    SAGISAKA, K., IWASA, M. and YOKOI, T. On the New Secretor Status Recognized by Saliva Specific ABH Antigens. Tohoku J. exp. Med., 1984, 143 (3), 255-259-The present authors have reported that each of ABH antigens of saliva consisted of saliva specific antigen and common antigen between saliva and red cells. Using saliva specific anti-A and -B, 77 samples of A, B and AB groups could be determined as strong secretor (Se) or weak secretor (se). Frequency of Se was 62% for A group saliva, 45% for B, and 76% for A antigen and 75% for B antigen of AB group saliva, respectively. The secretor status of twenty-four O salivas could not be determined by saliva specific anti-H. Compared with the ordinary secretor system, some subjects belonging to Se in the ordinary system were typed as se in the new system, whereas all se saliva (three cases) in the ordinary system were typed as se in the new system.

    MORI, K., SUZUKI, S. and SUGAI, K. Electron Microscopic and Functional Studies on Platelets in Gray Platelet Syndrome. Tohoku J. exp. Med., 1984, 143 (3), 261-287-The ultrastructure and functions of platelets and megakaryocytes in 8 patients of a family with gray platelet syndrome were investigated. Hemostatic examinations on these patients revealed prolonged bleeding time, decreased platelet retention rates and decreased platelet aggregation rates induced by ADP, collagen, Ristocetin and epinephrine. Marked decreases in ATP and ADP release in response to these agents were also noted. Clinical and coagulation studies on this family suggested that the hereditary nature of the syndrome is autosomal dominant. Platelets and megakaryocytes in the peripheral and bone marrow blood smear from the patients showed peculiar gray color by May-Giemsa stain due to a deficiency of α-granules. Electron microscopic examinations revealed slightly enlarged platelets containing a deficient amount of α-granules, whereas dense bodies and mitochondria appeared normal. Several morphological abnormalities of patient's platelets, such as aggregates of dense tubular systems, circular arrays of dense tubular systems, an area of cytoplasmic sequestration with an enclosing membrane, clumps of dense material and remnants of Golgi apparatuses were recognized. Megakaryocytes showed normally developed Golgi zones, defective α-granule synthesis and liberation of abnormal platelets as shown in the peripheral blood smear.

    NAGAI, N., TAKAYAMA, S., HIRATA, Y. and MARUYAMA, H. Classification and Heredity of Diabetes among Japanese Patients with Diabetes Found before 30-Year- Old. Tohoku J. exp. Med., 1984, 143(3), 289-294-Classification and family history of diabetes were examined in 551 Japanese patients whose diabetes were found before the age of thirty. These patients were treated at the Diabetes Center of Tokyo Women's Medical College from 1976 to 1981. Among those 551 diabetics, 198 patients were classified into insulin-dependent diabetes mellitus (IDDM) and 337 into non-insulin-dependent diabetes mellitus (NIDDM). IDDM was found mainly in the group of diabetics whose onset of diabetes was before the age of fifteen. The number of NIDDM was significantly increased after the age of twenty. Family history of diabetes was much less in the IDDM group than in the NIDDM group in the first degree relatives. To compare the family history of IDDM with that of NIDDM, 148 diabetics were selected from those 551 patients. The selected diabetics had the onset of the disease before 20 years of age, their present age was between 20 and 40, and the duration of diabetes of these patients was three years or longer. Six of the selected 75 diabetics with IDDM had parents with NIDDM and two of these 75 patients had siblings with IDDM. Thirty-nine of the selected 73 diabetics with NIDDM had positive family history of NIDDM alone. And the selected diabetics with earlier onset of IDDM showed higher risk for IDDM case in their siblings.

    MIZUSHIMA, Y., WEPSIC, T., DESILVA, M.A., JANNS, G. and LARSON, C.H. Comparative Study on Tumor and Adjuvant (BCGcw)-Induced Nonspecific Suppressor Cells in Rats. Tohoku J. exp. Med., 1984, 143 (3), 295-304-Comparative studies on tumor and adjuvant-induced depression of in vitro mitogen responses were carried out using spleen cells obtained from syngeneic tumor bearing (TB) ACI rats or from rats which had been immunized with BCG cell walls attached to oil droplets (BCGcw). These in vitro studies demonstrated that: 1) the spleen cells from TB rats (TB-spleen cells) showed strongly depressed mitogen responses to concanavalin-A (Con-A), phytohemagglutinin-P (PHA-P) and lipopolysaccharide (LPS), 2) the mitogen response of lymph node cells from TB rats was slightly depressed, 3) the removal of plastic or nylon-wool adherent cells or phagocytic cells from TB-spleen cells resulted in a restoration of the mitogen response, 4) the Con-A response of normal spleen cells could be suppressed by the addition of TB-whole spleen cells, 5) the suppressor cell activity was not abrogated by the in vitro treatment with x-irradiation (2000 rails), 6) carbonyl-iron treated TB-spleen cells showed a normal level of mitogen response, and on addback to normal spleen cells no suppressive activity was detected in them. Similar results were observed when spleen cells were obtained from BCGcw immunized rats. These results suggest that in ACI rats tumor-induced nonspecific suppressor cells detected by in vitro assay are the same cell populations as BCGcw-induced nonspecific suppressor cells.

