    ASAKI, S., IIATORI, S., NISHIMURA, T., SATO, A., HANZAWA, K. and GOTO, Y. Diagnosis of Submucosal Tumor by Injecting a Water Soluble Contrast Medium: Development Pattern and Growing of Tumors. Tohoku J. exp. Med., 1984, 143 (4), 385-395 - With the application of our endoscopical method of contrasted X-ray imaging, gastric submucosal tumors were studied with chief regards to their modes of growing up and patterns of development. It has become known that, of extragastric developing tumors, larger ones tend to show Pattern IVa and smaller ones Pattern IVb. Generally, the tumors showed a tendency of becoming larger in older patients. Periodical checking disclosed about 15% of the tumors growing larger with the lapse of time. It has been surmised that as tumors grow up, their development possibly turns from intra-gastric to intramural and mingled in pattern. Their modes of growing could be classified as follows: 1) Abruptly growing large at a certain time and then remaining without notable changes, 2) step-by-step enlarging, and 3) gradually growing up straight forwardly with years.

    SENBA, M. A Reliable Silver Staining Method for Identification of Pneumocystis carinii in Histologic Sections. Tohoku J. exp. Med., 1984, 143 (4), 397-404 - The acquired immune deficiency syndrome (AIDS) is characterized by the development of Kaposi's sarcoma and several opportunistic infections including pneumonia caused by Pneumocystis carinii. As a staining method for Pneumocystis carinii, the methenamine-silver nitrate method has been used routinely. Most of fungi can be stained easily by this technique but the identification of Pneumocystis carinii is not always easy. The author has improved the method by using ammoniacal silver nitrate and found that this was a reliable staining method for Pneumocystis carinii. Moreover, this method proved to be superior to others in demonstrating Pneumocystis carinii in histologic sections.

    KIMURA, I. and AYYAR, D.R. The Eaton-Lambert Myasthenic Syndrome and Long-Term Treatment with Prednisolone. Tohoku J. exp. Med., 1984, 143 (4), 405-408 - A 34-year-old women with the Eaton-Lambert myasthenic syndrome was treated with prednisolone for over a year. Proximal muscle weakness and breathing difficulty due to weakness in the respiratory muscles showed improvements after long-term administration of prednisolone. Electrophysiological studies before treatment revealed decreased amplitudes of the compound muscle action potential (CMAP), a decrement in CMAP to low frequency nerve stimulation, an increment on high frequency stimulation and posttetanic facilitation over 300%. Following administration of prednisolone, these electrophysiological parameters became almost normal. This case suggests that immunosuppressive drugs such as prednisolone may have a place in the management of the patients with myasthenic syndrome, especially of those who has no evidence of malignant tumor.

    MORITA, T., SUZUKI, T., KONDO, S., SAEKI, H, and TSUCHIDA, S. Response of the Renal Pacemaker to β1-Adrenoceptor Agonist: Difference from the Cardiac Pacemaker. Tohoku J. exp. Med., 1984, 143 (4), 409-412 - Electromyographic responses of the canine-pyeloureter to dobutamine, a β1-adrenoceptor agonist were investigated in vitro. Debutamine produced a marked enhancement of potential in the pelvicalyceal (PC-) border, but failed to evoke noticeable responses in the renal pelvis and ureter. Dobutamine, however, made the pacemaker rhythm a little slow. This finding shows that the renal pacemaker is somewhat different form the cardiac pacemaker, pace of which is quickened by dobutamine.

Med., 1984, 143 (4), 413-420 - Sera from 109 Japanese infants with pneumonia were tested for antibody to Chlamydia trachomatis(C. trachomatis) L2 strain by an indirect immunofluorescence (IF) technique. Nasopharyngeal swabs were also collected to isolate C. trachomatis. Clinical specimens were inoculated onto cycloheximide-treated McCoy cells and DEAE-dextran-treated HeLa 229 cells. Of 109 patients, 32 (29%) were positive for IgM antibodies (titer, ≥1:16) to C. trachomatis. C. trachomatis was isolated from 21 (66%) of 32 IgM antibody-positive infants as compared with 5 (7%) of 77 IgM antibody-negative infants. Detectable levels of IgM antibody were common in infants during the first four months of life. Clinical characteristics of pneumonia of these IgM antibody-positive patients were also described. This is the first report of serology and clinical characteristics of C. trachomatis pneumonitis from Asian countries including Japan.

