    ASAKI, S., HATORI, S., IWAI, S., NISHIMURA, T., SATO, A. and GOTO, Y. Diagnosis of Submucosal Tumors by Injecting a Water Soluble Contrast Medium: Basic Research and Imaging of Tumors. Tohoku J. exp. Med., 1982, 138 (2), 121-130-With the aim of adequately distinguishing gastric submucosal tumors from other lesions such as gastric polyps, gastritis, and protuberances due to extragastric pressure, a new method was developed. A water soluble contrast medium is endoscopically injected into a submucosal lesion followed immediately by the application of roentgenography, thereby obtaining X-ray pictures of the lesion through frontal and lateral projections. Since the contrast substance is water soluble, the method can visualize submucosal microchanges. Highly diffusible, the substance usually has a fading time of about 40min, the present method is convincingly free of danger. This paper provides the results of basic research performed using adult mongrel dogs together with some comments on the applicability of the method in clinical use.

Med., 1982, 138 (2), 131-137-Triolein hydrolyzing activity in homogenates of the rat arterial wall was studied. Two optimal pH values were observed around 5.0 (acid lipase) and 7.0 (neutral lipase). Neutral lipase activity was enhanced by serum and was inhibited by 1M NaCl but was not inhibited by protamine. Heparin decreased the acid lipase activity. A 1-mM solution of Cu2+ ion and Zn+2 ion decreased both lipase activities. The activities of both acid and neutral lipases were increased by the addition of rat serum (2μg protein serum/ml) and were decreased by the addition of more than 5μg protein serum/ml. The activities of neutral and acid lipases were increased by the addition of 0.25mM phosphatidylcholine and phosphatidylserine. Both lipase activities were decreased by the addition of 0.25 mM phosphatidylethanolamine. The addition of 0.25mM sphingomyelin increased neutral lipase activity and decreased acid lipase activity. At concentrations higher than 2mM, these phospholipids decreased the activities of both lipases. These results suggest that at least acid and neutral lipases exist in the arterial wall cells, and their activities might be modified by phospholipids.

T Cell-Mediated Cytotoxicity against HBsAg- Coated Chang Cells in Patients with Chronic Hepatitis: Evidence for Cytotoxicity Mediated by Delayed Hypersensitivity T Cell Reaction. Tohoku J. exp. Med., 1982, 138 (2), 139-150-T lymphocytes from 7 (21%) of 34 patients with chronic hepatitis showed positive cytotoxicity against HBsAg-coated Chang cells. This positivity was observed in HBsAg-negative patients having positive blast transformation responses to HBsAg, as well as in patients convalescing and recovered from acute B hepatitis. Levels of S-GPT in these patients were not different from those showing no cytotoxicity. T cell-mediated cytotoxicity against HBsAg-coated hepatocytes in HBsAg-negative patients thus may have no significant pathogenetic role in destruction of hepatocytes and may represent anamnestic response of sensitized T lymphocytes to HBsAg. Positive cytotoxicity against HBsAg-coated Chang cells was also found in 3 of 17 patients with HBsAg-positive chronic hepatitis. All positive cases exhibited blast transformation responses to HBsAg and low HBsAg titers in their sera. Levels of S-GPT in these patients were significantly higher than in those showing no cytotoxicity, suggesting possible presence of T cell-mediated liver cell damage in these patients. T lymphocytes from asymptomatic HBsAg carrier showed no cytotoxicity to HBsAg-coated hepatocytes and no blast transformation responses of lymphocytes to HBsAg. From the results of the parallel occurrence of T cell-mediated cytotoxicity and blast transformation responses to HBsAg, and of presence of lymphotoxin in the supernatant co-cultured HBsAg and cytotoxicity-positive lymphocytes, it seemed likely that T cell-mediated cytotoxicity in our system might be mediated by lymphokine produced by T cells as a result of delayed hypersensitivity reaction in vitro.

Med., 1982, 138 (2), 151-160-A daily oral dose of 1-2mg of Carboquone was administered for remission induction to 38 patients with hematologic malignancy, and the following results were obtained: 2 of the 3 patients with malignant lymphoma, 1 of the 12 patients with multiple myeloma, 13 of the 14 patients with chronic myelogenous leukemia, and all 9 patients with polycythemia very attained complete remission.

Med., 1982, 138 (2), 161-165-A combination therapy consisting of daily intrarectal administration of 1, 500mg of FT-207 suppository, daily or every three day bladder instillation of 40mg adriamycin for a total of 20 applications, and once per week intramuscular administration of 5 KE OK-432 was described. The therapy was performed in 30 cases of advanced bladder cancer. The effectiveness evaluated by Karnofsky's criteria was as follows: 8 cases of 0-0, 2 of 0-A, 1 of 0-B, 2 of 0-C, 7 of 1-A and 10 of 1-B, showing an effective rate of 57%. Local pain was observed in 20 cases (67%), pancytopenia in 1 (3%), hepatitis in 4 (13%), stomatitis in 5 (17%) and anorexia in 6 (20%). From this clinical trial, it is presumed that combination therapy administering FT-207 intrarectally, adriamycin intravesically and OK-432 intramuscularly might be used for the treatment of advanced bladder cancer provided sufficient care is taken of general side effects.

