Med., 1983, 139 (2), 113-119-From the national and regional statistics on the body dimensions of schoolchildren at the 6th year grade in primary schools (i.e., 11-year-olds) and at the 3rd year grade in junior high schools (i.e., 14-year-olds), the secular gains in height and weight by the children of the same age were calculated for the three periods of 1960-4, 1972-76 and 1976-81. Compared with the first period (i.e., 1960-4) when the growth acceleration was maximal, the secular gain was apparently reduced in the second and third study periods (i.e. 1970's) to the level of 1/2 to 1/3 of that in the first period, and was next to nil in some districts. Such deceleration was coupled with changes in seasonal growth patterns and diminution of urban-rural gaps both in the body size and the secular gain. For example, the body dimensions of the children in the City of Sendai did not differ in 1981 between the core part of the city and the surroundings, while the difference had been observed till late 1970's. The 1979 measurements of height and weight of Japanese children are compared with counterpart values observed in other countries.

Med., 1983, 139 (2), 121-131 The present authors have made it a rule to use a modified Mellet-Guy's method for measuring pressure in the bile duct. The determined manometric pressure expressed in curve has been classified into (a) basal pressure, (b) rising time, (c) perfusion pressure, (d) dumping time and (e) residual pressure. The basal pressure (a) represents a static pressure free from any load in the bile duct. The rising time (b) varies with factors including the measuring apparatus, ductal elasticity and capacity, resistance at the choledochal terminal, and the volume of physiological saline infused. The perfusion pressure (c) is likely to undergo interference of a variety of factors such as the ductal diameter, mural elasticity and thickness, resistance at the choledochal terminal, along with the measuring apparatus. The dumping time (d), free from the influence of the measuring apparatus, is considered to be an important guide for us to guess the condition of choledochal terminal. The residual pressure (e) and the trend of manometric pressure curve help us to surmise the choledochal terminal resistance and the presence of ductal abnormalities. After all, among the above factors, the dumping time and the residual pressure are apparently available for objective evaluation of bile duct pressure.

    SAITO, Y. and UZUKA, Y. Colony Forming Ability of Bone Marrow Cells from Patients with Acute Leukemia. Tohoku J. exp. Med., 1983, 139 (2), 133-139 - Bone marrow cells from adult patients with acute leukemia were serially cultured using an in vitro agar culture method. Four categories of growth pattern were recognized according to growth characteristics in vitro in previously untreated patients with leukemia. Detailed cytological investigation was performed for the determination of cell morphology and the origin of colonies. In vitro agar culture may be useful for monitoring the remission-relapse status in acute leukemia.

Med., 1983, 139 (2), 141-150 - A 74-year-old man with brady-tachy syndrome showed split atrial activities in electrophysiologic studies. The split intervals of atrial activities varied spontaneously or in response to electric stimulations. The shorter were the coupling intervals of atrial extrastimuli, the longer the split intervals of captured atrial activities. At a critical timing of atrial extrastimuli, atrial flutter-fibrillation was initiated reproducibly. During atrial flutter-fibrillation, the split activities showed Wenckebach-like phenomenon. These findings seem to result from depressed conductivity. The split atrial activity may reflect a diseased state of the atrium which predisposes to atrial arrhythmias.

    NAKANOME, C., AKAI, H. and GOTO, Y. Serum Group I Pepsinogen Levels in Patients with Peptic Ulcer and Normal Subjects. Tohoku J. exp. Med., 1983, 139 (2), 151-158-Serum group I Pepsinogen (PG I) levels were measured by radioimmunoassay in patients with peptic ulcer and normal subjects. The mean (±S.E.) serum PG I level in 318 normal subjects was 79±3ng/ml. The level in males, 87±2ng/ml (n=246), was significantly higher than in females, 72±4ng/ml (n=72). The serum PG I levels in the patients with gastric ulcer and in those with duodenal ulcer were 91±7ng/ml (n=31) and 117±10ng/ml (n=31), respectively. Both values were significantly higher than the value in the subjects with endoscopically normal mucosa (63±5ng/ml). No significant change in serum PG I was observed after subcutaneous injection of tetragastrin or after ingestion of meal. A significant correlation was found between serum PG I and stimulated pepsin output, peak pepsin output, and maximal acid output and peak acid output. These findings suggest that serum PG I may be determined by the chief cell mass.

