    Myocardial protection by oxygenated crystalloid cardioplegia was studied in isolated working rat hearts. After normothermic and hypothermic ischemia, recovery of aortic output was good in rat hearts that were infused with oxygenated crystalloid cardioplegic solution. Under hypothermic ischemia by topical cooling, recovery of aortic output from low oxygenated crystalloid cardioplegia was not good after reperfusion. It was concluded that oxygenated crystalloid cardioplegia is well able to protect the myocardium even under hypothermia, because aerobic metabolism might be maintained in the hypothermic myocardium.

    To evaluate the effect of myocardial protection by oxygenated crystalloid cardioplegia, we investigated the recovery of cardiac function from the beginning of coronary reperfusion to the early postsurgical period after mitral valve replacement. Ten patients were set ected for this study excluding patients who had accompanying aortic valve disease or severe coronary artery disease. Five patients received oxygenated St. Thomas' Hospital solution and five recieved non-oxygenated St. Thomas' Hospital solution. At the beginning of reperfusion, hearts infused with the oxygenated crystalloid cardioplegic solution showed the good recovery of cardiac function, such as time to start the cardiac beat, the need of electric defibrillation, or pacemaker. The clinical evaluation could not be cleared but the five patients who received myocardial protection by using oxygenated cardioplegia showed higher cardiac index than the patients who received non-oxygenated cardioplegia.

    The changes and prediction of the prevalence rate of diabetes mellitus (DM), and the percentage of DM patients receiving therapy (rate of treated patients) during the remaining years until 2000 AD were obtained by exponential regression analysis on the basis of various data accumulated during the most recent 10 or more years. As a result, assuming that recent trends would continue, the DM prevalence rate and rate of treated DM patients were estimated to increase at a considerably faster rate than the corresponding increase rates of all diseases as a function of age-group, the expected rate of increase is especially large 'n the age group over 64 years. It was estimated that in the year 2000 the DM prevalence rate will be 4 times and the rate of treated DM patients will be 2 times their 1985 rates. In the study of formulizing estimation of the prefectural prevalence rate of DM, using Patient Survey results, a new method of estimation of the prevalence rate of DM was developed in which the number of total illnesses was considered. This method was found to be valid. Next, the results of health examination (the DM-positive rate, the urinary glucose-positive rate, the receiving rate) performed in accordance with the Health and Medical Services Law for the Aged and the DM prevalence rate calculated from the Patient Survey were subjected to correlation analyses. The analyses revealed a positive correlation between the positive rates of these parameters and the DM prevalence rate.

    Image contrast defined by the count density of nuclear medicine image is often used to evaluate abnormal accumulation of radioisotope (RI) due to disease. However, the image is not always a true radioactivity distribution of the patient, because of the presence of many factors that degrade the image quality. These are inadequate resolution of the scintillation camera, and absorption and scatter of γ-rays. Therefore, it is important to examine how well the image reflects the object radioactivity distribution. In the present paper, the relation between object contrast, defind by RI distribution, and image contrast, difined by count density, was investigated experimentally. The phantom 1 consists of a cube with low background activity and a sphere or circular disk with high activity. Phantom 2 (cold case) consists of a cube with high activity and a sphere without RI activity. These phamtoms were imaged with a scintillation camera and the image contrast (count density of the sphere or disk/background count density) was calculated. Correlation between the object contrast and image contrast was linear, supporting its use to evalute the abnormal accumulation of RI in a patient. However, on account of the effect of scatter, the absolute value of image contrast was somewhat smaller than the object contrast. It should then be important, considering above results, to use the image contrast in clinical study.

    The cross sectional areas of the trunk musculature observed in CT images at the level of E5 of Erdheim's grid (corresponding to the level of the mid-point of the sternum) were analyzed to elucidate its development and functional significance in situ. The subjects included 99 healthy adults (50 males, 49 females), whose ages ranged from the thirties to the seventies. The shape of the cross section of each muscle was traced on a CT image and the cross sectional area (CSA) was then measured. The data were analyzed against sex, laterality, age and body type. Results. 1. The CSAs of the scapular muscles and the m. latissimus dorsi tended to be larger in males than in females, and in the right than in the left. This suggests that differences in the medial rotation of the upper arm are related to sex and laterality. The CSAs of the m. serratus anterior, m. trapezius and m. rhomboideus, which appeared in fixation of the scapula, tended to be larger in males than in females. 2. The CSAs of the proper back muscles at the level of the mid-point of the sternum were 1/3 those in the lumbar region in males and 1/4 in females. The E5 level proper back muscles were observed to decrease with age of the male earlier than those of the lumbar back muscles. 3. The CSAs of the seventies age group were smallest. Decrease with aging was observed in muscles with phasic function. These tendencies suggested that decrease by aging would appear with the phasic function of the shoulder girdle. 4. Relation between body type and each muscle were also investigated. The CSAs of m. serratus anterior, m, trapezius and m. latissimus dorsi decreased in the order of the D, C and A body type. The CSAs of the m, teres major and m. subscapularis of the D body type tended to be larger than those of other types. These thus correlated with the demand for trunk support functions of the upper limbs because of obesity.