Stimulation of 1, 25-Dihydroxyvitamin D Production by Parathyroid Hormone and Dibutyryl 3', 5'-Cyclic AMP in Normal Subjects, Hypoparathyroidism and Pseudohypoparathyroidism. Tohoku J. exp. Med., 1982, 138 (2), 167-175 Parathyroid extract (PTE) or synthetic 1-34 human parathyroid hormone (1-34hPTH) was injected intravenously as a bolus in 4 normal subjects, 4 patients with PTH deficient hypoparathyroidism (HP) and 3 patients with pseudohypoparathyroidism (PHP). In normal subjects and HP, plasma 1, 25(OH)2D was markedly increased at 6hr and reached the peak at 12 or 14hr after administration of 200 units of PTE or 20 to 30μg of 1-34hPTH. On the other hand, 500 units of PTE or 20μg of 1-34hPTH failed to increase plasma 1, 25(OH)2D in PHP. However, 2.5mg/kg of dibutyryl cAMP remarkably increased plasma 1, 25(OH)2D in a patient with PHP. Maximal increments of plasma 1, 25(OH)2D in 3 patients with HP (21.7±5.6pg/ml, mean±S.D.) were nearly as high as in normal subjects (20.6±7.0pg/ml), whereas those in 3 patients with PHP (2.3±2.3pg/ml) were distinctly lower than in normal subjects or HP. It is suggested that 1, 25(OH)2D production by PTH is intact in HP, but is impaired in PHP mainly due to a defect in the activation of adenylate cyclase system.

    SEKIGUCHI, K. Further Observation on Empyema after Resection of Bronchogenic Carcinoma and Survival. Tohoku J. exp. Med., 1982, 138 (2), 177-185-Out of 11, 247 cases undergoing resectional therapy for bronchogenic carcioma at 39 institutions, 6, 348 were operated before July 1976 and the remaining 4, 899 thereafter. Analyzing both series, the following results were obtained: 1) The rate of pneumonectomies to lobectomies was 1:2.3 in the former and 1:3.9 in the latter. Pneumonectomies were more on the left side, whereas the right lobectomies were about twice as many as the left ones. 2) Postoperative empyema occurred in 180 cases (2.8%) in the former and 79 cases (1.6%) in the latter. The incidence was significantly higher in the cases of right pneumonectomy than in the cases of left pneumonectomy, and in the cases of pneumonectomy than in the cases of lobectomy. Although the incidence of postpneumonectomic empyema decreased only slightly, overall incidence decreased significantly because of marked increase of lobectomy cases. 3) Out of 180 cases of empyema in the former series, 17 died within 30 days. 4) The 5-year survival rate of the patients with empyema was 28 per cent. 5) The 5-year survival rate was 30 per cent in 93 cases of squamous cell carcinoma, 30 per cent in 50 cases of adenocarcinoma and 15 per cent in 13 cases of large cell carcinoma. There was no survivor among 7 cases of small cell carcinoma. 6) Out of 259 cases of empyema, micro-organisms were cultured in 161 cases, were negative in 20, and unknown in 78. Although various organisms were found, gram-negative bacteria and Pseudomonas were most frequently isolated, i.e. 37 per cent and 36 per cent, respectively. 7) As for the treatment of 5-year survival cases of empyema, thoracoplasty and bronchial closure were frequent as compared with open thoracic window technics such as Eloesser's and Symbas' operation. And, more than half of the cases of empyema seem to have been complicated with bronchial fistulae.

Med., 1982, 138 (2), 187-197 Since Kahn et al. reported in 1976 insulin resistant diabetes due to anti-insulin receptor antibodies, an unusual form of diabetes mellitus (type B) has been found in many countries. We had two diabetic patients with anti-insulin receptor antibodies, associated with either acanthosis nigricans or systemic lupus erythematosus. Remission occurred 15 months after the onset of insulin resistant diabetes. One patient unfortunately died of acute pneumonia and the other has been followed up. The anti-insulin receptor antibodies were measured according to the method of Omori and Hirata by using the pellet of human placental membrane. The anti-insulin receptor antibodies in both cases diminished as remission occurred. Reverse hemolytic plaque assay (PFG) detected immunoglobulin-producing cells. In Case 2, the plaque forming cells were twenty times as many as the normal value. Immunosuppressive therapy with cyclophosphamide reduced the immunoglobulin secreting cells as remission occurred. The patients with insulin resistance (type B) should be treated with enough insulin inspite of the presence of insulin resistance. Besides, cyclophosphamide, 6-mercaptopurine and prednisolone should be used with caution. Plasma exchange is a treatment to be tried. It is important to note that spontaneous remission may occur more than half a year after the onset of insulin resistant diabetes.

