Tohoku J. exp Med., 1986, 148 (4), 335-339-Two of our 5 patients with neuronal ceroid lipofuscinosis excreted a large amount of autofluorescent lipids in their urine. These autofluorescent lipids consisted of more than 5 bands in the thin-layer-chromatography. Excitation and emission spectra of these lipids showed that they had heterogeneous components. This finding was specific to neuronal ceroid lipofuscinosis, and never found in other neurological diseases. We also measured unesterified dolichol and dolichol fatty acid ester in urine, and 4 of our patients did not excrete a large amount of such compounds compared with control groups. This finding suggest that increased excretion of dolichol in urine of patient with neuronal ceroid lipofuscinosis may be the secondary effect, but there are some possibilities that certain abnormalities in glycoprotein metabolism exist in neuronal ceroid lipofuscinosis.

Relevance of Anti-α-Fetoprotein Antibody for Radioimmunodetection: Analysis with α- Fetoprotein-Secreting Rat Hepatoma and Antigen-Bound Acrylic Beads. Tohoku J. exp. Med., 1986, 148 (4), 341-351-Using AFP-secreting AH66 rat hepatoma cells and AFP-bound acrylic beads (AFP-B), artificial cells expressing surface antigen, the effect of blood AFP level on the accumulation of radiolabeled rabbit anti-AFP antibody in the site of tumor and beads implanted in rats was examined. Scintigrams of hepatoma-bearing rats with implanted AFP-B showed a marked localization in AFP-B site, but not in the tumor site on day 5 after the injection of radiolabeled antibody. The imaging of AFP-B was most satisfactory in normal rats with extremely low blood AFP. Tissue/blood radioactivity ratio in AFP-B on day 5 was found to be significantly higher in normal rats (17.43±5.55), compared with 6.66±1.67 in the tumor-bearing rats (p<0.01), while the ratio of AFP-B in tumor-bearing rats remarkably recovered to 11.20+0.25 (p<0.01) after whole blood exchange to reduce an elevated serum AFP level. The accumulation of anti-AFP antibody in the tumor was not observed irrespective of AFP-B. Microautoradiography of tissue sections showed the specific localization of radioactivity on the surface of AFP-B but not on the surface or the inside of the tumor cells. Immune complexes were detected in plasma of tumor-bearing rats 1min to 7 days after the administration of the radiolabeled antibody. In conclusion, the application of anti-AFP antibody for the diagnosis and treatment of tumors is limited to the case of the expression of AFP on the surface of tumor cells and lower AFP level in circulation.

Med., 1986, 148 (4), 353-360-A monoclonal antibody was prepared by hybridizing mouse myeloma cells with spleen cells from the mouse which was immunized with human colon cancer transplanted in nude mice. The reactivity of the monoclonal antibody, named A7, was tested by immunoperoxidase method. A7 reacted strongly with human adenocarcinoma cell lines and carcinoembryonic antigen (CEA). In surgical specimens, A7 reacted with 10 cancer tissues and 2 normal colon mucosa from 19 colorectal cancer patients. A7 did not react with other cancers. It was thought that A7 reacted with colon- or colon cancer-specific CEA. The reactivity of A7 with colorectal cancers was markedly reduced by preoperative irradiation.

Med., 1986, 148 (4), 361-364-Practically all myeloma cells had prominent vacuoles, around and within which the end product of acid phosphatase (AP) reaction was demonstrated. These vacuoles were considered to be autophagic. In remission, only a few myeloma cells were vacuolated. Serial determinations showed transition from an initial high AP score (3.86) to a low score (1.32) in remission. Moreover, there was a trend towards a lower average percentage of positive cells in remission (72%) as compared to the preinduction phase (100%). Intracellular Bence Jones proteins of the present case may be toxic and capable of causing vacuolation with activation of lysosomal enzymes.

Tohoku J. exp. Med., 1986, 148 (4), 365-371-The metabolism of [choline-methyl-14C] sphingomyelin in cultured skin fibroblasts from patients with different types of Niemann-Pick disease was measured 1 and 3 days after uptake from the media. The cell lines obtained from type A disease had more than 95% unhydrolyzed sphingomyelin in situ on day 3 while two cell lines obtained from type B had 36.3% and 43.3% unhydrolyzed sphingomyelin on day 3. The cell line derived from one patient with the transitory type disease had 48.1% unhydrolyzed sphingomyelin on day 3, and there was no significant difference in the sphingomyelinase activity measured in vitro or in degradation of sphingomyelin in situ between the type B and transitory type disease. In three cell lines from patients with type C disease, there was 18.5%, 29.6% and 31.1% unhydrolyzed sphingomyelin on day 3, which indicates that this type has a decreased ability to metabolize sphingomyelin. Cell from type E disease metabolized sphingomyelin normally.

