During the period from 1978 to March, 1983, 55 patients with carcinoma of the larynx were treated with the use of CO2 laser at Kurume University Hospital. Preliminary results are summarized as follows: 1) Twenty-two patients who had had no previous treatments were treated with laser alone. Of the 22, 2 were Tis cases, 14 were T1a, 5 were T1b and 1 was a T2. Eight patients developed recurrence. Three of these 8 were treated with laser again, 2 were treated with laser and radiotherapy, 1 underwent partial laryngectomy, 1, total laryngectomy, and the other refused to have any further treatment. The former 7 patients are alive with no evidence of disease. The latter is alive with recurrent carcinoma. 2) Thirty patients who had had no previous treatments were treated with laser and radiotherapy (6, 000 rad). There were 10 T1a, 13 T1b and 7 T2 cases. Four patients developed recurrence. Of these 4, 3 were treated laser surgery alone and 1 underwent total laryngectomy. All patients are alive without any evidence of recurrence. 3) Three patients were radiation failure. Two of them were rT1a and 1 was rT1b. They were all treated with laser alone. None of them have developed further recurrence. Two are alive and the other died of an intercurrent disease.
There are many studies about development of the utricular macula using the TEM. This study demonstrates the morphological development of the utricular macula in the mouse using the SEM. The utricular macula of CBA/CBA mouse from the 11.5th gestational day to the 2w postnatal day were studied. Numerous microvilli and primary cilium on the surface of otocyst were found between 11.5th and 12.5th gestational day. Sensory hairs appeared on the 13.5th gestational day. Its kinocilium and stereocilia were short and the length of stereocila were almost equal in size. The polarity of kinocilium was first noticed after 16.5th gestational day. The formation of otoconia started on the 14.5th gestational day. Its shape was dumbbell-like, egg-like columnar and so on. Otoconia with hexagonal form was recognized after 16.5th gestational day.
A case of T cell origined malignant lymphoma with Mikulicz's syndrome was reported. The patient, 56 years old male, visited our clinic on May 21, 1982, complaining of synmetrical and asymptomatic swelling of the lacrimal, parotid and submandibullar glands. In physical findings, he had impairment of saliva secretion and keratoconjunctivitis sicca. The laboratory data showed hypercalcemia, hypergammaglobulinemia, high serum LDH and high serum amylase levels. And the lacrimal, parotid and submandibullar glands showed high uptake of the isotope in 67Ga scintigraphy. The parotid and submaxillary sialograms demonstrate the irregular extravasation of contrast material into the glands. The ducts are delated and stretched. The histological findings of the parotid gland obtained by open biopsy proved the diffuse and mixed typed of malignant lymphoma. The tumor cells diffusely infiltrated into the grandullar tissue, and the structure of the glands was widely destroyed. These histological findings were simillar to those of the so-called Mikulicz's disease, which has morphologically been characterized by infiltration of abundant lymphocytes in glandullar tissue. The infiltrested lymphocytes of the case reported, however, were mostly malignant. In addition, the surface markers of the lymphocytes revealed T cell origin, and ATL antigen was positive.
Frontal sections by Polytome, 3 and 4cm posterior to nasion, were analysed in order to calculate the spatial distribution of the ethmoidomaxillary plate (EMP). 80 male adults and 68 female adults, all without the previous histories of sinus surgeries, were included in the study. Both the medial and lateral borders of EMP were analysed in order to calculate the angles between the borders and the anthropological base plane (Frankfort plane). These angles of the medial and lateral borders are as follows respectively; 8.0° and 15.6° in the male, 0.6° and 11.3° in the female, and 9.6° and 14.6° by the average of both the male and female. A labelling study of EMP by dry skulls was performed in order to determine the changing positions of the overlapping petrous ridge by various positioning in PA view. Each of the three corners of EMP was labelled with a tiny lead ball in the maxillary sinus. When the central X-ray hits the EMP tangentially, these labels will assume linear arrangement among each other. This linear arrangement is seen when the central X-ray makes an angle of 0° to -15° to the anthropological base plane. In order to avoid the overlapping of the petrous ridge, an angle exceeding -10° is necessary. These results suggest that EMP will be best demonstrated by -15° angulation of the central X-ray in the PA view. This is in accordance to the previous reports that Caldwell view is suited for the demonstration of EMP.
Ten rabbits with normal auditory brainstem responce (ABR) were used for the experiment to clarify the mechanism of auditory disturbance in cerebral apoplectic patients. They were injected 2ml of their own blood into the internal capsule by the stereotaxic method, and the place of injection was comfirmed histologically after the measurement of ABR. Abnormal ABR could be observed in 8 of 10 rabbits after the blood injection. Namely, the influence of the internal capsular bleeding on ABR was realized by this experiment, and it was suggested that a pressure of local bleeding to the brainstem would act as the indirect factor of auditory disturbance in apoplectic patients.
In order to elucidate the pathophysiological stage of Meniere's disease, experimental endolymphatic hydrops was produced in the guinea pigs, and the correlation between the parameters of the obtained electrocochleogram and the grade of pathological endolymphatic hydrops was analysed with postoperative survival time of 3 to 5 weeks. Furthermore, thus obtained results were applied on the clinical cases to establish the usefullness of electrocochleographic technique as a tool of Meniere's diagnosis, Following items are pointed out in this study. a) By the parameter of SP/AP ratio, it is possible to detect the existence of hydrops but not grades of the endolymphatic hydrops. b) By newly defined parameter of SP area (i.e.expression of Meniere's disease broad AP area), it is able to detect not only existence of hydrops but also to predict the grades of hydrops. c) When furosemide is applied to the experimental hydrops animals, SP/AP ratio can tell the grade of hydrops at 60-75 minutes after administration. d) The mean value of 0.50 of SP/AP ratio was measured in the preoperative patient of Meniere's disease and 1.94 of SP area.After the patient underwent to endolymphatic Sac surgery, the mean value of SP/AP ratios and SP area were changed to 0.46 and 1.85 respectively. From these results, the fiducial limits of electrocochleogram's parameters between the Mfnii re's disease and normal are defined as >0.37 in SP/AP ratio, >2.75msec in width and >1.49 in the SP area.