    GOTO, K. Transmembrane Macromolecule Rotation Model-Molecular Hypothesis of Membrane Excitation-. Tohoku J. exp. Med., 1983, 139 (2), 159-164 -A hypothesis concerning the molecular changes in the nerve cell membrane during excitation is presented. Based upon established principles about the interaction of hydrophobic and hydrophilic ligands and upon recent facts concerning the contraction of neurofilaments, the hypothesis accounts for both the initiation of the action potential and its conduction along the nerve axon without attenuation. The hypothesis can be summarized as follows: When acetylcholine combines with the acetylcholine receptor (AchR) α subunit, hydrophobic subsites are exposed at the bound surface of AchR. Since hydrophobic portions are quite unstable, the subsites move to the center of the membrane, which is hydrophobic. It is postulated that the rotation of receptor subunits makes the center an ion channel. Ca++ flowing in through the ion channel causes the undercoating filaments to contract and this contraction results in the conduction of an action potential along the axon. The contraction of the undercoating filaments induces transmembrane rotation of globular protein connected to the filaments. At right angles to the axon the force of the hydrophobic bond between membrane macromolecules is so strong that many cylinderical lipid structures are formed. The gaps between the rotatory cylinders serve as ion channels. Ca++ passing through the ion channels allows for the conduction of an action potential without attenuation.

    UENO, N., MATSUMOTO, T., OHKAWA, M., NAKAYAMA, M. and TONO-OKA, T. Urinary Arylsulfatase in Normal Children and in Patients with Pediatric Malignant Disease. Tohoku J. exp. Med., 1983, 139 (2), 165-169 - Urinary arylsulfatase (AS) activities were measured in 20 normal children and 10 patients with malignant disease, consisting of leukemia (6), Hodgkin's disease (1), neuroblastoma (2), and malignant teratoma (1). Seven of the patients showed a significantly high activity, and a serial measurement carried out in 4 patients showed a well correlated relationship between AS activity and the activity of disease. Thus the measurement of urinary AS activity could be a laboratory test for monitoring the activity of malignant diseases because of its simple and rapid procedure.

    TEZUKA, F. and SAWAI, T. Hyperplasia of Small Hepatic Cells in the Precancerous Condition of Cirrhotic Livers. Tohoku J. exp. Med., 1983, 139 (2), 171-177 - Karyometrical analysis was performed in order to characterize histologically the precancerous condition of cirrhotic livers. The following parameters of hepatocytic nuclei were estimated in 29 normal and 35 cirrhotic livers: Nvo, the number of nuclei in a unit volume of hepatic tissue; D, the mean of nuclear diameters; s, the standard deviation of nuclear diameters (the degree of anisokaryosis); and N/C, the nucleocytoplasmic volume ratio. Results indicate: (1) In normal livers, Nvo is inversely correlated with age (r=-0.53, p<0.01), D and s are positively correlated with age (r=0.68, p<0.01, and r=0.75, p<0.01, respectively) and N/C is almost constant regardless of age. (2) Each of the parameters is independent of age in cirrhotic livers. (3) Nvo and N/C are larger, and D and s are smaller in cirrhotic livers with hepatocellular carcinoma than in those without carcinoma (p<0.05). From these results, it is concluded that the parenchyma of cirrhotic livers with hepatocellular carcinoma, which is in strongly precancerous conditions, can be characterized by hyperplasia of small hepatic cells rather than by liver cell dysplasia which is defined by marked anisokaryosis and nuclear pleomorphism.

Diurnal Profile of Gallbladder Size in Diabetic Patients: Ultrasonographic Evaluation of Diabetic Neurogenic Gallbladder. Tohoku J. exp. Med., 1983, 139 (2), 179-186 The diurnal change of gallbladder size was clearly measured with a, real time ultrasonography. To visualize the gallbladder, 21 diabetic patients and 11 healthy subjects were scanned right subcostally in the supine position nine times between 8:00a.m. and 8:00p.m. Photographs of the images were produced on Polaroid film, and the area of gallbladder was calculated. The diurnal changes of the gallbladder size were expressed as the percentage of the size measured at 8:00a.m. after an overnight fast. The gallbladder in normal control started contracting immediately after breakfast and reached 57.2±7.6% at 9;00a.m. In the diabetic patients with and without autonomic neuropathy the gallbladder contracted to 80.9±14.5% and 72.4± 14.8%, respectively, at 9:00a.m. while the nadir of the diurnal profile curve in healthy subjects was 24.5±3.6% at 3;00p.m., in the diabetic patients with and without autonomic neuropathy those were 57.7±6.1% at 6:00p.m. and 41.3±8.3% at 8:00p.m., respectively. This implies that the gallbladder contraction was delayed and poor in diabetic patients. In this paper, the gallbladder tonicity index is proposed, which is useful for the easy evaluation of autonomic state of gallbladder in diabetes mellitus.