Med., 1982, 138 (2), 199-208-We investigated the occurrence of delayed neurotoxicity in domestic fowl following percutaneous application of leptophos. Five groups of 5 adult hens received daily percutaneous doses of 1.0ml/hen of leptophos emulsion (leptophos; 340mg/hen/day) for 2, 5, 10, 15 or 20 days. There was no abnormal gait in the 2-day group. Two out of 5 hens in the 5-day group showed mild ataxia from about 2 weeks after the final administration, but did not develop severe neuropathy. On the contrary, 4 out of 5 birds in the 10-day group and all hens in the 15- and 20-day groups were affected by various stages of neurotoxicity. Some of them died from neurotoxicity. Ten of young male chickens were given the same dermal dose for 5 or 10 days. Although no abnormal chicken was observed in the 5-day group, all chickens in the 10-day group showed severe paralysis and two of them died. We studied the incidence rates of delayed neurotoxicity resulting from respective applications of the emulsion and the acetone solution of leptophos. No significant difference was observed between them. These results suggest that the daily dermal application of the relatively high dose of leptophos, even if for the short term, can cause the same delayed neurotoxic effects as by the oral administration in hens or chickens.

Long-Term Follow-Up of Patients Who Have Undergone Valve Replacements for Acquired Valvular Disease: A Clinical and Hemodynamic Study. Tohoku J. exp. Med., 1982, 138 (2), 209-222 Thirty-three cases, comprizing 5 of mitral regurgitation (MR), 11 of mitral stenosis (MS), 11 of aortic regurgitation (AR) and 6 of AR and mitral steno-regurgitation (MSR), underwent both preoperative and postoperative heart catheterization in association with a muscle exercise test. Comparisons were made between preoperative and postoperative clinical symptoms, hemodynamics and cardiac functions. The following results were obtained: 1) Improvements in clinical symptoms and hemodynamics at rest were observed in all but two patients. 2) Abnormalities in hemodynamics were obtained after the muscle exercise test, even in cases exhibiting normal hemodynamics at rest. 3) An increase in pulmonary artery mean pressure was noted in MR and MS groups after the muscle exercise test. 4) The cardiac function index values in both the MR and MS groups showed lower levels than those in both the AR and AR+MSR groups. 5) The presence of atrial fibrillation and extent of myocardial fibrosis were related to improvements in postoperative cardiac functions.

    TORINUKI, W., KUMAI, N. and MIURA, T. Chronic Photosensitive Dermatitis Due to Phenothiazines. Tohoku J. exp. Med., 1982, 138 (2), 223-226-A 74-year-old man had eczematous eruptions on sun-exposed areas. Photopatch tests were positive to chlorpromazine and promethazine, and the action spectrum for the photosensitivity was in the long-wave ultraviolet light (UV-A) range. It was concluded that the chronic photosensitive dermatitis was due to phenothiazines.

Med. 1982, 138 (2), 227-229 The relationship of lipoperoxidemia and HDL-cholesterolemia to circulating zinc and copper was investigated in patients undergoing chronic hemodialysis. When the serum zinc level was low and the copper level was high, or when the zinc/copper ratio was low, decreased concentrations of HDL-cholesterole were observed. In view of the high incidence of atherosclerosis among patients undergoing chronic hemodialysis, zinc and copper may somehow contribute to atherogenesis by affecting the serum HDL-cholesterol concentrations.

Med., 1982, 138 (2), 231-236-In healthy subjects aged 20 to 50 years, the urinary excretion of carnitine and its serum concentration increased rapidly and markedly after synthetic β1-24ACTH-Z was injected. Their serum triglyceride levels changed inversely. In contrast, healthy subjects older than 70 and patients aged 45 to 50 with atherosclerosis exhibited lower and delayed changes of carnitine excretion and serum concentrations of carnitine and lipid after ACTH injection. When the aged subjects and the atherosclerotic patients were administered with triiodothyronine prior to ACTH, their metabolic responses to ACTH improved towards normal. The results suggest that the aging process impairs the homeostatic regulation of serum lipid through a “lipid-carnitine” system and that thyroid hormone plays a contributory role in the activation of this system.

    NAKAMURA, S. Serological Markers of Hepatitis A and B Virus Infection in University Students. Tohoku J. exp. Med., 1982, 138 (2), 237-238-Serological markers of hepatitis A and B virus infection were determined by radioimmunoassay in 162 university students. Antibody to hepatitis B e antigen (anti-HBe) was demonstrated in 11 of 15 students positive for both antibody to hepatitis B e antigen (HBsAg) and antibody to hepatitis B core antigen (anti-HBc). The presence of anti-HBc alone seemed to denote previous hepatitis B virus infection. Antibody to hepatitis A virus was demonstrated only in 3%. Study of 23 stored HBsAg-positive sera showed high titer anti-HBc significantly more frequently in sera positive for anti-HBe than in sera positive for hepatitis B e antigen. Incidence of hepatitis B e antigen was significantly higher in male students with HBsAg, and that of anti-HBe in female students.
