    Starvation-induced mucosal erosions usually occur in the forestomach but seldom arise in the glandular portion of rat stomach. In order to assess this mucosal resistance of the rat glandular stomach against starvation, we investigated the effect of starvation on the mucosal concentrations of prostaglandins (PGs) and hexosamine in rat glandular stomach. Mucosal PGE2, thromboxane A2 (TXA2) and PGF2α increased in 5-day fasting rats; mucosal hexosamine also increased in 5-day fasting rats compared with controls (1-day fasting rats). These results suggest that the gastric mucosa of strayed rats protects itself by accumulating endogenous mucosal prostaglandins (mainly PGE2) and gastric mucin, and this is possibly one of the reasons why starvation seldom induce mucosal lesions in the rat glandular stomach.

  • V. Alpha-Fetoprotein Producing Human Gastric Poorly Differentiated Adenocarcinoma Transplanted Serially into Nude Mice

    Alpha-fetoprotein (AFP) producing human gastric carcinoma was transplanted into BALB/c AJcl-nu nude mice.
    The original tumor tissue was obtained by the gastrectomy from 72-year-old patient with gastric carcinoma. The tumor had been transplanted serially 20 passages in nude mice. The transplantability was 100%. The doubling time of the tumor was ranged 5.6 days to 11.2 days.
    AFP appeared in the serum of nude mice 3 to 4 weeks after subcutaneous (s.c.) transplantation and 6 weeks after intraperitoneal (i.p.) transplantation of the tumor cell suspension prepared from s.c. transplanted tumors. Positive correlation was observed between serum AFP level and tumor growth rate. Serum AFP level of mice received i.p, transplantation was elevated as the function of time after transplantation.
    AFP was stained positively with immunochemical technique in the original and transplanted tumors and it was located in the cytoplasms of the tumor cells. The histology of original tumor was retained.
    The malignant potential of these transplanted tumor cells was expressed as carcinomatous peritonitis and liver and/or lung metastases.

    Lower esophageal sphincter pressure (LESP), gastric emptying, small bowel transit time and plasma motilin levels were measured in diabetics and normal subjects in order to investigate the disturbances of the alimentary tract motility and the participation of motilin in these motility disorders. Hypermotilinemia was observed in all the diabetics with or without autonomic neuropathy. Low response of LESP to tetragastrin found in diabetics with autonomic neuropathy could not be explained by motilin. Gastric emptying was strongly correlated with fasting plasma motilin levels and a significantly accelerated gastric emptying observed in diabetics without complications or diabetics with diarrhea was considered to be due to hypermotilinemia. On the contrary, no significant correlation was observed between small bowel transit time and plama motilin levels, suggesting no participation of endogenous motilin in the regulation of small bowel transit.

    One hundred thirty-three cases (Stomach: 83 cases, Small intestine: 39 cases, Large intestine: llcases) of non-Hodgkin's lymphoma arising from the gastrointestinal tract were clinicopathologically and immunohistologically studied.
    The 5 year survival rate was 45.6% for the stomach, 33.6% for the small intestine and 65.6 for the large intestine. Most of the deceased cases lived no longer than one year after operation.
    The important prognostic factors were the number of tumor lesions, the histological type by proliferative pattern, the depth of invasion into the wall, the involvement of regional lymph nodes and the reaction of non-neoplastic lymphocytes around the tumor tissue.
    Immunohistologically, 15 cases of examined 50 cases were positive for intracytoplasmic immunoglobulin in the tumor cells and there were 4 cases with polyclonal pattern.
    Most of lymphomas of the gastrointestinal tract were thought to be derived from B-cell, because seventy percent of them histologically showed nodular or vague nodular pattern, intracytoplasmic immunoglobulins were positive even in some cases of diffuse lymphomas, and they showed better prognosis than T-cell lymphomas in Kagoshima prefecture.

