    A histological study was performed on the prostates in 20 patients with prostatic calculi removed through surgical procedure.
    Remarkable inflammations were observed in 14 cases (70%), which were classified into such as ductal-, periductal- and stromal-type according to their localizations of cellular infiltrations. Both the calculi and the inflammations were frequently accompanied with squamous cell metaplasia or stratification of the epithelium.
    Nodular hyperplasia designated histologically or macroscopically were found in 8 cases (40%).
    In one case (5%) a small infiltrative glandular lesion was found at the posterior lobe and diagnosed as latent carcinoma.

    I. A 48-year-old man is described with an renal arteriovenous fistula secondary to a renal cell carcinoma of the left kidney. The invasion of the tumor thrombus into the remarkably dilated main renal vein was demonstrated preoperatively by retrograde femoral arteriography, and was removed by careful operative procedure.
    II. This case is the 14th case of malignant renal arteriovenous fistula, and the literatures described 125 cases of various renal arteriovenous fistulas were briefly discussed, furthermore, a new etiological and clinical classification of arteriovenous fistula (Table 1) was presented.

    In an attempt to investigate the prognosis in malignant tumors of the kidney, follow-up studies in 29 cases of hypernephroma and 11 cases of carcinoma of the renal pelvis were made over a period ranging from 3 to 9 years, and the following results were obtained.
    1) The 3-year and 5-year survivals of hypernephroma treated by nephrectomy averaged 39% (9/23) and 30% (4/13), respectively, while the corresponding values for patients with pelvic tumor were 33% (3/9) and 33% (2/6).
    2) In either of these malignancies the prognosis was generally poor for those patients a) who visited us late after the first symptoms had appeared, b) in whom the tumor was found to be already large at the first visit, c) in whom the tumor, upon extirpation, was found to be heavy, d) in whom the tumor had already spread outside the renal capsule, or e) in whose condition there were demonstrable metastatic lesions.

    Two cases of true hermaphroditism were experienced in two-year- and three-year-old siblings. Exploratory laparotomy and histological examination revealed that each sibling had ovary on the left side and ovotestis on the right side and that each had a fallopian tube on the ovarian side and maldeveloped spermatic cord on the ovotesticular side. They were both sex chromatin positive and their sex chromosomes consisted of XX. Their external genitalia were quite similar to each other.

  • III. WITH SPECIAL REGARDS TO ALDOLASE ISOZYMES IN HUMAN TISSUES

    1) The method of horizontal starch grain zone electrophoresis was employed for the studies on ALD isozyme in the normal kidney, the bladder, the prostata as well as the respective organ tumors, the normal liver and the iliopsoas muscle.
    2) The ALD isozymes, ALD I and II were confirmed for the first time in the normal prostata, the bladder, prostatic tumors (cancer and adenoma). Especially, in the normal prostata and prostatic cancer, ALD II which was failed to demonstate with veronal buffer but borate buffer afforded the excellent electrophoresis. Protein patterns with a lower albumin peak and A/G ratio were observed in a group of kidney, liver and iliopsoas, while that with a higher albumin peak and A/G ratio, in a group of bladder and prostata.
    3) Kinetics of ALD isozyme were determined as Optimum pH being 8.5-9.0, Km 0.64-0.81×10-3M, and relatively thermostable, which were observed with little difference among isozymes.
    The effectors employed included Na, K, Ca, Mg, Mn, Zn, Cu, Co, Fe++, Fe+++, cysteine-HCl, glutathione of reduced type, ascorbic acid, 2-mercaptoethanol, EDTA, diethylstilbesterol, PCMB, hexestrol, testosterone propionate, among which only diethylstilbestrol was able to demonstrate the differences in the isozymes with ALD II more strongly inhibited.

  • IV. WITH SPECIAL REGARDS TO ALDOLASE F-1, 6-P/F-1-P ACTIVITY RATIO IN HUMAN TISSUES

    Activity ratio of ALD F-1, 6-P/F-1-P was determined and discussed of the normal adult organs (the iliopsoas muscle, the liver, the kidney, the bladder and the prostata), tumors (kidney tumors, bladder tumors, prostatic tumors and adenomas) and foetal organs (the iliopsoas muscle, the liver, and the kidney).
    The normal adult organs studied could be divided into two groups i. e. a group with higher enzymatic activity ratio such as the muscle, the bladder, and the prostata and the other with lower activity ratio such as the liver and the kidney.
    A similar tendency was also true with the foetal organs except the iliopsoas muscles and tumors in having higher enzymatic activity ratio of ALD F-1, 6-P/F-1-P.
    A metabolic pathway different from that of glycolysis in the normal or adult organs is considered to be responsible for the similarity in ratio of enzymatic activity.

