  • Appendix: The statistic view of the genuine diverticulum

    The authors reported a case of the genuine diverticulum of the urinary bladder, healing with the complete excision and in addition made statistic observation on ten cases (male 8, female 2) of the genuine vesical diverticulum.

    Dr. Earle had read paper concerning stricture of the female urethra in 1828. From that time little attention has been paid to that problem as a rare condition of small importance, but most authors agree that its occurrence is very rare as compared with urethral stricture in the male. And as a further contribution to the study of the subject, I propose to describe nine cases of our clinic, which in respect to symptomatology and pathology are of interost.
    Case. I. The patient, a widow, age 50. She complained of the pain on the both region and the turbidity of urine. On examination of the urethra there was definite contraction of urethra at the anterior third of it which admitted a No. 15 bougie with some difficulty.
    Case. 2, The patient, age 42. She was attacked by frequent urination, retention of urine and the pain before and after urination. Upon the examination of the urethra, there was a stricture at 0.5-1.0cm. above the external orifice, which permitted the passage of a N. 15 bougie.
    Case. 3. The patient age 69. She was suffered from the retention of urine. There was the formation of polyp at the orifice of urethra and the ureter catheter was introduced with greatest difficulty.
    Case. 4. The patient, age 50., complaining of the retention of urine. The urethra was pressed by the Esthiomène in which small multiple tumor masses surround the meatus.
    Case. 5. The patient, age 60. She had noticed the very small stream and the haematuria. By urethral examination there was the hyperplasia of clitoris.
    Case. 6. Patient, age 53, complaining the obstruction of urination. Near the clitoris the small swelling was seen.
    Case. 7. Patient, age 61, suffered from the retention of urine and the small stream. There was tha definite obstruction one centimeter above the external meatus to a No. 8 bougie.
    Case. 8. Patient, age 62. She was seized by the pain after the urination and the turbidity of urine. No. II bougie was inserted with some resistance which was at the anterior third of urethra.
    Case. 9. Patient, age 60, suffered from the formation of polyp at the orifice.
    By these there were 294 urethral strictures found in the male to nine in the female, and the frequency by age was much among 50-60 years old, the youngest 42 and the eldest 69. The etiology was by polyp 2, neoplastic I, Esthiomène I, hyperplasia of clitoris I and unknown 3 in nine cases. But among the latter the typhoid fever was seemed as the course in one case. The stricture may be located in any part of the urethra, but the external meat-us and the anterior third the favorite sites. As the treatnent gradual dilation of the stricture with bougie was performed in 4 cases, removal of tumor in 2 cases and retrograd catererization after the Sectio alta in one case. Cystitis and pyelonephritis were the most serious complication and required the attention necessary in this disease.

    Die Nebenhodentzündung ist die häufigste Komplikation des Harnröhrentrippers und entsteht durch Fortschreiten des Entzündungsprozesses von der hinteren Harnröhre durch Vas deferens hindurch. Geht die Entzündung auf die Umgebung des Vas deferens über, so führt sie zur Perideferentitis resp. Funiculitis. Aber die Erkrankung des Vas deferens ohne Beteiligung des Nebenhodens ist sehr selten und nach Csillag als Deferentitis partialis gonorrhoica beschreibt. Ich habe diese Krankheit für 5 Fälle an der unseren Klinik und möchte gern diese Fälle kurzfassend beschreiben.
    1) Alter: 28, 31, 35, 38 und 40 Lj.
    2) Seite: links 3 Fälle, rechts 2 Fälle.
    3) Komplkation: Chronischer Tripper 1, Akuter Tripper 3, Chronische Prostatitis 1, Akute Prostatitis 3, Speamtocystitis 1 und Reste der Epididymitis gonorrhoica I Fall. Bemerkenswert ist die Gonorrhoea der hinteren Harnröhre in meisten Fälle zu finden.
    4) Als die subjectiven Symptome der von den Leisten ziehende und stechende Schmerz odes ein Druckgefühl im Bereich des Samenstrangs kommt zur Beobachtung
    5-6cm. lang, bleistiftdicker, knorpelharter Strang vorfand sich in der Richtung des Funiculus spermaticus im Jnguinalkanal. Oberfläche glatt, druckempfindlich. Hoden und Nebenhoden intact.
