    Recently we encountered a male bearing a XX sex chromosome constitution. The patient is a 25-year-old unmarried male. He complained of loss of sexual potency. He is 154cm in height and 50Kg in weight. No gynecomastia was noted. There were no webbed neck and no cubitus valgus. The external genitalia appeared normal in size. A small testis measuring 2×1.5×1.5cm was felt in both scrotal sacs. An exploratory laparotomy failed to reveal any female internal sexual organs. No urogenital sinus was detected. Histology of both testes showed no spermatogenesis. The basement membrane of seminiferous tubules showed hyalinization. In interstitial tissue, clusters of Leydig cells were observed.
    Sex chromatin was positive in cells of buccal mucosa smears. Chromosomal analysis was performed in peripheral blood, skin and testis. The majority of cells are found to contain 46 chromosomes including a pair of apparently normal XX sex chromosomes in these three tissues. It was concluded that, this patient, although phenotype is a male, has normal female karyotype.

  • THE IN VIVO DYNAMIC GRADIENT OF THE URETERAL CONTRACTION

    Investigation of the peristaltic movement in twenty one healthy humans and eighteen mongrel dogs was undergone by means of the electric strain gauge manometer. The dynamic gradient of the amplitude, frequency and resting pressure versus various recording sites of the ureter were recorded in both humans and dogs. The relationship of the ureteral dynamic gradient to the rate of urine flow, travel rate of ureteral contraction and rhythmicity of the peristaltic waves were established and discussed, to clarify the physiology of the ureter.

  • RENAL FUNCTION AND URETERAL FUNCTION IN PARAPLEGICS

    The relationship of the ureteral contraction to involvement of the pyelogram and ureterogram was studied in 18 paraplegics. Observations have been performed by the electromanometer with sensitive strain gauge.
    The amplitude and frequency of contraction decreased in magnitude and in number as the grade of hydronephrosis and hydroureter developed. The resting pressure rose with advance of the grade, however, it remained at, or a little above, the standard value.
    The renal functions of 75 paraplegics were studied by use of excretory urograms. The impairment of the renal function developed much more in patients with lumbosacral lesion.
    The contraction of the ureter is thought to be damaged, not by a direct cord injury, but by the numerous factors subsequent to the cord injury.

  • BALOON TRACTION METHOD OF BLADDER NECK ROENTGENOGRAPHY

    1. Measurement of the diameter of the vesical neck and posterior urethra, at the portion of verumontanum in voiding cystourethrography was made for the differential diagnosis of dysuria. The results are as follows;
    2. The author devised a new roentgenological method to show the shape of the bladder neck.
    A soft baloon tied to the tip of an urethral catheter was inserted into the bladder. The baloon was filled with 20ml of contrast medium, and the end of urethral catheter was tracted by a 250g weight.
    The author divides the form of the baloon at the portion of the vesical neck into two types.
    Type I; the curve of the baloon at the bladder neck is inward convex.
    This type is subdivided into I-a and I-b according to the degree of convexity.
    Type II; the curve is outward convex.
    In a group of 15 cases without dysuria 9 cases belonged to type II and the other 6 cases to type I-b. However, in the 22 cases with dysuria, 21 cases belonged to type I and 14 of them belonged to type I-a.
    We think that the roentgenollogical finding of the bladder neck by this method may be as useful for judging the condition of the bladder neck as the test of finger-insertion into the posterior urethra during suprapubic operation.
    Though our experience is still limited, this new method is useful in differential diagnosis and an indication for operation of bladder neck obstruction.

