  • 単発矩形波励振による共振特性

    In order to examine the resonance characteristics of the vocal fold in a wide frequency zone, a single rectangular pulse of 0.5 msec duration was given to the vocal fold via the thyroid cartilage and the vibration pattern of the vocal fold was recorded by the ultrasonic pulse variant M-mode display. Healthy side of the vocal fold in 13 specimens removed surgically due to cancers of the larynx, hypopharynx and of the tongue were used for the study. The power spectra of the decremental waves recorded by this method were analyzed by the Fast Fourier Transform and resonance characteristics including Q-values were obtained using mini-computer.
    The results were as follows;
    1) Resonance characteristics in freshly excised larynx The mean value of the lowest resonance frequency of the normal vocal fold was 117.5±12.9Hz and Q-value 4.92±0.45. The second resonance frequency was observed at 180.3± 17.7Hz with Q-value 6.38±1.39. Resonance frequencies of the vocal fold were a little higher in females than in males.
    2) Effect of mass factor As the weight of the loaded mass increased, the resonance frequency and Q-value tended to decrease.
    3) Effect of fold tension A higher resonance frequency was observed when a longitudinal tension was applied to the vocal fold. When a longitudinal tension became over 50gr, however, the resonance frequencies increased non-linearly. Q-values were well correlated with the resonance frequency variation.
    4) Mechanical factors of the vocal fold Effective mass, effective stiffness of human vocal fold can be estimated within the range of 0.141±0.02gr and 73.7±12.0kdyn/cm, respectively.

    Normal development of the facial nerve in the mouse was investigated morphologically and histologically, especially at the middle ear region, with a light microscope.
    The materials were 59 mouse fetuses, belonging to ddN strain, from the day 8 to 20 of pregnancy. Their heads were serially sectioned and stained with hematoxylin and eosin.
    The results were as follows:
    1) On the day 9, the anlage of the acoustic ganglion was developing just anterior to the otic vesicle and the cells of the superficial ectodermal thickening around the region of the 1st branchial cleft appeared to differentiate into the neuroblasts of the future geniculate ganglion.
    2) The geniculate and acoustic ganglia became clearly differentiated on the day 10. The main trunk of the facial division located posteriorly to the anlage of the stapes and ran across the mesenchyme of the 2nd branchial arch vertically.
    3) The peripheral and central nerve trunks from the geniculate ganglion were recognizable on the day 11. The chorda tympani and the greater petrosal nerve were also present at the day 11.
    4) The identification of the horizontal and vertical portions of the facial nerve was clear on the day 12.
    5) The development of the otic capsule and the stapedial footplate seemed to have much influence upon the running course of the horizontal portion of the facial nerve and the development of Reichert's cartilage upon the running course of the vertical portion between the day 12 and 13.
    6) The development of the facial canal belonging to the horizontal and vertical portions began as a sulcus formed with the precartilaginous otic capsule and the laterohyale at the day 13.
    7) The development of the lateral wall of the horizontal facial canal began by forming the connective band at the day 16 and the vertical segment at the day 18.

    The utterance intelligibility test should quantitatively evaluate the postoperative articulatory function of the patients with carcinoma of the tongue. The authors propose a new list consisting of only 25 monosyllables for an appropriate use in everyday clinics. The selection of the monosyllables was performed taking account of the following points:
    1) The results of the intelligibility test using the new list should highly correlate to those using the conventional list consist of 100 monosyllables.
    2) The results of the test should reflect well the postoperative articulatory disorders.
    3) The list should include monosyllables which appear frequently in daily conversation.

    The triads for aspirin-induced asthma (AIA) are asthma, aspirin intolerance and nasal polyps.
    Eighteen cases, 6 males and 12 females, who complained asthmatic attack and/or rhinorrhea after ingestion of non-steroidal anti-inflammatory drugs, were seen within the past several years. Mean age of onset was 34.2 year old in asthma and 25.4 year old in rhinitis. It seemed that AIA is related to a disease of middle age. Asthma was found in 17 of 18 cases (94.4%) and nasal polyps was found in 13 of 17 cases (76.5%) examined. The polyps were recurrent and 7 patients had undergone polypotomies. Urticaria was seen in 44.4% and sinusitis diagnosed by X-ray studies in 81.8%. Sensitivity to at Least one inhalent allergens was found in 7 of 9 cases (77.8%) and 6 of 11 cases (54.5%) showed positive reactions for RAST. Eosinophilea was seen in 11 of 13 patients (84.6%) and highest level of eosinophil count was 27%.
    The etiology of this disease is unknown but probably relates to inhibition of prostaglandin synthesis. The hypothesis is that aspirin inhibit the cyclo-oxygenase pathway, and arachidonic acid is directed along the lipoxygenase pathway to form SRS-A, an extremely potent bronchoconstrictor.