  • 1ST REPORT: THE RELATIONSHIP BETWEEN THE FIBRINOLYTIC ENZYME SYSTEM IN THE BLOOD AND THE MICROHEMATURIA DUE TO EXERCISE

    The change of the fibrinolytic enzyme system (F. E. S.) in the blood associated with micro-hematuria caused by exercise as a lesson at the college of women's physical education was observed.
    1) The change of F. E. S. in the blood after the exercise varied according to the quantity of the exercise.
    In general, after exercise the value of the plasma-fibrinogen (Fbg) and concurrently the plasminogen (PLg) or proactivator (Proact.) in the blood increased, and these PLg. or Proact. were activated partially. But the manifest activation of F. E. S. in the blood was not observed due to inhibitors.
    2) The micro-hematuria was observed according to the quantity of the exercise relatively increased, the micro-hematuria after the exercise was brought about or increased.
    3) In relation between the hematuria and F. E. S. in the blood, when after the exercise the microhematuria first appeared or increased, the increasing of the plasma Fbg and PLg. or Proact. in blood and the partial activation of these precursors were more frequently seen.
    Regardless of before and after exercise, the microhematuria associated with F. E. S. in the blood was observed.
    The mean value of the plasma Fbg was 336.5mg/dl in the positive group of the microhematuria and 296.2mg/dl in the negative. The increasing of PLg. or Proact. in the blood and its partial activation of the positive group were observed more than the negative.
    4) The change of F. E. S. was studied, determined with simple methods used the whole blood, and its result was compared with Fbg and PL activated by SK in the serum and the euglobulin fraction.

    The estimation of metachromatic reactions (MR) in the urine was made in 27 male and female cases of renal tuberculosis and cystophthisis, 77 cases of urosis, and 86 cases of pulmonary tuberculosis. Cases of renal tuberculosis and systophthisis were classified into two groups: one group in which chemotherapy was performed in conjunction with an operation (group 1), and the other group which received chemical treatment alone (group 2). Long-term observations on the healing processes in both groups led to the following conclusions:
    1) The pre-treatment MR of renal tuberculosis and cystophthisis showed a maximum of an eightfold value in both sexes.
    2) For group 1, postoperative MR values became negative one month later at the earliest and eight months afterwards at the latest.
    3) The negativity of MR in group 2 occurred 14 months later at the earliest and 36 months afterwards at the latest. When MR became negative in each case, the picture of the pelvis renalis was improved on at the same time.
    4) When the negativity of MR was noted in the urine, improvements were seen, including decreases in red and white blood cell counts, as compared with findings at the onset of treatment. There was a decrease in the MR values after tubercle bacilli disappeared from the urine even on cultivation.
    5) The slower occurrence of MR negativity in group 2 than in group 1 seems to be due to the longterm continuous excretion of an MR-positive substance, an ester type of higher fatty acid, into the urine.
    6) For the assessment of the effects of treatment in renal tuberculosis effects of treatment in renal tuberculosis and cystophthisis, the MR qualitative method alone is not sufficient, but the estimating technique should be adopted for observation of healing processes.

  • REPORT X. THE CHARCOT-LEYDEN CRYSTAL FORMING CAPACITY OF MARROW EOSINOPHILS AND THEIR CYTOCHEMICAL ANALYSIS

    In 35 male cases of Urosis, marrow eosinophils were tested for their Charcot-Leyden crystal forming capacity and their cytochemical analysis.
    Results
    (1) Eosinophilia was noted in the peripheral blood of 10 out of the 35 cases.
    (2) Ten cases of peripheral eosinophilia comprised 3 cases of “prostatitic eosinophilia” (PE).
    (3) The 3 cases of “prostatitic eosinophilia” showed a Charcot-Leyden forming capacity in peripheral eosinophils.
    (4) Among the 35 cases the massage of the prostate produced a marked increase in marrow eosinophilia in 4 cases, 3 of which showed positive reactions to “PE”.
    (5) For the 35 cases the marrow eosinophil counts ranged from 81 to 875.
    (6) Cytochemical findings in the marrow eosinophils of 3 cases of “PE” were the positive reactions of the silver test, millon's test, the azo test and the chromatin test, whereas normal findings were noted in the remaining 32 cases.
    (7) The marrow of 3 cases of “PE” was as normal as in the 32 cases, except that the eosinophil count was great.
    (8) Following the massage of the prostate in 3 cases of “PE”, marrow eosinophils increased over 50%, as compared with those before the massage.
    (9) After developing into promyelocyes, marrow eosinophils from 3 cases of “PE” were specific eosinophil count increased over 50% due to the massage of the prostate. The Charcot- Leyden crystal forming capacity was also present.
    (10) The author wishes to agree with Yamamoto et al. who have interpreted the “PE” phenomenon as a reactive phenomenon due to pathological changes in the allergy of the prostate, including infection or induced allergy through candida, trichomonas, parasites and the like.
