    With respect to the pathological histological observation on a tuberculous kidney which was administered of renal pelvical irrigation with antibiotic solution (Streptomycin and INAH), and fluorescens microscopic observation on tubercle bacillus, following conclusions were obtained from comparing and Study of comparative examples in this research.
    1. The effect of the administration of the irrigation therapeutics on the pathological histological observation, is conspieuous on the foci of pelvic renalis. That is to say mainly purification and disappearance of tubercls bacillus accompained by the decollement of the caseation is conspicuous on the tuberculous foci which spread out in the pelvis renalis.
    Its degree was not always proportional to the amount of streptomycin of intra muscular injection which was used at same time, but was proportional to the number of the days the irrigation was administered. The more the number of the days of the irrigation was administered, was better the result. However, proportional to the amount of streptomycin of intra muscular injection administered at same time, the effect of therapeutics on the parenchyma is great. also, better the recovery of the focus of the pelvis renalis the recovery of the focus of renal parenchyma is good.
    2. As the observation which approximately corresponds with the description of predecessors, tuberculousknots in the renal parenchyma are contabescence of epithelioid cyst and appear to be dispersed.
    There are many of those which indicate the penetration of lymphocyte and cellulor infiltration in circumference is scarce. Only slight degree in small area or it could not be found at all.
    Langhans's giant cell also indicates the inclination of atrophy, deformation and disappearance.
    Circling of the gitter faser against the knot is good and there are many which penetrate in the knot. The inclination of fibrosis or reduction was conspicuous. also in the knot which accompanies the caseation at the center, the circling of the gitter faser is well but the penetration into focus of center caseation is less. At the place where the fibrosis or hyalinosis of Glomerus, hypertrophy of Bowman's capsule, disappearance of nephritic duct, and the diffusible proliferation of the connective tissue, are strongly observed, the infiltration of small round cell is strong and inclination of recovery is slight.
    There are many cavitary wall which schowed fibrillary hypertrophy, and large collagen fiber wrapped them, and fine gitter faser runs between them and appeared to be a fine net.
    3. The regeneration of epithelium was recognized at the recovered place of the urcerative wall or cavitary wall. Commencement of the regeneration is started from the fine gitter faser covered the epidermis cuticle which was expanded hydropically on exposed tuberculous granulation tissue and on top of that, by basing it, the epidermis cuticle was regenerated in the imperfect shape of monoptychic or stratification from healthy edge portion.
    The uniformed formation of transitional epidermis cuticle was not observed.
    Submucous coat of renal pelvis generally shows hydropic appearance. However, between the example of irrigation and that of Streptomycin with intra muscular injection, the great difference was not always recongnized by the observation of lesion in renal parenchyma only.
    4. Concerning tubercle bacillus Tubercle bacillus, in control the focus of the renal pelvis which is the main position of many tubercle bacillus, is purified by the administration of irrigation therapeutics and in accompanying it, tubercle bacillus is decreased suddenly and has great difference from control. Because of this, the progress of the lesion is rapidly ceased and as a whole the inclination of the recovery became conspicuousely great.

    The removal process of partial renal is the better process than the removal of kidney which leaves only the healthy part, removed the focus. It is congratulatable that by doing this the focus of the residual renal was also made best of and such process increases with the advance of urinery technics and the improvement of chemical therapy. Many persons announced such a condition of recovery of renal function after the removal of kidney but it is very rare to refer of the difference between the case of bi-renal and single, and they regard the case of single renal as dangerous unreasonably. Under the view that there may be the case of organ with the compemsasory function or not from the clinical case and the following case was gained afted testing renal function that by the experimental test of various animals centering around the renal clearance test.
    1) When the executive of the excision process of partial renal, there was quite a lot of number of death from immediately after to the next day, but the imcompleteness of renal function is considered to be the cause of it. Namely one who was cut off about 2/3 of single renal died next day.
    This process that renal function is obstacled in comparison with the renal quantity of kidney which was said by Karoner is proved.
    2) In case of surgical desease due to higher parasises, the isolation of kidney the digital. compression or the rubber hose around the clamp is used in order to minimize the obstacle of renal function caused with it. The time is 5-7 minutes. In this case the digital compression and clamp compression were not recognized.
    3) The highest of the renal obstacle are PRF, PBF, GFR. All of them are 1-3 days after operation. The recovery condition is RFF, RBF which are recovered within 2 weeks in single kidney and GFR is recovered a little late. The comparison of bi-kidney and single kidney untill 3 days after the operation the residual renal is bigger in the obstacle then bi-renal but the recovery is 1 or 2 weeks faster than bi-renal in every test.
    4) In the pathological and histological examination until 3 or 4 days after single kidney is much more remarkable in systematic change, namely it is remarkable in hyperemia of the skin, increase of kernel, atrophy of ansa, about 7 days a slight cange can be seem in glomerulus, no big change and so both of them were not recognized in their change.
    5) The above mentioned fact compensated on behalf of another in case of other kidney is healthy namely the removal of part of kidney when the bi-renal, as the recovery of the renal function was compensated with the affiliated kidney at the latest and so there is no anxiety at all, but in case of single kidney it recovers far more speedily than expectation. As it is revealed soon after 3 days that the obstacle of the renal function, it is necessary to be done with. prudent care or preparation. Namely the artificial kidney or peritoneum current was prepared and at a time one should be taken for preparation with the imcompleteness of renal function. Also the excision process is looked to be called the operation which is to be done positively without feeling any fear to the person who has only single kidney.

