    HORIE, K., HOSHI, H., HAMANO, K. and KANEKO, T. Morphological Changes in the Mouse Popliteal Lymph Node after Local Injection of Dextran Sulfate. Tohoku J. Exp. Med., 1994, 172 (3), 175-193-Dextran sulfate 500 (DS) is known to be a potent B-cell mitogen. Injection of this compound (500μg) into the footpads of mice produced characteristic changes in the draining popliteal lymph nodes, including elimination of macrophages from the lymphatic sinuses and lining the subcapsular sinus, a marked blast reaction throughout the nodal parenchyma, damage to the stroma of the outer peripheral cortex, and disorganization of all the existing primary and secondary lymph follicles. On day 6 after DS treatment, small lymphocytes bearing surface Ig were detected and distributed loosely and broadly in the deeper part of the peripheral cortex without any tendency to reorganize lymph follicles. On day 21, these small lymphocytes populated the outer and deeper layer of the peripheral cortex, sometimes forming nodular accumulations representing regenerating lymph follicles, and on day 35, various numbers of regenerated lymph follicles were present in the peripheral cortex. Tracer studies using FITC-conjugated human serum albumin showed that intraparenchymal diffusion of labeled HSA was highly blocked at the outer peripheral cortex on days 4 through 14, and that this pathway began to reopen at around 21 days. Disorganization of lymph follicles could be considered due to extensive damage to follicle stromata which became unable to bind B lymphocytes, and follicles seem to be re-organized in response to lymph-carried antigen in addition to restored follicle stromata and repopulating B lymphocytes. Macrophages had little effect on the lymph follicle formation in this study.

Morphological Analysis of Multinucleated Giant Cells Occurred in Experimental Autoimmune Myocarditis. Tohoku J. Exp. Med., 1994, 172 (3), 195-204-Our previous study reported the rich existence of multinucleated giant cells in an autoimmune myocarditis experimentally induced in rats. The present study investigated the histochemical and ultrastructural characteristics of these giant cells. Histochemistry for an acid phosphatase clearly demonstrated multinucleated giant cells dispersed at the inflammatory foci. Ultrastructurally, the giant cells were shown to be single cells, but not clustered cells. Their ultrastructural characteristics were very similar to the basic features of macrophages, except that the giant cells were poor in lysosomes and phagosomes. It was noticeable that some macrophages possessed three or more nuclei, displaying an intermediate form between mononuclear macrophages and multinucleated giant cells. These findings suggest that the giant cell in the experimental autoimmune myocarditis is a single multinucleated cell, and possibly derived from macrophages by cell-to-cell fusion.

Results of Multi-Centric Retrospective Studies. Tohoku J. Exp. Med., 1994, 172 (3), 205-208-The efficacy of long term consecutive treatment of clomiphene citrate (CC) was investigated in 608 infertile women associated with anovulatory disorders (classified as WHO group II amenorrhea). This study was performed in multi-centric facilities of obstetrics and gynecology by a retrospective randomized assessment. Infertile women associated with anovulation were treated by 50-150mg of CC for five consecutive days in each treatment cycle. In this study, 200 pregnancies were observed and the total pregnancy rate was 32.9%. The number of abortions were 33 out of the 200 pregnancies (16.5%). The cumulative pregnancy rate within the pregnant subjects reached 90% in initial 10 treatment cycles and there was no difference between 50mg and 100mg of CC treatment. There were remarkable differences among the pregnancy rates achieved during consecutive treatments at the primary (clinics and hospital), secondary (regional center hospitals) and advanced facilities. Consecutive treatment by CC for infertile women associated with anovulation may be effective during initial 10 cycles. Although ovulation can be confirmed during the following cycles, the stimulation protocol should be reconsidered after 12 consecutive cycles therapy with CC, because cycle fecundity decreases. Hence, to improve pregnancy rates, it is important to check the previous administration of CC which reflects adversely on the subsequent conception rate especially in advanced facilities where these patients are reffered to.

Med., 1994, 172 (3), 209-220-Twenty-one cases of atypical bronchplasty were selected from a series of 125 tracheobronchoplastic procedures by the same surgeon between 1979 and 1993 and were reviewed to assess the indications and technical problems. The procedures were classified as follows: Type A (3 cases) was anastomosis between the right main and lower bronchi with upper and middle lobe resection. Type B (4 cases) was anastomosis between the left main and upper segmental bronchi with resection of the lower lobe and lingula or between the left main bronchus and the basal bronchus with resection of the upper lobe and superior segment of the lower lobe. Type C (5 cases) was resection of the right lateral wall of the trachea with various types of lung resection. Type D (4 cases) was sleeve segmentectomy. Type E (2 cases) was bronchial reconstruction without lung resection. Type F (3 cases) was miscellaneous procedures. All these procedures except one achieved favorable results. It is emphasized that lung-preserving surgery must always be considered under strict observation using frozen section study, even if an unusual procedure is required. If lung tissue has to be resected, as little as possible should be removed.