  • Effect of Enzymatic Preparation

    The purpose of this study is (1) to estimate nutritional status after gastrectomy and (2) to evaluate whether malabsorption after gastrectomy is corrected or not by the administration of enzymatic preparation. The subjects selected for this study consist of 10 cases of total gastrectomy containing each 5 cases of double tract (D-T) and Rouz-en Y (R-Y) reconstruction, 6 cases of Billroth I (B-I) and 5 cases of Biliroth II (B-II) gastrectomy. Methods: 1) BT-PABA test; 0. 5g of BT-PABA and 50g of Sustagen dissolved in 250m1 of water were orally administered and urine was collected for 6 hrs. Urinary PABA was determined by Bratton & Marshall's method modified by Smith. 2) Fecal fat and nitrogen were measured by the method of van de Kamer and semimicroKjeldahl, respectively.
    Results were as follows;
    1)Though loss of the weight and increase of fecal fat and nitrogen were found in most of the cases, total protein and total cholesterol were normal. Serum carotene decreased in onethird of the cases.
    2)In BT-PABA test followed by intravenous administration of pancreozymin, urinary PABA excretion ratio increased in cases of D-T and B-I.
    3)In BT-PABA test with simultaneous administration of enzymatic preparation, urinary PABA excretion ratio increased in cases of D-T, R-Y, and B-I.
    4)On the enzymatic replacement therapy, fecal fat reduced in cases of R-Y, B-I and B-II. 5)In 13 cases out of 17 cases whose urinary PABA excretion ratio increased in BT-PABA test with simultaneous administration of enzymatic preparation, fecal fat reduced on the enzymatic replacement therapy.
    Conclusion: From these results, it was strongly suggested that decrease of intraluminal pancreatic enzymes was one of the important causes of malabsorption after gastrectomy, and enzymatic replacement therapy was useful for the treatment of malabsorption after gastrectomy.

  • The Effect of Cysteamine on the Duodenal Mucus

    We studied the mechanisms of cysteamine-induced duodenal ulcer, investigating the effect of cysteamine on the duodenal mucus.
    Forty adult, male Wistar rats received subcutaneous injection of cysteamine (30mg/100g body weight) and were sacrificed at various intervals. Seven rats were prepared for the control study. Morphological changes of the duodenal mucosa were observed by stereomicroscopy followed by hiatoligical investigations of the Brunner's glands and crypts of Lieberkuhn on the semi-thin H-E and PAS stained sections. For the quantitative analysis of PAS-positive mucin, a newly developed computerized color image analyzer was used. Eighteen rats with truncal vagotomy were also examined by the same method.
    Erosion developed at 6h. and ulcer at 12h. after cysteamine administration. On the other hand, the PAS-positive mucin of the Brunner's glands was moderately reduced at 3h., then markedly at 6 and 12h.. At these periods, the lumina of the glands were found to be dilated, which suggested serous hypersecretion. Goblet cells and brush borders of crypts of Lieberkuhn also showed marked reduction in PAS-positivity at 6 and 12h., when dilatation of some crypts were observed. The changes of these glands were recovered at 24 and 48h.
    In the vagotomized rats, no ulcerative lesions developed by the cysteamine administration. However, PAS-positive mucin of the Brunner's glands was also markedly reduced at 6 and 12h., whereas neither dilatation of the glands nor remarkable changes of crypts were observed.
    It was demonstrated that reduction of PAS-positive mucin of the Brunner's glands and crypts of Lieberkuhn occurred before the ulcerative lesions developed and that the mucus-reducing effects of cysteamine on the Brunner's glands was not inhibited by vagotomy.

    The effects of the partial hepatectomy on Langerhans islet cells were studied by elecrtron microscope and morphological findings of B cells were estimated by morphometric analysis, in an effort to correlate biochemical date, including IRI, IRG and glucose. Empty granular sacs were increased in number and Golgi apparatus and SER were proliferated. Mitochondoria were increased in number and were globular in shape. These changes of cytoplasmic organellae in B cells were prominently observed on the 3rd day after partial hepatectomy and gradually return to normal ultrastructure by 7 days. These morphological findings seem to reflect a hyperfunction of B cells. The morphologic changes of Langerhans A cells after partial hepatectomy revealed similar findings of changes of B cells. These Results suggested that hyperfunction of Langerhans islet cells induced by partial hepatectomy was closely asociated with multiplication of hepatocytes.

