    The objectives of this analysis are to investigate if counting the number of dishes consumed per day is a rapid procedure for estimating daily nutrient intake in China, and to explore if urban-rural differences exist in the daily number of dishes. A nutritional survey (including dish number counting) was conducted on 499 adult women in six urban and four rural communities in China. The number of dishes was an influential variable in estimating intake of protein, animal protein, fat, animal fat, and some vitamins (e.g., vitamin B2) and minerals (e.g., zinc), but not of energy and carbohydrate. Intake of some nutrients (e.g., protein) was sufficient when people consume more than 10 dishes per day. Thus, the number of dishes is a useful indicator of sufficient intake of animal protein and fat as well as some micro-nutrients. The application of the dish number counting method showed that there was a significant difference between urban (11.9 dishes per day) and rural populations (7.9 dishes).

    With improvement in immunosuppressive drugs, the beneficial role of donor-specific blood transfusion (DST) in the preconditioning of renal allograft recipients has been diminished. This retrospective study was conducted to investigate the influence of DST on long-term graft survival in successful one haplotype-mismatched kidney transplantation in the cyclosporine (CsA) era at Iwate Medical University. Between August 1983 and October 1996, 52 one haplotype-mismatched living related first renal transplants were performed. Fifty grafts survived beyond the first year after transplantation. These 50 patients were divided into two groups according to maintenance immunosuppression, 12 kidney graft recipients received azathioprine (AZA), prednisolone (PSL), CsA, and DST, and 38 recipients received AZA, PSL and CsA. Our DST protocol consisted of three transfusions of 30 ml of donor-specific buffy-coat at 4-week intervals, without immunosuppressive coverage. In recipients receiving DST and CsA, the 5-, 10-, and 13-year graft survival rates were 100%, 83%, and 67%, respectively. In recipients without DST, the 5-, 10-, and 13-year graft survival rates were 95%, 74%, and 69%, respectively. There was no significant difference between the two groups in long-term graft survival. In conclusion, DST and CsA combination treatment in our protocol may not induce long standing donor-specific immunologic hyporesponsiveness. Other strategies are expected to induce immunotolerance.

    Recently, we found that hyperosmolality acutely inhibited atrial natriuretic peptide (ANP) dependent cGMP production by reducing ANP binding sites in cultured rat inner medullary collecting duct (IMCD) cells. Therefore, the present study was undertaken to evaluate the chronic effect of hyperosmolality on ANP dependent cGMP production in IMCD cells. Cell culture was carried out either in an iso-osmotic or hyperosmotic solution consisting of equi-isomolar NaCl and/or urea. Incubations with ANP and/or other agents were performed under the same osmotic conditions. ANP or SNP stimulated cGMP production was enhanced in a chronically hyperosmotic medium. These changes occurred in an osmolality-dependent manner. Hyperosmolality with sodium alone or with sodium and urea, but not with urea alone, was effective for the enhancement of ANP action. There was no significant difference between 125I-ANP specific bindings under iso-osmotic and hyperosmotic conditions. Incubation with cytoskeleton modulators did not affect ANP-dependent cGMP production stimulated by hyperosmolality. On the other hand, both actinomycin D, an inhibitor of transcription, and cycloheximide, an inhibitor of protein synthesis, prevented the stimulatory effects of hyperosmolality. The results suggest that chronic hyperosmolality enhances ANP-dependent cGMP production via stimulation of transcription and protein synthesis in cultured rat IMCD cells.

    In this study, we determined curcumin’s anticancer and chemopreventive effects in mice bearing Ehrlich ascites tumor by evaluation of cancer biomarkers, sialic acid level and sialidase activity. Both plasma sialic acid level and sialidase activity increased significantly in the mice group with Ehrlich ascites tumor. When the tumor groups fed with curcumin and fed with sesame oil were compared, sialic acid level and sialidase activity in ascites fluid significantly reduced in the group fed with curcumin in addition to the increases of plasma sialic acid level and sialidase activity. The tumor group fed with curcumin lived twice longer than the one fed with sesame oil. Curcumin as a phenolic compound decreased all these parameters in Ehrlich ascites tumors and lengthened survival by 88% in the mice with tumor. We concluded that curcumin has anticancer activity.

    Medial superior cluneal nerve entrapment neuropathy causes pain radiating from the low back down to the posterior thigh. It tends to be misdiagnosed as a lumbar spine disorder. Patients in previous reports were in the middle or old age at the onset. Proposing simultaneous full flexion of the ipsilateral hip and knee joints as a provocation test, we present two cases of teenager females who spent long before diagnosis of their condition. Both of them had engaged in vigorous sports activities and completely recovered from the disability following local anaesthetic and corticosteroid injection at the trigger point.

    Cholesterol granuloma is usually associated with middle ear disease and is very rare in the paranasal sinuses. We report a case of cholesterol granuloma originating in the maxillary sinus of a 52-year-old female. Endoscopic sinus surgery was performed on the left maxillary sinus, and the cholesterol granuloma was successfully removed by the middle meatal antrostomy. Light microscopic examination showed granulomatous tissue with typical cholesterol clefts, multinucleated foreign body giant cells, small areas of hemorrhage, hemosiderin-laden macrophages and plasma cells. We also describe the details of the endoscopic surgical techniques employed in the treatment of this disorder.

    Bilateral congenital inferior tibiofibular diastasis is an extremely rare anomaly of unknown etiology. A few cases have been reported previously. The author presents a case in which bilateral tibiofibulotalar arthrodesis was performed.

    In the present study, 1476 adult women in 6 prefectures in Japan volunteered to offer peripheral blood and spot urine samples, and to complete questionnaires on social habits and health. Blood samples were analyzed for iron, ferritin and TIBC in serum in addition to RBC, Hb and Cd in whole blood. Urine samples were analyzed for Cd, α1-MG, and β2-MG; the measures were corrected for creatinine and were expressed as e.g., Cd-Ucr. Among 1212 never-smokers, 37 women with <25 ng ferritin/ml serum and <10 g Hb/100 ml blood were classified as the anemics, whereas 701 women with ≥25 ng/ml ferritin and ≥10 g/100 ml Hb were taken as controls. Matching by age and the prefecture of residence was successful for 34 anemics. Comparison (by paired t-test) of Cd in blood, and Cd, α1-MG and β2-MG in urine (as corrected for creatinine) of the anemics with that of matched controls showed no significant differences. Thus, it appeared likely that the current level of iron insufficiency among general women population in Japan may not induce substantial increase in Cd absorption or Cd-associated kidney dysfunction.