    FUKAYA, T., SUGAWARA, J., YOSHIDA, H. and YAJIMA, A. The Role of Macrophage Colony Stimulating Factor in the Peritoneal Fluid in Infertile Patients with Endometriosis. Tohoku J. Exp. Med., 1994, 172 (3), 221-226-Macrophage colony stimulating factor (M-CSF) concentrations in the peritoneal fluid of 44 infertile patients were measured. The concentration of M-CSF was significantly higher in the patients with endometriosis than in those without endometriosis. There was no significant difference between the M-CSF concentration and stage of endometriosis classified by revised American Fertility Society (rAFS) classification in the 44 patients. However, in primary infertility patients with endometriosis there was a significant correlation between peritoneal M-CSF levels and rAFS points of endometriosis. We speculate that there is some relationship between the increased levels of M-CSF in the peritoneal cavity and the primary infertility associated with endometriosis especially progression of endometriosis.

Med., 1994, 172 (3), 227-235-To evaluate the effects of altered preload on the Doppler flow pattern of right ventricular inflow in the first day of life, serial Doppler echocardiography of the pulmonary artery (PA) and tricuspid valve was performed in 16 normal neonates at 2, 12, and 24hr. A computer-interfaced digitizer pad was utilized to measure the followings: PA flow velocity-time integral, total transtricuspid flow velocity-time integral, flow velocity-time integral of early diastolic filling (E area), and flow velocity-time integral of atrial contraction (A area). The PA flow velocity-time integral, total transtricuspid flow velocity-time integral, E area, A area, peak E, and peak A were increased significantly by 24hr compared with the values at 2hr of age. However, the peak E/A, E/A area and peak E/total transtricuspid flow velocity-time integral did not show significant changes from 2 to 24hr. The ductus arteriosus size was inversely correlated with the peak E (r=-0.46, p<0.05), E area (r=-0.37, p<0.05), and A area (r=-0.34, p<0.05). Therefore, the increase in right ventricular preload at 24hr is considered to be due to the cessation of the left-to-right shunt through the ductus arteriosus. These results suggest that the pattern of early and late diastolic filling of the right ventricle were dependent on the increase in preload. We can therefore conclude that the redistribution of transtricuspid flow induced by the preload condition should be taken into account to interprete the transtricuspid velocity pattern in terms of diastolic function.

Med., 1994, 172 (3), 237-242-Time-dependent ovulation-blocking effect of ether anesthesia was investigated using female Wistar-Imamichi rats showing a 4-day cycling rhythm. A 6-hr (1400-2000) and 8-hr (1400-2200) anesthesia with ether vapor on proestrus caused ovulation blockade in 85% and 100% of the rats, respectively. On the other hand, pentobarbital injections at 1400 and 1500 completely blocked the spontaneous ovulation, while the injections at 1600 and 1700 did not. Ether (1400-2000) and pentobarbital anesthesia decreased the mean serum LH levels at 2000 on proestrus. The LH levels were 13.26ng/ml for the control, 0.36ng/ml for the ether group and 0.56ng/ml for the pentobarbital group, respectively. These findings indicated that ether and pentobarbital similarly suppressed the LH surge, but difference between ovulation-blocking effects by ether and pentobarbital is possibly due to factor(s) other than suppression of the LH surge.