    When the lymphnode cells from the sensitized guinea pigs were stimulated with the specific antigen in vitro, both lymphocyte transformation and production of lymphokines, such as choles-tatic factor and macrophage activating factor were induced. However, lymphocyte transforma-tion was clearly blocked by treatment of cells with bromodeoxyuridine followed by visible light radiation. In contrast, the productions of both cholestatic factor and MAF were not affected but rather enhanced by such bromodeoxyuridine treatment.
    These results suggest that lymphocyte that produces cholestatic factor and MAF differs from the cell that proliferate in response to antigen, and indicate that lymphocyte transformation may not be prerequisite to the production of lymphokine.

    The abnormal lipoprotein-X (LP-X) observed in patients with cholestasis was deviled into two subfractions (LP-X1 and LP-X2) by zonal ultracentrifugation. To clarify the mechanism of LP-X formation, the changes of two subfractions of LP-X and their lipid and apoprotein compositions were studied.
    In patients with extraheptic cholestasis LP-X1/LP-X2 ratio was increased as cholestasis progressed: LP-X1 was accumulated predominantly. And the ratio was decreased as the cholestasis reduced by PTC-drainage.
    On the other hand, in patients with intrahepatic cholestasis LP-X2 was markedly increased, whereas LP-X1 was not detectable. There were no significant differences in the lipid and protein compositions between LP-X1 and LP-X2. In the composition of apoproteins, however LP-X2 was abundant in Apo E which is assumed to be derived from hepatocyte.
    These findings suggest that LP-X1 is formed by the direct regurgitation of the bile components into the circulation and LP-X2 is synthesized and secreted by the liver.

    Change of local hepatic blood flow of the rat liver under various conditions was measured by hydrogen clearance method.
    The results were summarized as follows.
    1) There was no difference in local hepatic blood flow in different portions of hepatic lobes.
    2) In hypotensive rats, those blood pressure was decreased to 65% of the normal following phlebotomy, local hepatic blood flow was decreased to 70% of the normal at the region adjacent to porta hepatis and to 63% at to peripheral region of the same hepatic lobe.
    3) After ligation of hepatic artery, 22% decrease of local hepatic blood flow was observed.
    4) Twenty one percent decrease of local hepatic blood flow was noted following the postural change from the supine to the upright position. When the position was returned to the supine, local hepatic blood flow was again increased to 18% of that in the upright position.
    5) Local hepatic blood flow was increased to 21% following intraportal venous injection of glucagon and insulin.
    6) Hepatic regeneration rate in the hepatectomized rat was accelerated following intraperitoneal injection of glucagon and insulin, but the degree of hepatic regeneration was not correlated to local hepatic blood flow.
    7) Local hepatic blood flow was increased to 22% by the injection of CCL4. The flow was increased to 53% by the administration of CC14 with p-Hydroxyphenyl salicylamide and the liver injury was milde.

    The clinical features of 51 patients with hepatocellular carcinomas (HCC) smaller than 5cm in diameter, including 26 with resection and 4 with autopsy, were studied. All 51 patients were clinically diagnosed after the introduction of real-time linear scan.
    In 62.7% of them, no subjective symptoms were seen and in the remainder abdominal fullness and general malaise were frequent.
    Serum α-fetoprotein (AFP) was normal in 25.5% and above 200ng/ml in 33.3%. HBs antigen was positive in serum in 20%. Determination of the ratio of sGOT/sGPT and the level of alkaline phosphatase were not useful for diagnosing small HCC.
    Ultrasound (US) had the highest detection rate (92.2%) for small HCC in comparison with other imaging modalities.
    In 12 patients with no resection, the survival rate in one year was 75%. It was expected that patients with small HCC will live for a comparatively long time.
    It may be possible to detect small HCC by a combination of US and AFP measurements as a routine for check-up examination in patients with chronic liver disease.

    Hepatic secretory proteins in the serum, including α1-fetoprotein were sequentially measured following abstinence in 30 cases with alcoholic liver injury. Serum prealbumin levels on admission in alcoholic liver disease with slightly decreased hepatic capacity, evaluated by ICG maximal removal rate were significantly higher than those in the corresponding non-alcoholic liver disease group. Following abstinence, many cases of alcoholic liver injury showed decreases of serum prealbumin levels. In alcoholic liver injury, serum transferrin levels on admission were low and an increase of serum transferrin following abstinence was frequently observed. In alcoholic liver injury serum α1-fetoprotein levels on admission were within normal range and did not show any clear increase following abstinence except in one case.
    These results indicate that responses of hepatic secretory proteins to abstinence is different depending on the differences of the proteins. It is also sugges ted that changes in serum prealbumin may be attributed to the drinking of ethanol, and that changes in serum transferrin may reflect disturbances of protein secretion from the liver due to the impairement of microtubles polymerization by alcohol. Changes in α1-fetoprotein may also have resulted from a release of inhibitory mechanism of alcohol on protein secretion from the liver.

