Roentgenographically Occult Bronchogenic Squamous Cell Carcinoma: Location in the Bronchi, Depth of Invasion and Length of Axial Involvement of the Bronchus. Tohoku J. exp. Med. 1986, 148 (3), 241-256 - Forty patients with roentgenographically occult lung cancer underwent surgical resection. In thirty-five, cancer was detected by mass screening and in five at outpatient clinic. Bronchoscopic localization was accomplished in all cases. Serial block-sections of bronchi of all surgical specimens were prepared and examined microscopically. Forty-two lesions of bronchogenic squamous cell carcinoma were found including two of concurrent occult carcinoma. The site of tumor in the bronchi, the depth of mural invasion and the axial length of involved portion were investigated microscopically. The depth was measured with a micrometer. Fifteen lesions occurred in a subsegmental or a sub-subsegmental bronchus and 27 in a segmental or a central bronchus. Based on the depth of invasion (DI), occult bronchogenic carcinomas were classified microscopically into six categories (DI0 to DI5). Of the 42 lesions, 6 were judged as carcinoma in situ (DI0), 4 as suspicious invasion (DI1), 7 as intramucosal invasion (DI2), 17 as extramuscular invasion (DI3), 5 as extracartilaginous invasion (DI4) and 3 as extrabronchial invasion (DI5). Microscopic analysis revealed the presence of at least two types of infiltration in the bronchial wall. One was the creeping type in which carcinoma extended flatly along the superficial layers of bronchus and was presumed to stay as an occult lung cancer for a long time. Med., 1986, 148 (3), 257-266 - Sixteen cases with carpal tunnel syndrome (CTS) were reported out of 412 patients on long-term hemodialysis due to chronic renal failure. Clinical symptoms included numbness in the area innervated by the median nerve in 100% of symptomatic hands and pain in the wrist and hand in 81%. This pain usually became severer at night and during hemodialysis. Muscle atrophy was noted in 41% of the hands. Nerve conduction studies revealed prolonged distal sensory latency, slowed sensory nerve conduction velocity across the wrist and normal distal motor latency in 18%, prolonged distal motor latency in 51% and no response in motor or sensory stimulation in 31%. Evidence of denervation on electromyography was seen in 36% of the hands. Patients were conservatively treated avoiding daily activities precipitating the condition with volar wrist splint only at night in 18% and in 90% with steroid hormone injection in the carpal tunnel. Median nerve release was performed in 18% of the hands. Amyloid deposit was demonstrated in 3 of 4 operated hands. Although the relation between long-term hemodialysis and the occurrence of the amyloid deposition in the carpal tunnel has not been established, the present data along with other recent reports strongly indicate that amyloid deposit in the carpal tunnel on hemodialysis patients could be one of the most possible cause of CTS.View full abstractDownload PDF (3417K) -

Med., 1986, 148 (3), 267-273 - The effects of diphenylhydantoin and phenobarbital on lipids metabolism were studied by quantification of lipids in human serum with GC-MS. The ratios of palmitic acid to stearic, oleic and linoleic acids decreased in patients on diphenylhydantoin therapy. Increases of free palmitic, stearic and oleic acids concentrations as well as higher cholesterol ester levels were observed in patients receiving either diphenylhydantoin or phenobarbital. With respect to fatty acids composition of cholesterol esters and phosphatidyl-choline, higher concentrations of palmitic, oleic and linoleic acids were observed in patients on diphenylhydantoin and phenobarbital therapy. Although the concentration of palmitic acid in serum phosphatidylcholine remained the same for all groups, a marked decrease in stearic, oleic and linoleic acids concentrations in serum phosphatidylcholine was observed in diphenylhydantoin treated group, and oleic acid in phenobarbital treated group. These results suggested the relationship to lecithin-cholesterol acyltransferase activity by these drugs.View full abstractDownload PDF (684K) -

Med., 1986, 148 (3), 275-283 - Nine anti-herpes compounds, idoxuridine (IDU), bromovinyldeoxyuridine (BVDU), bromovinylarabinofuranosylurasil (BVaraU), carbocyclic bromovinyldeoxyuridine (C-BVDU), chloroethyldeoxyuridine (CEDU), acyclovir (ACV), dihydroxypropoxymethyl-guanine (DHPG), adenine arabinoside (Ara-A), and phosphonoformate (PFA) were examined for their inhibitory activities against the replication of twenty-six newly isolated clinical strains of varicella-zoster virus (VZV) in human embryonic fibroblast cell cultures. The order of (decreasing) efficacy was: BVaraU>BVDU ≥C-BVDU>CEDU≥IDU>ACV>Ara-A>DHPG>PFA. Tohoku J. exp. Med., 1986, 148 (3), 285-294 - In order to investigate the applicability of liver transplantation after warm ischemia, a partial auxiliary auto-transplantation of the liver after 30min of warm ischemia was carried out in 9 mongrel dogs. Among them, 6 dogs survived longer than 7 days. Liver functions, blood coagulability and histological changes were investigated before and after the transplantation. Serum GOT, LDH, and OPT levels were elevated immediately after transplantation, and returned to normal within a few days. Hemostatic tests also showed significant abnormalities after transplantation. Although PT and thrombotest became normal within a week, PTT and antithrombin III remained slightly abnormal for longer periods. Microscopically, vacuolization and degeneration of hepatocytes were observed after transplantation but they recovered completely within 4 weeks. Based on these results, it could be said that the livers subjected to warm ischemia for 30min ischemia were still acceptable for auxiliary transplantation.View full abstractDownload PDF (1410K) -