Med., 1984, 143(3), 305-313-Wistar female rats housed under conditions of 12hr dark/12hr light were pinealectomized (PX) or underwent sham-operation (SO) 21 days after ovariectomy, on the 7th or on the 15-17th day of pregnancy. Serum and pituitary prolactin (PRL) levels in ovariectomized (OVX) rats were determined 9 days after pinealectomy. In the case of OVX rats receiving estrogen and progesterone injections (OVX-EP), PRL levels were determined 48hr after injection administered 7 day after pinealectomy. PRL levels in pregnant rats were determined on the 20th day of pregnancy and in postpartum rats, on the 3rd day following parturition. As compared with the SO control, pinealectomy resulted in a significant decrease in the serum PRL level in the OVX-EP rats but in a significant increase in that level in the OVX, pregnant and postpartum rats. In OVX-EP rats, exogenous estrogen raised the serum PRL level less in PX than in SO rats, probably because the pineal gland is closely related to the facilitation of PRL secretion by estrogen. The high estrogen level in OVX-EP rats seemed to trigger pineal stimulation of PRL release, but low estrogen levels in OVX and postpartum rats or markedly high levels of progesterone in pregnant rats on the 20th day are thought to cause pineal inhibition.

The Effect of Protamine on Serum Complement Activity in Aminonucleoside Nephropathy. Tohoku J. exp. Med., 1984, 143 (3), 315-318-Combined administration of protamine sulfate (Ps) and aminonucleoside (AN) to rats causes severe nephrotic syndrome and, histologically, focal glomerular sclerosis. These changes are more distinct than those produced by AN alone. While the exact mechanism of Ps acting on AN nephropathy is not known, marked hypocomplementemia was observed regularly after the Ps+AN injection, suggeting that complement is activated and consumed in the kidney. It is suggested that complement may play an important role in AN+Ps nephropathy.

Med., 1984, 143 (3), 319-326-Indomethacin, an inhibitor of prostaglandin synthesis, depressed the incorporation of [3H] uridine into RNA of mouse preimplantation embryo, when it was injected in vivo or added in vitro. In contrast, prostaglandin F decreased the inhibitory effect of indomethacin, when it was added in vitro with indomethacin. The results of these experiments suggest that prostaglandin F plays a role in the process of implantation and that the mouse preimplantation embryo may be able to produce prostaglandin F.

Angiotensin II Analogue Infusion Test in Renovascular Hypertension under Low Sodium Intake and under Spironolactone Administration: Is Angiotensin II Analogue Infusion Test Useful in Determining the Mode of Treatment ? Tohoku J. exp. Med., 1984, 143 (3), 327-333-An angiotensin II antagonist (1-Sar, 8-Ileu-angiotensin II) was infused into 5 hypertensive patients with unilateral renal artery stenosis under 5-day low sodium diet (2g NaCl/day) and under 7-day spironolactone administration (300mg/day). In the sodium depleted state, 1 case showed depressor response, 2 cases pressor response and the other 2 no response. During the spironolactone administation, 3 cases showed depressor, 1 case pressor, and anothor case no response. Angiotensin II analogue (A II A) infusion test under spironolactone administration was more effective than under sodium depletion. However, there were cases which showed pressor response, i.e. false negative, under low sodium diet and under spironolactone administration. The hypertension of these patients was cured by converting enzyme inhibitor, percutaneous transluminal renal angioplasty (PIRA) or bypass surgery. A II A infusion test has been used for screening, diagnosis and determination of the surgical repair of renovascular hypertension. However, the proportion of depressor response was low in our cases, yet pressor responders and non-responders were also cured by PIRA. PIRA is a painless and low risk procedure of the treatment of renovascular hypertension, so PTRA will become the preferential mode of therapy for the treatment of renovascular hypertension of the cases with no depressor response by AIIA infusion.

Med., 1984, 143 (3), 335-344-Twenty-two vagotomized peptic ulcer patients were studied on the influence of vagotomy on the release of GIP. Moreover, the relationship between acid output and GIP release was analyzed. These patients underwent one of the three types of operation; truncal vagotomy with pyloroplasty (TV+P) selective vagotomy with pyloroplasty (SV+P) and selective vagotomy with antrectomy (SV+A). Before and after surgery, a gastric juice study and an oral glucose tolerance test were performed on separate days. Maximum acid output in response to tetragastrin correlated significantly with integrated GIP response after oral glucose loading. In the SV+P group, the response of GIP was slightly greater after surgery. In the SV+A group, the integrated GIP response diminished postoperatively, although, after surgery, GIP reached its peak sooner. The integrated GIP response was greater in the SV+P group than in the TV+P group. The response of GIP was less in the SV+A group than in the SV+P group. These changes were insignificant. In conclusion, it is presumed that GIP release is affected by gastric acid, gastric emptying time, intestinal transit time and vagal nerve action.