  • A MICROANGIOGRAPHIC AND HISTOLOGIC STUDY

    Senile lesions of the human testes were histologically observed and the microangiographic vascular patterns were significant in the etiology. The materials used for microangiographic and histological studies were the right testes obtained from thirty-nine and eighty-two autopsy cases respectively.
    The results were summarized as following:
    1) After joining with the deferential artery the spermatic artery divides into one to a few main arteries, which penetrate the capsula albuginea in the hilus and go down along the medial surface of the testis.
    Then they turn up along the lateral surface up to the upper pole. On the upward way the main arteries extend branches towards the mediastinum testis along the course of the seminiferous tubule.
    2) The venous blood of the anterior half of the testis flows along the course of the seminiferous tubule to the main veins which run parallel with the main arteries. In the posterior half veins drain to the venous plexus of the mediastinum testis which join with the main, veins from the anterior part at the hilus of the testis.
    3) The senile changes of the testis are prominent in the upper and centripetal areas of the testis which correspond to the periphery of the arterial supply.
    4) The reduction of the spermatogenesis with thickening of the basement membrane of the seminiferous tubules focally appears in the forth decade and is prominent in the fifth decade. The increment of lipofuscin pigment in the Leydig cells appears late and is focally observed in the fifth to sixth decade.
    5) The hernia-like protrusion is increased in number up to the sixth and seventh decades and decreased thereafter. The lesion is regarded as one step of the course in the development of the seminiferous tubule, a bud to develop to a branch or a bridge between the tubules.
    6) It is concluded that circulatory disturbances would be most prominent in the etiology of the senile lesions of the testes.

  • UTETERAL FUNCTION AT VARIOUS URINE FLOW RATES

    Attempts were made to analyze through a computer system data concerning urometrograms of an apparently healthy man as well as electroureterograms of healthy dogs with the following results:
    1. On urometrographic analysis in an apparently healthy man, discharge intervals of action potentials averaged 23.3sec., contracting pressure 27.91mmH2O and resting pressure 11.77mmH2O respectively at a constant urine flow.
    2. In normal dogs with constant urine flow, discharge intervals averaged 2.945sec., travel rate 30.65m/sec. and amplitude 0.87mV respectively as electroureterographic recordings revealed.
    3. In normal dogs, action potentials of the ureter continuously increased or decreased in discharge interval or, otherwise, suddenly at a different pace, depending on the degree of increase in intraureteral pressure as urine flow increased.

  • (I) VITAMIN AND GONADAL FUNCTION

    Concerning the relationship between gonadal function and vitamins, reports by various investigators are already available. All of these deal with gonadal function in vitamin deficiency. We have therefore caused a disturbance of spermatogenic function through X-ray irradiation experimentally using adult rats of Wistar strain to study the influence of V-B1, V-E, V-B12, V-B1, B6, B12 complex, and pantethine on their recovery process. In order to study the relationship between the testicular storma and vitamins, these vitamins were administered following cadmium chloride administration to see the effect of these vitamins on the recovery process of the stroma. Administration of these vitamins facilitated the spermatogenetic function but administration of vitamin E and vitamin B12 alone in a large dose rather inhibit the spermatogenetic function. Upon administration as the complex, even at a dose producing disturbance in administration of a single substance, stimulation of the spermatogenetic function was rather found to our interest. The effect of vitamins was not so pronounced against a disturbance of spermatogenetic function such as X-ray injury. As compared with the group treated by X-ray irradiation alone, the revovery of spermatogenetic function was more pronounced. In the metabolic aspect, changes in the metabolic pattern corresponding to various vitamins might indicate the effect of vitamins in the metabolic aspect. The changes in the stroma might also indicate the facilitation of recovery due to vitamin administration.

    β-glucuronidase activities of serum, urine and exudate from the bladder, which were collected from the patients with genitourinary diseases, especially bladder tumors, were studied.
    The results were as follows:
    1) β-glucuronidase activity in urine indicated a high level in the morning and low in the afternoon.
    2) There were not so remarkable changes between urine volume and β-glucuronidase activity in one day.
    3) An adequate incubation time for determination of β-glucuronidase activity was 24 hours. No bacterial affects on β-glucuronidase could be found.
    4) In normal, they have shown almost the same levels of β-glucuronidase activity in separate urine from the kidneys and bladder. The other hand, the patients with bladder tumors, especially cancers, their β-glucuronidase activity showed abnormal high levels in bladder urine and exudate from the bladder after urinary diversion. But after extirpation of bladder tumors, it indicated a normal β-glucuronidase activity in bladder urine.
    5) A normal level of β-glucuronidase activity in urine collected from the kidneys and bladder was below 1.30μg/ml/hr.
    6) A high urinary β-glucuronidase activity of bladder tumors was inhibited by treatments with SLA and other various anti-cancer drugs.
    7) We found two obvious peaks, during study of β-glucuronidase isozymes pattern. We consider that there are two isozymes in β-glucuronidase.