Diagnosis of Submucosal Tumors by Injecting a Water Soluble Contrast Medium: Judgement of Tumor Developing Pattern and Endoscopic Lumpectomy. Tohoku J. exp. Med., 1984, 143 (4), 421-429 - With the remarkable increases in the number of cases of endoscopical removal of submucosal tumors spurred by the application of high-frequency current, it has become more and more important to apply beforehand some reliable methods like ours to differentiate between tumors and other tumescent lesions due to extra-gastric pressure similar in configuration to tumors and also to confirm the development patterns of tumors either intra-gastric or extra-gastric. Such preparatory arrangements seem indispensable to minimize the risk of perforation incidental to the endoscopical treatment of tumors. To cope with another danger of major bleeding following tumor resection, securing on hand an effective hemostatic is indispensable as well. In our care of the patients after their undergoing endoscopical lumpectomies, we usually subject them to quiet rest and fasting for two or three days; then when they are to resume a regular diet, we endoscopically examine them before and after each meal to confirm the presence or absence of bleeding. In 12 subjects of this series treated with our routine and finally with lumpectomies for gastric submucosal tumors, no serious adverse reactions developed.

Med., 1984, 143 (4), 431-439 - A 24-year-old male with chronic atrial standstill underwent an electrophysiologic study. No atrial activity was recorded from the right atrium which did not respond to stimulation with up to 10 volts. After the administration of atropine, non-localized split atrial potentials appeared, and intraatrial phase 4 block and atrial flutter with intraatrial 2:1 block were observed. The atrium then responded to electrical stimulation (less than 2 volts). Persistent sinus node activity with intraatrial block was found during post-pacing atrial pauses.

Influence of Various Experimental Conditions on the Inhibitory Effects of (E)-5-(2-Bromovinyl)-2'-Deoxy-uridine on Varicella-Zoster Virus Replication in Cell Culture. Tohoku J. exp. Med., 1984, 143(4), 441-449 - Inhibition of varicella-zoster virus (VZV) replica-tion by (E)-5-(2-bromovinyl)-2'-deoxyuridine (BVDU) has been examined in vitro under various experimental conditions. The 50% inhibitory dose (ID50) of BVDU for VZV replication in human embryonal fibroblast (HEF) cultures was 0.016μg/ml, if the assay was based on the reduction of visible foci. If the assay was based on the reduction of immunofluorescent foci, the ID50 was 3 times higher. There was no significant difference in ID50, whether the HEF cultures were infected with cell-free or cell-associated VZV. When the HEF cells were infected with VZV at different multiplicities of infection (MOI), the ID50 of BVDU increased in parallel with the increase of MOI. BVDU was normally added immediately after virus infection. However, the addition of BVDU could be delayed until 8hr after infection without substantial decrease of activity. On the other hand, BVDU did not cause any inhibition of focus formation when it was removed within 8hr after VZV infection. If removed at 24 or 48hr after infection, BVDU caused a significant reduction in focus formation. BVDU had no effect on focus formation if added to the HEF cells before virus infection. BVDU was also found to inhibit VZV focus formation in Vero cells, but only at an ID50 that was 5-10 times higher than the ID50 noted in HEF cells.

    Adenocarcinoma. Atypical Glands as a Basis for Histopathologic Diagnosis. Tohoku J. exp. Med., 1984, 143 (4), 451-465 - The basic architectural pattern of adenocarcinoma was established by 3-D reconstruction of carcinomatous glands and their lumina from serial histologic sections in four gastrectomy specimens to establish strict morphologic criteria of adenocarcinoma and related lesions on structural basis. The carcinomatous glands in the moderately differentiated type had multiple anastomoses with one another forming a 3-D network, a pattern quite different from the arborescent normal glands, whereas the lumina were separated into many small parts, giving the cell masses a porous character. Well differentiated tumor had more connections between lumina, giving rise to the formation of dense luminal network, while in the poorly differentiated variety the glands lost unity, broken up into fragmental nests. These analyses provided a new viewpoint from which to establish a basis for the discrimination of dysplastic from overtly malignant lesions.