    MATSUE, Y. and OKUMA, T. Relative Advance of Eye Movement to the Target in the Rightward Tracking in Schizophrenics. Tohoku J. exp. Med., 1984, 143 (3), 345-349-Time relationship between the horizontal smooth pursuit eye movement (SPEM) and tracking target in the leftward or rightward tracking was investigated in 28 normal and 25 chronic schizophrenic subjects. Normal subjects were more liable to show a relative advance of SPEM in relation to the target in the leftward pursuit than in the rightward pursuit. These findings suggest that the spatial task such as SPEM depends more on the right hemispheric function in normal subjects. On the other hand, many of schizophrenic subjects showed a relative advance of SPEM to the target in the rightward pursuit. These findings suggest the dominance of the left hemispheric function in eye tracking tasks in schizophrenic subjects. The finding in the present study would support the hypothesis of the left hemispheric overactivation in the schizophrenic patients.

Med., 1984, 143 (3), 351-359-The effects of hypothermia upon electrocochleogram (ECochG) and auditory evoked brainstem response (ABR) were studied in 10 children who underwent the open heart surgery under hypothermic anesthesia. The latencies of N1, waves III and V in response to click of 115 dB p.e. SPL were prolonged at the rate of 0.12±0.02, 0.25±0.04 and 0.43±0.05msec/°C, respectively. The N1 amplitude was initially increased, returned to the precooling value at about 26°C and decreased thereafter. SP was increased in amplitude by hypothermia and never disappeared throughout the observation. At the time of rewarming, SP appeared first, followed by N1 and finally by waves III and V. The results obtained in the present study were discussed in the light of the sensitivity of the sensory hair cells, peripheral nerves and CNS to the hypothermic condition.

Direct Measurement of Urinary Bile Acids of Infants by High-Performance Liquid Chromatography Connected with an Enzyme Immobilized Column. Tohoku J. exp. Med., 1984, 143, (3), 361-371-An accurate and sensitive method by high-performance liquid chromatography connected with 3α-hydroxysteroid dehydrogenase immobilized column for the measurement of urinary bile acids of noncholestatic and cholestatic infants was performed in this study. The purification and group separation of urinary bile acids were done through a piperidinohydroxypropyl Sephadex LH-20 column. By ultraviolet spectromonitor, non 3α-hydroxy bile acids such as 3β-hydroxy-5-cholenate were determined, and 3α-hydroxy bile acids by fluorescence spectromonitor. Urinary bile acids in the specimens obtained from noncholestatic and cholestatic infants were determined by this procedure. Major bile acids were cholate, chenodeoxycholate conjugates and cholate unconjugate in healthy infants. In cholestatic infants, total and individual bile acids were extremely increased, and 3β-hydroxy-5-cholenate conjugates appeared in the sulfated fraction.

Med., 1984, 143 (3), 373-379-Urometry provides a valuable test of ureteral function wherein intraureteral pressure changes associated with ureteral peristaltic contraction are recorded via a fine catheter inserted through the bladder into the ureter. An attempt was made to evaluate the function of the intramural ureter by analysis of isometric wave pattern of resting pressure recorded during the phase of no ureteral peristaltic contraction with continual instillation of sterile water via the catheter at a constant rate. By this technique, the length of functional intramural ureter (FUimL) was estimated to be 10-15mm and the resting pressure in the intramural ureter (Pr) was between 5 and 10mmHg in patients with possibly normal intramural ureter (4 ureters). In cases of chronic dysuria with cystographic evidence of morphologic changes in the bladder and with no vesicoureteral reflux (VUR), the FUimL was 5mm and the Pr 2-3mmHg. The corresponding values were 5mm and 1mmHg with a ureter showing VUR. The data obtained indicated that the product of these parameters, FUimL×Pr, may reflect the function of intramural ureter. The method reported herein being procedurally simple requiring no particular apparatus provides a remarkably valuable test for the evaluation of function of intramural ureter which has been virtually impracticable.

Med., 1984, 143 (3), 381-382-The yields of platelet-rich plasma (PRP) obtained by centrifugation of CPD (citrate-phosphate-dextrose) blood and ACD (acid-citrate-phosphate) blood were compared. The volumes of PRP from 5ml blood in test tubes and 200ml blood in bags were larger by 4% and 4.5%, respectively, when CPD was used as an anticoagulant. In addition, the number of platelets in PRP from CPD blood was higher than that from ACD blood. These data suggest that the optimal centrifugal condition for CPD blood should be different from that for ACD blood.

Med., 1984, 143 (3), 383-384-The number of sodium pump units of erythrocytes measured with ouabain binding assay was significantly lower in 17 patients with essential hypertension (0.538±0.020pmol/109 cells) than in 13 normotensive controls (0.673±0.031pmol/109 cells) (p<0.01) and it was inversely correlated with erythrocyte sodium concentration (r=-0.86, p<0.01).
