, foreign body granuloma and amyloidosis to malignant lymphoma is occurred. We must be kept in mind, under certain circumstance, especially at local long-standing immune reaction foci, that it may happen malignant change of lymphocyte population. For prevention it, early diagnosis should be done by using not only routine histological examination but cytochemical study and immunological analysis.

    Three diseases which show complicated histological pictures, Hodgkin's disease of mixed cellularity (19 cases), lymphoplasmacytoid lymphoma of pleomorphic type (5 cases) and immunoblastic lymphadenopathy (11 cases), were investigated by ANAE-staining to estimate T-cell and plasma cell series, moreover, to find out some changes for differential diagnosis. Two quantitative differences were obtained. 1) T-cell rest population of immunoblastic lymphadenopathy was relatively fewer than those of two other diseases. 2) Plasma cell series of Hodgkin's disease was shown considerably decrease comparing with the other two. As mentioned above, though two difference were detected, those percentages were dispersed wide distribution from case to case. We could not observe an absolute diagnostic indicator. Combined methods may be needed such complicated histological diseases to differentiate each other.

    We reported here the difference of T-cell rest population between 11 cases of immunoblastic lymphadenopathy (IBL) and 18 cases of chronic nonspecific lymphadenitis with predominant infiltrate of plasma cell series (CNL-pl) by using ANAE staining of lymph node imprints. The both diseases were shown almost the same mean values about plasma cell series, but the T-cell rest population of IBL was indicated a significant decreasing by statistic analysis (p 0.1) . Decreasing of ANAE activities is reported in various neoplastic process and suppressor T-cell predominant states. The report here may be suggested a phase of neoplastic finding of IBL.

    We reported here the results about the difference of ANAE activity between good prognostic group (LP & NS) and poor prognostic group (MC & LD) of Hodgkin's disease. ANAE stains were done using lymph node imprints. There have been many reports that the diminution of ANAE droplet reaction products is occurred in the process of blastic transition of lymphocytes. ANAE droplet positive cells were better preserved in LP & NS than those in MC & LD. ANAE granular positive cells were almost the same mean values in the both groups. It may be suggested in this report that the neoplastic process from lymphocyte to lymphoblast in good prognostic group is not so prominent as that in poor prognostic group.     We reported here ANAE activities of lymphocytes in peripheral blood of aleukemic malignant lymphomas. The peripheral blood of 26 cases of chronic nonspecific lymphadenitis were compared with that of various types of malignant lymphoma. 20 cases of Hodgkin's disease and 2 cases of lymphoblastic lymphoma of T-cell type showed no remarkable quantitative difference of ANAE activities, both droplet positive and granular positive cells, compared with the lymphadenitis. On the other hand, immunoblastic lymphomas, immunocytomas and several cases of malignant lymphoma of FCC origin, which are constantly or often accompanied the differentiation to plasma cell, were shown relative increase of plasmacytoid lymphocytes in peripheral blood. Tendency of ANAE activity-diminution was seen in T-zone lymphoma, CLL and Burkitt lymphoma. In general, such as Hodgkin's disease which shown the proliferation of various cell components may be not so correlated the lymphocyte's change between lymph node focus and peripheral blood picture, whereas malignant lymphomas shown monotonous neoplastic proliferation is suggested to reflex the foci, in even aleukemic states, to peripheral blood in some degree.

  • —OBSERVATION OF THE FEMORAL HEAD DEFORMATION RELATED TO VARIOUS TREATMENT AND OBSERVATION OF EXCISED LIGAMENTUM TERES—

    During the past 12 years, studies on clinical and surgical findings in CDH and on excised ligamentum teres examined by means of light microscope (LM) and scanning electron microscope (SEM) were performed on 63 hip-joints of 60 patients with surgically treated CDH. The following clinical results were obtained: many of the patients brought to us were too old (mean age 12.3 months) ; almost 80% of the acetabulum of patients received operations were of the flat-sharp type or the protruding type; a reratively high portion of cases (24%) needed adjustment operation because a centripetal position could not be achievd by open reduction alone. With respect to the relationship between previous treatments and deformation of the femoral head, some deformation, in which ligamentum teres seemed to be concerned, were observed in three groups, two treated with Pavlik harness (Riemenbügel: RB) and overhead traction (OHT), and one group which had received no treatment. Other groups treated with plaster fixation, etc., Histologically, the ligamentum teres could be classified into three parts; zone I (proximity of the apex), zone II (intermediary of the ligament), and zone III (base of the ligament) . In zone I, histopathologic changes were observed mainly in chondroid tissues, in zone II, in connective tissues and in zone III, in vascular system. The age and various treatments caused the different histological changes in the ligament, which were chiefly observed in connective tissues with vascular changes throughout all zones or in some parts of a zone, or with no vascular changes. SEM revealed wavelike structures of the ligamentum teres, showing suitable structures for joint movement. Histologically degenerating ligaments had no fibrous structures, indicating that these changes could not be thought of as reversible, and that they might affect cartilaginous tissues on the femoral head during early developing processes. Accordingly, the RB method and OHT are preferable for treating CDH, because they affect less the ligaments.