Rapid Radioimmunoassay of Plasma GH and Its Application to the Judgement of Surgical Results in Patients with Acromegaly. Tohoku J. exp. Med., 1984, 143 (4), 467-471 - To improve the surgical results in patients with acromegaly, rapid radioimmunoassay (RIA) of plasma GH was developed and this method was applied for quick judgement of whether transsphenoidal hypophysectomy was performed successfully or not. The best reaction temperature and the shortest incubation time were proved to be 40°C and 16min, respectively. The GH values obtained by this method and the usual RIA method were correlated well (r=0.9, p<0.001). Using the rapid RIA it became possible within 35min just after the termination of the hypophysectomy to report the data to the neurosurgeons. This RIA method was applied for 5 patients with acromegaly undergoing transsphenoidal hypophysectomy. In two cases plasma GH decreased markedly (62.0-71.4% decreases from preoperative value) at the termination of the hypophysectomy, and later fell to normal ranges. The other three cases showed only slight decreases (7.7-17.3%), and the operation was judged as unsuccessful. Therefore, two of these cases were followed by further removal of residual tumors which resulted in steep declines of GH. In the remaining one the further operation was not performed, because surgical removal of the adenoma was believed by the surgeons to be complete. In conclusion, rapid RIA of plasma GH is possible within 16min and this method is of significant value to improve the surgical results of acromegalic patients.

Med., 1984, 143 (4), 473-480 - We have investigated the frequencies of sister chromatid exchange (SCE) induced by pulse-wave ultrasound in lymphocytes obtained from the human umbilical vein. The following results were obtained: 1) There is an increase in the frequency of SCEs with increases in acoustic power. 2) The critical acoustic power was 37.3-73mW/cm2 (spatial average temporal average, SATA). 3) Regardless of changes in pulse duration and repeated pulse frequency, if the acoustic power remains constant, the SCE frequency does not change significantly.

    ABE, T., SUZUKI, M., MORITSUKA, T. and BOTAN, Y. Statistical Factor Analysis and Cluster Analysis in the Etiology of Climacteric Symptoms. Tohoku J. exp. Med., 1984, 143 (4), 481-489 - Factor analysis and cluster analysis were applied to a set of 17 climacteric symptoms data obtained from 194 premenopausal and postmenopausal women. Six distinct factors were extracted and the women were divided into 7 groups by hierarchical cluster analysis in terms of their factor scores. Only one representative factor was identified in 3 groups out of the 7. These 3 groups including 87 women were tentatively designated as vasomotor symptom, neurotic, and sensory disturbance groups. These results confirmed statistically that at least 3 simple etiological factors exist among climacteric symptoms.

Med., 1984, 143 (4), 491-499 - We have investigated lectin-binding profiles of the human breast epithelia and the structural atypism in intraductal carcinoma. The lectino-histochemical probes using biotin-labeled peanut agglutinin (PNA), soybean agglutinin (SBA) and wheat germ agglutinin (WGA) with avidin-biotin peroxidase complex were performed on formalin-fixed and paraffin-embedded specimens from 24 patients. While normal and benign hyperplastic epithelia were stained almost in luminal borders and apical cytoplasmic surfaces, intraductal carcinoma additionally showed conspicuous staining of intracytoplasmic lumina and supranuclear cytoplasmic margins. The lectin-binding pattern found in papillary hyperplasia was completely different from that in intraductal carcinoma; the former was an irregular network structure along luminal borders, whereas the latter was not only a cribriform but also a rosette structure along intracytoplasmic lumina and supranuclear cytoplasms. Such lectin-binding patterns seemed to reflect the change of cell polarity in malignant transformation. The significance of these observations in elucidation of structural atypism was discussed in relation to 3-D structure of intraductal carcinoma.

    OKUYAMA, S. and MISHINA, H. Consecutive Bestatin-Hormone Regimen Induces Redifferentiation of Cancer Cells in Vitro. Tohoku J. exp. Med., 1984, 143 (4), 501-502 - When undifferentiated murine mammary adenocarcinoma cells were consecutively exposed to bestatin and estradiol or testosterone in vitro, morphological signs of redifferentiation of the cells were observed along with cellular enlargement and subsequent cell loss. These observations suggest that similar treatments can be applied clinically to cancers of the breast and prostate.

Med., 1984, 143 (4), 503-504 - A histopathological survey of surgical materials from 143 patients with rectal carcinoma subjected to chemotherapy and low-dose radiotherapy with tegafur and bleomycin was carried out. The chemotherapy combined with radiotherapy brought about better results than chemotherapy alone. In 4 of the 58 patients treated with radiochemotherapy, no cancer cells could be found in surgical specimens. Exfoliation into the rectal lumen of cohorts of cancer cells may occur in some cases of rectal carcinoma thus treated, resulting in cancer cell elimination.