    Physiological characteristics of first order horizontal canal neurons, especially in their spontaneous activity, were studied in 11 unpigmented guinea pigs. Ninety-three single neurons were recorded and their inter-spike intervals and regularity of the resting discharge were analyzed. Mean inter-spike intervals was 40.5 msec with a range of 11 to 227 msec.
    The neurons were classified into three groups according to their regularity of resting dis- charge which was determined by coefficient of variation (CV). Fourty neurons (43.0%) showed regular type of resting discharge and their mean inter-spike intervals was 20.2 msec. Thirty-six (38.7%) neurons exhibited intermediate type and had 32.2 msec mean inter-spike intervals. Remaining seventeen neurons (18.7%) were classified as irregular type and showed mean interspike interval of 95.5 msec.
    The basic physiological characteristics of spontaneous activity in unpigmented guinea pigs were same as pigmented guinea pigs reported by Curthoys (1982), but the mean inter-spike intervals in unpigmented guinea pigs was clearly greater than that of pigmented guinea pigs.

    Twenty normal hearing young adults ingested 1.4ml alcohol per kg body weight over 60 minutes.
    Then the blood alcohol levels, breath alcohol levels were measured by Dupont ACA blood analyzer and DPA-2 breath analyzer.
    Also the thresholds and the amplitudes of the acoustic reflex were measured by impedance audiometer ZO-72, using 1.5sec., 500Hz., pure tone stimulation contralaterally in 13 persons, and ipsilaterally in 7 persons. First, the thresholds were decided and then the amplitudes were recorded after the stimulation by the sounds of 20dB more than those of before drinking alcohol.
    The results were as follows:
    1) In 13 persons who the stimulation were given contralaterally, the amplitudes were decreased at the point of just finishing drinking. Four persons showed remarkable decrease, while five showed no significant change.
    2) In 7 persons, who the stimulation were given ipsilaterally, the thresholds were increased at the points of just finishing drinking, 30 minutes later, 60 minutes later. No variability was found among them.
    3) No significant relations were found not only between the blood alcohol levels and the changes of the thresholds, but also between the blood alcohol levels and the changes of the amplitudes. From the above mentioned, it was concluded that the acoustic reflex was depressed when people drink alcohol, and people will get morepossibilities of suffering from acute sensorineural healing loss if they are exposed to loud sounds while drinking, like in discos.

    A 19-year-old female had been treated with DPH medication, and on the first visit to our clinic, she presented with gaze nystagmus to the right and left side, saccadic ETT and total depressed OKP pattern suggestive of cerebellar dysfunction.
    Concentration of DPH in blood reached the toxic level.
    By the withdrawal of DPH medication, the concentration of DPH in blood was reduced and the findings of ETT, OKP and gaze nystagmus were improved.
    In this case, it was speculated that a reversible lesion due to DPH intoxication existedinot only in the cerebellum, but also in the brain stem.

    99mTcO4- and 67Ga-citrate scintigraphies were preoperatively applied to 26 cases of parotid adenolymphoma. 99mTcO4- scintigraphy revealed focal hot image on 18 sites (69%), normal image on three sites (12%), and cold image on five sites (19%). Macroscopic cystic degeneration and microscopic collagenous degeneration were suspected to be the factors by which 99mTcO4- scintigraphy resulted in normal or cold scintigram.
    67Ga-citrate scintigraphy showed normal scan in 21 sites of tumor (81%), and diffuse of focal hot scan in five sites (19%). Three out of five Ga-positive cases were preceeded by sialography. Other two cases had the evidences of abscess formation in the tumor or acute inflammatory response around the tumor.

    The authors reported three cases of the foreign bodies of button batteries in the nasal cavity. The first patient was a 2-year-old boy who put a small alkaline cell into his nostril, and septal perforation and the nasal atresia was observed due to chemical burn. The second patient was a 2-year-old girl, and alkaline cell caused the nasal atresia. The third case was a 5-year-old boy, who put the button battery into his nostril, but neither septal perforation nor nasal atresia was observed.
    The foreign bodies of button batteries in the nasal cavity caused the specific clinical symptoms by alkali burn. It was suggested even if there were no leakage of the alkaline cell contents, the button battery might give rise to a current external to the battery and generate sodium hydroxide at the cathode of the cell.