    TAKEMATSU, H. and TAGAMI, H. Lack of Correlation between Interleukin 6 and Interleukin 1 Levels in Psoriatic Lesional Skin. Tohoku J. Exp. Med., 1994, 172 (3), 243-252-The histopathologic features of active Psoriatic plaques are characterized by epidermal hyperplasia and the presence of inflammatory cells. Recently it has been strongly suggested that an increased local production of cytokines and growth factors by keratinocytes or by activated inflammatory infiltrates play an crucial role in the induction of these changes. Particularly keratinocytes are demonstrated to secrete several different immuno-inflammatory mediators, including interleukin 1(IL-1), IL-6 and IL-8. IL-1 induces IL-6 production in vitro, which in turn stimulates the proliferation of human keratinocytes. Although IL-1 has been reported to be reduced in samples from psoriatic lesions, enhanced immunohistochemical expression of IL-6 has been shown in psoriatic lesional skin. Thus, we designed the present study to elucidate the paradoxical situation of IL-1 and IL-6 expression in psoriatic skin lesions by directly measuring these cytokines in tissue fluids collected from suction blisters as well as in horny tissue extracts. As reported before, the levels of immunoreactive IL-1-α or -1β tended to be reduced in the scale extracts of psoriasis and related pustular dermatoses. In contrast, the levels of immunoreactive IL-6 tended to be increased, despite the presence of great variations between samples, a significant elevation being found only in the scale extracts of pustular psoriasis. Although the mean of IL-6 levels in the suction blister fluids from psoriatic involved skin was higher than those from normal or psoriatic uninvolved skin, again no statistical significance was noted. Moreover, no significant correlation was observed between the levels of immunoreactive IL-1 and IL-6 in these materials. Our direct measurements of these cytokines in lesional tissue samples do not provide evidence suggesting any close interrelationship between IL-1 and IL-6 nor provided evidence suggesting a pivotal role for IL-6 in the pathogenetic mechanism of psoriasis.

Med., 1994, 172 (3), 253-262 -Two temporary occlusions of the bilateral common carotid artery for 15min with an interval of 15min induced severe brain tissue damage in normotensive Wistar rats. Although no lesions were seen after a single 15-min occlusion, severe ischemic lesions were found in 68% (49/72) of the rats subjected to two consecutive occlusions for 15min with an interval of 15min, and the incidence was significantly higher than that in the rats subjected to a single 30-min occlusion (32%, 6/19) or two 15-min occlusions with an interval of 30min (11%, 1/9). The local cerebral blood flow (1CBF) in the caudoputamen, however, was decreased by only 40-60% of pre-occlusion level during occlusions. These results suggest that intermittent incomplete disruption of CBF with short intervals could be more hazardous than a continuous interruption of the same duration.

Med., 1994, 172 (3), 263-273-Localization of guanine nucleotide binding protein (G-protein) α chains in human kidney was examined using immunoblotting and immunocytochemical techniques. Immunoblot studies revealed Gαs, Gαi1, 2 and Gαi3 proteins in membrane fractions of glomeruli, cortical tubules and medullary tubules. Immunocytochemical studies confirmed the presence of these G-proteins in several parts of human nephron. The results demonstrate the presence of a large amount of G-protein α subunits in human nephron segments. We speculate that G-protein in human kidney may play an important physiological role well-defined in other mammmalia.

Tohoku J. Exp. Med., 1994, 172 (3), 275-282-One hundred and sixty-two esophageal carcinomas curatively resected and followed by postoperative irradiation were analyzed to investigate whether the delay of postoperative irradiation affected the patients' prognosis. The period of 40 or more days between surgery and postoperative irradiation caused significantly worse survival rates, and this fact was true even if the survival rates were corrected by the other significantly important prognostic factors: age and tumor staging. The incidence of infield recurrence and cancerous death were higher in the patients with the waiting period of 40 or more days. Significant repopulation of residual tumor cells in curatively resected esophageal carcinomas begins 40 days after surgery, therefore, postoperative irradiation should be started within 40 days after a radical resection. The difference of survival rates as related to the waiting period became clear after the follow-up of more than four years. The prognostic difference of subclinical tumors seems to be uncovered several years after treatment.

Med., 1994, 172 (3), 283-289-Genetic alteration of the p53 gene was examined in tissue specimens of primary gallbladder cancer (GBC) using PCR-single strand conformation polymorphism (SSCP) and direct sequencing analyses. The p53 gene mutation was detected in five of the 16 GBC cases examined (31%). All five cases showed single point mutations; four were mutations resulting in amino acid substitution and the other was one base-pair deletion. Of the five mutations detected, four occurred at evolutionarily conserved regions and the remaining one at a well conserved region among species. Our result is similar to those of previous reports for various human cancers in mutation scattering over four of the five conserved domains.

Med., 1994, 172 (3), 291-293 -C3H mice bearing subcutaneously transplanted mouse bladder carcinouma (MBT-2) were treated with intramuscular injection of bleomycin followed by local delivery of electric pulses at the tumor site. The tumors markedly reduced and even disappeared for several days after this treatment. Neither electric pulses nor Bleomycin administration alone showed a significant inhibitory effect on the tumor growth. Thus, it is demonstrated that in mouse bladder tumor the antitumor effect of bleomycin can be considerably potentiated by local electric pulses.
