    Natural biological response modifiers such as lignin, tannin, and protein-bound polysaccharide (PSK) stimulate the production of tumor necrosis factor (TNF), interleukin-1α (IL-1α), and IL-1β by cultured human peripheral blood mononuclear cells. However, these agents induce no detectable amount of these cytokines in cultured human skin fibroblast, stomach cell carcinoma, renal cell carcinoma, glioma, glioblastoma, or myelogenous leukemic cell lines. When human myelogenous leukemic cell lines (ML-1, THP-1, HL-60) were treated with 12-O-tetradecanoylphorbol 13-acetate (TPA), they were differentiated into adherent macrophages expressing higher NBT-reducing activity, but only ML-1 cells became IL-1β producing cells. If ML-1 cells were sequentially treated with TPA and then PSK, further increase of IL-1β production was observed. When HL-60 cells were treated with vitamin Ds, another monocyte/macrophage inducer, their NBT-reducing activity was significantly increased, without induction of IL-1β production. These data suggest that (i) the types of differentiation-associated properties expressed might be determined by the inducers, (ii) the cytokine production might be stimulated via direct action of the inducers or a secondary result of differentiation induction, and (iii) the expression of conventional differentiation markers does not necessarily reflect mature cell functions.

    Between 1980 and 1989, 112 patients had curative resection for solitary carcinoma of the rectum. Of these 112 patients, 12 (10.6 %) who developed local recurrence were clinicopathologically studied. The average interval from operation to recurrence was one year and 11 months, and there was recurrence within 3 years in 83.3 %, The risk of local recurrence increased in cancers of the lower rectum (Rb), tumor diameter of 4.1 cm or larger, type 3 (infiltrating and ulcerating type), invasion depth of a2 (s) -ai (si), and lymph node metastasis, including carcinoma infiltration (ew) of not more than 4 mm at the external surgical surface. Local recurrence rate increased with increase in positive lymph node metastasis. There was recurrence in 9 out of 63 cases (14.3 %) after Miles' operation, and in 3 out of 33 (9.1 %) after anterior resection. This difference was not significant. There was a trend toward decreased survival of patients who developed local recurrence. In operating on rectal cancers, the importance of appropriate lymph node dissection and excision of the carcinoma with sufficient surrounding tissue should be emphasized.

    Incidence of Methicillin resistantStaphylococcus aureus (MRSA) in Showa University Hospital from July 1990 to September 1992 was surveyed.S. aureuswas most frequently isolated, and about 50 % was identified as MRSA. The ratio of MRSA inS. aureusisolated each month was 60-80 % from inpatients, and 15-30 % from outpatients. The incidence of MRSA inS. aureuswas 84% and 75%, respectively, in the specimens of urine and pus from inpatients The incidence of MRSA was 18 % and 19 %, respectively, in specimens of pus, or from the respiratory tracts of outpatients.In vitrocombination effects of arbekacin (ABK) and β-lactams against clinical isolates of MRSA (56 strains), whose minimum inhibitory concentcations were over 4μg/ml, were examined. Synergistic and additive effects were observed in, respectively, 82 % and 11 % by ABK plus ampicillin, in 63 % and 0 % by ABK plus fosfomycin, and in 52 % and 25 % by ABK plus clavulanic acid/ticarcillin. No antagonism was observed. Synergism or addition were observed against the MRSA producing β-lactamase. The ABK dose would be decreased by antibiotics combination.

    Numerous factors contribute to cerebrovascular diseases. Among the more significant of those factors is sexual intercourse. In the past 13 years, 14 cases of cerebrovascular disease have been studied. The cases included 13 male subjects and 1 female. Of these subjects, 8 had intracerebral hemorrhage, and 6 had subarachnoid hemorrhage. Results of the studies suggest that the diseases may have attacked as results of sexual intercourse. A most significant point of interest during the studies was that only 3 cases resulted from sexual intercourse with the subject's spouse. All other cases resulted from sexual relation that were extramarital or with prostitutes. The sexual acts were performed in the lovers's dwelling, a hotel or a brothel. Preliminary studies show evidence that promiscuous or indiscrimimate sexual behavior may be a risk factor leading to cerebrovascular disease. More cases must be exposed and studied to give researchers opportunity to statistically determine the causes and nature of these diseases in their entirety, so instructions and countermeasures for their prevention can ultimately be offered.

    The usefulness of CT-guided percutaneous lung biopsy using an automated biopsy gun (ABG) and an 18-gauge cutting needle (18 G needle) was studied and compared with the use of an ABG and a 20-gauge cutting needle (20 G needle) . Materials consisted of 83 examinations (83 patients, 83 lesions) of CT-guided lung biopsies using an ABG ; 20 G needles were used in 66 of the 83 examinaeions (Group-A), and 18 G needles were used in the other 17 (Group-B) . There were no statistically significant differences between the two groups in terms of biopsy success rate, complication rate, mean examination time, or diagnostic accuracy in the cases of malignant tumors. Biopsied specimens were graded according to both tissue quantity and quality from 0 to 4 ; The mean score in Group-B was superior to that in Group-A (p<0.05) . Accuracy in histological diagnosis tended to be higher in Group-B than in Group-A, especially in the diagnosis of benign lesions. The results suggest that CT-guided percutaneous lung bopsy using an ABG and an 18-gauge cutting needle will be useful when detailed histological investigation is needed.