    We measured time related changes of lung functions, plasma substance P, histamine and leucotriene C4 (LTC4) in 18 asthmatic patients and 5 normal subjects following inhalation of ultrasonically nebulized distilled water (USNDWI) and of isotonic (0.9 %) saline to clarify the mechanism of bronchial response after USNDWI. None of the subjects investigated had obvious changes in pulmonary functions due to the isotonic saline. However, while normal subjects had no apparent change after USNDWI, among the asthmatic patients, 9 had an immediate bronchial response (JAR alone group), 5 had immediate and late bronchial responses (dual AR group), and 4 had no brochial response (nonresponder group) . In the dual AR group, plasma substance P tended to increase in JAR and LAR, and plasma histamine and LTC4 tended to increase in JAR. The results suggest that the mechanism of JAR after USNDWI may be due to provocation of a chemical mediator release from mast cells through axonal reflex, and that the development of LAR itself after USNDWI revealed relations between these mechanisms and mast cells.

    The shape and myofibrous organization of the tensor fasciae latae muscle was investigated quantitatively to elucidate functional characteristics of the muscular morphology. The present data were compared with data accumulated in our laboratory. Right tensor fasciae latae muscles were obtained from 27 cadavers for dissecting practice (18 males, 9 females, mean age 68.6 yrs) . After the length, width and thickness of the muscle were measured, H.E. stained preparations were made for measurement of the muscle fiber. Results : 1. The muscle masses and volumes were larger in male than in female. Trends of change due to aging were observed. These trends were remarkable in both males and females older than 60. Cross sectional areas of the muscle belly in the oldest group were also very small. 2. The mean number of the muscle fibers in a unit area (1 mm2) was greater in females (1176) than in males (1012) . These values were smaller than those of the psoas major muscle, and similar to those of the iliacus muscle, but were larger than those of the quadratus lumborum muscle. 3. The mean numbers of total muscle fibers per muscle were 188112 in males and 170531 in females. Trends of change due to aging were observed in males. The trend in males was similar to those of the quadratus lumborum and iliacus muscles, but less than that of the psoas major muscle. The trend in females was similar to that of the psoas major muscle, but less than that of the quadratus lumborum muscle. 4. The mean cross sectional areas of the muscle fibers were 802 μm2 in males and 632 μm2 in females. Derease of thickness due to aging was observed in males. These values were similar to those of the psoas major muscle in both sexes, but smaller than those of the quadratus lumborum and iliacus muscles. 5. The mean density of muscle fiber was larger in males (74.5 %) than in females (65.8 %) . These values were similar to those of the psoas major and quadratus lumborum muscles in both males and females.

    To study the form and functions of the fallopian tubes, ciliary movement was investigated, and scanning electron microscope views during the sexual cycle, pregnancy, postpartum, after castration, and after administration of sexual steroid hormones were examin-ed. 1) Cliliary movement was frequent during proestrus, metestrus, and diestrus, but infrequent during estrus. The form of the ciliated cells did not change. Secretory cells were markedly distended in metestrus, suggesting the effects of estrogen and progesterone. 2) Ciliary movement increased on the 1st day of pregnancy, but decreased on the 2nd and the 3rd days and increased again from the 4th day on. The form of the ciliated cells did not change during pregnancy. Secretory cells were active in the 2nd day and, in cooperation with the decreased ciliary movement, controlled the transport of fertilized ova. Epithelial cells were exhausted by the 20th day of pregnancy, and on the 1st postpartum day the epithelia were devastated. However, both ciliated cells and secretory cells had regenerated by the 5th postpartum day. 3) Three weeks after castration, ciliated cells in the fallopian tubes were short and did not move briskly. The surfaces of the secretory cells were exfoliated, and the microvilli disappeared. Administration of estradiol caused both ciliated cells and secretory cells to regenerate well. Administration of progesterone increased secretory cells, but they appeared wasted.

