ジャーナル フリーWe carried out the outpatient appointment system in Saitama National Hospital to control the number of outpatients for five years and studied these economic and medical evaluation.
We produced the good results as follows:
1. the hospital income was always more than expenditure.
2. the mean number of outpatient per day was able to be controled always about 450 and the mean number of admission per day was able to be increased to about 340 from 280.
3. the number of outpatient on one doctors'duty per day saved down to 37 from 47.
4. the number of outpatients on one nurses duty day saved down to 39 from 51.
5. the outpatient-nursing-time was able to be curtailed 195 minutes from 240 minutes per day per one nurse ore day.
the efforts were rewarded and were able to put in practice the outpatient appointment system and to place great emphasis on the admission treatment.抄録全体を表示PDF形式でダウンロード (1337K) -

ジャーナル フリーManagement of the upper urinary tract, especially protection of the upper urinary tract infection is one of the most important problem during treatment of the bladder cancer.
Here we report that obtained from the inspection of vesicoureteral reflux(VUR)of bladder cancer patients as a factor which initiates the upper urinary tract infection. VUR were found in 12 cases (22.6%) out of 53 cases of the bladder cancer. There are no statistical significance between VUR and pyuria, tumor site&tumor grade.
We have treated bladder cancer patients by external irradiation with 6 MeV linear accelerator. In 4 cases out of 5 cases with reflux VUR disappeared after the irradiation. In 7 cases out of 14 cases without reflux VUR were newly found after the irradiation. Contract bladder were found in 3 cases out of these 7 cases.抄録全体を表示PDF形式でダウンロード (4113K) -

ジャーナル フリーWe have analyzed 71 cases of selective angiographies; that is subclavian, bronchial, celiac, superior mesenteric, inferior mesenteric and intra-iliac arteriographies on the 57 patients.
Fifteen (21%) of 71 cases we have not succeeded. Among the 15 failed cases, 4 cases had arteriosclerosis and thrombosis in the extra-iliac artery and the common iliac artery. The other failed cases were 5 celiac artery, 1 superior mesenteric artery, 2 bronchial artery, 1 esophageal artery and 2 intra-iliac artery. The 5 failed arteriographies were 3 cases of cancer of stomach and choledochus, 1 giant liver by metastasis of cancer and 1 penetrative gastric ulcer. The failed 1 superior mesenteric arteriography was due to invisibility of catheter caused by previously administered 76% urographin in the huge cystoma pseudomucinosum.
We have succeeded in celiac angiographies on 36 (86%) of 43 cases. Vascular anomalies have been seen in celiac angiographies on 9 (25%) of the 36 cases. In 18 (60%) of 30 cases, the aberrant finding of the celiac angiographies were confirmed by operation.
Angiographic findings of hepatic metastasis have been made on 2 of 5 cases.

ジャーナル フリーFor blood gas analysis, we attempted to clarify the clinical usefulness of arterialized capillary blood obtained by ear lobe method as a substitute for arterial blood. The blood sample was collected simultaneously from the brachial artery and from the warmed ear lobe of patients with mild or severe pulmonary diseases. In collecting blood from ear lobe there are following important things; first, the sample must be collected directly into heparinized capillary tubes from the punctured wounds of the ear lobe, second, the ear lobe was cleaned and warmed at least 5-10 minutes by radiant heat or rubbing by the finger tips, and third, the ear lobe was punctured 5 mm in length and 2 mm in depth, then the capillary blood gave sufficient supply, so we must never press out the blood sample.
The comparison of gas-tension and pH of arterial and arterialized capillary blood was shown in table 1-6. The correlation coefficient of both observed values was significant, and it was proved statistical significance.
The ear lobe method is satisfactory for routin clinical use and screening procedures for room air breathing.抄録全体を表示PDF形式でダウンロード (383K) -

ジャーナル フリーIn attempt to overcome some disadvantages of the determination of cholesterol based on the Liebermann Burchard reaction (Sperry Webb's method), an improved dextran sulfate precipitation method was devised and applied to automatic determination (Levine's method).
The method consist of two procedures: precipitation of cholesterol with dextran sulfate and replacement with NaCl solution after removal of supernatant.
To improve the second procedure a narrow graduated testtube was used. NaCl solution was brought to 0.3ml, instead of adding of 0.3ml.
From the results of correlation with Technicon N24 method, reproducibility, and quality control, it was concluded that the method is simple and satisfactory as a routine method.抄録全体を表示PDF形式でダウンロード (392K) -

ジャーナル フリーThe author reported here two cases of systemic lupus erythematodes, and they are sister. Their parents are cousins. The elder sister was first attacked with this disease five years ago (19-years-old), and took remissions and recurrences by turns. One year ago she was complicated in autoimmune hemolytic anemia. The younger sister fell ill one year ago (15-years-old). At first she had no systemic signs except the erythema on cheeks, nose and palms. But one month ago she had remittent fever, general fatigue, tenderness and spontaneous pains of systemic lymph nodes. So the author diagnosed this case as systemic lupus erythematodes also. The author investigated parents'gammaglobulin percentage, Wassermann reaction, complement value, immunoelectrophoresis and antinuclear-antibody and found out a slight hypergmmaglobulinemia of both parents.抄録全体を表示PDF形式でダウンロード (5648K) -
