  • ÜBER DIE PYELO-URETERITIS CYSTICA

    Ich habe einen Fall der Pyelo-ureteritis cystica beobachtet und in 3 unter 55 Sektionsfällen die Zystenbildung in der Nierenbeckenschleimhaut angetroffen. Aus der histologischen Untersuchung beider Nierenbeckenschleimhaut habe ich die folgenden Schlüsse gezogen.
    1) Die Epithelnester (Sog. Limbeck-Brunnsche Epithelnester) und die Zystenbildung der Schleimhaut sind eine Folge von Veränderungen, die aus den chronischen Entzündungsvorgängen entstehen, und zwischen beiden Gebilden befindet sich kein wesentlicher Unterschied.
    2) Der Inhalt des Epithel-Gebildes, das den drüsenähnlichen Bau zeigte, hat keine Schleimreaktion.
    3) Ich habe in neuen Untersuchungen das Epithel-Gebilde in der Nierenbeckenschleimhaut als Normalbefund in Abrede gestellt and bin der Meinung, dass alle Veränderungen als pathologisch aufzufassen sind.
    4) Der zystoskopische Befund und die Pyelographie sind die beste Hilfe bei der Diagnostik von Pyelo-ureteritis cystica.

  • ÜBER DAS ERSCHEINEN DER LYMPHOZYTEN IM NIERENBECKEN UND DIE PYELITIS FOLLICULARIS

    Durch die histologische Untersuchung der Nierenbecken von einem der Pyelitis follicularis und 55 Sektionsfällen bin ich zum folgenden Schluss gekommen.
    1) Ich bestätigte in seinen Nierenbeckenwänden die nicht geringe Entzüngsveränderung.
    2) Ich konnte in 27 unter 55 Sektionsfällen in den Nierenbeckenwänden die Lymphozytenhäufen, die den ähnlichen Bau wie Lymphfollikel haben, feststellen. So bin ich der Meinung, dass jene sich “Pseudo-lymphfollikel” im histologischen Befund nennen sollten.
    3) Diese Lymphozytenhäufen d. h. “Pseudo-lymphfollikel” entwickeln sich nach den Entzündungsvorgängen aus dem Lymphozyten-infiltrat zu den lokalisierten Zellhäufen, und werden allmählich dem Lymphfollikel ähnlich.
    4) Ich beschrieb einen Fall der Pyelitis follicularis und, den Befund in seiner Nierenbeckenwand mit dem histologischen Untersuchungsbefund von 27 Sektionsfällen vergleicht, bin zum Schluss gekommen, dass die Pseudo-lymphfollikelneubildung in meinem Operationspräparat der Pyelitis folliculars sich aus den chronischen Entzündungsvorgängen entsteht.

    I. Clinical Studies
    I have compared the following 4 kinds of clearance test (GFR, RPF, RBF, FF) with phenolsul-fonphthalein, Fishberg's concentration, blood creatinine, indigo-carmine tests and excretory urography in several kinds of urological disease.
    The conclusions are the following:
    1. Endogenous creatinine clearance can be able to apply as GFR.
    2. I have studied on renal function in urological diseases, as follows: renal tuberculosis, renal calculus, prostate hypertrophy and the other miscellaneous diseases. I have confirmed that these clearance tests are one of a sensitive renal function tests.
    3. I have observed that a high-power correlation existed between GFR, RPF, RBF and phenolsulfonphthalein test, concentration test, blood creatinine.
    4. According to the degree of renal lesion, the quantity of phenolsulfonphthalein during one hour and GFR, RPF, RBF showed the parallel results.
    5. I have studied the postoperative renal function in five cases of renal tuberculosis and in two cases of prostate hypertrophy.
    II. Experimental Studies
    I have observed prae-and postoperative renal function of unilateral nephrectomy, unilateral and bilateral partial nephrectomy, and renal-bisection with rabbits. Renal function has recovered in three or four weeks in unilateral operative cases. In the rest of kidneys or in bilateral cases the postoperative prognosis has been most discouraging. I have confirmed these clearance tests have been one of a sensitive renal function tests also in experimental studies.

    In one patient who suffered from urethrorectal fistula and in another who had his right and left ureter transplanted, respectively, into sigmoid and to skin, the urine obtained from the bowel and bladder or ureter was chemically analyzed. In one dog with an isolated distal colon the chemistry of saline solution instilled into the rectum was studied.
    The results obtained were as follows:
    The reabsorption of sodium, potassium, ammonia and chloride from the bowel, particularly the higher reabsorption of chloride than the fixed base, was potent in the disturbance of the normal regulation of the constancy of the internal environment.
    Evidence of increased demand on the kidney function was our finding that the bladder urine was very acid.
    In the exchange between CI- and HCO-3 in the colonic mucosa, which might be a cause of increased loss of body alkali, there resulted an increase of bicarbonate concentrations, a higher pH and an excessive loss of chloride in the bowel urine.
    The reabsorption of sodium from the bowel exceeded that of the chloride and the amount of bicarbonate in the bowel urine decreased when the extremly alkaline urine following the administration of diamox passed into the colon.

  • REPORT OF A CASE

    We recently experienced a case of 17 years old patient with actinomycosis of the ovary perforating into the urinary bladder.
    In addition to left oophorectomy with segmental resection of the urinary bladder, large doses of penicillin were administered and the result of the treatment was quite satisfactory.
    From a review of the literature this case was found to be the 7th reported case of actinomycosis of the ovary in our country and the 7th case perforated into the urinary bladder in the world.