    Changes in serum free-thyroid hormones after administration of thyroid hormone preparations were studied in 48 patients with primary hypothyroidism. When l-thyroxine (l-T4) was given orally or intravenously, a slight increase was observed in the concentration of serum FT4, but no significant change was found in the concentration of serum T3, or FT3, unless the dose of l-T4 exceeded 150 μg. The concentrations of serum T3 and FT3 were quickly increased to super normal levels by even a relatively small dose of l-triiodothyronine (l-T3) . When more than 50 mg of dessicated thyroid was given orally, the concentrations of serum T3 and FT3 exceeded the normal upper limit. These result indicate that the best and safest medicament among the thyroid hormone preparations for the treatment of hypothyroidism is l-thyroxine, either oral or intravenous.

    A patient's computer for medical information should contain information aimed at effectively managing all data in the patients database. However, it is not cost effective to line up each instrument to scan the various medical data in different media simultaneos-ly, and even if instruments are connected, it requires too much time to scan each data image, CT, Echo, MRI, Endoscope, etc. We have introduced a computer-compatible still video medical image capturing system to resolve these problems. The new system consists of a still video camera and a personal computer with a full-color, video digitizer board and a 32 bit CPU. The still video camera can take a 1/15 to 30 second exposure that is controlled as an NTSC signal, and the image data is highly compatible to these machines. The still video system controls the taking of medical images and the computer controls the capture of image from the vide system. Introduction of the still video system releases us from these problems. There are also other advantages : This system can be used for preoperative conferences, to illustrate explanations for the patient's family after a surgical operation, and to lecture to medical students. The system is also able to capture micro findings on a plate through a CC D camera attached to a microscope and manage these image data captured by the computer as an image database.

    We introduce here three kinds of reconstruction post anterior resection for patients with rectal cancer using an auto stapler machine. PCEEA (Premium Curved EEA Stapler) an improved EEA Stapler has several functions : Two-part shaft assembly, removable trocar, cartridge shield trocar, and others. These new functions allow us to reconstruct the intestinal tracts of patients with rectal cancer in the following ways without colostomy :
    1) End to end anastomosis ;
    2) End to side anastomosis ;
    3) Double Stapling.
    Adoption of a technique depends on the anastomotic level. Double stapling is adopt at the lower level of the technique. Leakage for these technques : Two of 13 cases of end to end anastomosis, three of 9 cases of double stapling, and no case of side to end anastomosis. We concluded that although leakage rate is highest the double stapling method, leakage is also caused by other factors such as blood flow to the retained rectum, fragility of the wall, tension at the anastomosis, and other reasons.

    Although there may be no clinical signs of tumors in patients with cob-rectal cancer, intestinal contents are sometimes blocked on the oral side of the tumor. This could lead to the following consequences during prepreparation of patients with cob-rectal cancer : intestinal flora may be reduced to 0.1-0.001 that in appear patients without cancer, and cancer cells from the tumor may appear in the content. We have found class N positive in 40 % of cytology in irrigation water after washing out the colonal lumen. We have engaged ED or IVH, for one week with no chemical or mechanical preparation of patients with colon cancer, but intestinal contents remained in 20 % of the patients. These contents can sometimes cause postoperative complications. There are two methods of decompression before and during surgical operation for colon cancer, with and without stenosis. One method is rectal prothesis ; a prosthetic instrument placed at the stenosis of the rectum with irrigation through the lumen of the instrument. Irrigation is possible in the whole colon for 3 to 4 days, after which a barium enema is possible to get information from the oral side of stenosis. Another method is irrigation of the whole colon from the oral stump after removing the tumor. As a result of either method of irrigation, remarkably fine operative maneuvers are possible, and we have seen no post operative comoplications. By using either of the two methods, the incidence of anastomotic leakage has been diminished compared operations without irrigation or rectal prosthesis. Thus, for preoprative patients with stenotic rectal cancer, a rectal prosthesis developed us has greatly improved prognosis.

    Changes in the numbers of operative cases in the central operating room of Showa University Hospital from 1987 to 1989 were studied. The total number of operations remained constant, but the number controlled by anesthesiologists increased. Emergency cases tended to be fewer. By age, fewer infants and more aged people received anesthesia. Operations using systemic anesthesia exceeding 4 hours increased. More cases per month, in all departments, received anesthesia in July, March and August, and fewer received anesthesia in January, February and November. Especially, large increases in numbers of cases were noted in operations on patients 6 to 16 years old. Moreover, increases in the Department of Otolaryngology and Department of Plastic Surgery were large, while increases in the Department of Surgery and Department of Orthopedic Surgery were smaller.