    We performed isoflurane-induced hypotensive anesthesia for 8 patients, and isoflurane-induced normotensive anesthesia for 8 patients undergoing radical mastectomy. We measured the hemodynamic parameters : systolic arterial pressure, diastolic arterial pressure, and heart rate, plasma catecholamine concentration (epinephrine, norepinephrine), plasma renin-activity, and plasma-aldosterone concentration in each group. All patients were premedicated with 0.5 mg atropine sulfate, and 50 mg hydroxyzine. Anesthesia was induced with thiamylal and the trachea was intubated after vecronium or succinylcholine. Isoflurane was administered until systolic blood pressure decreased to 80-90 mmHg in the hypotensive group. In the normotensive group (control group), a concentration of 1-2 % isoflurane was administered. Hemodynamic data (blood pressure, heart rate) and blood samples for measurement of plasma epinephrine (EP), norepinephrine (NEP), plasma renin activity (PRA) and plasma aldosterone (PA) were collected : 1) before induction (S1), 2) 30 min after starting to operate (S2), 3) 60 min after starting to operate (S3), and 4) 90 min after starting to operate (S4) . Decreases in mean blood pressure below baseline were significantly greater in the hypotensive group than in the control group. Heart rate did not change in the control group, but in the hypotensive group it increased significantly. NEP concentration did not change in the control group but increased significantly above the baseline (S1) in the hypotensive group. NEP concentration (S2·S3) was significantly greater in the hypotensive group than in the control group. AD was significantly above the baseline (S1) in the control group, but did not change in the hypotensive group. PA increased significantly in both groups, and PA (S3) increased significantly more in the hypotensive group than in the control group. The results suggest that isoflurane-induced hypotension preserves the effect of homeostatic response, and this increases stress hormones. The cardiac depressing effect of isoflurane was believed to be less than those of halothane or enflurane, and at higher concentration the direct vasodilation effect of isoflurane predominated. We conclude that isoflurane can be employed safely and effectively as a hypotensive agent.

    A 21-month-old male patient with chronic renal failure and eosinophilic peritonitis is reported. Examination of the patient revealed a retarded growth pattern and a diagnosis of chronic renal failure, the etiology of which, an afebrile convulsion, was established for the first time. Laboratory tests presented evidence of azotemia, hypocalcemia and acidosis. Abnormal findings were also evident in the EEG and ECG results. However, when the serum calcium levels were returned to the normal range, these abnormal findings disappeared. The patient has had no afebrile convulsion since. At age 42 months, continuous ambulatory peritoneal dialysis (CAPD) was initiated due to exacerbation of the renal failure. The patient concomitantly developed bacterial peritonitis and shortly thereafter eosinophilic peritonitis. Examination revealed the presence of eosinophilia in the peritoneal fluid. Administration of antibiotics was discontinued, and this resulted in disapperance of the eosinophilia from the peritoneal fluid.

    A 25-year-old male was admitted to the hospital complaining of epiabdominal pain, vomitting and palpitation for one month after a common cold. Chest X-ray and ultrasonography (US) examination revealed cardiomegaly with pericardial and pleural effusion. Electrocardiogram examination revealed frequent PVC, and laboratory data showed the high antibody titer of cytomegalovirus. In spite of symptomatic therapy, severe cardiomegaly on US developed dilated cardiomyopathy (DCM) on the 3rd hospital day. Cardiac disfunction persisted and he expired in the about 3rd hospital month. Autopsy revealed severe dilated cardiomegaly and, microscopically, diffuse fibrosis with massive granulomatous foci, chiefly on the endocardial side, of the heart. Pathomorphological study suggested that granulomatous myocarditis after cytomegalo-viral infection developed into DCM like lesion, but viral infection was not proved pathologically.

    The patient was a 36-year-old man suffering from fever and right hypochondralgia. The white blood cell count was 25, 900/μl and CRP value was 21.9mg/dl. Abdominal ultrasound examination revealed a large abscess about 120 mm in diameter. Neither cyst nor trophozoite could be found in the stool or the liver pus from the drainage tube. He was homosexual and suspected to have amebic liver abscess. He was treated with metroindazole. Finally, the titer of antibody toEntamoeba histolyticawas 1: 6400 and we made a diagnosis of amebic liver abscess. The patient's condition improved, but the abscess cavity did not disappear until 17 months after onset on CT scan.

    We report a case of W erner's syndrome with diabetes mellitus and hyperlipidemia. The patient was a 54-year old man. Diabetes or impaired glucose tolerance and other metabolic abnormalities are frequently associated with this syndrome. An intravenous glucose tolerance test showed reduced glucose tolerance coupled with low K-value. The urinary C-peptide level was almost normal. The quantification of tissue insulin sensitivity by insulin injection (0.1U/kg regular insulin) showed a decrease in IST (insulin sensitivity index) of 1.47. There was no evidence of increased levels of insulin counterregulatory hormones. No anti-insulin antibodies were detected. The affinity and number of erythrocyte insulin receptors were normal. This reduced insulin sensitivity seems to be due to a defective post recep-or mechanism. This patient had hypertriglyceridemia and hypercholesterolemia. Special attention should be paid to prevention of complications of the several metabolic disorders that are evident in this syndrome.