    Peritoneal irrigation for positive renal compensation of a temporaly renal insufficiency way first devised by Ganter and have were attempted by many investigators. Oder and others have reviewed electrolytic composition of irrigating fluids for peritoneal lavage from 101 clinical cases.
    Recent man had regard to electrolyte balance at the peritoneal irrigation. It is ideal that electrolyte pathern of irrigating fluid change in proportion as a state of electrolyte unbalance at anuria. But it is difficult that man make a fit solution for a burning clinical case.
    I maked experiment to prepare a better solution for anuria. Rabbit, which cocured nephrectomized anuria and anuria of mercuric poisoning, have were irrigated two solutions; one include potassium and another not it. The results obtained were as follows.
    1) There is no difference of Na+, Cl-, HCO3--pathern in serum bitween use of two solutions. HCO3-pathern in serum approached to normal and latent acidosis was compensated.
    2) Hyperpotassemia drew down to normal, however the solution, that it not included potas sium, had better ability of dialysis than another.
    3) Excretion of toxic product; Non-Protein-Nitrogen are equal.
    4) Ascent of bloodsugar and excreation of Glucose in urine were recognized.
    5) Peritoneal irrigations employed 3 clinical cases and 2 patients recovered. One case was anuria which appeared to be due to food intoxication and another was acute glomerulonephritic oliguria. Last case of uraemia by chronic nephritis died.

  • PART 2. STUDIES ON THE EXTRACTS OF PROSTATIC GLAND

    In the present research, the effect of prostatic extracts on nitrogen metabolism was studied. The extracts of prostatic gland which was used in this research was made by Omura Maruo's methods and it was administrated for 10 days daily. The summary of the results obtained was as followed.
    1. Total protein showed decrease at the beginning of the administration of the extracts and then gradually increased until the end of the administration.
    2. Non-protein nitrogen showed considerable increase from the initial dosage and maintained. azotemia over two weeks after the end of the administration.
    3. Urea and creatinine showed also the same as showed in nonprotein nitrogen.
    4. Different extracts showed different results, but polypeptide like extract showed much stronger results than the other saline and polysaccharide extracts.
    From the above results, the author demonstrated the influence of the prostatic extracts onn nitrogen metabolism and the property of stimulating nitrogen metabolism was possessed by polypeptide like extracts and moreover the inner secretion would be expected to have a stimulating effects on nitrogen metabolism.

  • PART 4. STUDIES ON THE EXTRACTS OF PROSTATIC GLAND

    This paper presented the effects of the surgical operation and treatment of prostatic hypertrophy and prostatic cancer on phosphate metabolism.
    Experimentally, phosphorous determination was made by the injection of the prostatic extracts into rabbits.
    1. Serum inorganic phosphorous, acid soluble phosphorous, lipoid phosphorous showed no essential differences between prostatic hypertrophy and prostatic cancer.
    2. Nucleic acid phosphorous level showed much higher in prostatic cancer than prostatic hypertrophy.
    3. The effect of the surgical operation on phosphorous determination was also observed. Inorganic phosphorous, acid soluble phosphorous, lipoid phosphorous and, especially, nucleic acid phosphorous level were decrasing after the operation.
    4. On the effects of the prostatic extracts, these phosphorous level were increasing after the administration of the, prostatic extracts, especially, nucleic acid phosphorous level showed much effective with it.

    This is a case diagnosed as that of primary urethral cancer, in which the cancer cell are chiefly of squamous epithelial nature involving the Bulbus Urethralis and Fossa Navicularis.
    Priapism was manifest throughout the course of the disease.
    Removal of the entire sexual organ was followed by urethroplasty. No recurrence or metastasis have been noted during the past one year of observation.