Med., 1986, 148 (4), 373-384-Complete transection of the midthoracic spinal cord was performed on adult female dogs. Development of reflexes and locomotive movements in the hindlimbs after spinal transection was investigated over several months. In the earlier stages after the surgery, dogs showed flaccid paralysis of the hindlimbs. Within several weeks, muscle tone of the hindlimbs was gradually increased accompanied by development of flexion reflex with after-discharge in addition to monosynaptic reflexes. Alternating stepping movement also began to develop. Afterward, extensor thrust and crossed extension reflex were observed. Standing behavior of the hindlimbs was found after enough development of the extensor thrust and correct placement of the pads of the toes. Steady development of stepping and standing caused forward locomotion using fore- and hindlimbs. This walking behavior of the hindlimbs was never inhibited by additional spinal cord transection. These findings indicate that the isolated spinal cord caudal to the transected site is responsible for the locomotive behavior of the hindlimbs in adult spinal dogs. Spinal automatism and development of responses induced by afferent inflow from outside the cord may help to explain such functional recovery of the paralyzed hindlimbs.

Med., 1986, 148 (4), 385-391-Two cases of citrullinemia were reported. Case 1 was an one month old female. Her clinical course and findings were different from the fluminant type of neonatal citrullinemia reported in predominantly Caucasian countries. Our patient was well controlled under a low protein diet and essential amino acids till 9 months of age, but unfortunately she died of Reye's like syndrome. Case 2 was 31 year old male (at the time of death). He was admitted to our hospital because of hyperammonemia and mental retardation. By subsequent laboratory investigations he was diagnosed as having adult type of citrullinemia and died of hepatoma. Enzymological analysis revealed that argininosuccinate synthetase (ASS) activities in the liver tissues of the patients decreased to 40% (Case 1), 20% (Case 2) compared with those in control liver tissues. The other urea cycle enzyme activities were all within normal range. ASS activities in the kidney and brains of the two cases were within normal range. The kinetic constant values of ASS for three substrates in the tissues of liver and kidney were all normal. Results of immunochemical analyses indicated that citrullinemia in our patients was caused by a quantitative deficiency of ASS associated proteins of the liver and kidney tissues as to the molecular weight.

Tohoku J. exp. Med., 1986, 148 (4), 393-401-The fasting serum levels of α-tocopherol were determined by high-pressure liquid chromatography in 13 patients with chronic pancreatitis of whom 7 were positive for pancreatic calcification (CCP) and 6, negative (NCP) and 10 healthy subjects. The fasting serum levels of α-tocopherol were significantly lower in patients with chronic pancreatitis (7.2±1.1μg/ml for CCP and 7.9±0.6 for NCP) than in healthy subjects (11.3±0.7μg/ml). Vitamin E absorption was determined in those with chronic pancreatitis and in healthy subjects after postprandial oral administration of 400mg of vitamin E, using soft capsules which contained tocopherol nicotinate along with an appropriate amount of a suspension of an ester of fatty acids with glycerol and middle chain triacylglycerol. The mean absorption of vitamin E was 12.7±2.0μg/ml•hr for healthy subjects, 9.1±3.1μg/ml•hr for CCP and 13.0±2.7μg/ml•hr for NCP, respectively. There was no significant difference in vitamin E absorption between patients with chronic pancreatitis and healthy subjects. Further, the rate of hydrolysis of tocopherol nicotinate did not significantly differ between healthy subjects and patients with chronic pancreatitis. It is of interest to note that vitamin E absorption in patients with chronic pancreatitis was increased by the postprandial use of an oily suspension type preparation of tocopherol nicotinate.

Med., 1986, 148 (4), 403-409-The occurrence of Bowen's disease on unexposed area of the body in a 68-year-old man with multiple myeloma is reported. Not only non-reticular but also reticular malignancy should be taken into consideration as internal malignancy associated with Bowen's disease. The previous reports of Bowen's disease associated with reticular malignancy are also reviewed.

Free Thyroxine Estimation for the Screening of Hyper- and Hypothyroidism in an Adult Population. Tohoku J. exp. Med., 1986, 148 (4), 411-420-Serum free thyroxine (FT4), total T4 (T4) and total T3 (T3) were determined by radioimmunoassay in 1, 114 adults during a periodic health evaluation to detect unsuspected thyroid dysfunction. As a result, 3 patients with hyperthyroidism, 4 with hypothyroidism, 3 taking thyroid medication, 4 with chronic thyroiditis, 1 with simple goiter and 3 women under estrogen administration were found. Free T4 values were within the normal limits in the present four subjects taking estrogens, but all of them showed high T4 and one high T3 values. FT4 and T4 were low in four patients with hypothyroidism, but two of them showed normal T3 values. For the screening of hyperthyroidism, FT4 was the most reliable measurement and determination of either FT4 or T4 was suitable for the screening of hypothyroidism, but T3 measurement did not diagnose all patients with hypothyroidism.