Med., 1986, 148 (3), 295-302 - The accumulation and efflux of nickel from three kinds of cultured pneumocytes were studied. Nickel rapidly penetrated the cells examined. The concentration of nickel in the cells was related to the volum of cytoplasm and surface area of the cells. Some metabolic inhibitors (NaN3, NaCN, NaAsO2) reduced the accumulation of nickel by up to 74% . Extracellular calcium regulated the accumulation of nickel into the cells. CaCl2 at 10mM inhibited partially the accumulation of nickel, while nickel accumulation was enhanced in calcium free medium. From the efflux experiment, nickel was shown to be distributed to three different compartments in the cells, i.e., cell membrane, cytoplasm and intracellular constituents. Three rate constants were determined as k1=1.67-5.33, k2=0.031-0.037 and k3=0.002-0.007min-1. These results suggest that cell membrane is readily permeable to nickel and that saturable nickel-binding sites might exist in the pneumocytes examined.View full abstractDownload PDF (783K) -

Med., 1986, 148 (3), 303-312 - The effects of paraquat (PQ) on cell growth, DNA, RNA and protein synthesis and on superoxide dismutase (SOD) activity were investigated in cultured pneumocytes of type II cell origin and human embryonic fibroblast cells of the lung. The incorporation of 3H-thymidine, 3H-uridine and 14C-leucine into DNA, RNA and protein, respectively, were all reduced to 25%-70% of the control by PQ at 10-3M, but not at 10-5 M nor 10-7M. The activity of SOD in the cells was increased to 130%-270% by 10-3M of PQ. Among the cells studied, A-549 cells, which were most resistant to the inhibitory effect of PQ on cell growth and the synthesis of DNA, RNA and protein had the highest induction of SOD activity by PQ. In contrast, L-2 cells in which the cell growth and the synthesis of nucleic acids and protein were most inhibited had the lowest induction of SOD activity by PQ. These results indicate that nucleic acids and protein synthesis are possible targets for lethal effects of PQ in the pulmonary cells, and that the specificity of PQ toxicity in pulmonary cell lines might be related to the ability of induction of SOD by PQ.View full abstractDownload PDF (916K) -

Med., 1986, 148 (3), 313-325 - To establish the role of hormone receptors in patients with polycystic ovary (PCO), PCO rats were prepared by treating with testosterone propionate (TP). Five-day old immature female rats were subcutaneously injected with 1.25mg TP in sesame oil. They were then killed at the age of 12 weeks. The ovarian receptors for LH and FSH as well as serum hormone levels were investigated in PCO rats and also in control rats at the various stages of the estrous cycle. The LH receptor binding in the TP-treated ovaries was elevated almost as high as that of proestrus control, and was observed to be higher than the other control. The FSH receptor binding of PCO rats was elevated to 173% of that of diestrus control, which showed the highest value throughout the cycle. Thus, the gonadotropin receptors in PCO rats appeared to be in an activated state. High levels of the receptor binding were due to an increase in receptor binding sites. Serum LH level was significantly higher than that of diestrus control but still remained lower than that of proestrus control. In contrast, FSH level was as low as that of diestrus control. Prolactin level was markedly elevated and 17- and 2-fold higher than that of diestrus and proestrus control, respectively. Estradiol level was higher than that of diestrus control, increasing to almost the same level during proestrus control. While progesterone level was largely depressed to 23 and 13% of that of diestrus and proestrus control, respectively, testosterone level was almost the same as that of diestrus control. From these results, it was suggested that tonic secretion of LH, low level of FSH, and markedly high levels of prolactin would increase the gonadotropin receptors and result in extremely low production of progesterone in rat ovaries. Clinically, elevated levels of the LH and FSH receptors may be a relevant ocurrence in PCO patients.View full abstractDownload PDF (2874K) -

Med., 1986, 148 (3), 327-331 - A single blind cross over trial was conducted of thyrotropin-releasing hormone treatment in 7 patients with spinocerebellar degenerations. Patients received daily 2mg of thyrotropin releasing hormone intramuscularly during the 2 week treatment period. Tohoku J. exp. Med., 1986, 148 (3), 333-334 - An antibiotic FOM was found to be preventive of the side effect of an antineoplastic drug CDDP on the peripheral T lymphocytes in lung cancer patients. Whereas finding a significant decrease in T3+ cells (p<0.01) and T4+ subset (p<0.05) with CDDP alone, the initial levels of these cell populations were kept unchanged in combination with FOM. T8+ subset showed no substantial change. A protective effect of FOM on T4+ subset was suggested to be an additional aspect of its action at least in combination with CDDP.View full abstractDownload PDF (205K)