Med., 1983, 139 (2), 187-193-A strain of SHR which develops hypertension spontaneously is marked by a selective depression of T-cell functions associated with an early appearance of natural thymocytotoxic autoantibody and a deficiency of thymic hormone. Present results demonstrate that various immunopotentiators (IPs) such as thymostimulin (TS), PS-K, SPG, neurotropin (NSP), and K-247 partially or almost completely reversed the T-cell depression in these SHR as detected by a rosette forming test, plaque-forming assay and blastogenesic response to PHA and Con A. In contrast, these IPs had no effect on the immune responsiveness of WKA rats with normal T-cell functions. Among the IPs, NSP, which had an almost complete restorative effect on the T-cell functions of SHR, induced significant transplantation resistance to the syngeneic tumor challenge. A new synthetic product K-247 also induced a significant suppression of lethal tumor growth in SHR, though its restorative effect on T-cell functions was weak. The fact that K-247 had a suppressive effect on tumor cell growth in vitro indicates that biochemical modifications rather than immunological ones may be involved. However, none of the IPs induced antitumor resistance in normal WKA rats. These results suggest that this strain of SHR provides a useful animal model for evaluation of various IPs.

Med., 1983, 139 (2), 195-203 -Exogenously administered glucagon causes a decrease of plasma gastrin concentration, but the mechanism has not been fully elucidated. We measured antral gastrin concentrations and plasma gastrin levels before and during glucagon infusion in nine patients free of gastrointestinal diseases. In addition, we measured plasma gastrin levels in both the renal artery and renal vein in six patients with normal renal function before and during glucagon infusion. Glucagon caused a significant increase of antral gastrin concentration concomitant with a significant decrease of plasma gastrin level and a significant decrease of percent extraction of gastrin in the kidney. These observations suggest that glucagon causes the decrease of plasma gastrin level by inhibiting the release of gastrin from the antrum, but not by accelerating the renal extraction of gastrin.

Disturbances of the Alimentary Tract Motility and Hypermotilinemia in the Patients with Diabetes Mellitus. Tohoku J. exp. Med., 1983, 139 (2), 205-215-Lower esophageal sphincter pressure (LESP), gastric emptying, small bowel transit time and plasma motilin levels were measured in diabetics and normal subjects in order to investigate the disturbances of the alimentary tract motility and the participation of motilin in these motility disorders. Hypermotilinemia was observed in all diabetics with or without autonomic neuropathy. Low response of LESP to tetragastrin found in diabetics with autonomic neuropathy could not be explained by motilin. Gastric emptying was highly correlated with fasting plasma motilin levels and a significantly accelerated gastric emptying observed in diabetics without complications or diabetics with diarrhea was considered to be due to hypermotilinemia. On the contrary, no significant correlation was observed between small bowel transit time and plasma motilin levels, suggesting no participation of endogenous motilin in the regulation of small bowel transit.

    HASHIMOTO, Y. Interaction of Sisomycin with Pancuronium. Tohoku J. exp. Med., 1983, 139 (2), 217-218-The effect of sisomycin sulfate (75mg) on the neuromuscular junction was studied in anesthetized patients. Sisomycin sulfate alone did not depress twitch tension. When twitch tension after pancuronium had recovered to 50% of the control level, i.v. administration of sisomycin sulfate caused a decrease in twitch tension. The combined sisomycin-pancuronium-induced blockade was antagonized by calcium chloride (400mg) or edrophonium (10mg).

Med., 1983, 139 (2), 219-220-Seven normal subjects were asked to flex the left elbow as quickly as possible, responding to a tone stimulus under three conditions: 1) during passive flexion of the elbow, 2) during passive extension and 3) without passive movements. The EMG activities in passive movements reduced significantly during first 60 to 70msec, compared to the condition without passive movements.

    KANEDA, H., TAKADA, K., MAETA, T., SHITOMI, K. and MURATA, T. Dialysis Encephalopathy Relieved by Hemofiltration: A Case Report and a Review. Tohoku J. exp. Med., 1983, 139 (2), 221-222-Dialysis encephalopathy (DE) is characterized by a symptom complex of stuttering speech, myoclonic jerk, fit, psychosis and dementia observed in patients undergoing hemodialysis. Its cause is believed to be accumulation of aluminium in the brain. No effective treatments are known to date. A female patient, 61 years of age, with DE was successfully treated by hemofiltration in our hospital. She had been hemodialysed twice weekly. At 13 months after the start of hemodialysis, she began to suffer from finger tremor, myoclonic jerk, stuttering and irrelevant speeches and disorientation. Her EEG showed, from time to time, slow waves with high amplitude which were compatible with DE. Soon after the start of hemofiltration instead of hemodialysis, these symptoms and EEG abnormalities were dramatically improved. She had received aluminium hydroxide (4-6g/day) before and after the development of DE. As for the cause of her DE, it is reasonable to think that the etiological factors can be removed by hemofiltration, but not by hemodialysis, and so their molecular weights may be less than 30, 000. Aluminium itself may not be responsible for the development of her DE.

    TORINUKI, W, and MIURA, T. Total and HDL-Cholesterol Levels in the Serum of UVA-Irradiated Porphyric Rats. Tohoku J. exp. Med., 1983, 139 (2), 223-224 - Total and HDL-cholesterol concentrations were measured in hexachlorobenzene-induced porphyric rats before and after repeated exposure to UVA for a month. No significant change in either concentration was seen.