    In recent years, it is often stated that preoperative evaluation of the mediastinum should be by imaging techniques such as computed tomographic scanning (CT scan) and magnetic resonance imaging (MRI) which offer high resolution. These are, however, indirect diagnostic techniques for assessing the presence of metastasis to mediastinal lymph nodes. Mediastinoscopy was first described in 1959 by Carlens. We introduced this method of preoperative diagnosis by mediastinoscopy to the selection for operation of patients with lung cancer and other diseases. In the present paper, we discuss three cases with mediastinal disease, two patients with lung cancer and one with esophageal duplication cyst. We conclude that mediastinoscopy with imaging techniques, such as CT scanning and MRI-CT should be obligatory for preoperative evaluation of patients with lung cancer, among other diseases, in whom there is the slightest suspicion of metastasis to mediastinal lymph nodes.

    We report here, two cases of cecal cancer with appendicitis-like symptoms.
    Case one was a 75 year old male. He complained of ileocecal pain with high fever for two days, and his WBC was 21200/mm3. The diagnosis was acute appendicitis. He refused an appendectomy, so he was given chemotherapy for 3 days and his symptoms completely disappeared. Later, a barium enema, as part of a thorough exmamination before his discharge, revealed a focal cancer of the cecum. The cancer was removed by ileocecal resection. In this case there was no pathological connection between the cecal cancer and acute appendicitis.
    Case two was a 78 year old male. The diagnosis was acute appendicitis, and appendectomy was performed. During the operation a tumor was found near the formis appendix. Biopsy indicated pathological adenocarcinoma. Two weeks later, the tumor was removed surgically.
    When a patient complains of acute appendicitis-like symptoms, the following possibilities must also be considered : 1) acute appendicitis due to obstruction of the lumen by a tumor ; 2) acute appendicitis and cecal cancer at the same time ; and 3) cancer may be causing the appendicitis-like symptoms.

    A case of hepatocellular carcinoma with Otsu-type liver cirrhosis is reported. A 61 years old man was admitted our hospital with complaints of general fatigue and weight loss. He had a blood transfusion 31 years ago. Laboratory data showed elevation of hepato-bile enzyme and abdominal CT, US, angiography revealed evidence of hepatocellular carcinoma. Antithrombim III (AT-III) level was high from first admission and increased to 84.62 mg/dl. He died of hepatic failure, but the AT-III level did not decrease. At autopsy, the liver weighed 4340 g. The surface of the liver was rough, and cut surfaces showed diffuse nodules of hepatocelluar carcinoma. Microscopically, tumor cells were trabecular and pseudoglandular type with Edmondson grade III atypism. Immunohistologically, the tumor cells showed positivity against anti AT-III antibody. This case suggests the potential of AT-III synthesis of hepatocellular carcinoma.

    We encountered an esophageal cancer (mp) with a marked cauliflower like tumor. A 69-year-old female was admitted to our associated hospital with complaints of heartburn and anterior chest pain. An upper GI series revealed a huge filling defect, 9.5cm in the major axis, in the middle esophagus. Endoscopic findings showed profuse bleading on the surface of the tumor. Biopsy findings from the polypoid lesion indicated well differentiated squamous cell carcinoma. Histological examination of a resected specimen, 8.0×5.0 cm showed that cancer invasion was limited to the mucosa in the major part of the cauliflower like tumor, and to sm and mp only in a small part of the base of the tumor. No vessel invasion or lymph node metastasis were observed.

    A thirty nine-year-old female with progressive systemic sclerosis was admitted to our hospital. She complained of Raynaud's phenomenon and had a severe headache. While trying to induce digital vasospasm with cold pressor stimulation, we simultaneously measured each areas of the artery and vein from the first branch to the second branch of the fundus with a PC-8801-MARK II Digitizer. The artery of the fundus dilates in patients with headaches. This suggests that Raynaud's phenomenon is associated with cerebral artery changes.

    A 73-year-old male complaining of dysphagia was admitted to our hospital and diagnosed by endoscopy as having esophageal carcinoma at the middle intra-thoracic esophagus. The presence of metastatic lymph node swelling at the bifurcation of the trachea caused renal and respiratory dysfunctions. Combination therapy with 1200 ng/body/day 5'-DFUR (5'-deoxy-fluorouridine) for about 7 months, irradiation with a total dose of 61.2 Gy, and hyperthermia, for 40 minutes once per week for 6 times was selected for treatment. With this combination therapy, complete response was obtained 10 months after beginning the treatment, which continued for 10 more months. Effectiveness of this therapy on the metastatic lymph nodes at the bifurcation was observed by CT scan. Thus, this combination therapy is recommended for patients with inoperable esophageal carcinoma.