Pharmacokinetics and Pharmacodynamics of Conventional and Slow Release Forms of Nifedipine in Essential Hypertensive Patients. Tohoku J. exp. Med., 1986, 148 (4), 421-438-In patients with essential hypertension, the pharmacokinetics of nifedipine in 2 forms (capsule, 10mg nifedipine dissolved in an organic solvent; slow release tablet, 20mg nifedipine) and their pharmacodynamic effectiveness on arterial pressure were studied. The maximum plasma concentration was higher and achieved more rapidly after application of the capsule than after the retard tablet (p<0.01). The plasma half-time of nifedipine in the capsule was shorter than that in the retard tablet (p<0.05). The absorption rate of nifedipine from the capsule tended to be larger than that from the retard tablet. Plasma concentrations of nifedipine, measured 12hr after the dosing of the retard tablet during chronic treatment, were not different from those after the acute administration of the retard tablet, suggesting that no accumulation of nifedipine occurs. The capsule of nifedipine elicited prompt and profound hypotension with short uration, while the retard tablet produced a hypotensive effect with relatively slow onset and long duration. Arterial pressure reduction was maintained throughout the day using 12 hourly dose of the retard tablet. During the chronic treatment the maximum hypotensive effect was observed 4 weeks after the start of treatment. Twelve hourly administration of the retard tablet is a convenient regimen for the long-term control of mild to severe essential hypertension.

Med., 1986, 148 (4), 439-447-Using a radioimmunoassay for atrial natriuretic peptide (ANP) we studied plasma concentrations of immunoreactive ANP in order to investigate the pathophysiological role of ANP in patients with various diseases. Plasma ANP levels were elevated in patients with congestive heart failure (394±260pg/ml, n=8) and chronic renal failure (219+86pg/ml, n=11). In patients undergoing hemodialysis plasma ANP levels were markedly high and decreased after hemodialysis from 433±166pg/ml to 204±92pg/ml (n=11). ANP was removed from blood to dialysate (21±13pg/ml of dialysate, n=6, dialysate flow: 500ml/min). Plasma ANP level was conversely correlated with creatinine clearance (r=-0.812. p<0.001) in patients with renal diseases (n=29). In patients with atrial fibrillation, pace maker implantation, lung disease, chronic glomerulonephritis, nephrotic syndrome, essential hypertension, liver disease and cerebrovascular disease, plasma ANP levels were not significantly different from those in normal subjects (70±32pg/ml, n=28). These results suggest that ANP may be a circulating hormone playing pathophysiological roles in congestive heart failure and chronic renal failure.

Relapsing Process in FAB Subtypes of Adult Acute Leukemia and Its Relationship to Treatment Regimens. Tohoku J. exp. Med., 1986, 148 (4), 449-458 The relapsing process in the bone marrow was studied in those 77 patients with adult acute leukemia, diagnosed according to the FAB classification who achieved complete remission (CR) and then received intermittent multi-drug intensification treatment. Relapse occurred in most of the patients who exhibited Auer rods or Ph1 chromosomes in the bone marrow, or in whom blasts increased to 8% or more, but some patients remained in CR by subsequent treatment, that is, relapsing process was reversible. With our conventional treatment, the relapse or relapsing process occurred in most of the patients with L1, L2 and M1 subtypes within 6 months and was irreversible. It occurred mainly between 5 and 13 months in those with M2, M3 and M5 and was reversible in some cases. Patients with M4 subtype received intensified treatment due to the difficulty of achieving remission; relapse was seen in only 3 of 7 cases. To prevent relapse and attain a potential cure, the treatment should be intensive before the relapsing process with adequate supporting care. In view of the above-mentioned observations, our new treatment protocols for acute leukemia were designed to be more intensive than those conventionally employed, and to be discontinued within approximately 10 months in lymphoblastic leukemia and approximately 8 months in myeloid leukemia

Med., 1986, 148 (4), 459-460-Gastrin (10 μg/ml) enhanced 14C-leucine uptake in cultured tissue of stomach and colon carcinomas and content of 14C-labelled protein in the medium was increased. One of 2 stomach and one of 3 colon carcinomas transplanted into nude mice were enhanced in growth by pentagastrin injected in a dose of 250μg/kg every day. On the other hand, secretin (100U/kg) inhibited the trophic action of pentagastrin.