    Carcinoma and intestinal metaplasia were induced by insertion of paraffin pellet containing N-ethyl-N'-nitro-N-nitrosoguanidine (ENNG) in the gallbladder of hamsters. One hundred of male hamsters were divided into three groups. Group-I; Seventy hamsters were operatively inserted 5 mg of paraffin pellet with 1.2mg ENNG into their gallbladder. Group-II; Twenty of animals received insertion of 5mg paraffin pellet without ENNG. Group-III; Ten were shamoperated. All animals were sacrificed and autopsied at 6 months after the treatment. The gallbladder histologically showed hyperplasia, intestinal metaplasia, papilloma, and carcinoma. Adenocarcinoma was classified into 3 types, papillary, infiltrative, and mixed type according to Edmondson's classification.
    Twenty one (38%) out of 55 in Group-I developed carcinoma; 9 papillary, 7 infiltrative, 5 mixed type. Seven out of 9 papillary carcinoma were associated with intestinal metaplasia, whereas only one in infiltrative carcinoma showed intestinal metaplasia. In Group-II, hyperplasia, intestinal metaplasia and papilloma were encountered, but no developement of carcinoma was confirmed. Moreover, no above mentioned changes were observed in Group-III.
    These results suggest that papillary carcinoma might be caused from papilloma with intestinal metaplasia and that infiltrative carcinoma might develop sudddenly from intact and hyperplastic epithelium.

    Obstructive jaundice was produced in male Wister rats by ligation and transection of the bile duct outside the, liver, and the pancreatic weight, at ten days and four weeks, was compared to that of control animals after laparotomy alone.
    The pancreatic wet weight of obstructed rats was 1.27 times of control at ten days and 1.4 times at four weeks. The amylase, lipase and protein contents in obstructed animals were respectively 1.68, 1.57, and 1.38 times of control rats and histrological examination showed tendency of hypertrophy of pancreatic acinar cells.
    Obstructive jaundice produces pancreatico-trophic effect in rats.

    To elucidate the cause of the increase in the echogenicity of the pancreas in the elderly, the ultrasonograms of 116 patients were studied for echogenicity, and the echogenicity of the ultrasonograms in living patient and that of ultrasonograms of immersed autopsied pancreas were compared with the histological findings.
    In approximate 65% of the patients, the echogenicity of the pancreas was greater than that of the liver, and 24% were remarkably greater than it. In comparison with the echogenicity and histological findings, it was revealed that the level of fatty replacement among the high echogenicity group was significantly greater than that in the low echogenicity group, and there was a close relationship between echogenicity of the pancreas and fatty replacement of this tissue. However, there was no relationship between echogenicity of the pancreas and pancreatic fibrosis.
    These results suggest that the increased echogenicity of the pancreas in the elderly is attributable to the histological increase in fatty replacement of the pancreas.

    We investigated role of IAA, which is a plant growth-hormone, in animal cell proliferation and discussed its possibility for clinical application.
    Experiments with LP3 and 3T3 cell-culture revealed the following results: The LP3 and 3T3 cells produced IAA during their cell-proliferation and its concentration per cell reached high level on the 2nd culture-day, then decreased thereafter. However, the IAA concentration of the culturemedium was rather constant such as 10-7 Mol, and the cell-proliferation progressed normally under that concentration but if it exceeded that levels, it suppressed cell-proliferation.
    Studing serum and urinary IAA concentration from normal individuals as well as G.I. cancer and non-cancer patients, we found the following results: 1) In patients with early cancer, the IAA was generally elevated (p⟨0.01) but it decreased in the terminal cancer patients. 2) Among the liver tumor, primary liver cancer showed decreased IAA level but metastatic liver cancer showed elevated IAA level (p<0.02).
    Furthermore, the cancreous tissue and its marginal area were found to have elvated IAA than non-cancerous remote area, suggesting close relationship between the IAA and cell-proliferation.