    In the morphological analysis of the functional developement of upper extremity, the myofibrous organization of upper extremity muscles in man was studied and the results were compared with those of monkey. The muscles were dissected from the right arm of the adult human (male, 55 years, 60kg.) and the specimen were embedded in celloidin. The transverse sections were stained with hematoxylin and eosin and the calculation and measurement of the muscle fibers were applied the method which had performed in monkey previously. The following results were obtained; 1) In the upper arm muscles, Triceps brachii was developed remarkably and was followed by Brachialis in man. In the each head of Triceps brachii, concerning the cross-sectional area, total number of muscle fibers and mean size of muscle fibers, the predominant order of three heads was caput longum, madiale and laterale, caput mediale, laterale and longum and caput longum, laterale and mediale, respectively. Relatively, the caput mediale was most developed and the mean size of muscle fibers in the caput longum was the thickest in upper extremity muscles. On the other hand, the developement of Biceps brachii was following to Triceps brachii which was the most developed in monkey. In Triceps brachii, the caput longum was the most predominant concerning the cross-sectional area and total number of muscle fibers. 2) In the forearm muscles, concerning the muscle weight, cross-sectional area and total number of muscle fibers, Flexor digitorum profundus and Flexor digitorum superficialis were following to upper arm muscles and the mean size of muscle fibers was belonged to the relatively small group and large group in upper extremity muscles, respectively. Relatively the former tended to develope in monkey and the latter in man. 3) In Supinator and Brachioradialis, concerning the cross-sectional area and total number of muscle fibers, the former was equal to those of Flexor digitorum superficialis and tended to develope in man. The latter was predominant relatively in monkey than in man. 4) Concerning the extensor muscles of forearm, the mean size of muscle fibers was larger in man and these muscles tended to develope remarkably in man than in monkey. 5) It was considered that the facts described above were related closely with the characteristic of upper extremity movements in man and monkey.

    Highly purified interferon-α and -β (IFN-α, -β) are ineffective in induction of differentiation of human myeloid leukemia cell lines, ML-1 and HL-60. On the other hand highly purified natural interferon-γ (IFN-γ) induced differentiation having characteristics that are associated with the human promyelocytic leukemia cell line, HL-60. Monoclonal antibody to IFN-γ neutralized its activity. However, the natural IFN-γhad almost no inducing activity in ML-1, a human myeloblastic leukemia cell line. Similar results were obtained using recombinant IFN-γ. Mitogen stimulated human leukocyte conditioned medium (LCM) induced differentiation of both ML-1 and HL-60 cells. After treatment of LCM with monoclonal antibody to IFN-γ, LCM activity was reduced more than 50% in ML-1 cells, and 80% in HL-60 cells. Even if IFN-γ was eliminated from LCM by affinity chromatography, the LCM induced differentiation of ML-1 and HL-60 cells, but IFN-γ markedly enhanced the ML-1 cell differentiation induced by IFN-γ free LCM. The results suggest that leukocytes produce differentiation inducing factor (s) other than IFN-γ, and that IFN-γ is both an inducer and an enhancer of induction of human myelogenous leukemia cells.

    It has been common procedure for surgens to use the mucoperiosteum flap of the vomer when the cleft palate is closed. However, there have been some problems of the facial growth disturbance after injuring to the vomer area, also. Then I tried some experiments by means of the 186 infant Wister male rats in order to find out the consequence by operating to the vomer region. I divided into four groups. Group A: the only periosteum removal in a half posterior vomer region. Group B: the vomer removal lefting periosteum intact in the same region. Group C: both vomer and periosteum removal in the same region. Group D: injuries to a half anterior vomer region. Dry skull bones were made for measurement in one, two, three months after operation. Sagittal deviation and anterior growth disturbance rate were evaluated statistically. The biggest sagittal deviation rate were seen in group A and C. But its inclination was not so much. There were no significant differences among all groups in anterior length rate. These results were pointed out, at the same time, that the rats were not suitable as a experimental material in this study.

