This paper deals with clinical research of personal sensitivity to alcohol intakes: average daily alcohol intake (ADAI) (g/day) and total alcohol intake (TAI) (kg) with a use of serum γ-glutamyltranspeptidase (γ-GTP) activity. In contrast to a positive correlation between γ-GTP and TAI (r=0.4856,P<0.001) in social drinkers, a significant negative correlation (r=-0.4999,P<0.001) was observed in chronic alcoholics, strongly suggesting a "poor responder" to drinking. In addition, the "poor responder" is defined to be a significant elevated alcohol intakes such as ADAI(160g/day) and TAI(1122kg), in spite of almost normal range of γ-GTP activity (34mU/mL). Moreover, the "poor responder" is characteristic of muscular body image in Kretchmer's classification ,less withdrawal syndrome and almost none of intorelable signs even on the occasion of initial drinking. Personal sensivity to drinking is also discussed in special reference to polymorphism of genomic types of alcohol metabolyzing enzymes (alcohol dehydrogenase ADH2 and acetoaldehyde dehydrogenase ALDH2).

Present recommended dietary allowance (RDA) is based on average requirement obtained by statistics of large population, but the range of 2 times standard deviation may not cover the vitamin requirements owing to the genetic polymorphisms. For example, loaded vitamin C excretion by AA homozygotes of glutathione S-transferase 1-1 polymorphism (A342G(exon5)) was 66% larger than that by GA heterozygotes (p<0.007). The daily folate intake of 400 μg instead of the Japanese RDA (240 μg), was recommended for TT homozygotes of methylene tetrahydrofolate reductase gene polymorphism (C677T) in "Sakado Folate Project". This health interventions increased the average serum folate concentration of residents from 5 ng/mL to 8 ng/mL (p<0.0001), with a particularly large increase after genome announcement during a half year. Food fortification may be needed to provide enough vitamins for polymorphic individuals.

Warfarin is the most commonly-used oral anticoagulant for treatment of thromboembolism, but adjustment of the dose that is appropriate to each patient is not so easy because of large inter-individual variation in the dose requirement. We analyzed SNP genotypes of the candidate genes, VKORC1 and CYP2C9 using DNAs of 828 Japanese patients treated with warfarin, and classified the patients into three categories, on the basis of genotypes of the two genes, which we call "warfarin-responsive index". The median warfarin daily dose varied significantly by the classification according to the warfarin-responsive index (2.0 mg/day for the index 0 group, 2.5mg/day for the index 1 group, and 3.5mg/day for the index 2 group; P=4.4×10^<-13>). Thus, a combination of VKORC1 and CYP2C9 genotypes should be applicable to predict appropriate dose of warfarin, leading to safety and cost effectiveness for the personalized warfarin treatments.

Single-nucleotide polymorphisms (SNPs) of the mitochondrial NADH dehydrogenase subunit-2 (ND2) andβ3-adrenergic receptor (BAR3) genes are well established to be associated with life style-related diseases. The ND2 gene variant (Mt5178A) is associated with longevity or less susceptibility to adult-onset diseases. The BAR3 gene allelic variant (Trp64Arg and Arg64Arg) is linked with obesity or non-insulin-dependent diabetes mellitus. We investigated whether the combination of these 2 genes affect eating behavior, and found that the BAR3-normal (Trp64Trp)+ND2-variant (Mt5178A) group preferred much carbohydrate and less animal protein, Vitamin B_2 (B_<2)>, calcium in comparison with the other genetic groups. Less intake of B_2 was principally associated with Mt5178A in this group. Taking account that flavin adenine dinucleotide is a coenzyme of the NADH dehydrogenase which composes the proton-pumping complex I of the mitochondrial electron transport system, the association between Mt5178A variant and less B_2 intake is tremendously interesting. In this paper, we discuss a possible association between genetic polymorphism and eating behavior by summarizing our own research data.