    Observation were carried out on the cross-sectional areas of the subcutaneous fatty layer, muscular layers, osseous tissue layers and abdominal cavity in the lower abdominal plane, and of the subcutaneous fatty layer thickness, using abstract X-ray computed tomography. The changes due to sex and age were investigated. The subjects of the study comprised of sixty-nine Japanese adults (male: 26; female: 43) and 4 minores (less than 10 years old: 2, teenagers: 2) . The following results were obtained; (1) The cross-sectional area in the adult lower abdomen increased with age until a certain age and it decreased rapidly with old age in male, but there was little decreasing tendency in old age in female. (2) As for the cross-sectional area of the various tissue layers, that of the abdominal cavity was the greatest for both upper and lower planes, and this was followed by the subcutaneous fatty layer, muscular layer, osseous layer, in terms of size, in the upper plane. On the other hand, the cross-sectional area of the abdominal cavity was followed in size by the muscular layer in males and the subcutaneous fatty layer in females in the lower plane. (3) Concerning the age-related changes of the cross-sectional areas of the various tissue layers, it was observed in the elderly subjects that the males showed decreasing tendencies in the cross-sectional areas of the abdominal cavity and subcutaneous fatty layer, while the females showed increasing tendencies in the abdominal cavity but decreasing tendencies in the subcutaneous fatty layer. (4) As for the subcutaneous fatty layer thicknesses in the cross-sectional area, the gluteal line area was the largest and followed by the lumbar area in the upper plane. In contrast to this, in the lower plane, the thickness in the gluteal region was the largest for both males and females, and this was followed by the lumbar line region in males and the lateral line region in females. (5) As for the female minors, the total cross-sectional areas were 70% of that of the adults, and the cross-sectional areas of the respective tissue layers also showed the same proportions under 10 years old. In the teenagers, too, the total cross-sectional areas were considerably smaller compared to the adults, but the cross-sectional areas of the various tissue layers were roughly equal to the adults except for the abdominal cavity area which alone was smaller.

    Semi-Alkaline-Proteinase, Seaprose-S (SAP), produced by Asperigllus mulleus, was investigated anti-inflammation, using Carrgeenan's phlogogenic action to the experimental sciatic perineuritis of rats. Carrageenan (CRG) has been shown dramatic model of pen-and/or neuritis on the sciatic nerve at 0.05% of concentration that increase lymphotic leucocyte (lymphocytosis) arround the nerve tissues, fragility of neurolemma membrane and neurotelangiectodic findings, etc. Results of SAP treatment for anti-CRG effects showed that pre-administered group with 500mg/kg (p. o.) inhibited 70% of 0.05% CRC's phlogogenic action, and SAP treatmented group with antibiotics Cefamezin (CEZ) showed complete inhibition, while the mechanisms of these anti-CRG effects of SAP (and/or SAP+CEZ) have not known yet. Therefore SAP might be expected to have clinical application in the relief of the phlogosis, including neuritis, perineuritis and so on.

    Forty-nine cases with esophageal carcinoma are treated by radiotherapy and are followed with radiological examination from 1979 to 1984. 19 cases of them were evaluated by radiological and endoscopic findings. Japanese Society of Esophageal Diseases indicated the evaluation on esophageal cancer by chemotherapy and radiotherapy. 1. CR (Complete Response) : Disappearance of the primary focus on the barium filled view for a period longer than 4 weeks. 2. PR (Partial Response) : The decrease in the involved areas on the barium filled view after the treatment is more than 50% for a period longer than 4 weeks. 3. MI (Minor Improvement) : The decrease in the involved areas on the barium filled view after the treatment is more than 25% and less than 50% for a period longer than 4 weeks. 4. NC (No Change) : Improvement of the involved areas on the barium filled view after the treatment is no more than 25%. 5. PD (Progressive Disease) : Increase in the involved area. Progression of subjective and objective symptom. The evaluated cases were judged on base of this criteria. There were three CR, eight PR, two MI, six NC and six PD in result evaluated 19 cases. Evaluation of the response of chemotherapy or radiotherapy were performed by both change of radiological and endoscopic findings of esophageal cancer by radiotherapy. The value of the long diameter by short diameter were compared with that of only the long diameter by the criteria for evaluation of the radiological findings (improvement of stenosis) . The values of the long diameter by short diameter were corresponded to that of the long diameter except 3 cases. Therefore, it is considered that evaluation with the criteria should be decided by value of the long diameter by short diameter and change of randwall and concavity on radiological findings.

    We reported a 53-year-old female with unknown fever and polyarthritis, who had a history of breast augmentation with injection of silicone 25 years ago. Laboratory examinations revealed accelerated ESR, leukocytosis, strong positive CRP, positive anti-nuclear antibody (speckled pattern) and biological false positive. But RA test and anti-DNA test were negative. We suspected this disorder as adjuvant disease of man. The removal of silicone injected in bilateral breast resulted in the partial remission of the symptoms and laboratory findings. The administration of the immunosuppressant resulted in almost complete improvement of them. It may be suggested that the injected silicone played a role in pathogenesis in this case.

    Immature teratoma of ovary is a relatively rare tumor and has difficulty to expect its prognosis and histological maturity. Most immature teratomas are classified in the intermediate group of ovarian tumor, but there are a few cases which have much worse prognosis with wide spread metastases. We have experienced three cases of immature teratoma of ovary with disseminating metastases to the great omentum and the peritoneum. We report the clinical and pathological studies of these three cases, especially neural tissues of the primary tumors.
